GENERAL NOTES 105-P01 UTILITIES: The vertical and horizontal utility locations shown in the plans are
approximate. Plan locations should not be interpreted as exact for bidding or construction purposes.
105-P02 UTILITIES: Potentially two utility relocations/adjustments are planned as identified in
the Utility Conflict Summary Sheets (provided as a Supplemental Design Data for the project). All other utilities on the project need to be protected and remain in place.
107-P01 MAINTAINING TRAFFIC – DROP-OFFS: If, at the end of the work-day, drop-offs
greater than 2 inches and less than 18 inches or slopes steeper than 4:1 exist between the edge of a traffic lane and the outside edge of the proposed roadway, perform one of the following actions: Construct a traversable wedge in the area of the drop-off or steep slope; or Close the lane adjacent to the drop-off or steep slope and provide 24-hour
flagging or pilot car operations.
When constructing a wedge, construct a wedge composed of aggregate or earthen materials with a 4:1 or flatter slope along the entire length of the area. Compact materials using Type C compaction, as specified in 203.04 E.4, "Compaction Control Type C." Install stackable vertical panels that meet the requirements of Section 704.03 H, "Stackable Vertical Panels", along the edge of the driving lane closest to the wedge. The Engineer will measure stackable vertical panels as specified in Section 704.05, "Method of Measurement" and will pay for panels as specified in Section 704.06, "Basis of Payment." The Engineer will not measure material used to construct the wedge. Include the cost of materials, equipment, labor, and incidentals required for this operation in the price bid for “Aggregate Base Course Cl 5.” If a 4:1 or flatter wedge is not installed, provide 24 hour flagging or pilot car operations and associated traffic control at no additional cost to the Department. The requirements of Section 704.04 O, "Traffic Control for Uneven Pavement" apply to drop-offs created by milling or the placement of hot mix asphalt.
108-100 WEEKLY PLANNING & REPORTING MEETING: A weekly planning and reporting meeting is required.
202-P01 REMOVAL OF PIPE-ALL TYPES & SIZES: At locations where a portion of metal pipe
is removed, vertically cut metal pipe at the removal limit prior to the removal and extension. Include all costs associated with cutting the pipe in the unit price bid for "Removal of Pipe-All Types & Sizes.”
202-P02 REMOVAL OF BITUMINOUS SURFACING: At approaches where bituminous
surfacing is removed, saw bituminous surfacing at 20 feet LT or RT and at the tie in location.
203-010 SHRINKAGE: 25 percent additional volume is included for shrinkage in earth
embankment. 203-P01 COMPACTION AND DENSITY CONTROL: Compact material for sliver grading as
specified in Section 203.04 E.4, Compaction Control, Type C. 203-P02 COMMMON EXCAVATION-SUBCUT: Incorporate Common Excavation-Subcut
material into sliver widening. Include all cost for subcut, hauling, placing and compacting material in the unit price bid for “Common Excavation-Subcut.” Engineer will measure Common Excavation-Subcut at plan quantity.
203-P03 ROADWAY OBLITERATION: Obliterate section line roadway from Sta 5978+00 Lt to
5982+50 Bk Lt and approaches at Sta 5976+85 Bk Lt and Sta 5980+30 Ahd Rt as shown in section 60. At approach locations, grade ditches to drain and at section line road location, grade to connect adjacent wetlands. Excavation, including gravel, and topsoil quantities have been included in the bid items for “Common Excavation-Type C” and “Topsoil.”
256-P01 REMOVE AND REPLACE RIPRAP: Remove and relay existing riprap at locations
shown on the plans. Include all costs for removing and relaying the existing riprap in the unit price bid for "Remove and Replace Riprap.” Geosynthetic Material Type RR will be paid separately.
261-P01 TEMPORARY EROSION CONTROL WITHIN WETLANDS: Fiber Rolls and Silt
Fence have been provided for placement at the back-side of the earthen berm at the perimeter of the work area at wetlands. If there is no standing water within the adjacent wetland, immediately seeding and mulching of the earthen berm may replace the fiber roll. If seed/mulch is not immediately applied, fiber rolls or silt fence are required. Fiber rolls will still be required at the weirs within the berm. Temporary seed mix and mulch for this use will be paid for as “Temporary Cover Crop” and “Straw Mulch.”
261-P02 EROSION CONTROL MATERIALS: Prior to ordering erosion control materials, verify
quantities with the Field Engineer.

430-P01 SUPERPAVE FAA 43: Use approximately ¼” of the 3” overlay for blade leveling before the overlay is applied. Place the blade leveling in a single motor grader pass across the full width of the driving lane. Compact the material with a pneumatic roller prior to the overlay. Place 1” of material for bottom lift of overlay and compact as specified in Section 430.04 I.3, Ordinary Compaction. Place 1 ¾” of material for top lift and compact as specified in Section 430.04 I.2, Calculated Density. Include all costs in the unit price bid for "Superpave FAA 43” and “PG 58S-34 Asphalt Cement.”
704-100 TRAFFIC CONTROL SUPERVISOR: Provide a Traffic Control Supervisor. 704-255 TRAFFIC CONTROL FOR SHOULDER DROP-OFF: If the shoulder and adjacent
driving lane are not even at the end of the day, the following criteria will apply: Place the following sign assembly at the locations listed below. Sign Assembly: Sign No. W8-9a-48 "Shoulder Drop Off" and supplemental plate Sign No. W20-52-54 to identify the distance. Locations: In advance of the drop off; Spaced at each mile from the advance sign; and At major intersections (CMC routes, state and US highways, and Interstate
Ramps).
If the difference in elevation between the shoulder and the driving lane is 2" or greater, construct a slough on the driving lane that is 4:1 or flatter. If the difference in elevation between the shoulder and driving lane is less than 2", no slough is required. Sign assemblies will be measured and paid for according to Section 704 "Temporary Traffic Control."
704-500 PORTABLE RUMBLE STRIPS (PRS): Use PRS made of rubber or engineered
polymers.
Install PRS that meet the following criteria: Have no adhesives or fasteners required for placement; Have a manufacture’s speed rating that meets or exceeds the posted speed
limit; and Each strip in the array must weigh a minimum of 100 pounds.
Use individual PRS constructed in one of the following manners:
A single piece; Inter locking segments; or Two pieces hinged at the midpoint.
An installed array of PRS consists of a minimum of 3 individual strips.
Move rumble strips with the flagging operation. Do not place rumble strips on horizontal curves. The Engineer will count and measure each array as one unit. Include the cost of providing, installing, maintaining, and relocating PRS in the unit price bid for “Portable Rumble Strips”.
704-P01 TRAFFIC CONTROL FOR BITUMINOUS PAVEMENT: Provide traffic control consisting of a temporary road closure, flagging, and a pilot car. Traffic control device quantities are based on a 6 mile limitation and the list below. Provide additional devices at no additional cost to the Department.

704-P02 TEMPORARY WIDENING AT PIPE REPLACEMENTS: Use Borrow-Excavation to
construct temporary widenings at pipe replacement locations. When temporary widening is no longer required, incorporate material into sliver widening. Include all cost for hauling, placing, compacting of this material for both uses in the unit price bid for “Borrow-Excavation.” Payment will be made one time for the double use of this material.
