When replacement of Water Heater is required, customer will be responsible for code upgrade items including flues and vents, expansion tank, earthquake straps, gas/water flex lines, drip leg and any other code items determined by Service Contractor.     Breakdowns caused by unknown defects and conditions that predate the effective date of this contract such as 1.) insufficiently maintained systems, 2.) rust or corrosion, 3.) mismatched systems, and 4.) improper installation or repair so long as the mechanical failure was not known, was not caused by the current contract holder, and would not have been detectable by a state certified home inspection or a visual inspection and simple mechanical test. A visual inspection is an observation of the covered item to verify that it is structurally intact, without missing parts, and without damage or conditions that could cause imminent failure (i.e. corrosion, sediment build-up, mechanical damage, etc.). A simple mechanical test consists of operating the covered item and performing simple diagnostic tests to ensure that it operates properly and without irregular sounds, smoke, or other abnormal outcomes. A home inspection qualifies as a visual inspection and simple mechanical test.

    Note: The No Fault Coverage is intended to cover unknown pre-existing conditions caused by the previous owner. For purposes of determining whether a breakdown was caused by an unknown pre-existing condition, EHW will accept a licensed contractor’s written diagnosis or a state-certified home inspection as proof that the problem was unknown so long as the inspection was performed within 60 days of the effective date and so long as the contractor/inspector uses industry standard diagnosis guidelines. For example, HVAC industry standards suggest that a properly functioning A/C system should have a temperature differential split between 16 degrees and 22 degrees. A temperature split differential higher or lower than that standard should not be considered to be in “working condition” in the contractor’s/inspector’s written report. This coverage is not intended to take responsibility for normal maintenance and proper use of appliances and equipment by the contract holder. The contract holder must have all known pre-existing defects correctly repaired by a service professional and deliver to EHW any paid invoices evidencing the repairs before the defective item can be covered by this contract. This coverage also does not cover undersized systems.

    THIS SERVICE CONTRACT COVERS

    NO FAULT COVERAGE

  • Air Conditioning Unit: For the standard service call fee, EHW will perform one basic A/C tune-up in accordance with local industry standards. EHW will calibrate thermostat, check freon levels and system pressures, EHW will also test temperature split, perform amp draw on condenser motor, evaporator motor and compressor, clean condenser coils, check condensate lines, clean or replace filters (must be supplied by owner), clean and tighten electrical connections, check contactors, test capacitors and safety switches.Heating Unit: For the standard service call fee, EHW will perform one basic heating tune-up in accordance with local industry standards. EHW will calibrate the thermostat, check heat operations, clean and tighten electrical connections, clean or replace filters (must be supplied by owner) inspect pilot system, test safety switches, clean burners, and test limit switches.

    Note: Tune-ups are covered for 1 unit. Any additional units inspected will require an additional $30 service call fee unit. Properties outside of these counties will be assessed an additional fee; Weber, Davis, Cache, Salt Lake, Utah, Summit, Wasatch, Tooele, Iron and Washington. If a contractor determines that a repair is needed while performing tune-up an additional service fee will be required to perform repair. AC tune-up may only be scheduled between March 1st and May 1st. Heating unit tune-up may only be scheduled between October 1st and December 1st. Homeowner is responsible for scheduling tune-ups online at ElevateHW.com.

    THIS SERVICE CONTRACT COVERS

    PRE-SEASON TUNE-UPS FORAIR CONDITIONING AND HEATING UNITS

    RE-KEY SERVICE

    Note: EHW is not responsible for picking locks. Locks must be standard residential locks. We will not replace doors, doorknobs, or handles. Properties outside of these counties will be assessed an additional fee; Weber, Davis, Cache, Salt Lake, Utah, Summit, Wasatch, Tooele, Iron and Washington.

    For the standard service call fee, EHW will re-key up to six keyholes, including deadbolts and provide up to four copies of the key.

    THIS SERVICE CONTRACT COVERS

    This option covers all parts and components that affect the operation.

    WASHER AND DRYER - $100 (PER SET)

    BUYER UPGRADE OPTIONSHomeowner may elect to receive additional protection and coverage on one or more of the following items by requesting the specific additional coverages and making additional applicable payment at closing of sale. Coverage is limited to buyer only.

  • This option covers all parts and components that affect the cooling operation, including refrigerant recapture, recovery and recharge, ice maker/dispenser, water dispenser, and auger motor.

    KITCHEN REFRIGERATOR - $75 (BUILT IN OR FREE STANDING)

    Note: Coverage is limited to a maximum of $2,000 in the aggregate per Service Contract. Repair or replacement of ice maker/dispenser, water dispenser, or auger motor will only be completed if parts are available.

    This option covers a combined total of four appliances: wet bar refrigerator, additional refrigerator, wine refrigerator, free standing ice maker, and freestanding freezer.

    Free Standing Ice Maker: All parts and components are covered that affect the primary function of the ice maker and water dispenser.

    ADDITIONAL REFRIGERATION UNITS - $60 (AVAILABLE WITH KITCHEN REFRIGERATOR PURCHASE)

    Note: Coverage available for up to four additional refrigeration systems and is limited to a maximum of $1,000 in the aggregate.

    This option provides coverage for above-ground and accessible working parts, and components of heating and filtration system as follows: heater, pool pump, motor, filter housing, filter timer, gaskets, blower, back flush valve, pool sweep motor and pump, above-ground plumbing and wiring.Additional Coverage For Salt Water Pools: Salt water control unit, salt cell, and flow sensor for the salt water chlorinator.

    SWIMMING POOL AND HOT TUB/SPA EQUIPMENT - $190SALT WATER SWIMMING POOL AND HOT TUB - $350

    Note: Swimming Pool and Hot Tub repairs will be limited to $1,000 per Service Contract for diagnosis, labor, parts, and/or materials.Salt Water Swimming Pool and Hot Tub repairs will be limited to $2,000 per Service Contract for diagnosis, labor, parts, and/or materials.

    This option provides coverage for failures due to Mismatched Systems, Improper Installation, Permit Fees, Code Violations/Upgrades, Crane Costs, and Haul Away Service of failed appliances or systems, as set forth below.

    Code Violations/Upgrades: EHW will pay to correct code violations, if required, to effect repair or replacement under this service contract.Haul Away: EHW will pay the cost to remove a covered appliance, system, or component when we are replacing a covered system or appliance.Permits: EHW will cover the cost of obtaining local building permits when required to commence replacement of a covered item, EHW will not be responsible for replacement service if permits cannot be obtained.Mismatched System and Improper installation: EHW will repair or replace a covered item that was improperly installed, modified, or repaired prior to the Effective Date of this Service Contract or items pertaining to undersized cooling and heating systems relative to the square footage of area being cooled/heated.Crane: EHW will cover the cost of the use of cranes or other lifting equipment required for service of a covered rooftop air conditioner or heating unit.

    ORANGE UPGRADE - $50

    Note: Coverage under this option (Orange Upgrade Coverage) is limited to $250 in the aggregate to correct code violations/upgrades, improper installation and mismatched systems, permits, haul away, and crane costs. EHW may provide cash in lieu of repair/ service.

    This option covers the mechanical parts & components, which affect the operation of the water softener.

    WATER SOFTENER - $75

    Note: $500 limit to diagnose, repair, or replace.

Coverage will commence upon receipt of payment and will expire one year after the Effective Date of Standard Coverage.     This option covers the aerobic pump, jet pump, septic tank and line from house to septic, and sewage ejector pump.

    SEPTIC SYSTEM - $75 (ONE TANK PER OPTION)

    Note: Coverage for diagnosis, access, repair or replacement of sewer lines from house to septic tank, sewage ejector, aerobic and jet pumps, septic tank is limited to a maximum of $500 in the aggregate.

    ELEVATED PIPE LEAK COVERAGE - $100

    This option covers all parts and components of well pump utilized exclusively for domestic use.

    WELL PUMP - $100 (INCLUDES ONE PUMP PER SERVICE CONTRACT)

    This option covers a one time pumping per service contract if the stoppage is due to septic tank backup.

    SEPTIC TANK PUMPING - $35 (ONE TANK PER OPTION)

    This option covers concrete encased or underground pipe leaks that are located outside the foundation of the covered home, including water, gas, and drain lines that service the main home.

    Note: Coverage not available to condominiums and multi-unit buildings. EHW will not be responsible to restore landscaping as a result of accessing underground plumbing. Coverage is limited to $2,000 in the aggregate.

    This option provides additional coverage on all kitchen appliances including refrigerator (must have refrigerator coverage) by matching for brand and color of appliance where normally excluded otherwise.

    BRAND FOR BRAND APPLIANCE UPGRADE - $200

    Note: Coverage is limited to $10,000 in the aggregate.

    This option covers leaks and breaks of PVC lines (unless caused by freezing), sprinkler heads, bubbler heads, timers, gate valves, shut-off valves, solenoid, and other remotely activated control valves.

    SPRINKLER SYSTEM AND TIMER - $80

    Note: Sprinkler heads and system timer is replaced with builder’s standard when necessary. Covers systems with six (6) valves or less. Systems with more than six (6) valves please call for quote.

    **Included in Totally Elevated Plan or must be purchased as an upgrade optionWhen EHW replaces an R-22 condenser or air handling unit, if any replacement parts become incompatible with the existing equipment due to changes in the federally mandated refrigerant standards (R22 to R410A) EHW will pay to upgrade or modify the existing covered air conditioning equipment to maintain compatibility, and to meet the new federal standards.

    HVAC EQUIPMENT INCOMPATIBILITY COVERAGE - $150

    Note: The Federal Government is mandating the phase out of R-22 refrigerant (Freon) because of its ozone-depleting properties (see phaseoutfacts.org for more details). As a result of this federal mandate, air conditioning manufacturers have stopped producing air conditioning systems that utilize R-22 refrigerant. Instead, manufacturers are producing air conditioning systems that utilize a non-ozone depleting refrigerant called R-410a (Puron) for residential applications. The phase-out of R-22 refrigerant and R-22 air conditioning equipment will have a significant financial impact for homeowners who have R-22 air conditioning systems. If an R-22 condenser or air handler must be replaced, it must be replaced with R- 410a air conditioning equipment or components. This will cause existing R-22 air conditioning equipment to become incompatible with any new replacement equipment unless the existing equipment is upgraded. This coverage is meant to cover those upgrade costs due to incompatibility when EHW replaces an R-22 condenser or air handler.

  • TERMS & CONDITIONSThis home Service Contract (“hereinafter referred to as Service Contract”) is offered, administered, and serviced by Elevate Home Warranty, LLC. (“EHW”), a residential service provider of home service plans that provide breakdown coverage on household systems and appliances. EHW agrees to repair or replace covered items in accordance with the terms and conditions of this Service Contract or, at EHW’s sole discretion, to provide a cash payment in an amount equal to EHW’s actual cost to repair or replace such item(s). As used herein, “Per Plan” means that EHW will provide coverage up to the specified dollar amount per system in this Service Contract.Subject to the terms, conditions, exclusions and limitations specified below, EHW will provide service coverage to repair or replace selected covered items so long as they:

    Are located within the confines of the Main Foundation of the Structure and Garage with the exception of the following items: pool/spa, well pump, septic tank, and air conditioning units that are in place as of the effective date of this Service Contract. *Pool/spa, well pump and septic tank options only included with additional purchased coverage. See upgrade section for pricing.Are in proper, safe working order and correctly installed in the home on the effective date of this Service Contract;Have become inoperable as a result of proper usage and normal wear-and-tear in accordance with manufacturer’s instructions and specifications;Are not covered by a manufacturer, distributor, builder, extended warranty, or homeowner’s insurance policy;Are reported during the term of this Service Contract.

    This Service Contract covers breakdowns caused by unknown defects and conditions that predate the Service Contract Effective Date, see No Fault Coverage on page 8. Known pre-existing conditions are not covered by this service contract. The Service Contract holder must have all known or detected pre-existing defects correctly repaired by a service professional and deliver to us official paid invoices evidencing the repairs before the defective item can be covered by this Service Contract. This Service Contract does not cover cleaning or maintenance services due to insufficiently maintained covered equipment or problems caused by undersized systems.This Service Contract covers single-family dwellings, including manufactured homes, new construction homes, condominiums, townhomes, and mobile homes, under 5000 sq. ft., (unless otherwise approved by EHW). Coverage is for occupied, owned or rental residential property and does not include commercial property.Pricing, coverages, and terms listed are for properties involved in a real estate purchase transaction. For a home not involved in a real estate purchase transaction, please call us for a quote at (801) 804-5132.We may use the personal contact information provided for setting up and processing your warranty. We may also use this contact information provided for communicating with you about your account activities and events and for marketing purposes (such as providing you with special offers, additional coverage options and renewal programs.)Replacement: If EHW determines, at its sole discretion, that your covered item must be replaced, EHW will be responsible for providing equipment comparable in features, capacity, and efficiency, but not for matching dimensions, color, or brand except as provided by the Brand for Brand Upgrade (see Buyer Upgrade Options on page 11). EHW reserves the right to offer cash in lieu of replacement according to the terms of this Plan as described in paragraph 6.f of this Service Contract. EHW is not responsible for matching any feature of an existing item that does not contribute to the primary function of that item. When replacement of an appliance is required EHW will contribute up to $75 for installation costs.Cash in Lieu: EHW reserves the right to offer cash in lieu of repair or replacement in the amount of EHW’s actual cost (which may be less than retail) to repair or replace any covered item. If EHW makes the determination to provide the contract holder cash in lieu of repair or replacement, we will make payment within twenty (20) days from the time that determination is made. If we provide cash in lieu of repair or replacement of a covered item, that item will not be covered until you have provided us with paid invoices or receipts evidencing that it has been correctly repaired or replaced by a service professional.

    LIMITATIONS OF LIABILITY & EXCLUSIONSThis Service Contract is intended to cover the costs to repair or replace your covered home systems and appliances when they fail due to normal wear and tear; however, coverage is not all inclusive. There may be situations in which you

    will be responsible to pay additional costs for parts or services not covered by this Service Contract. In those cases, we will work with you to determine the best course of action to reasonably minimize your out-of-pocket costs.

    a. General Exclusions: This Service Contract does not cover: Known defects that existed on or before the Service Contract effective date. The contract holder must have all known pre-existing breakdowns and defects correctly repaired by a service professional and deliver to us any official paid invoices evidencing the repairs before the defective item can be covered.Breakdowns that are caused by any condition that is not considered to be normal wear and tear, such as, but not limited to:

    Misuse, abuse, or improper usage;Lack of capacity or insufficient or undersized systems or components;Improper previous repairs or modifications;Missing parts, components, or equipment;Fire, freezing, hail, wind damage, water damage, mold, lightning, smoke, earthquakes, mudslides, soil movement, other acts of nature, power surges, accidents, or any other risk covered by homeowner’s insurance;Manufacturer or builder defects;Chemical, soap, or sediment build-up (except water heaters);Pest or pet damage; orOdors or Noise.

    Failure of parts or components caused by the lack of manufacturer recommended maintenance is not covered unless it’s a Buyer’s Coverage Plan and the condition pre-dated the effective date, was not known by the contract holder, and would not have been detectable by a state certified home inspection or visual inspection and simple operating test.EHW will not be held liable for repairs or replacements for shared equipment, common areas or facilities of condominiums, mobile home parks, or common interest communities. Cosmetic damage is not covered by this Service Contract.EHW shall not be liable for consequential, incidental, or secondary damages resulting from the malfunction of any covered item, such as, but not limited to, food spoilage, loss of income, utility bills, additional living expenses, or the restoration or repair of walls, ceiling, flooring, cabinets, countertops, or painting.Under no circumstances shall EHW be responsible to perform service or repairs involving toxic or other hazardous materials or substances including but not limited to asbestos, mold and other pathogenic organisms, and other contaminants.EHW shall not be responsible for the repair or replacement of any system, appliance, or other item that is covered by a manufacturer’s warranty. Homeowner is required to contact manufacturer to determine coverage. If manufacturer determines there is no coverage then homeowner will be required to provide proof of non-coverage from manufacturer to EHW. EHW shall not be responsible for the repair or replacement of any system, appliance, or other item which has been deemed to be defective by the Consumer Product Safety Commission or which has been recalled by the manufacturer.EHW does not provide coverage for Commercial grade equipment, even when used in residential applications.EHW shall not be liable for computerized or electronic systems or alarms (including intercom systems, security systems and fire alarms) or their accessories and controls.

    b. General Limitations of Liability:Permits and other fees: With the exception of coverage granted by the Orange Upgrade, (see Buyer Upgrade Options on page 11), EHW shall not be responsible for the costs of obtaining legally required permits or other fees associated with covered repairs and replacements including, but not limited to, application fees; costs related to refrigerant recapture, reclaim, and disposal; haul away or disposal fees; variance requests; crane costs; costs to correct code violations; costs of legally required tests; the cost of construction or other structural modifications made necessary by repairing or replacing covered equipment; or other indirect costs.EHW is not responsible for providing access to covered items, closing access to covered items, or for costs of restoration of any wall, floor, cabinets, countertops, tile, stone, brick, paint or the like. EHW is also not responsible for costs to locate leaks, blockages, breaks, or other mechanical failures that are not accessible.

  • EHW is not responsible for delays in providing timely service due to conditions outside its control including, but not limited to delays in obtaining parts or equipment. EHW is not responsible for any damage, injury, sickness or death caused by any failure or any delays in providing repair service. EHW is not responsible for repairs, replacements, or modifications of covered equipment that is merely inefficient.With exception of coverage granted by the HVAC Equipment Incompatibility Coverage option, (see Buyer Upgrade Option on page 12). EHW is not responsible for upgrades, components, or parts required due to the incompatibility of the existing equipment with the replacement system, appliance, component, or part thereof or new type of material or chemical utilized to run the replacement equipment, including, but not limited to, differences in technology, refrigerant requirements, or efficiency as mandated by federal, state or local governments.Systems and appliances that are installed at the premises any time after the effective date of this Service Contract are not covered unless EHW agrees to provide coverage for such items.

    COVERAGE TYPE AND COVERAGE PERIODBuyer’s Coverage: A "Buyer’s Coverage" Service Contract is purchased in conjunction with a real estate transaction by or on behalf of a home buyer up to seven (7) business days after the closing date. Buyer’s Coverage Service Contract term shall be effective as of the Close of Sale (“Effective Date”) and will remain in effect for one full year from the Close of Sale. “Close of Sale” means the earlier of the Settlement Date listed on (a) the HUD-1 (or Settlement Statements or Closing Disclosure) or (b) the recording of the deed conveying title to the Buyer. The Service Contract fee must be received by EHW within fourteen (14) days after the Close of Sale.This agreement will not be considered valid for any reason until payment of the coverage premium has been received and processed by EHW.Seller’s Coverage (for listing/escrow period): A “Seller’s Coverage” Service Contract is ordered by or on behalf of a prospective home seller on or after the date when the property is listed for sale with a real estate broker. By enrolling in a Seller’s Coverage Service Contract, seller agrees to provide the homebuyer with a EHW Service Contract paid out of the proceeds of the closing. The “Effective Date” begins the day the listing is received by EHW and continues until close of sale, the listing is terminated, or expiration of initial listing period (up to 180 days) whichever occurs first. If the close of the sale does not occur in 180 days, we may choose at our sole discretion to extend Seller’s Coverage. Pre-existing Conditions are not covered for the Seller. Optional coverages set forth in this Service Contract are not available to Seller, and Seller’s Coverage is not available on multiple unit dwellings. Seller’s Coverage is limited to $1000 in the aggregate.New Construction Coverage (Buyer’s Coverage Only): A “New Construction” Service Contract is purchased in conjunction with a real estate transaction of a brand-new home by or on behalf of a home buyer up to seven (7) business days of the Close of Sale. A New Construction Coverage Plan begins one (1) year from the original closing date and continues for three (3) years from that date. Plan fee must be received within fourteen (14) business days of the Close of Sale.Existing Homeowner’s Coverage: An “Existing Homeowner” Service Contract is available to homeowners not immediately purchasing or selling their home (i.e., when the home is not listed for sale and/or it is more than seven (7) business days after closing), is effective thirty (30) days after we receive the required payment (unless we otherwise agree in writing) and continues until the Expiration Date as indicated on the Service Contract. Pre-existing breakdowns and defects, both known and unknown, are not covered under the Existing Homeowner Service Contracts. The Service Contract holder must have all known pre-existing breakdowns correctly repaired by a licensed service contractor of his/her choice and deliver to us any paid invoices evidencing the repairs, before the defective item can be covered by an Existing Homeowners Service Contract.

    REQUESTING SERVICETo request service, simply submit your request for service online at www.ElevateHW.com or call EHW at (855) 349-8824. We are available to accept service calls 24 hours a day 7 days a week. After we receive your request for service, EHW will assign a local SERVICE CONTRACTOR to arrange a mutually convenient day and time during normal business hours, M-F 9:00 am – 5:00 pm, to

    diagnose the problem and perform covered repairs. EHW shall have the right and discretion to select its own service contractors. However, EHW may authorize you to contact an independent contractor directly. You may contact EHW for specific contractor guidelines. EHW will not reimburse any costs or fees incurred by your own contractor without express prior authorization by EHW.Service Call Fees: A service call fee will be required and collected by EHW when setting up a new service request. The standard service call fee is $65 for each new request for service. If a claim is denied the service call fee shall not be refunded. We will not respond to new service call requests until all previous service call fees are paid. It is your responsibility to select the appropriate service trade type when requesting service. If you select the wrong service trade type when requesting service, an additional service call fee may be charged to you to re-dispatch the appropriate contractor. You will also be responsible for overtime charges incurred as a result of after-hours services for any non-emergency repairs.Emergency Claims outside of Normal Business Hours: In the event of a time sensitive emergency, EHW will make every reasonable effort to expedite service calls within 24 hours or less. An emergency situation is defined as a failure as a result of:

    Plumbing failure causing significant interior leaking or flooding.A complete loss of heating or cooling systems when temperatures inside the home are below 45 degrees Fahrenheit or above 90 degrees Fahrenheit.Any covered condition that poses an immediate or imminent threat or danger to health or safety.A failure that may be causing ongoing damage to the home. EHW cannot be held liable or responsible for any damage, injury, sickness or death caused by any failure or any delays in providing repair service.

    Expediting Normal Service: EHW will accept your request to expedite non-emergency service if an authorized EHW contractor is available. However, requests for non-emergency service outside of normal business hours, M – F 9:00 am – 5:00 pm as well as holidays or weekends will require a $120 service call fee.Service Guarantee: Services performed by contractors authorized by EHW shall be guaranteed for a period of thirty (30) days from the date of service. Should failure of that item occur within thirty (30) days, we will not charge you another service call fee.Second Opinions: EHW reserves the right to send out a 2nd opinion on any service request at EHW’s expense. In the event that EHW informs you the malfunction is not covered; you have the right to request a second opinion of the cause of the malfunction. You must ask EHW for a second opinion from another EHW contractor within seven (7) days from the time EHW informed you the malfunction was not covered. If the second opinion concludes that the malfunction should be covered, you will not owe an additional service call fee. If you request a second opinion and the outcome of the second opinion is the same as the initial opinion, you will be responsible for the payment of an additional service call fee.

    CONTRACT HOLDER RESPONSIBILITIESYou are responsible to operate covered systems and appliances and perform any manufacturer-required maintenance on covered equipment according to the guidelines located in the owner’s manual.When a failure occurs, turn the covered item off and protect it from further damage and immediately initiate a service request via our website or by calling EHW at (855) 349-8824. Should you fail to protect the covered item from further damage, the consequential damage is not covered.You are obligated to provide information relating to the cause, nature, and timing of any breakdown. This information may include inspection reports, real estate contracts, sellers disclosures and repair invoices.

    TRANSFER OF THIS SERVICE CONTRACTThis Service Contract may only be transferred to a subsequent purchaser of a covered structure within the Coverage Period. In order to transfer this Service Contract, you must notify EHW prior to the sale of the covered structure or EHW shall have no obligations to the subsequent purchaser under this Service Contract.

  • RENEWALS & CANCELLATIONRenewals: This Service Contract may be renewed at our discretion only. In that event, you will be notified of the prevailing rate and terms of Renewal. Payment Rates may increase upon renewal. To ensure there is no lapse of coverage, payment must be received prior to the Service Contract expiration. Cancellation: This Service Contract shall be cancellable by the contract holder at any time. This Service Contract may not be cancelled by EHW during the coverage term, except when:

    Payment of the contract fees are not received;Material misrepresentation has occurred;A substantial change in the risk has occurred after the effective date; orA substantial breaches of contractual duties, conditions, or warranties has occurred.

    If EHW cancels this Service Contract, you will be entitled to receive 100% of the unearned pro-rated premium amount minus an administrative fee of $50.00. In cases of non-payment EHW will send you a notification letter ten (10) days prior to cancellation. If EHW elects to cancel this Service Contract for any other reason, EHW will send you a notification letter 30 days prior to termination of coverage. There will be a $50.00 administrative fee to cancel this Service Contract and EHW will only refund 75% of the pro-rated unearned premium amount of this Service Contract minus any amount spent towards diagnosis or repairs. In the event that Seller’s Coverage contract expires or is cancelled after a service request has been made on the covered property, and if the contract fee has not been paid, the contract holder will be responsible for the purchase of the selected EHW contract or reimbursement to EHW for any service costs incurred and unpaid service fees, whichever is less. Cancellation of this contract must be in writing.

    UTAH RESIDENTSThe coverage provided under this Service Contract is not guaranteed by the Property and Casualty Guaranty Association. This Service Contract is subject to regulation by the Utah Insurance Department. To file a complaint, contact the Utah Insurance Department.
