Qualification Statement

Tab 4 - Key Proposed Personnel and Team Organization

Vendor Project Team Resumes Resumes shall be provided for the implementation team as well as any additional personnel involved in live operation and ongoing support and maintenance. Resumes shall be specific to the actual personnel to be assigned to this Project for all primary roles (Project Manager, Trainer, Conversion Lead, etc.). A Team Member Resume Table shall be completed for each named resource included in the Proposal.

For each Project referenced in the Table below, a Project Background Form (provided with Tab 13) shall be completed. In the event multiple named personnel worked together on the same Project, only one (1) Project Background Form is required to be completed for that Project.
