Welcometo the 50th offering of the annual Southwestern clinical pharmacy sminar. We hope that the weekend will provide and opportunity for great educational activities as well as quality time with old friends and colleagues.

Asa a 1984 graduate of the University of Arizona College of Pharmacy, I was thrilled to return last August in my new role as Dean. The partnership between the College of Pharmacy and the Arizona Pharmacy Association is important in many ways and we are excited to provide this longstanding program as support. We also thank those who are joining us for the first time. Our goal is to blend tradition with new opportunities and we are confident that all attendees will continue to find value in the programming and occasions to network with other professionals.

Since this is a milestone year for the program, I would like to acknowledge that 50 years ago Henry W. Winship III, Ph.D. provided direction and leadership in the establishment and subsequent continuation of this seminar. Hank Winship received a pharmacy degree from the college in 1961 and had the misfortune to pass away in 1977. I know that some of you knew Dr. Winship and may have attended one of the early seminars. We recently received a call from a 1968 alum sharing that he attended the very first offering held at the Arizona Inn in Tucson. This weekend let’s remember Dr. Winship and the leadership that led to comradery and education one weekend each year for the past 50 years.

MEET OUR SPEAKERSAzPA Anticoagulation Certificate Program

Lindsay E. Davis, Pharm.D., BCPS, [email protected] Lindsay E. Davis is an Associate Professor at Midwestern University College of Pharmacy - Glendale (CPG). Dr. Davis received her Doctor of Pharmacy degree at The University of Arizona in 2003 and completed her residency training at The Phoenix VA Medical Center in adult acute care in 2009-2010. Dr. Davis has practiced in both hospital and retail pharmacy. Her clinical site is a private cardiology practice with 16 cardiologists where she follows patients in both the inpatient and outpatient setting focused on care transitions, medication therapy optimization, nursing and mid-level provider education, and protocol

development. Dr. Davis serves on the Banner Estrella PGY1 Pharmacy Practice Residency Advisory Committee. Over the previous 7 years she has precepted over 85 students and residents in cardiology, internal medicine, and academia rotations.. Dr. Davis is honored to have received the Mentor of the Year, Teacher of the Year, and Outstanding Educator Awards during her tenure at CPG. She has provided over 30 presentations at the state and national level including ASHP midyear, ASHP NPPC, APhA, AACP, McKesson ideaShare, and Lily Teaching Conferences. She has been an invited speaker at the University of Michigan, Ferris State University, Wayne State University, Western University, University of Arizona, University of Toledo, Idaho Society of Health-System Pharmacists, and the Arizona Pharmacy Association. Her scholarly publications are focused on teaching and precepting methods as well as cardiovascular pharmacotherapy. Dr. Davis is the Program Coordinator for CPG’s Clinician Educators Program, a post-graduate longitudinal teaching and learning curriculum for pharmacists, which began in 2013, has 250 enrolles to date, and provides more than 30 hours of classroom and mentored teaching opportunities annually to resident and preceptor participants across the state of Arizona.

Kayla Kloc, Pharm.D., [email protected]. Kloc graduated from Midwestern University College of Pharmacy- Glendale in 2009. Dr. Kloc started at Cigna as a staff pharmacist soon after graduating. In 2011, she became PIC at the Cigna Medical Group Clyde Wright Pharmacy downtown Phoenix. Since 2012, she has been practicing as a clinical pharmacist at Cigna Medical Group in the Anticoagulation Services Department. Dr. Kloc focuses on management of patients on warfarin therapy, helping her team monitor approximately 2,100 patients across the

valley. Dr. Kloc is a Certified Anticoagulation Care Provider.

Elisabeth Palmer, PharmD, [email protected] Elisabeth M. Palmer is currently an Emergency Department Clinical Pharmacist at Banner Estrella Medical Center in Phoenix. After receiving her undergraduate degree in Biology from Arizona State University in Tempe, Arizona in 2009, Dr. Palmer earned her Doctor of Pharmacy degree at Midwestern University College of Pharmacy in Glendale in 2014. She completed her residency training at the Banner Baywood Medical Center in Mesa, Arizona 2014-2015. Dr. Palmer has practiced exclusively in a clinical setting. Banner Estrella Medical Center is a rotation site for Advanced Pharmacy

Practice Experience (APPE) students as well as a required emergency medicine rotation for PGY1 residents. In addition to her clinical duties at Banner Estrella, Dr. Palmer also acts as a preceptor for residents and students during their rotations. Among her primary clinical interests are emergency medicine, critical care, and cardiology.

Elizabeth Pogge, PharmD, MPH, BCPS, [email protected] Dr. Elizabeth Pogge is an Associate Professor at Midwestern University College of Pharmacy—Glendale (CPG). She got her Doctor of Pharmacy from the University of Nebraska Medical Center and her Masters of Public Health in Nutrition from the University of Massachusetts- Amherst. She is a Board Certified Pharmacotherapy Specialist and is a Fellow of the American Society of Consultant Pharmacists. Her practice site is an ambulatory care clinic at Banner Health Center in Sun City West where she provides

anticoagulation, medication therapy management, and transition of care services.

AzPA Diabetes Certificate Program

Betty Louton, PharmDReceived her Doctor of Pharmacy degree from Midwestern University, College of Pharmacy Glendale, AZ. She is a Population Health - Clinical Pharmacist at Banner Health Corporate and a preceptor for MWU-G. She became a community partner with MWU-G/COP which is a unique position whereby a Community Practice Pharmacist works with the University one day a week to help enhance the educational experience of our student pharmacists. She is a trainer for Diabetes Self Management, Medication Therapy Management, and Immunization Certification. She is currently

serving as the Community Practice Chair for the Arizona Pharmacy Association. Dr. Louton also maintains on call status as a pharmacist at John C Lincoln Hospital.

Grace Akoh-Arrey, BS, Pharm D, CDE, BCACPDr. Akoh-Arrey received her Bachelors of Pharmacy Degree from the University of Oklahoma and Doctor of Pharmacy Degree from the University of Florida. She is an Adjunct Assistant Professor of Pharmacy Practice at Midwestern University, College of Pharmacy in Glendale, Arizona. She is a Pharmacy Manager at Fry’s Food and Drug Stores and now has the role of Patient Care Coordinator. Dr. Akoh-Arrey is a Board Certified Ambulatory Care Pharmacist, a Certified Diabetes Educator and a Certified Diabetes Technology Clinician. She has a Collaborate Practice with Panda Medical Associates, an Endocrinology Specialty Practice, where she oversees

the Disease Management Program, provides drug information, and compliments the physician evaluation and diagnosis by developing and maintaining individualized patient care plans with emphasis on diabetes care. She serves as the Chair of the Local Networking Group on coordinating Body of the Arizona Association of Diabetes Educators (AZAADE). She has served on the Board of Directors at the Arizona Pharmacy Association as Chair of Community Pharmacy Academy and represented as a House of Delegate member for the American Pharmacist Association (APhA). Dr. Akoh-Arrey received Pharmacist of the Year Award from Fry’s, Service to Pharmacy Award from Community Pharmacy Academy, Exemplary Patient Care Award from the Arizona Pharmacy Association, and Preceptor of the Year Award from Midwestern University College of Pharmacy-Glendale. She is passionate about patient care and the advancement of pharmacy practice in Arizona.

AzPA Immunization Certificate Program

Holly Van Lew, Pharm DHolly Van Lew is clinical pharmacist at Phoenix Indian Medical Center (PIMC), an Indian Health Service (IHS) facility. She is the Pharmacy Based Immunization Clinic Director and the Vaccines for Children (VFC) coordinator at her medical center. She manages all inventory, procurement, storage and distribution of vaccines for the adult private supply, as well as the VFC supply for the medical center. She is a trainer for the Pharmacy Based Immunization Delivery Certificate Programs for the American Pharmacists Association (APhA) and Arizona Pharmacy Association (AzPA). Holly facilitates the certificate program course several times per

year to students, technicians and pharmacists. Holly works at the local level within her practice site to educate pharmacists, providers and nurses regarding immunizations. Her national level efforts include coordinating with the CDC/IHS Liaison to collect and evaluate IHS immunization data to improve vaccine forecasting within the electronic health record.

Kelly Fine (Ridgway), R.Ph, FAzPA, Chief Executive OfficerKelly joined AzPA as CEO in October 2011. A graduate of the University of Toledo College of Pharmacy, she came to Arizona in 2001. Prior to her position with AzPA, Kelly was Pharmacy Care Manager for a major drug chain where she was responsible for patient care programs including immunizations, medication therapy management, community health fairs, and in-store screening services. Kelly served as the 2010-2011 President of AzPA and served on the Board of Directors in various capacities since 2006. In 2013 she was named an AzPA Fellow. Kelly is also a preceptor and adjunct faculty at Midwestern University College of

Pharmacy Glendale and the University of Arizona College of Pharmacy.

Sophia Galloway, Pharm DSophia Galloway is a 2014 graduate of Midwestern University and works as a staff pharmacist for Fry’s Food Stores in Phoenix. In the past, she served on the AzPA Board of Directors as Student Pharmacist Academy Chair and more recently serves the Community Pharmacist Academy as Legislative Chair. Sophia has a passion for education and works part-time as an adjunct faculty member for the College of Pharmacy at Midwestern University. She also helps teach the AzPA Immunization Course to students and pharmacists and is helping to pilot a certificate program for

pharmacy technicians at the AzPA Annual Convention. Sophia is lives in Phoenix with her husband, Spencer and beagle, Wally.

General and Breakout Sessions

Alan Barreuther, Pharm.D. FASHPis a recently retired pharmacist who practiced in academia (University of Arizona College of Pharmacy), hospital pharmacy, managed care and the pharmaceutical industry. He received his early education in Tucson, including a pharmacy degree from the University of Arizona and a Doctor of Pharmacy and Pharmacy Residency from the University of Michigan College of Pharmacy/University of Michigan Medical Center. He most recently worked in Medical Affairs for multiple pharmaceutical companies (i.e. Bristol-Myers Squibb, Purdue Pharma, Cephalon, and TEVA). He has served on Hospice Advisory Board for Tucson Medical Center

and is on the Casa de la Luz Foundation Board of Directors.

Lieutenant Commander (LCDR) Andrew DeMotto, Pharm Dhas been serving as a clinical staff pharmacist at the United States Public Health Service (USPHS) Whiteriver Indian Hospital for over 5 years. He was highly involved in establishing and continues to coordinate the Pharmacy-Based Immunization Clinic at his practice site that immunizes patients of ALL ages for virtually ANY vaccine. He also manages the Vaccines For Children (VFC) program for the hospital. Working with the both the Deputy Chief and Pharmacy Director, LCDR DeMotto established a National Clinical Pharmacy Specialists (NCPS) approved protocol for the Pharmacy-Based Immunization Clinic. He also trains and

maintains competencies for over 15 pharmacists currently credentialed to vaccinate patients in the pharmacy department. LCDR DeMotto has delivered two National Indian Health Service (IHS) webinars and provided a formal presentation at the IHS Southwest Regional Annual Pharmacy Continuing Education Meeting regarding pharmacy immunization practices at the Whiteriver hospital.

Christi Jen, PharmD, BCPSis an Emergency Medicine Clinical Pharmacist and PGY1 Residency Program Director at Banner Boswell Medical Center in Sun City, Arizona. Dr. Jen is a board certified pharmacotherapy specialist. A graduate from the University of Illinois Chicago College of Pharmacy, she completed her pharmacy practice residency program at Maricopa Medical Center in Phoenix, Arizona, a Level 1 trauma facility. In addition, she serves as a clinical instructor in emergency medicine for Midwestern University College of Pharmacy – Glendale, the University of Arizona, and Roseman University. She also provides education

to the West Valley Prehospital (EMS) Group. She also serves as a member of the ASHP Preceptor Skills Section Advisory Group and American College of Clinical Pharmacy’s Emergency Medicine PRN group and has given presentations on emergency medicine topics at local and national meetings. Her professional interests include emergency/disaster management, toxicology including drugs of abuse, and infectious diseases. She is one of the three Arizona representatives to the ASHP House of Delegates.

Ghazwa Korayemis a second year pharmacy resident in internal medicine at Banner University Medical Center Tucson. She completed a first year pharmacy residency at the same institution and received her Doctor of Pharmacy degree from Nova Southeastern University. Dr. Korayem is a Board Certified Pharmacotherapy Specialist. She is involved with the internal medicine team at the institution. . Dr. Korayem was formally a faculty member at Princess Noura University, Saudi Arabia. This is her first time speaking at the Southwestern Clinical Conference.

Carol J. Rollins, M.S., Pharm.D., BCNSP, FAzPA, FASHPreceived her B.S. in Pharmacy and Pharm.D. from the University of Arizona. She completed a Nutrition Support Pharmacy Practice Residency at University Medical Center in Tucson, Arizona. Carol received her M.S. in Food Science and Nutrition from Purdue University, West Lafayette, Indiana, and her initial Bachelor’s degree from Concordia College, Moorhead, Minnesota. Rollins is currently the clinical pharmacist responsible for coordinating the nutrition support team at Banner University Medical Center Tucson and is Clinical Professor at the University of Arizona College of Pharmacy. She is Director of an ASHP

accredited PGY2 Nutrition Support Pharmacy Residency and a preceptor for PGY1 and PGY2 residencies. Carol is an active member of the Health System Academy of AzPA and serves on several committees. She is one of the three Arizona representatives to the ASHP House of Delegates.

Hayley Meyeris currently a PGY2 Internal Medicine Pharmary Resident at Banner University Medical Center. She received her Doctor of Pharmacy from the University of Iowa in 2015 and completed a PGY1 General Pharmacy Residency at the Iowa City VA in 2016. Her interests include internal medicine, cardiology, infectious disease and academia.

Jeannie Hong, PharmD, NCPS, BCPS is a Rheumatology Specialty Pharmacist at the Phoenix Indian Medical Center. Given the specialty, Jeannie works with 13 other Indian Health Service sites in Arizona, Utah, Nevada, and New Mexico and also collaborates with clinical pharmacists at other health systems to promote coordination of care and shared knowledge of best practices. Previously, Jeannie worked as a clinical pharmacist at the Parker Indian Health Center upon graduation from North Carolina at Chapel Hill Eshelman School of Pharmacy in 2011. In addition to her normal duties, she served as a clinical preceptor and mentor to 4th year students from 5

Schools of Pharmacy, anticoagulation clinic manager, pharmacist immunizer and pharmacy informaticist. She was also involved with local outreach to reduce youth drug abuse. As a United States Public Health Service (USPHS) officer, Jeannie is also involved in committees and task forces, one of which is to promote pharmacists’ status as providers.

Joseph Zeppa, CPhT.was born in Oakland California in 1984. He served in the infantry as an assault man with the Marine Corps from 2002 to 2006 with 2 combat deployments to Iraq ending in an honorable discharge. He graduated from Arizona College of Allied Health and became PTCB certified. He worked at Women’s International Compounding Pharmacy as a lab technician doing aseptic and non-aseptic compounding prior to joining CVS Pharmacy. He is finishing up his bachelor’s degree in Health Care Management and plans to apply for pharmacy school shortly after.

Juan Villanueva is a second year pharmacy resident in infectious diseases at Banner University Medical Center Tucson. He completed a first year pharmacy residency at the same institution and received his Doctor of Pharmacy degree from the University of Texas at Austin College of Pharmacy. Dr. Villanueva is a Board Certified Pharmacotherapy Specialist. He is involved with the infectious disease team and antimicrobial stewardship program at the institution. This is Dr. Villanueva’s first time speaking at the Southwestern Clinical Conference.

Julie L. Webb, R.Ph.Senior Vice President, Office of Professional DevelopmentAmerican Society of Health-System Pharmacistshas extensive experience in pharmacy practice, association management, and educational meeting planning. As Senior Vice President for ASHP’s Office of Professional Development, she oversees ASHP educational enterprise including the ASHP Midyear Clinical Meeting and ASHP’s Summer Meetings. In addition she is responsible for the exploration and implementation of products and services to support ASHP’s overall mission as a membership association. She oversees ASHP Advantage branded educational offerings, ASHP’s Certification Resources and

Professional Certificate product lines, and ASHP’s eLearning platform. Prior to assuming the role of Senior Vice President, Julie served as the Vice President and Senior Managing Director for Resources Development and Senior Director of Business Development for ASHP Advantage where she was responsible for program development, promotion, and marketing activities. Julie has also served as the Director of the ASHP Section of Home Care Practitioners, a membership component group of the American Society of Health-System Pharmacists (ASHP). Julie’s pharmacy practice experience includes home, acute care, and veterinary medicine. She has also served as a consultant to the home infusion industry.

LCDR Lynette Wasson, PharmD., BCPS I am Chief Pharmacist at Peach Springs Health Center, in Peach Springs, Arizona. I’m also an officer in the United States Public Health Service (USPHS), with the Indian Health Service (IHS). Peach Springs Health Center Pharmacy serves the people of the Hualapai Indian Reservation, and provides pharmacy services for Supai Health Station, located at the bottom of the Grand Canyon and serving the Havasupai Tribe. I graduated from the University of New Mexico College of Pharmacy in 2009. Prior to working at Peach Springs, I worked 6 years in South central and Western Alaska. I’ve had the opportunity to work in

a variety of settings that include Inpatient, Outpatient, Village Pharmacy, and Long-Term Care. Throughout my career, I’ve tried to maintain professionally active in local and national organizations, serving as Board Member of the Alaska Pharmacists Association (AKPhA). I was selected to represent AKPhA as a Delegate to the APhA House of Delegates for 3 consecutive years (2013-2015), and served as AkPhA Membership Chair for 2 years.

Mindy (Throm) Burnworth, PharmD, FASHP, FAzPA, BCPS Dr. Burnworth graduated from the University of Missouri Kansas City School of Pharmacy. She then completed ASHP-accredited residencies in Pharmacy Practice (PGY1) and Adult Internal Medicine (PGY2) at Grady Health System in Atlanta, Georgia. Dr. Burnworth is a Professor of Pharmacy Practice at Midwestern University College of Pharmacy-Glendale, AZ. She precepts pharmacy students on advanced pharmacy practice experiences at Maricopa Medical Center in Phoenix, Arizona, where she provides clinical pharmacy services for an adult internal medicine team. Dr. Burnworth serves ASHP on the Political Action Committee

and Advocacy Advisory Board (2015-Present) and makes annual contributions to the national PAC. She is recognized as a Fellow of ASHP and is one of the three Arizona representatives to the ASHP House of Delegates.

Wannisa Dongtai is a second year pharmacy resident in critical care at Banner University Medical Center Tucson. She completed a first year pharmacy residency at the same institution and received her Doctor of Pharmacy degree from the University of Maryland, College of Pharmacy. She is a Board Certified Pharmacotherapy Specialist. Dr. Dongtai was formally a faculty member of Ubon Ratchathani University, Thailand.

Michael B. Bottorff, PharmD, FCCP, FNLA, CLSProfessor and ChairDepartment of Pharmacy PracticeManchester College of PharmacyFort Wayne, INMichael B. Bottorff, PharmD, is currently Professor and Chair in the Department of Pharmacy Practice at Manchester University College of Pharmacy in Fort Wayne, Indiana. After earning his doctorate in pharmacy from the University of Kentucky (1981) in Lexington, Dr. Bottorff completed his pharmacy residency there, serving as Chief Resident. He has held faculty positions

at the University of Tennessee (1983-1989), the University of Cincinnati (1989-2009), the University of Charleston (2009-2011) and South College (2011-2015).

Dr. Bottorff ’s research interests have focused on the clinical pharmacokinetics and pharmacodynamics of cardiovascular drugs, and he has served as principal or co-principal investigator on numerous clinical trials. Dr. Bottorff has presented both nationally and internationally on such topics as drug therapy for acute coronary syndromes, management of dyslipidemias, understanding and predicting clinically important drug-drug interactions, and new strategies for managing atrial fibrillation.

Dr. Bottorff is a member of the editorial board of the Journal of Clinical Lipidology. He is also a reviewer of manuscripts submitted to The Annals of Pharmacotherapy, Archives of Internal Medicine, British Journal of Clinical Pharmacology, Pharmacotherapy, and The American Journal of Cardiology, among others. He has published over 85 original research articles, book chapters and refereed reviews, along with over 50 research abstracts. Dr. Bottorff is a member of the American Heart Association, and a Fellow of the American College of Clinical Pharmacy and the National Lipid Association. He has also served as Chairman of the National Pharmacy Cardiovascular Council and is certified as a Clinical Lipid Specialist.

Dr. Bottorff is a recognized specialist in cardiovascular therapeutics. He has assisted in the implementation of guideline-based therapies and served on the first national panel to develop guidelines for the management of heart failure for AHCPR.

Dr. Bottorff has published extensively on the impact of drug interactions on drug safety. Specifically, he authored the drug interaction paper for the National Lipid Association statin safety report, published in the American Journal of Cardiology in 2006 and updated in the Journal of Clinical Lipidology in 2014.

Stacy L. Haber, Pharm.D is an Associate Professor at Midwestern University College of Pharmacy-Glendale and is also the Director of Midwestern University’s Drug Information Center. Previously, she was as an Assistant Professor at The University of Arizona College of Pharmacy. Collectively, she has served as an educator of pharmacy students and residents in the state of Arizona for almost 20 years. Dr. Haber received her B.S. in pharmacy from West Virginia University and her Pharm.D. from the Medical University of South Carolina, where she also completed pharmacy practice and drug information residencies. As a drug information specialist, her interests include evidence-based health care, formulary management, and drug safety. She is active in the Arizona Pharmacy Association, the American Society of Health-System Pharmacists, and the American College of Clinical Pharmacy. She has given presentations on drug information topics locally and nationally and has authored numerous publications in her field.

Robert Lipsy, PharmD, FASHP, BCPS Dr. Lipsy is the Senior Clinical Pharmacist and Medication Management Specialist at the Medication Management Center, College of Pharmacy, University of Arizona. He is an Assistant Professor at the college where he coordinates the drug information and literature evaluation course. Dr. Lipsy has over 30 years of clinical pharmacy experience. He is a fellow of the American Society of Health-System Pharmacists and in 2001, was awarded the Elias Schlossberg Lifetime Achievement Award by the Arizona Society of Health-system Pharmacists. Dr. Lipsy has authored numerous articles, books and book chapters and

given over 100 local, regional, and national presentations and lectures.

Roger Morris, R.Ph., J.D. is the national chair of Quarles & Brady’s Health & Life Sciences Industry Team, former chair of the firm’s Health Law Practice Group, and a member of the firm’s governing Executive Committee. His practice focuses on the representation of pharmacies, including retail pharmacies, pharmacy wholesalers, specialty pharmacies, compounding pharmacies, mail order pharmacies, veterinary pharmacies, pharmacy benefit managers (PBMs), and pharmacy manufacturers. He also represents health care providers, including hospitals, health systems, and medical staff. Roger’s previous

career as a pharmacist enables him to provide unique insight to clients on a wide range of issues, including licensing new pharmacy locations, responding to regulatory inquiries and investigations before state review boards, and the development of complex new programs that include multiple points in the pharmacy distribution process and multi-state jurisdictional matters.

Dr. Ryan Gries graduated from Midwestern University – College of Pharmacy Glendale in 2014 and completed an Acute Care PGY-1 Residency at Maricopa Medical Center in Phoenix, Arizona. Ryan is a Board Certified Pharmacotherapy Specialist and is employed with Maricopa Integrated Health Systems as an HIV/ambulatory care clinical pharmacist at the McDowell Family Health Clinic in Phoenix. Prior to relocating to Arizona, Ryan served in the US Army and Wisconsin National Guard deploying overseas several times in support of Operations Enduring and Iraqi Freedom. Currently he has the honor of serving as CPG’s founding Alumni

Council President, Adjunct Faculty and preceptor for Midwestern University, Southwestern States Residency Conference planning committee member, and is an active member of Kappa Psi Pharmaceutical Fraternity.
