What are Long-Term Support Systems?
Individuals, public and private agencies that deliver, oversee, screen for, or pay for services for seniors, children and adults with disabilities who seek some type of assistance with activities of their daily lives.

Why Transformation?
TODAY….
• Long term support systems are fragmented
• Planning, management and administration of related activities are not coordinated as well as they could be
• Housing and long term support agencies plan and deliver services in different systems
• Modern methods for gathering data are limited
• Some people have no services at all, and others have less than they may need
• The number of Virginians over 60 will double by 2030

Demonstration o State Long-Term Support Profile Tool
Grant (to measure improvements in quality of long term supports over time)

• Call 2-1-1 Virginia: A call specialist will assist you by telephone 24 hours a day, 7 days a week, 365 days a year

Faster Access to SupportsThrough Development of…
• An on-line Medicaid application
• Streamlined processing of applications for certain Medicaid “waivers”
• An on-line form that you can fill out to tell you if you might be eligible for certain supports

No More “Agency Hopping”
• Local agencies are currently joining together in 11 “No Wrong Door” implementation sites to improve your experience in obtaining and providing long term supports
• Goal is a network of No Wrong Door resource providers statewide by 2010

• The vision:o A comprehensive network of aging
and disability service providers that collaborate and plan together
o Electronic sharing of information with your permission to avoid your having to fill out multiple applications for supports at multiple agencies

Planning Based on Individual Needs and Preferences
• All systems will put individuals at the center of planning for long term supports.
• Individuals will control more of the services and supports they use.
• Family, friends and neighbors can play a more active role supporting the people they care about.

More Choices for Community Living
• If you live in a nursing home, Intermediate Care Facility for Individuals with Mental Retardation (ICF/MR) or long stay hospital, you will have more options for moving to the community if that is your choice.
• If you currently use waiver services in the community, you will be able to access new waiver services that you need.

New Medicaid Waiver ServicesUnder “Money Follows the Person”
• Transition Services – a one-time, life-time maximum $5,000 benefit for individuals transitioning from institutions to the community for up-front household expenses
• Transition Coordination - responsible for supporting the individual with the activities associated with transitioning
o Local government agencies, private non-profit 501(c)(3)s and fiscal services agencies
o Same rate as MR case management• Consumer-Directed Supported Employment -
consumer-directed option for individual supported employment services

• Supplemental home modifications• Rent to cover time between signing a lease
and moving in after home modifications are completed
• 24-hours-per-day, 365-days-per-year emergency back up service provided by 2-1-1 VIRGINIA for the first 12 months after someone moves back into the community

Closing theHousing Gap
• Housing, transportation and long term support stakeholders formed a Task Force
• Developed an Annual Housing and Transportation Action Plan to increase options for seniors and people with disabilities in September
