Coming out for Middle School Football in 2013… In order to start practice on August 27th, you must turn in:

A Completed Physical, by a Doctor, dated after 4/15/2013.

Mass Sports Physical Night @ Lincoln HS 6 – 8 p.m. Tuesday, August 6th, 2013 East /Main Gym

Bring completed 4 page MHSAA Form $25 Cash – Or register online at www.lincolncommunityeducation.com

Dr. Corey Dean – Proceeds benefit Lincoln Sports Medicine Pre-register for a FAST PASS to the front of the line by paying online at www.lincolncommunityeducation.com!

Completed MHSAA Medical History & Parental Consent Forms – Due to Athletic Office!

These forms can be found in the high school athletic office, or on the MHSAA website at

http://www.mhsaa.com/Schools/FormsResources.aspx. The athlete and parental permission signatures, as well as the medical history portion of the form must be completely filled out.

You must have a physical completed in order to start practice.

What Equipment Does the School Provide?

The school provides ALL necessary football

equipment EXCEPT SHOES! Football cleats should be all or mostly black, with

replaceable style cleats.

Do not spend money on excessive wrist bands & straps – Most are illegal.

We suggest you don’t buy a visor, most block

vision – but any visor MUST be clear.

What do you pay for?

Every Middle School player will pay $40 for a Spirit Pack consisting of:
