The definition of a disease changes over time. Based on the disease definition, we use disease staging as a classification system that uses diagnostic findings to produce clusters of patients who require similar treatment and have similar expected outcomes. Because metabolic disorders including obesity and type 2 diabetes (T2D) are heterogenous and influenced by social and economic environment, the definition and staging system of disease might be changed or progressed. For better understanding and giving insight on clustering of clinically homogeneous patients for assessment of quality of care, analysis of clinical outcomes, review of utilization of resources, and assessment of efficacy of alternative treatments, we invited 4 distinguished speakers to solve these matters.

Diabetes dramatically increases the risk of various cardiovascular complications, including coronary artery disease, stroke, and atherosclerosis. In this session, four prominent researchers will give lectures about recent advances in diabetes-associated nervous and cardiovascular consequences. Prof. Seong-Su Moon, from Dongguk University, will present recent advances focused on the brain and diabetic neuropathy. Prof. Eun Ho Choo, from The Catholic University of Korea, will focus on the extent and severity of coronary atherosclerosis in asymptomatic diabetic individuals evaluated by coronary CT angiography. Prof. Adam R. Wende, from University of Alabama at Birmingham, will present emerging evidence for epigenetic transcriptional regulation in diabetic cardiomyopathy. Prof. Seungjoo Lee, from department of neurosurgery in University of Ulsan, will deliver a lecture on the mechanism by which the endothelium-specific transcription factor Sox17 regulates vascular homeostasis.

Over the last 15 years, islet allotransplantation has evolved into an established treatment for type 1 diabetic patients with hypoglycemic unawareness, and the procedure is currently reimbursed for this indication in several countries. However, human islet allotransplantation will never supply sufficient islets for the treatment of the many millions of patients with type 1 diabetes. Preliminary safety data obtained in recent pilot clinical trials, and preclinical safety and efficacy data obtained in the last 10 years in the stringent pig-to-nonhuman primate islet xenotransplantation setting, suggest that xenogeneic porcine islets can possibly be developed an alternative β-cell replacement therapy with broad applicability. In addition, the recent results reveal the potential impact of genetic modification of the xenogeneic islet donor and offer promise that an unlimited source of transplantable β-cells may be possible. In this session, recent progress and current perspective of islet xenotransplantation will be discussed.

Recently, many pathophysiological mechanisms have been suggested for links between metabolic diseases and an impaired control of energy balance, including gut microbiota, autophagy and immune response. In this session, 5 outstanding speakers will cover each issue. For the first topic, Prof. Rossignol from INSERM will talk about bioenergetic modulation in rare and common disease. Next, Prof. Lee from Yonsei University will talk the effect of autophagy modulation on the whole body metabolism. Thirdly, Dr. Ryu from EPFL will introduce the recent progress on the role of gut microbiota metabolites on host health. Fourthly, Prof. Shong from Chungnam University will share the current understanding about the critical role of Th2 cytokines in systemic glucose homeostasis. Lastly, Dr. Fan from Salk Institute will talk about the role of PPARδ in muscle and its potential implication in metabolic syndrome.

Emerging adults with type 1 diabetes are at risk for poor glycemic control, gaps in medical care, and adverse health outcomes. There are many challenges that face the emerging young adult with type 1 diabetes. The transition to adult medical care has long been defined as “the purposeful, planned movement of adolescents and young adults from child-centered to adult-centered health care systems.” In recent years, the topic of transitioning pediatric patients with type 1 diabetes to adult diabetes care has been discussed in the literature and there have been many studies. This session will deal with the recent progress on transition and management to adulthood in type 1 diabetes.

Most of the Asian countries still accept the clinical practice recommendations for diabetes mellitus based on the evidence from western people. However, we are in desperate need of different, and optimized clinical practice recommendations for Asian diabetes mellitus based on the evidence from Asian people. The Committee of Clinical Practice Guideline of KDA published “2015 Clinical Practice Guidelines for Diabetes in Korea” last year. At this juncture, KDA invites speakers from 5 Asian countries to share and discuss their own guidelines. This session will be a meaningful start for the consensus for clinical practice guidelines for diabetes in Asia.

This particular session is organized by the collaboration between the Korean Diabetes Association (KDA) and world renowned Steno Diabetes Center in Copenhagen, Denmark. This session will provide participants with the latest advances and perspective on gut microbiome and metabolic alterations. Three speakers from Steno Diabetes Center and two speakers from Korea will share their data and perspectives on gut microbiome. We also believe that this symposium will serve as a distinctive opportunity to initiate a firm alliance to fight diabetes and promote friendship between KDA and Steno Diabetes Center.
