The XXI Meeting of the Directing Council of the Pan American HealthOrganization, XXIV Meeting of the Regional Committee of the World HealthOrganization for the Americas, was held at the Casa de la Cultura "GabrielaMistral" in Santiago, Chile, from 10 to 13 October 1972, as convened by theDirector of the Pan American Sanitary Bureau, in accordance with ResolutionXIX 1 approved by the Executive Committee at its 68th Meeting.

The Meeting was opened by Dr. Jesse L. Steinfeld, Surgeon General of thePublic Health Service of the United States of America and President of the XXMeeting of the Directing Council. The following Governments were represented:Argentina, Barbados, Bolivia, Brazil, Canada, Chile, Colombia, Costa Rica, Cuba,Dominican Republic, Ecuador, El Salvador, France, Guatemala, Guyana, Haiti,Honduras, Jamaica, Kingdom of the Netherlands, Mexico, Nicaragua, Panama,Paraguay, Peru, Trinidad and Tobago, United Kingdom, United States ofAmerica, Uruguay, and Venezuela. The Executive Committee was representedby its Vice-Chairman, Dr. Hugo V. Alquéres Baptista, in accordance with Rule14 of the Rules of Procedure of the Committee.
The meeting was also attended by Dr. Thomas A. Lambo, AssistantDirector-General of the World Health Organization as well as by observers of theOrganization of American States (OAS), the Inter-American Development Bank(IDB), the United Nations Children's Fund (UNICEF), the United NationsDevelopment Program (UNDP), the International Labour Organisation (ILO),and the Food and Agriculture Organization of the United Nations (FAO).Observers from 11 nongovernmental organizations were also present.

AGENDA
In accordance with Rule 10 of its Rules of Plrocedure, the Directing Council,at its first plenary session, adopted the provisional agenda (Document CD21/1,Rev. 2)3 submitted by the Director, with the addition of Item 29, Comrmunicable Diseases in Border Areas. It was agreed to postpone consideration ofItems 20 and 21, on the Technical Discussions, until the next Meeting of theCouncil.
WORKING PARTY
A working party, composed of the Representatives of Argentina, Colombia,and Mexico, was established to study the application of Article 6-B of theConstitution concerning the suspension of the voting privileges of anyGovernment two years in arrears in the payment of its quotas. The workingparty presented a report and a proposed resolution in plenary session.

In establishing the contributions of Member Governments and ParticipatingGovernments, their assessments shall be reduced further by the amount standingto their credit in the Tax Equalization Fund, except that credits of thoseGovernments who levy taxes on the emoluments received from the PanAmerican Health Organization by their nationals and residents shall be reducedby the amounts of such tax reimbursements by the Organization.
3. That, in accordance with the Financial Regulations of the Organization,amounts not exceeding the appropriations noted under Paragraph 1 shall beavailable for the payment of obligations incurred during the period 1 January to31 December 1973 inclusive. Notwithstanding the provision of this paragraph,obligations during the financial year 1973 shall be limited to the effectiveworking budget, i.e., Parts I-VI.
4. That the Director shall be authorized to transfer credits between parts ofthe effective working budget, provided that such transfers of credits betweenparts as are made do not exceed 10 per cent of the part from which the credit istransferred. Transfers of credits between parts of the budget in excess of 10 percent of the part from which the credit is transferred may be made with theconcurrence of the Executive Committee. All t:ransfers of budget credits shall bereported to the Directing Council and/or the Conference.

Resolution II
Assessments of the Member Governments and Participating Governmentsof the Pan American Health Organization
The Directing Council,
Taking into account that Member Governments appearing in the scaleadopted by the General Assembly of the Organization of American States areassessed according to the percentages shown in that scale, in compliance withArticle 60 of the Pan American Sanitary Code; and
Considering that other Member Governments and Participating Governmentsare assessed on the basis of percentages which would be assigned to suchcountries if they were subject to the OAS scale,

Resolution III
Proposed Program and Budget Estirriates of the World HealthOrganization for the Region of the Americas for 1974
The Directing Council,
Having considered Official Document 114, submitted by the Director of thePan American Sanitary Bureau, which contains the proposed program andbudget estimates of the World Health Organization for the Region of theAmericas for 1974;
Bearing in mind that the proposed program and budget estimates aresubmitted to the Directing Council as Regional Committee of the World HealthOrganization for the Americas, for review and transmittal to the Director-General of that Organization so that he may take them into account whenpreparing the proposed budget estimates of WHO for 1974; and
Noting the recommendations 4 made by the 68th Meeting of the ExecutiveCommittee,
Resolves:
1. To approve the proposed program and budget estimates of the WorldHealth Organization for the Region of the Americas for 1974, appearing inOfficial Document 114, and to request the Regional Director to transmit themto the Director-General of that Organization so that he may take them intoaccount in preparing the WHO budget estimates for 1974.
2. To request the Director General that, when preparing those proposedbudget estimates, he give favorable consideration to increasing the proportionassigned to the Region of the Americas.

Provisional Draft of the Proposed Prograni and Budget Estimates of thePan American Health Organization for 1974

Having examined Official Document 114 submitted by the Director of the

Pan American Sanitary-Bureau, which contains the provisional draft that is toconstitute the basis for the preparation of the proposed program and budgetestimates of the Pan American Health Organization for 1974, to be consideredby the 70th Meeting of the Executive Committee and by the XXII Meeting ofthe Directing Council; and
Recognizing that the provisional draft of the proposed program and budgetestimates contains much-needed projects prepared in cooperation with thenational authorities,
Resolves:
1. To take note of the provisional draft of the proposed program and budgetestimates of the Pan American Health Organization for 1974, appearing inOfficial Document 114.
2. To request the Director to use the provisional draft as a basis for thepreparation of the proposed program and budget estimates for 1974, afterfurther consultations with Governments to determine their latest desires andrequirements with reference to the health priorities of the countries.
3. To request the Executive Committee to make a detailed examination ofthe revised program and budget estimates for 1974 to be presented by theDirector, after further consultation with Governments, and to submit itsrecommendations thereon to the XXII Meeting of the Directing Council.

Resolution V
Percentage Relationship of Contributions to theRegular Budget of PAHO
The Directing Council,
Having studied the constitutional basis as well as policies established by theDirecting Council for calculating assessments to the regular budget of PAHO,and noting that a constitutional amendment would be required to change thesystem;
Recognizing that as long as there are Governments in PAHO which are notmembers of the OAS, the true percentage of each Government's contributionwill be less than that shown in the assessment resolution, but that eachGovernment can easily calculate or obtain from the Director its actualpercentage; and
Considering that the present situation does not justify a change in theConstitution of the Pan American Health Organization,

To request the Director to furnish annually to the Executive Committee adocument similar to that provided to the 68th Meeting of the Committee(Document CE68/12).5

Amendments to the Financial Regulations of PAHO

Having considered the recommendation 6 of the Executive Committee and themodifications proposed in the Financial Regulations as shown in DocumentCD21/14;7
Taking into consideration that similar changes in the Financial Regulations ofthe World Health Organization were approved8 by the Twenty-Fifth WorldHealth Assembly; and
Bearing in mind the provisions of Financial }Regulations 15.1,
Resolves:
To approve the amendments in the Financial Regulations of the PanAmerican Health Organization as they appear in the annex to DocumentCD21/14.

Having considered the Financial Report of the Director and the Report of theExternal Auditor for fiscal year 1971 (Of]i.cial Document 113), as well as'Resolution IO approved by the Executive Committee at its 68th Meeting,
Resolves:
1. To approve the Financial Report of the Director and the Report of theExternal Auditor for fiscal year 1971 (Official Document 113).
2. To commend the Director for having consistently pursued long-termfinancial policies that have enabled the Organization to achieve and maintain asound financial position.

Engineering and Maintenance of Medical Care Servicesand Other Health Services
The Directing Council,
Having considered the report presented by the Director on engineering andmaintenance of hospitals and other health services (Document CD21/10),' 1 aswell as Resolution XIV12 of the 68th Meeting of the Executive Committee;
Recognizing the importance of hospital maintenance services to ensure goodquality of health care delivery; and
Bearing in mind that, to keep pace wit:h the increasing demands for aneffective health care delivery system measures must be taken to upgrademaintenance and engineering in all areas of he alth,
Resolves:
1. To coqmmend the Director for the effective support extended to theHospital Maintenance and Engineering Center in Caracas, Venezuela, the

activities of which have been essential for promoting interest in this fieldthroughout the Region.
2. To urge ministries of health of all the countries of the Region to establisha maintenance unit at the national level and to request financial assistance fromthe United Nations Development Program (UNDP), whenever necessary; toassign qualified personnel and an adequate budget to each health maintenanceactivity; and to train personnel to effectively handle and operate themaintenance program within the health service.
3. To recommend to the Director that he continue to promote a regionalmaintenance and engineering program for medical care services and other healthservices and that he explore possible national or international sources offinancing to help interested countries in the expansion of these services.

PAHO Award for Administration

Having considered the report of the Committee on the PAHO Award forAdministration (Document CD21/17 and Annex);13 and

To declare Dr. Eduardo Zapata Salazar of Peru the winner of the 1972 PAHOAward for Administration.

Having taken note of Document CD21/16 and annexl4 on country reports

on health conditions, prepared by the Director of the Pan American SanitaryBureau; and
Bearing in mind the interest expressed by the countries in continuing topresent to Pan American Sanitary Conferences a report on the progress achievedin the health field,
Resolves:
1. To request the countries to send to the Pan American Sanitary Bureau notless than 60 days before the date of the meeting of the Pan American SanitaryConference a report setting forth the most important developments in the healthfield in the past four years, with special emphasis on changes and innovations inthe organization and administration of health services and on novel aspects ofprograms in operation which are of general interest.
2. To recommend to the XIX Pan American Sanitary Conference that duringplenary sessions, the delegates limit their speeches to a statement of the mostimportant points in the above-mentioned report or of any other informationthey consider to be of interest to the Conference. The statement shall notexceed four double-spaced typewritten pages and the time for reading it shallnot exceed 10 minutes. For the purposes of this presentation, delegates are to beentered previously on the list of speakers.
3. To request the Director of the Pan American Sanitary Bureau tocommunicate with the health authorities of the countries not less than sixmonths before the date of the meeting of the Conference reminding them of theterms of this resolution.

Resolution XIII
Report on the III Special Meeting of Ministers of Health of the Americasand Steps Necessary to Implemenl the Decisions Adopted
The Directing Council,
Having heard the statement by the Director of PASB on the III Meeting ofMinisters of Health of the Americas, held in Santiago, Chile, 2-9 October 1972;and
Considering that the recommendations I s made by the Meeting constitutevaluable guidelines on the direction to be taken by the programs of the countriesduring the next decade,

Resolves:
1. To incorporate into the policy of the Pan American Health Organizationthe recommendations of the III Meeting of Ministers of Health of the Americas.
2. To request the Director of PASB to study the implications of thoserecommendations and the consequent modifications in the objectives andpriorities of the program of the Organization, and to report thereon to the XXIIMeeting of the Directing Council.
3. To recommend to the countries that, within their planning process, theyidentify the priority health problems and establish objectives for each of them inaccordance with the manpower, physical, and financial resources available,taking into account the regional health goals.
4. To request the Director of PASB to convene as soon as possible a workingparty of personnel responsible for planning and information and of healtheconomists, with a view to designing an evaluation system that can be adapted tothe conditions of each country and still be flexible enough to give comparableresults and in turn permit a continent-wide evaluation of the achievements of thedecade.
5. To recommend to PAHO that, in consultation with experts of thecountries, general guidelines be drawn up for determining the present financingof health investments and the changes required to carry out the plans andprograms envisaged in the Ten-Year Health Plan for the Americas, 1971-1980.
6. To suggest to the health authorities of the countries that they initiate coststudies of their health services and, whenever possible, cost-benefit studies,particularly in areas with the largest investment.
7. To recommend to PAHO that it prepare the necessary designs to ensurecomparability of the cost studies.
8. To recommend that PAHO furnish advisory services to countries request-ing them, in all matters relating to the financing of the health sector, such as thefinancial analysis of the sector, analysis of health expenditures, programming ofinvestments, management and financing of specific projects and of externalcredits, so that the countries will gradually come to know what financialresources are earmarked for health and how they are related to the benefitsobtained.

Considering the provisions of Article 9-B of the Constitution of the PanAmerican Health Organization; and

Considering that the Governments of Haiti, Mexico, and Panama were electedto the Executive Committee on the termination of the periods of office ofGuyana, Peru, and Venezuela,
Resolves:
1. To declare the Governments of Haiti, Mexico, and Panama elected to theExecutive Committee for a period of three years.
2. To thank the Governments of Guyana, Peru, and Venezuela for theservices rendered to the Organization by their representatives on the ExecutiveCommittee.

Resolution XV'
Financing of the Regular Budget of thePan American Health Organization
The Directing Council,
Considering that at present approximately 99 per cent of the funds for theregular budget of the Pan American Health Organization are derived from thequota contributions of Member Governments and Participating Governments,i.e., that income from other financial sources represents only just over 1 per centof the total regular budget;
Bearing in mind that, among other factors, the high rate of natural populationincrease in the Hemisphere, the commitment entered into by the Governmentsat the III Special Meeting of Ministers of Health of the Americas to attainrealistic but ambitious goals, and the continuous increase in the production costsof health services due to scientific and technological development, are sufficientreasons for considering the problem of the :necessary increase in the PAHObudget a matter of the utmost seriousness and urgency;
Recognizing that it is not possible for the burden of that increase to continueto fall almost exclusively upon the quota contributions of the MemberGovernments and Participating Governments, fi'om which it can be deduced thatthe most advisable and feasible course is to look for means of achieving a rapidand substantial expansion in the sources of financing in addition to quotacontributions, so that the major part of the budgetary increase would be derivedfrom those new sources, apart from the historical process of growth in quotacontributions that has been taking place and will continue to occur; and
Considering that in this regard the efforts which the Pan American SanitaryBureau has been making toward establishing a financial institution to promote

the raising of supplementary financial resources for the Organization's budgetare commendable,
Resolves:
1. To instruct the Director of the Pan American Sanitary Bureau to establisha special group of experts to study the problem of obtaining financial resourcesto supplement the quota contributions of Member Governments and Partici-pating Governments, and that the group propose concrete alternative solutions,feasible and capable of immediate application, to the 70th Meeting of theExecutive Committee.
2. To request the 70th Meeting of the Executive Committee consider thereport of the special group of experts and on the basis thereof present suitablerecommendations to the Directing Council at its XXII Meeting so that the mostappropriate alternative solution may be implemented as rapidly and effectivelyas possible.

Recognizing the importance of strengthening the coordination of activitiescarried out by the ministries of agriculture and of health in the field of humanand animal health;
Mindful of the need for continued efforts in manpower development inanimal health planning;
Being aware of the seriousness of the public health hazards resulting from theindiscriminate use of pesticides, particularly in view of the lack of adequatelegislation or of analytical laboratories to aid in the needed remedial action; and
Noting that the forthcoming completion of the Pan American Highwaythrough the Darien Gap will increase the risk of introduction of certain diseasesof human and animal significance into areas now free of them,
Resolves:
1. To take note of the Final Report of the V Inter-American Meeting,.at theMinisterial Level, on Foot-and-Mouth Disease mnd Zoonoses Control (DocumentCD21/5).
2. To emphasize the advisability of having Iepresentatives of the ministries ofhealth attend future Inter-American Meetings on Foot-and-Mouth Disease andZoonoses Control, in order to obtain a better coordination of activities in thisfield.
3. To express its thanks to the ministries of agriculture for their endeavors inthe control of these diseases of human and animal health significance, which, ifallowed to persist, would hinder the economic and social development of thecountries.
4. Quota Collections and Application of Article 6-Bof the PAHO Constitution
The Directing Council,
Having considered the report on the collection of quota contributions(Document CD21/12 and Addendum I);1 8 and
Having received and considered the report of the working party on theapplication of Article 6-B of the Constitution of the Pan American HealthOrganization, which indicated:
a) That two countries, Bolivia and Haiti, have made payments in 1972toward fulfillment of plans for payment of arrears within a definiteperiod, previously approved by the Directing Council; and that bothintend to make further payments before the end of this year;
b) That two countries, Paraguay and Uruguay, have made partial quotapayments in 1972, and each intends to make further payments so as tocomplete their quotas for 1972. In addition, each Government proposes toadopt a plan, starting in 1973, to pay an amount equal to its current quotaplus 10 per cent of the arrears outstanding at the end of 1972;
c) That the Government of the Dominican Republic plans to pay the amountof its current quota, US$54,065, before the end of 1972. Furthermore, itproposes to adopt a plan, starting in 1973, to pay an amount equal to itscurrent quota plus 10 per cent of the arrears outstanding at the end of1972,
Resolves:
1. To take note of the report on the collection of quota contributions(Document CD21/12 and Addendum 1).
2. To thank the Governments that have already made payments in 1972, andto urge all Governments to pay remaining balances of arrears and current yearquotas before the end of the year.
