n 0125-2013 I was assigned the original case or 13100306. During my investigation I determined the aboveinormation about the Protection rder through court records Ater speaking with the witness Laura Peters who

works or the State ar o Nevada at 9456 Double lvd # I ound that Zachary violated the order on twoseparate occasions

n 01032013 at about 1700 hours as Laura was closing the oice at the above location an unknown maleindividual contacted her through the ront glass door which aces to the south o the building The male asked toile a document with the oice and he was denied due to the hour and the oice closing The male said nothingelse but placed the document through the mail slot. Laura picked up the document and saw it was a motion byachary Coughlin representing himsel in a case that he believes is being handled by this oice. Laura knew thisto be a violation o the above listed order as she has had previous contact with achary and knew o the eistingorder

Later that same night as Laura was leaving the above oice she saw the unknown male get in to a vehicle thatshe knew was own by achary Coughlin Laura knew this as she has had previous contacts with Zachary and this

vehicle Laura said she saw the unknown male get in as a passenger o this vehicle but did not see the driverThis corroborated that this motion was being iled on behal o Zachary by an agent o his

n the temporary order it states that "t is uher ordered that you, the Adverse Party Zacha Coughlin), areprohibited, either directly or through an agen rom contacting intimidating using, attempting to use, orthreatening the use o physical force or otherwise inteering in any way with the employer, an employee o theemployer while the employee is peorming his duties o employment and any person while the person is presentat the workplace o the employer including but not limited to in person by telephone, through the mai, through_electronic mail emai acsimile fax or through another person.achary violated this by having an agent contact the oice located at 9456 Double lvd

hile speaking with Laura she told me she had already attended the hearing or the etension o the protectionorder and had received the etension on the protection order until 01-042014

Laura went on to tell me that on the morning o 0117-2013 when she came in to work at about 0700 hours therewas a a being printed rom the a machine that was supposed to be 371 pages Laura saw that the cover pagewas addressed rom "zachcouchlin" and canceled the rest o the a and unplugged the machine Laura knew thiswas a violation to the etended protection because o the ollowing

In the Etended rder or Protection Against Harassment in the orkplace it states "the Adverse Party ZachaCoughlin) are prohibited, either directly or through an agen rom contacting intimidating, using attempting touse or threatening the use o physical force, or otherwise interering in any way with the employer an employee_o the employer while the employee is performing his duties o employment and any person while the person ispresent at the workplace o the employer including, but not limited to, in person, by telephone, through electronicmail (e-mail), acsimile fax), or through another person. Adverse Party may submit documents to the State aronly through the S mail but those documents must not contain threats or other intimidating statementshile reering to the etended protection order it is a violation or Zachary to have sent a acsimile (a) to theState ar o Nevada located at 9456 Double . lvd Zachary could have mailed this to the oice utilizing theS Mail and it would not have been a violation o the protection order

Ater the two above mentioned violations one or the temporary order and one or the etended protection orderLaura advised that there was also a phone call rom a subject representing themselves as Zachary Coughlin on01-302013 I have not been able to identiy this subject positively as Zachary Coughlin
