12.1.12 Carer’s consent to be contacted by the Foster Carers’

Association of Tasmania The Foster Carers’ Association of Tasmania (FCAT) is a non- government organisation funded to support carers in their role: • by informing them about changes in law relating to children in care; • by providing advice about benefits and allowances available to them; • by giving carers the opportunity to present ideas to policy makers and

services; and • by safeguarding and promoting the interests of children and young people in

care. As of May 2009, as part of becoming a foster carer, contact details are automatically sent to FCAT for the carer to become a member of FCAT. However, please note that membership is voluntary. If the carer wishes to opt out of FCAT membership, please tick the box below. If the box is left empty, membership of FCAT will be arranged.

The Carer does NOT consent for the Department of Health and Human Services to forward their contact details being forwarded to FCAT.
