THOMAS CHARTER SCHOOL FOUNDATION Supporting the College And Career Pathways for MPTCS High School of Culinary and Performing Arts Graduating Seniors 2018-2019 SCHOLARSHIP APPLICATION The Marion P. Thomas Charter School [MPTCS] Foundation was established exclusively for charitable educational purposes. An independent nonprofit organization, the Foundation’s fundraising efforts primarily focus on providing tuition support for Newark students who successfully graduate from the Marion P. Thomas Charter High School and are accepted into colleges, universities or are pursuing certification in accredited Culinary or Performing Arts certificate or degree programs. Scholarship awards range from $500 - $10,000.00. ACADEMIC EXCELLENCE AWARD: (2 available) *Award Range: $7,000-$10,000 Awarded annually to scholars who demonstrate the values, character and integrity of MPTCS; with a minimum cumulative GPA of 3.5; SAT Score of at least 1000; accepted/attending an accredited college/university; has served the community through community service. REVEREND CHARLES E. THOMAS LEGACY SCHOLARSHIP (2 available): *Award Range: $5,000-$7,000 Awarded annually to scholars who have positively demonstrated the values, character and integrity of MPTCS; has a minimum cumulative GPA of 3.0; has been enrolled continuously at MPTCS since elementary school; accepted/attending an accredited college/university or other accredited career training program. DR. KAREN THOMAS LEADERSHIP SCHOLARSHIP (2 available): *Award Range: $5,000-$7,000 Awarded annually to scholars who demonstrate the values, character and integrity of MPTCS and has exhibited leadership qualities within MPTCS and/or the Newark community-at-large; accepted/ attending an accredited college, university, culinary schools or other accredited career training program; has a minimum cumulative GPA of 3.0 ERNESTINE WATSON COMMUNITY SERVICE SCHOLARSHIP: (1 Available) *Award Range: $3,000-$5,000 Awarded annually to a scholar who demonstrates the values, character and integrity of MPTCS and has performed noteworthy service to the MPTCS and/ or the Newark community at-large (must have a minimum of 40 volunteer hours for the school year); accepted to an accredited college, university, culinary school or other accredited career training program; has a minimum cumulative GPA of 2.75 MPTCS FOUNDATION SCHOLAR OF PROMISE SCHOLARSHIP: (3 available) Dr. Doreen Land Scholar of Promise Scholarship (for scholars attending 2 or 4 year institutions) Chef Dean Rawls Scholar of Promise Scholarship (for scholars attending culinary school) Kema Roberts Scholar of Promise Scholarship (for scholars attending 2 or 4 year institutions) *Award Range: $1,000-$3,500 Awarded annually to scholars who have consistently demonstrated the values, character and integrity of MPTCS; have had an upward trend in academic performance; accepted/attending an accredited college, university or other alternative career training program (such as culinary school)

CAREER/ENTREPRENEUR PATHWAYS SCHOLARSHIP (2) *Award Range: $1000-$2500 Awarded annually to scholars who have positively demonstrated the values, character, and integrity of MPTCS; accepted into trade/technical schools or submit a comprehensive business plan outlining first 2 years of startup ; must present MPTCS their final course schedule and verification of enrollment; must possess a 2.0 Cumulative GPA MPTCS SENIOR BOOK AWARDS (10) *Award Range: $500 Awarded annually to scholars who have positively demonstrated the values, character, and integrity of MPTCS; have a minimum cumulative GPA of 2.5; admitted/ attending a college or university or enrolled in an accredited alternative career pathway training/ trade program. *Monetary amounts for each scholarship will be determined based on the amount of money raised.

~ General Information ~ Applicants are evaluated based on completed applications received by April 27, 2018. Completed applications must include ALL of the following:

1. Completed application form 2. High school transcript 3. One (1) Personal Statement 5. One (1) Academic Recommendation* 6. Community Service Hours Log** 8. Signed Student-Parent/Guardian Contract

(NOTE: Other requirements for each scholarship are outlined within this application)

Applicants are awarded based upon thorough completeness of each application component and their demonstrated commitment to academic excellence, collaborative teamwork and community service. Scholarship awards range from $500-$10,000 and are based upon fundraising efforts throughout the year. Further stated, the monetary allotment and quantity of scholarships is dependent upon the amount of money raised during the school year. *Please return each recommendation in an individually sealed envelope signed by the recommender. **You must complete 20 hours of community service on or before May 31, 2018 (at least 5 hours must be completed at MPTCS or in support of a MPTCS Fundraising activity). Please note, applicants for the Ernestine Watson Community Service Scholarship must complete 40 hours of community service. See the attached community service form. Failure to complete these hours will impact your application. However, you must submit your community service form along with your completed application by April 27, 2018, indicating your commitment to assist with Fathers For Our Children on June 8, 2018, if applicable. An interview with the MPTCS Foundation Scholarship Committee will be required for all finalists. Finalists will be notified and interviews will be scheduled for late-May 2018.

2018-2019 INITIAL SCHOLARSHIP APPLICATION

~ Student-Parent/Guardian Contract ~ The NEW Marion P. Thomas Charter School Scholarship Program provides undergraduate scholarships to Marion P. Thomas Charter High School seniors who will enroll in 4 year colleges and universities or are pursuing certification in accredited Culinary or Performing Arts certificate or degree programs. The new MPTCS Scholarship Program is governed by the following Policies and Guidelines:

• Scholarships are based on annual fundraising outcomes and scholarship monetary distributions may change in the future based on subsequent fundraising efforts.

• Scholarships are awarded to high school seniors only • Scholarships may not be used to repeat a course • Scholarship Awards are must be applied to tuition, fees, room and board, books and other

course-related expenses. • Students who withdraw from school must notify the MPTCS Foundation in writing

immediately. Any scholarship balance must be refunded to MPTCS. • Awardees may contact MPTCS to request deferral of their scholarship during a leave of

absence. An award may be deferred up to one year at the discretion of MPTCS. • Scholarship awardees must exhibit behavior that is consistent with MPTCS ideals and core

ethical values, both on and off campus. Scholarships could be forfeited for reasons such as poor grades, academic misconduct or other forms of improper behavior. This includes student arrest and conviction. In these situations, scholarship awards may be revoked and recipients and/or parent-guardian asked to repay a portion or the entire award. Recipients will lose the privilege to apply for a MPTCS scholarship for 1 year.

• Eligibility will be determined after all requested documentation has been completed and submitted before the deadline date. All applications will be reviewed by representatives of the MPTCS Foundation Scholarship Committee.

• Awardees and parents/guardians must also agree to support at least one MPTCS sponsored event, activity or project for a minimum of 5 hours before actual receipt of award.

• After student enrollment has been verified, scholarship funds will be forwarded directly to the school and applied directly to tuition or to the student’s school account.

• All individuals who apply will receive the acceptance or denial of their request in writing. • Scholarships may be forfeited for falsified information or failure to comply with policies

and guidelines. __________________________

Thomas Legacy Scholarship: No additional information needed. We will review your school records to make sure you have been enrolled at MPTCS since elementary school Dr. Karen Thomas Leadership Scholarship: Include leadership roles and awards on application Ernestine Watson Community Service Scholarship: Community Service log (with a minimum of 40 hours) and Community Member Recommendation MPTCS Foundation Scholar of Promise Scholarship: Acceptance letter to college, culinary school or accredited training or trade program Career/Entrepreneur Pathways Scholarship: Must submit a comprehensive business plan (must be a minimum of 500 words, include overview, objective and accomplishable goals) MPTCS Senior Book Award: No additional information needed

*REMEMBER-- you must complete 20 hours of community service on or before June 8, 2018 (at least 5 hours must be completed at MPTCS or in support of a MPTCS fundraising activity). See the attached details on community service form. Failure to complete these hours will impact your application. ~ ~ ~ ~ ~ ~ An interview with the MPTCS Foundation Scholarship Committee will be required for all
