Q: What can you share about genetic test-

ing for women who may be predisposed

for breast cancer?

Dr. Holmes: This topic has been widelydiscussed in the media for the past year

since actress Angelina Jolie decided toundergo a preventive double mastecto-my after learning she was a carrier of abreast cancer gene mutation, calledBRCA 1.

Q: How common is the BRCA gene

mutation?

Dr. Holmes: BRCA mutations occur inabout 1 out of every 500 women inthe general population. On average,BRCA 1 mutations are associated withan 80 percent lifetime risk of breastcancer and a 40 percent lifetime riskof ovarian cancer. BRCA 2 mutationshave a slightly lower risk of 60 per-cent lifetime risk of breast cancer and25 percent lifetime risk of ovariancancer, but are also associated with anincreased lifetime risk of melanoma,pancreatic cancer, male breast cancerand prostate cancer. BRCA mutationcarriers are also more likely to bediagnosed with breast cancer in their30s and 40s as opposed to after age 50for most non-mutation carriers.

Q: What role does family history play?

Dr. Holmes: One critically importantmessage that comes from Jolie’s storyis the importance of paying attentionto your family’s cancer history, bothmaternal and paternal, since it influ-ences your own cancer risk. Jolie’smother had ovarian cancer and pro-vided a clear sign that BRCA genetesting should be considered.

Similarly, if you have a family or per-sonal history of breast cancer diag-nosed before age 50, ovarian cancer,or breast cancer in a male relative,you should speak with your doctorabout your family history and seekBRCA testing if you meet criteria. Allthat is required is a simple blood or

mouthwash test that is covered bymost insurance carriers.

Q: What do you think of Angelina

Jolie’s decision to undergo a double

mastectomy?

Dr. Holmes: No doubt, the decision toundergo preventive double mastecto-my was a difficult one for Jolie. But aword of caution is in order. Jolie’sdecision should not be heard by thegeneral public as a call for doublemastectomy for the average woman.

Jolie’s particularly genetic circum-stances made a double mastectomy areasonable personal decision for herdue to her high risk of developingbreast cancer as she grows older.What one can learn most from Jolie’sdecision is that her diagnosis of aBRCA 1 mutation and her decision to

undergo preventive double mastecto-my came before she was ever diag-nosed with breast cancer. This is thegreatest value of BRCA testingbecause empowers BRCA-mutationscarriers to take control of their ownhealth destiny by allowing them toopt for preventive surgery orincreased surveillance. By going pub-lic with her story, Angelina Jolie notonly inspires other women but shehas shared a tremendous gift withwomen and men who until now mayhave been unaware of this condition.

mothers, sisters and daughters, friendsand family members. With 3D imaging, which gained FDAapproval in 2011, clinicians can nowsee even smaller tumors more clearly,while reducing the number of false pos-itives. A recent study published in theJournal of the American MedicalAssociation showed a 41 percentincrease in the diagnosis of invasivebreast cancer and a 15 percent drop in“call backs” – incidents when womenare called in for additional testing toevaluate suspicious results.

At PVHMC’s own facilities, where ithas had all-3D machines for severalyears, there has been a 40 percent reduc-tion in callbacks. This technology hassaved hundreds of women from thestress and anxiety of inconclusive results.

For women who are diagnosed withcancer, PVHMC’s ability to get clearerimages of new cancer growth oftenmeans less invasive surgery, fewerrounds of chemotherapy or radiation,and more healthy years with lovedones. More studies and clinical trialsneed to be conducted to see how thenew imaging equipment affects long-

term outcomes, but in the short-term,PVHMC is already seeing significantbenefits to its patients.

I NTEGRATED Health Systems (IHS), suchas Kaiser-Permanente, are the obviousgoal of healthcare. The coordination

of care, under one IT umbrella system,allows for birth-to-death care with a singlepoint of medical contact for better clinicalsupervision and improved efficiency. Pioneering a Virtual Health Platform

Virtual integrated health is somethingInglewood Imaging Center pioneered inLA County developing some of the firstradiology-to-EMR (electronic medicalrecords) partnerships in the USA to betterconnect patients, MD’s, treating facilitiesand IPA’s by partnering with independentEMR vendors frequently used in the com-munity. EMR’s, rapidly adopted by MD’sin the USA following the implementationof the 2009 American Recovery and

Reinvestment Act, provide incentive pay-ments to eligible professionals, hospitals,and critical access hospitals (CAHs) as theyadopt, upgrade or demonstrate meaning-ful use of certified EMR technology.

IIC realized EMR’s could hold the keyas the future of healthcare coordinationoutside the conventional IHS ecosystemmodel particularly for leading independ-ent physician associations (IPAs). Yetthere was a key challenge, of the 658EMR vendors certified since virtuallynone of them “spoke” to each other.Therefore, Inglewood Imaging developedsome strategic early partnerships withEMR vendors to develop secure interfaceconnections that allow fluency betweenthe fragmented IT platforms.

Benefits to MD, Insurance and Patient

One of the immediate benefits noticedin a virtual IHS environment was howstreamlined care became for the verybusy ordering MD. In the past, finalresults were faxed, collated, scanned,attached, and uploaded into a medicalchart. IIC’s virtual approach routesreports electronically, allowing alladministrative work to be automated andenabling MD’s to receive our resultsinside their EMR inbox in as little as 30minutes. Reports are never lost. Eachreport also allows access to our “cloudimage” web viewer which enables MD’sto view images for better coordination.

Cost containment for IPAs and insur-ance payors are another benefit to a virtu-al approach as an estimated $30 billion iswasted annually due to inappropriateexam utilization and duplication of radi-ology studies necessitated by lost imagesor reports. Quality scorecards, vitallyimportant in today’s care, can also bemeasured that allow a balance of cost andquality not previously available. HealthInformation Exchange (HIE), once univer-sally adopted in LA County, can also easi-ly receive a copy of our virtual record toensure a continuity of care countywideshould a patient appear unexpectedly atthe office of another care provider.

The final benefit to virtual health isconnecting the patient through a patienthealth record in collaboration with theirprimary care physician. All “meaningfuluse” EMR’s are required to have a patientportal to instantly access a patient’sresults, track healthcare costs, locate aMD and schedule appointment, allowinga patient more power to manage theirpersonal health records and share infor-mation with his primary care physician.

Looking Through a Radiology Lens

Since cancer imaging is a priority forInglewood Imaging, collaborating virtual-ly with all the independent MD’s, cancercenters, hospitals and treating oncologistsis tremendously important. Our medicaloncologists receive EMR results via a

secure interface. Surgeons receiveimages through our PACS-cloud hyper-link portal if surgery is required.Radiation oncologists receive images thatcan be imported into their targeted can-cer treatment planning software to betterpinpoint radiotherapy and IPAs cansupervise care through a primary carephysician to ensure appropriate protocoland communication is maintained.

Summary

IH Systems understand change is nec-essary, which is why innovative new pro-grams such as Anthem’s Vivity, account-able care program, integrating seven dif-ferent fragmented health systems, mightbe very promising. Yet very often novelprograms don’t reach the patient popula-tion that needs the care most.

Ultimately I believe there will be a clos-er collaboration between the “virtual” and“integrated” health systems, with some-one like IIC operating in a similar role toSouthwest Airlines, connecting advancedcare back to a larger network as needed.Until this happens we are excited to bedelivering transformative care through avirtual network to provide state-of-the-artassistance to the 1,100 MD’s and 70 insur-ance plans we are honored to serve.
