TACKLING MALNUTRITION

NEARLY 1 IN 2 CANADIANS IS MALNOURISHED UPON ADMISSION TO HOSPITAL1†

Abbott is dedicated to supporting the fi ght against malnutrition in Canada. Since 2010, we have been a visionary partner of the Canadian Malnutrition Task Force (CMTF), collaborating to provide science-based nutrition solutions to hospital teams.

The CMTF’s research has shown that malnutrition is a serious problem. If left untreated, malnutrition leads to negative outcomes. These include: • Loss of muscle mass, strength, and patient mobility2

• Increased hospital length of stay3

• Increased readmissions4

• Increased mortality5

To improve nutrition care, the CMTF developed the Integrated Nutrition Pathway for Acute Care, or INPAC. This evidence-based approach leads to appropriate detection and treatment of malnutrition, which has resulted in:6• More patients being screened and diagnosed with malnutrition• Increased use of advanced nutrition care practices• Appropriate use of oral nutritional supplements• Better nutrition after discharge Studies show that providing oral nutritional supplements (ONS) as part of care improves nutrition status and patient outcomes compared to standard care.7,8,10 By providing nutritional supplementation, you are helping patients meet their energy and protein needs, and this may lead to:7,8,11

• Reduced patient complications• Improved handgrip strength• Reduced likelihood of readmission• Reduced mortality‡

TACKLING MALNUTRITION

ABBOTT’S “FEED THE PATIENT” INITIATIVE

Follow the 3-step approach, based on INPAC, to identify and treat patients with malnutrition:

Find patients at nutritional riskFeed them targeted nutritionFollow them throughout treatment

For more information on INPAC and on best practices for nutrition care in Canada from CMTF, visit nutritioncareincanada.ca and consult the INPAC Implementation Toolkit.

• Within 24-48 hours of admission, use a valid screening tool to identify patients who need nutrition care.

• The Canadian Nutrition Screening Tool (CNST) is validated in medical and surgical hospitalized patients, and can be completed by a dietetic technician, clerk, or nurse.9

• Provide early nutrition interventions based on nutrition assessment and severity of malnutrition.6,8,9,11

• Abbott’s family of oral nutritional supplements (ONS) and enteral formulas can help meet your patient’s specifi c requirements.

• A medication pass of supplements program provides patients with small amounts of ONS throughout the day with or without medication.9

• Monitor your patient’s nutritional status to optimize compliance and adjust nutrition care to preferences and needs.

• For continuity of care after discharge, recommend ongoing use of ONS at home.9o In the NOURISH study,

malnourished patients who consumed up to 2 servings per day of Ensure® Protein Max for 90 days post-discharge had improved nutritional intake without affecting their usual food consumption.10

• Recommend our Ensure® or Glucerna® Clubs to support nutritional intake after discharge.

WHAT ARE scFOS?

Short-chain fructooligosaccharides (scFOS) are a source of dietary fi bre that are naturally found in common foods such as bananas, asparagus, and onions.1,2 scFOS are recognized as a source of dietary fi bre by Health Canada3 and are defi ned as prebiotics by the scientifi c community because they stimulate the growth of benefi cial intestinal bacteria.1

scFOS are not digested in the small intestine, remaining intact until they reach the colon, where they are fermented by benefi cial bacteria (such as bifi dobacteria and lactobacilli) to short-chain fatty acids (SCFAs).1,2,4

ALLERGENS

Abbott realizes the importance of product information for you and your patients, especially when managing allergies and allergic-type reactions. Priority food allergens are listed following the ingredients list of each Abbott nutritional product in this guide.

The following foods or protein derived from one of these foods are considered priority food allergens in Canada:1

• Peanuts• Eggs• Milk• Mustard• Tree nuts (almonds, Brazil nuts, cashews, hazelnuts, macadamia nuts, pecans, pine nuts, pistachios, or walnuts)• Wheat and triticale• Soy• Sesame seeds• Crustaceans• Shellfi sh• Fish

Please refer to the statement of food allergens, gluten source, and added sulphites on the label to confi rm the presence in the product of any priority allergens, gluten, or added sulphites in a total amount of 10 ppm or more.

LACTOSE CONTENT

All Abbott's adult nutritional products contain some milk protein, with the exception of Pedialyte®, Pedialyte® Powder Sticks, Pedialyte® AdvancedCare® Plus Powder Sticks, and Pedialyte® Freezer Pops. With improvements in testing methodology, low levels of lactose in the products can now be detected.

Most people with lactose intolerance can tolerate some amount of lactose in their diet. A review of the medical and scientifi c literature2,3 suggests that most lactose intolerant patients could tolerate up to 6 g of lactose per serving.

Abbott’s adult nutritional products contain less than 500 mg of lactose per serving and are therefore suitable for most people with lactose intolerance.

RECOMMENDED HANG TIMES

OPEN SYSTEM:Ready to use (commercially sterile)When carefully poured from container into an enteral-feeding setup:†• 8-12 hours for adult patients.• 4-8 hours for pediatric patients.

Addition of modularsIncludes: Any product to which a modular component has been added.• Prepare using aseptic technique.• Hang for no more than 4 hours.

It is generally recommended for feeding sets and accessories to be changed after no more than 24 hours. Consult manufacturer recommendations.

CLOSED SYSTEM:Ready-to-hang products• Follow directions for use provided by manufacturer of

feeding sets.• Unless a shorter hang time is specifi ed by the set

manufacturer, hang product for up to 48 hours after initial connection when clean technique and only one new set are used. Otherwise hang for no more than24 hours.

Each health care facility is responsible for developing/following procedures for using safe techniques in the preparation and use of tube feedings and in setting quality assurance protocols for monitoring those techniques.

PEDIALYTE® DOSAGE CHART

Pedialyte® is scientifi cally formulated for fast, effective rehydration for both children and adults.

Refer to dosage chart for the recommended amount of Pedialyte® required for maintenance of fl uids and electrolytes. Total daily intake should be adjusted to meet individual needs based on ongoing losses, thirst, and response to therapy.

RECOMMENDED DOSE

Infants < 1 year of age Consult a physician before using Pedialyte®.

Children 1 year of age 50 – 100 mL per episode of diarrhea and/or vomiting, up to a maximum of 1 L over a period of 24 hours.

Children 2 to 9 years 100 – 200 mL per episode of diarrhea and/or vomiting, up to a maximum of 1 L over a period of 8 to 24 hours.

Adults, adolescents, and children ≥ 10 years

200 – 400 mL per episode of diarrhea and/or vomiting, or as much as wanted.

• Attempt to give/take Pedialyte® as soon as diarrhea begins.• Give/take frequently in small amounts (e.g. 5 mL) gradually increased, as tolerated. If more solution is requested/desired, give/take more, as tolerated.

For children, including infants < 1 year old, the Canadian Pediatric Society recommends that 50 to 100 mL/kg of oral rehydration solution (ORS) be given over 4 hours, to treat mild to moderate dehydration due to acute gastroenteritis.1 It is also suggested to:• Give small amounts at frequent intervals;• Use a spoon or dropper for very small infants;• Increase the amount of ORS based on ongoing losses,

such as diarrhea and vomiting;• Feed an age-appropriate diet after rehydration;• If breastfeeding, continue during rehydration therapy.1
