A successful break-up Alka Kumar, MS; Atul Kumar, MS CASE NOTES A 24-year-old woman had a 3-year history of primary infertility and a 1-year history of men- orrhagia. Hysteroscopic evaluation revealed a sessile polyp arising from the posterior uterine cavity wall (Figures 1 and 2). The polyp was bright-red compared with the relatively pale en- dometrium surrounding it. Prominent white glandular orifices speckled its surface. Closer vi- sualization disclosed that each gland opening was surrounded by well-defined regions of in- tense hyperemia (Figure 3). An especially de- tailed picture of the hyperemic gland openings was provided when a Hamou Micro-Hystero- scope II was used in the contact mode, providing a magnification at source of 80x (Figure 4). 1 This structure arose from the posterior uterine cavity wall. Kumar. A successful break-up. Am J Obstet Gynecol 2007. CONCLUSIONS The polyp was broken mechanically with the cutting loop of the resectoscope. Pyogenic aerobic bacteria grew in a cul- ture of the biopsy tissue after it was incu- bated for 2 days at 98.6°F (37°C). Histo- pathology identified an adenomatous polyp with areas of extreme inflamma- tion. Suitable antibiotics were adminis- tered. The patient was relieved of men- orrhagia, and she spontaneously conceived 3 months after surgery. f REFERENCE 1. Kumar A, Kumar A. Microcolpohysteros- copy.
