specific health needs ofgeriatric patients in emergencyrooms is cutting their rates ofhospitalizations and leadingto better medical outcomes. About one-third of patients

older than 65 who visit anemergency room nationwideend up being hospitalized,but this new program can cutthat rate, according to a studyby researchers from MountSinai Health System in NewYork City; St. Joseph’s

Regional Medical Center inPaterson, New Jersey; andNorthwestern MemorialHospital in Chicago. The Geriatric Emergency

Department Innovations inCare through Workforce,Informatics and StructuralEnhancements, or GEDIWISE, program launched in2012. At the three hospitalsthat were part of the study, apatient was 10 percent morelikely to be discharged fromthe hospital when comparedto the same type of patient notseen by such a transitionalnurse. Interventions initiated

during an older patient’sarrival in the emergency roomhave a significant impact oncare, said lead investigator UlaHwang, MD, associateprofessor of emergencymedicine and of geriatrics andpalliative medicine at theIcahn School of Medicine atMount Sinai. “Specifically trained

advanced-practice nursestarget older adults throughquestions and assessments tosee if they can face a safedischarge,” Hwang said.

patients for cognitive function,delirium, agitation, functionalstatus, falls risk and caregiverstrain. A score-based assessmentdetermines the extent of carethat will be delivered in theemergency department, Hwangsaid. Nurses ask questions to

understand the bigger picture ofwhat’s going on in a patient’slife, Hwang said: Whatmedications are you taking, anddo you have other underlyingmedical conditions? What levelof social support do you have athome? Can you care for yourselfor do you need help? If discharged, patients will

receive follow-up calls, receivecounseling from a social workeror sometimes be transferred to anursing home.

Ahead of a trend?Programs like GEDI WISE are

trending nationwide. In 2013,the American College ofEmergency Physicians,

American Geriatrics Society andSociety for Academic EmergencyMedicine joined with theEmergency Nurses Associationto develop and publish theGeriatric EmergencyDepartment Guidelines. In 2017the first ACEP GeriatricEmergency Accreditationbecame available for such ERs.“Bottom line, the population is

aging. Hospitals and health-caresystems have to focus on thisvulnerable aging population. Ata time when the nationalaverage for emergencydepartment admissions witholder adults is on the rise,programs that can reducehospitalization risk are crucial,”Hwang said. Hospitals can be a dangerous

place for older adults, Hwangsaid. Some risks include adversedrug effects, falls, cognitivedecline and ulcers. More than30 percent of older adultsdevelop a hospital-associateddisability after an acuteadmission, and many do notreturn to their previousfunctional state. Nurses are a central part of

making programs like this work,Hwang said. “They play aninterdisciplinary role working”with other medicalprofessionals including socialworkers, pharmacists, doctors,physical therapists and homehealth aides, she said.

hen it comes to apatient’s painmanagement,

nurses are leading the way.When zero pain is not anoption, nurses helpalleviate discomfort byunderstanding andacknowledging pain, beingresponsive and involved incare, and communicatingeffectively as patienteducators.

Considering herself new inthe area of pain research,Janiece Taylor, RN, assistantprofessor at the JohnsHopkins University Schoolof Nursing, lets frustrationdrive her instead of holdingher back. “Pain really is outside the

box,” Taylor said. For nurses who love

caregiving but struggle withdoubt about how to treatpain, Taylor says, “Don’tgive up.” She hopes her

research might one day “bemaking it just a little biteasier for that brand-newnurse” out there.

New solutions for painNurses play a huge role in

pain management. “We advocate for our

patients in so many wayswhether through research,at the bedside or asadvanced-practice nurses,”Taylor said. “It is essentialthat we stay up to date on

pain management, Taylor’scurrent work “focuses onnew and innovative ways toaddress pain amongcommunity-dwelling olderadults living with varioustypes of pain. I seek to findways that can be easily usedin the community/homesetting that are notnecessarily pharmacologicalin nature but cancomplement pharmacologyor serve as the main painmanagement tools in thosewho are not able to takepain medications,” she said. Her research is a crucial

piece of addressing theopioid crisis. “Pain researchers are

working tirelessly to findalternative options for painmanagement. Althoughthere is a place for opioids,however, it is important tofind practical alternatives toreduce the risk of not justaddiction but side effects,”Taylor said. Especially exciting are new

developments in managingpain through the use ofvirtual reality as well as inmental health, Taylor said.

Keep fightingPain can be overwhelming,

but Taylor urges people tonever give up on painmanagement.Communication betweenproviders and patients is asimple yet effectivecomponent to adequatelymanaging pain, she said. Patients should “advocate

for themselves with theirproviders and don’t just livewith the pain. Express anyconcerns, questions orchallenges to keepcommunication open withproviders. There are otheroptions that cancomplimentpharmacological painmanagement,” she said.For her fellow nurses,

Taylor also encouragestenacity. “It can seem very difficult

but we must try to identifyeffective and successfultreatments that can besustained over the longhaul,” she said. “Educationin pain management amongnurses is a vital componentto addressing pain in ouraging populations andthose experiencing chronicdisabling pain.”
