    services are billed on a UB billing form.• Data contributors and sponsors receive one copy of this full report and may

    request additional graphs/tables (fees may apply). Additional data points available include stratification by institution type (community or academic), center size, equipment compliment, hours of operation, different diseases, other drugs, additional “per patient” detail, etc.

    • We assume all data contributors have provided complete and accurate information.

    More Notes• Patients might be double counted if there were multiple encounters

    with different diagnoses/diagnosis order.• Centers with less than 12 months of data were not annualized.• “Visits” were excluded in all counts.• All data points are graphed, including outliers. Adjusted percentiles

    and means exclude outliers greater than 2 standard deviations outside the mean. On a few graphs, an outlier was excluded to improve the scale. If your center does not appear, contact us and we will provide complete graphs.

    Definitions• Diagnosis was assigned based on the 1st, or 2nd or 3rd code

    reported. Where only “V” diagnosis codes were reported, those patients were counted as “RT” or “Infusion.”

    • Patient = one unique medical record number.• DOS/Encounter = one date of service, regardless of the # of

    services provided.• FTE = 2,080 hours or 10 half-day physician clinic sessions.• Hours infusing was calculated according to the chart in the

    appendix.
