Underwritten by: Talbot Underwriting Ltd All Sport Protection insurance in a nutshell All Sport Protection covers you in the case of an accident, suffered whilst playing or training as part of your occupation as a professional rugby player, preventing you from participating in your occupation. Key benefits The following benefits are provided under your policy: 1. Monthly Income protection if you suffer an accident which results in you being temporarily unable to carry out your occupation. We will pay you the Monthly Income Protection benefit stated in your Schedule only after a deferment period of 6 months has expired. 2. Career Ending Injury benefit if you suffer an accident which results in you no longer being able to carry out your occupation ever again. We will pay you the Career Ending Injury Benefit stated in your Schedule. You can choose for this to be paid in one lump sum or in 12 monthly instalments. Pre-Existing Conditions are not covered A Pre-existing condition is any physical and/or psychological impairment, defect, infirmity, sickness or degenerative process which existed or was diagnosed within twenty-four months of the inception date of this Policy and for which medical treatment was rendered, recommended or prescribed by a Health Care Practitioner, or in the event that it has not been so diagnosed, in the opinion of a Health Care Practitioner, you should have reasonably been aware of its existence at inception. The small print: significant limitations and main exclusions Limitations The maximum career ending benefit is based on your age at inception of this policy. There is no cover available if you are 34 years of age or over. Income Protection benefit for Temporary Total Disablement. The monthly benefit you receive cannot exceed 60% of your gross monthly salary prior to the inception date of the policy. No benefit is payable until 6 months have lapsed from the date of injury.

Benefits are payable for a maximum of 12 months. Except that the maximum benefit period for 31 or 32 year old is 10 months and for a 33 year old is 8 months.

Career Ending Injury benefit for Permanent Total Disablement. The maximum amount payable is based on your age and the level of monthly benefit chosen at

inception of this policy. Any Income Protection benefit received will be deducted from the maximum benefit payable.

Main Exclusions Chronic conditions; Pre-existing conditions; Activities excluded under your Professional Sports Contract; Hazardous Activities; Drugs and Alcohol; Criminal Act.

Your side of the bargain Remember, your premium and insurance cover will be based specifically on the details you provide to us. So the information you give us will form a record of your unique combination of demands, needs and circumstances. Please make sure the information you send us is complete and accurate, and inform us immediately of any changes in circumstances that may affect the services provided by us or the cover provided by your policy. This is particularly important before taking out a policy, but it also applies throughout the life of a policy. You need to bear in mind: If you fail to disclose any information material to the insurance you could invalidate the

policy and claims may not be paid. You should read and check all insurance documents to ensure that you are aware of the cover,

limits and other terms that apply. Please be aware of all terms and conditions of your policy as failure to comply with them

could invalidate it. In the event of a claim, you should take note of the required procedures, such as prompt

notice to us of the claims, as stated in the policy documentation. As with any insurance, you have an obligation to take reasonable steps to mitigate any loss.

Policy length The policy period is annual as shown in the Schedule, unless otherwise agreed by underwriters.

Cancellation rights If, once you have checked your policy, you decide not to proceed with this insurance; under the terms of your policy you have a statutory right to cancel it within 14 days of the start of your policy or receipt of your policy documentation, whichever is the later. To cancel, please contact your broker All Sport Protection Ltd (details below). On receipt of your notice and, where applicable, the return of your policy documentation, we will refund any premiums already paid less a pro-rate charge for the time on risk (as set out in the policy documentation) except where you have already made a claim under your policy.

Claims You must notify us of a potential claim under this policy as soon as possible. If we are disadvantaged by any delay in notification, then we may reduce the benefit to reflect the disadvantage caused by the delay.
