Clinical Responsibilities for Care, MDS Compliance, and Quality Christopher Kennedy & Alicia Cantinieri | Zimmet Healthcare Services Group

1:00pm — 4:15pm

As dynamics have changed in the post-acute care environment, SNFs are under the microscope of surveyors, auditors, and consumers, and documentation to support quality care is more important than ever. From admission assessment to care planning and delivery to outcomes and discharges, the MDS and care plan can determine success or failure on various scales. As clinical teams struggle with the provision of care over paper, documentation serves as the communication link required for appropriate plans for care delivery to reimbursement. Facility documentation serves as a litmus test for solid policies, procedures, and organizational strengths. With the addition of PDPM and case-mix elements to the OBRA assessments, the MDS process requires review and evaluation before submission to third parties. This deep dive will highlight: • Core areas of the MDS and Care Planning• Processes that facilities should consider for operational and organizational success• Program will incorporate CMS, QIO and other readily available tools and formats for facilities to utilize in QA and QAPI programs

8:45am —9:45am Leveraging Lessons Learned in the Aftermath of a Pandemic Amy Lee | Coretactics Healthcare Consulting | Rexford, NY

For providers, caring for the elderly during a pandemic was filled with bold heroism in the face of unprecedented challenges such as supply chain disruptions, worsened staffing challenges, medical supply shortages, unexpected loss of life and exhausted caregivers.

Over a year later, CMS is still not giving us any indication of loosening regulations for nursing facilities any time soon. In fact, the direct opposite appears to be looming with the push for tougher regulations, loftier non-compliance fines and ongoing national facility closures. This session will bring forward discussion on lessons providers have learned, review recent survey trends and summarize how we can use all of this as steppingstones to prepare our facilities for success in “the new norm.”

11:30am — 12:15pm e Fabulous Four: Inspiring the People You Need to Succeed and rive Amy Pearl | Work Ignited | Buffalo, NY

Smart leaders like you know they can’t do it alone. That’s why they intentionally look for four people to support their vision and achieve their goals. In this presentation, you’ll meet those four people in order to find them on your own team. Then, you’ll learn the secrets to setting the stage for their success. Most importantly, you’ll learn how once you have that team firing on all cylinders, you’ll find time for those big ideas you’ve been putting off and, who knows…maybe even some time for some self-care!

One of the most distressing and powerful events any person can experience is learning that the death of someone they know occurred as a result of suicide. One of the darkest sides of the human experience is when a person’s suffering and hopelessness leads them to end their own life. Healthcare professionals can be challenged by their own capacity to engage and work with someone experiencing such extreme despair. Dealing with individuals who find themselves in such a dark place creates powerful emotions in others leading to feelings of helplessness and being confused about how to respond. As healthcare workers, you can play an important role in early identification, intervention, and prevention of such a catastrophic event.

This session will focus on increasing the knowledge of attendees regarding the prevalence of suicide in our community and the nursing home setting. Specifically, information about why individuals move towards suicide as a solution to their suffering will be discussed. Participants will learn about risk factors that contribute to an increased chance of a suicide attempt, as well as interventions to reduce the risk of such attempts, and institutional protocols to help your team respond to residents at such an extreme level of despair. Participants will leave the session with increased knowledge about risk factors for suicide, ways of responding, and helping the individual cope with this dark moment in their life.
