WHAT DO YOU HAVE TO DO? Once you have decided on a package you and your EYH

representative will need to do the following: You will have to fill out & sign a Sponsorship Confirmation Form and

make a 20% minimum deposit of the total package price you have purchased.

Your package is not officially reserved until that step is completed. All checks should be made payable to – Everest Youth Hockey The final payment of the balance will be due on 10/1/13\ You will need to provide us with a Hi-Resolution image of your logo or

artwork that you would like displayed on your signs. We will have services available with our graphics company to help you create this artwork if it doesn’t already exist.

If you chose a Top 10 Package you will also need to work with us to create your recorded commercial copy or provide us an already recorded commercial.

All signs and advertising will be up and visible by 11/1/13!
