WITNESS ETH:

WHEREAS, by Agreement dated April 28, 2017, Authority and Consultant entered into an agreement for Consultant to provide Continuing Program and Project Management Services (OAR Prime Entity) Services; and

WHEREAS, under the Agreement, Consultant agreed to perform such additional services for the Authority as are contained in any additional scope of work established by the Authority in any addendum to the Agreement and accepted in writing by the Consultant; and

WHEREAS, the Authority and the Consultant desire to enter into this Addendum to the Agreement to provide for additional services to be rendered by the Consultant under the terms of said Agreement.

NOW, THEREFORE, in consideration of the premises and the mutual covenants herein contained, the Authority and the Consultant do hereby agree as follows:

1 . Consultant shall perform additional services in accordance with the terms of the Agreement and the attached Exhibit "A." Consultant shall be paid for such additional services according to the payment terms set forth in the Agreement.

2. Consultant shall be compensated for such additional services in the NOT TO EXCEED amount of TWO HUNDRED TWENTY-NINE THOUSAND FIVE HUNDRED NINETY-FIVE AND NO/100 DOLLARS ($229,595.00), broken down as follows:

3. A. Consultant hereby certifies that it is not on the Scrutinized Companies that Boycott Israel List and is not engaged in a boycott of Israel , as defined in Florida Statutes§ 287 .135, as amended;

4. Authority may terminate the Agreement for cause and without the opportunity to cure if the Consultant is found to have submitted a false certification or has been placed on the Scrutinized Companies that Boycott Israel List or is engaged in a boycott of Israel.

In the event the Agreement is for One Million Dollars ($1,000,000.00) or more, Authority may terminate this Agreement for cause and without the opportunity to cure if the Consultant is found to have submitted a false certification or has been placed on the Scrutinized Companies with Activities in Sudan List or the Scrutinized Companies with Activities in the Iran Petroleum Energy Sector List or is engaged in business operations in Cuba or Syria .

5. Except as expressly modified in this Addendum, the Agreement dated April 28, 2017 and all prior addenda will remain in full force and effect.

A2 Group, Inc. is pleased to submit this proposal to render Continuing Program and Project Management services for the above-referenced project at the Orlando Executive Airport.

PROJECT DESCRIPTION The BP-045, ORL Taxiway F & G Rehabilitation , project consists of improvements to enhance airfield safety for aircraft operations at Orlando Executive Airport (ORL) . Specific work includes the modifications to existing taxiways' geometry for implanting the following: Rehabilitation of existing Taxiways F and Gas identified in the FDOT 2015 and 2019 (Draft) Airfield Pavement Management Program report; Removal of 400 feet of Taxiway F; Construction of Taxiway K1 , approximately 300 feet long, between Runway 13-31 and its parallel Taxiway K; drainage modifications to be made; Construction of new airfield edge lighting, signage, and markings along the constructed areas; and implementation of updates to the Airport Lighting Control and Monitoring System (ALCMS) and electrical vault as needed for the project.

Coordination with the Executive Airport staff and Operations will be required to confirm that proper notification and timely coordination are provided. We understand that the project duration has been set at 135 days to Substantial Completion. In addition, 45 days will be allowed for Final Completion.

SERVICES Our Scope of Services will be to provide the Authority with project management and inspection services during the project's pre-construction, construction, and close-out phases.

The services for this project will be performed in accordance with the provisions of GOAA's Continuing Program and Project Management Services contract and as indicated in the attached project schedule showing the proposed staffing levels for the duration of the project.

Our fee is based on regular time for our management and inspection personnel at our currently approved billing rates. We anticipate that construction activities will be performed during daytime and night-time shifts as coordinated with Airfield Operations in phases to minimize disruption to the functions of the airfield.

SMALL BUSINESS PARTICIPATION Based upon the scope of the BP-045, ORL Taxiway F & G Rehabilitation, we have identified several opportunities for small business sub-consultant services. An estimated $31,801.00 of our OAR fee will be awarded to Elipsis Engineering & Consulting, LLC. for oversight of compliance with federal requirements, materials testing including field sampling , laboratory testing, and asphalt plant inspection. John B Webb will provide quality control and inspection surveying services for an estimated total of $7,500.00. GCI , Inc. will provide field inspection support during the busiest part of the project for $22,692.00.

Our total projected small business participation for the BP-045 project is $61 ,993.00 which represents 27.00% of our proposal OAR fee. We will continue to identify opportunities to pursue our relationships with small business firms on future projects.

PAYMENT We will invoice in accordance with the terms of our contract with the Authority.

TRUTH IN NEGOTIATION CERTIFICATION

The Consultant hereby certifies, covenants, and warrants that wage rates and other factual unit costs

supporting the compensation for this project's agreement are accurate, complete, and current at the

time of contracting.

The Consultant further agrees that the original agreement price and any additions thereto shall be

adjusted to exclude any significant sums by which the Aviation Authority determines the agreement

price was increased due to inaccurate, incomplete, or noncurrent wage rates and other factual unit

costs. All such agreement adjustments shall be made within (1) year following the end of the contract.

For purposes of this certificate, the end of the agreement shall be deemed to be the date of final billing

or acceptance of the work by the Aviation Authority, whichever is later.

The Owner's Authorized Representative (OAR) services for this project will be performed in accordance with the provisions of GOAA' s Continuing Program and Project Management Services agreement. We anticipate that construction activities will be performed both during day-time shifts with some activities required during overnight hours as coordinated with Airfield Traffic Control Tower (ATCT). We anticipate full time daily inspections will be needed during construction operations during both day-time and night-time (as needed) operations. In addition to scheduled day and night work, the phasing anticipates a compressed schedule to tie in new taxiway Kl to Runway 13-31.

The total construction phase time to substantial completion is 135 days. 60 days will be allowed for mobilization and 45 days will be allowed after substantial completion for the contractor to reach final completion.

Coordination with Design Engineer: • Review the project phases to confirm access and maintenance of airfield operations as well

as operations of the airside ramps. • Perform independent schedule review to confirm that the appropriate contract time is

included and that all phasing and milestones are properly defined • Complete final constructability review of project plans. • Review the proposed project for completeness of the pay items. • Review design coordination of disciplines.

Plan, schedule and conduct conferences, meetings, and presentations, and take accurate minutes of same for distribution to the Owner and all conference participants. This will include pre-construction conference, bi-weekly Job Coordination Meetings, Disputes Review Board Meetings, EEO compliance meetings, and coordination meetings as needed to communicate with adjacent projects. Review the plans and specifications for the project to confirm that project documents are internally consistent and that all desired scope is described. Make recommendations to the Owner for such changes in the project as the OAR may consider necessary or desirable. Prepare independent cost estimates on work which may be added to or deleted from the project Review all requests for change orders. When appropriate, implement the Owner's procedures for the preparation and processing of change orders. Confirm available funding and review the change for grant eligibility. Keep accurate and detailed written records of the progress on the project

Maintain orderly files for correspondence, reports of job conferences, shop drawings, contract documents with revisions, change orders, as-built drawings, and all other project related documents.

Construction Oversight • Provide technical inspection and coordination of the work verifying that the materials

provided and the work performed are in accordance with the specifications and contract documents.

• Coordinate with the Engineer and report to the Owner, so that the Owner may make a "Final Determination" in such cases.

• Review and coordinate Contractor's proposed construction schedules and prepare progress schedule updates for inclusion with the monthly program schedule update. Review and evaluate contractor requests for extension of time and prepare change order when appropriate to adjust contract time.

• Coordinate, track and process all Requests for Information providing timely responses to the Contractor.

• Take necessary action to confirm that improper or non-conforming materials, work or equipment are not incorporated into the finished project.

• Organize and schedule both the substantial completion inspection as well as the final completion inspection. Provide inspection when partial occupancy is required. Coordinate the creation of punch lists and monitor the completion of all items noted.

• Ongoing review contractor's red lined plans. Review contractor's red lined final as-built plans.

• Provide photographic records of the project progress as well as of any issues encountered. • Monitor the contractor's maintenance of traffic devices on the airfield to confirm that

devices are checked and maintained as needed.

Survey

Analyze and evaluate all tests and inspections performed by the Contractor for soils, concrete, reinforcing steel, pipe, and asphalt under the terms of the contract documents or as required to demonstrate compliance of the completed work. Perform, analyze, and evaluate all tests and inspections performed by the OAR staff as part of the quality assurance program. Record the results of all quality control testing and quality assurance testing monitoring both for confirmation of results. Make recommendations and provide direction to the Contractor when materials tests are not in compliance. Monitor asphalt production at the asphalt batch plant. Prepare materials non-conformance reports when needed and work with contractor to resolve any materials discrepancy. Inspect the airfield lighting and airfield electrical equipment to confirm minimum requirements are satisfied. This will include not only fixtures but wire and grounding systems.

• Confirm survey control used by Contractor for layout • Facilitate comparison of design survey and grades to what is collected by the contractor. • Provide confirmation for final grades for pavement, earthwork and drainage



To help further describe the anticipated effort, I have created a list below of the expected tasks that will occur in each phase of the project.

Mobilization/Survey Phase 0: (Duration 60 Days) Expected Staffing: Limited part time Senior Project Manager, full time Project Manager, part time Assistant Project Manager, part time Senior Civil inspector, part time Project Coordinator, and part time Compliance Specialist.

• Schedule, organize and hold the pre-construction conference. • Prepare pre-work meetings for selected scope to encourage the contractor and major sub

contractors to be ready for critical work items. • Review the baseline schedule and provide comments. Work with the Contractor until an

acceptable baseline schedule has been provided. • Set up all project folders, electronic file system, and project field books. • Populate electronic project communication system with the appropriate participants. • Review all EEO and DBE submittals for accuracy and completion. Hold EEO compliance

kick off meeting. • Create project exhibits and summary for distribution to airport staff and FAA. • Assist contractor with attainment of badging and decals for staff and vehicles. • Collect and review project survey confirmation by Contractor. • Compare design to survey layout. • Compare design disciplines to survey to confirm coordination any grade adjustments across

electrical, grading, drainage, and signage. • Review survey data with Engineer of Record to confirm that design grades are

constructible.

Phase 1: (Duration 30 Calendar days concurrent with 75 days) Expected Staffing: Limited Part time Senior Project Manager, Full time Project Manager, part time Assistant Project Manager, full time Senior Civil inspector, part time Project Coordinator, and part time Compliance Specialist. The phase will allow for the refurbishment of Taxiway G including the tie in at the West Ramp and the tie in to Taxiway A. This scope will include milling and resurfacing of all the pavement, grading of the Taxiway Safety Areas, concrete reinforcement of the slope protected area at the canal

Phase 2: (Duration 40 Calendar days concurrent with 75 days) Expected Staffing: Limited Part time Senior Project Manager, Full time Project Manager, part time Assistant Project Manager, full time Senior Civil inspector, part time Project Coordinator, and part time Compliance Specialist. This phase will allow for the removal of the portion of Taxiway F between Taxiway K and Runway 13-31. It will then allow for the construction of Taxiway KI between Taxiway K 1 and Runway 13-31. 2014 - Present GOAA TWOs Include:

BP-486 Runway 18L-36R Rehabilitation and Related Work. The scope includes rehabilitation and improvements for the existing pavements for Runway 18L-36R at the Orlando International Airport (OIA). Construction work includes milling, crack seal and over1ay of existing asphalt pavement; removal and replacement of existing PCC pavement, joint/spall repair; reconstruction of 10'-0" of asphalt shoulders on each side of the runway; reconstruction of the asphalt blast pad at RWY 18L; seal coating of blast pad at RWY 36R and improvements to all associated lighting, markings and signage to comply with current FM standards. All light fixtures and electrical cabling on the runway will be replaced including the construction of 32 junction can plazas. The project includes major electrical vault work to install 6 new constant current regulators and replace the backup generator. The existing backup generator is mounted inside the vault building in a designated generator room with an underground fuel storage tank located outside. The proposed 450kWGenerator will be mounted adjacent to the vault in an enclosure with an above ground belly tank. After start up on the backup generator, the existing generator and tank will be removed. The generator room will be enclosed by the removal of ventilation louvers and converted to conditioned storage space. Mr. Brown is the Project Manager. Construction: $30 million. BP-468 Runway 17L-35R Rehabilitation, at the OIA. This highprofile airfield project included the rehabilitation and improvements for the existing pavements for Runway 17R-35L and connecting taxiways within the runway safety areas. Construction work included removal and replacement of concrete joints and cracked slabs, improvements on the runway and connecting taxiway pavement geometry, markings, lighting, and signage to comply with current FAA standards. T-1178 Virgin Trains Rail Corridor Design Phase. Working with Virgins Trains to coordinate design of 7 miles of railroad track to be constructed from the Florida east coast through the airport to the newly opened lntermodal Terminal Facility. The rail is to be constructed in a new corridor which had to coordinate conflicts with the AOA fence, RWY17LALSF-2, the Terminal Service Road, Tug Roads 2 and 4, primary power duct bank, fuel main line, TWY E and F Mid Crossfield Taxiway bridge underpass, and several

existing roadways. This project oversees the design of the construction of the rail corridor through GOM property from 528/Narcoossee to just south of the APM/ITF Complex. Mr. Brown served as the Project Manager.

T-1179Virgin Trains Maintenance Facility Program Design Phase. Construction of the rail car maintenance facility adjacent to Conserv 1 in the SW comer of the Orlando International Airport property. T-1246 Silver Airways Refurbishment of Former Comair Office Building #901 and Hangars. Silver Airways secured a grant from FOOT to refurbish the abandoned Comair hanger at Orlando International Airport for use as the maintenance headquarters for the airline. The building required full renovation and structural reinforcement to meet current building code. The complex included over 9,000 sf of office space, a 12,000-sf conventional hangar, and a 10,000 sf RUBB canvas hangar.

BP-455 Taxiways 89 & C Rehabilitation at the OIA. This project included rehabilitation of existing pavement for the south end of Taxiway C, Taxiway 89, and related areas. Work was done to restore and maintain the airfield pavement serviceability areas including milling of existing pavement, removal of deteriorated pavement, and a bituminous pavement overlay, along with associated markings, lighting, and signage. Mr. Brown was the Project Manager. Construction: $6 million.

BP-456 Airside 1 and Airside 3 Apron Rehabilitation at the Orlando International Airport. This rehabilitation project included replacement of the existing concrete slabs that were severely distressed with cracking, joint deficiencies, surface defects, water bleeding and pumping, etc. This was completed in 18 phases to keep airsides open. Mr. Brown was the Project Manager.

A GROUP, INC. BP-408 Cell Lot, Taxi Hold, Bus Hold, and Return to Terminal A Road at the Orlando International Airport. There were two main locations, the new north cell lot facility and the new restroom in the existing south Taxi Hold. Mr. Brown was the Project Manager.

H-260 Repairs to Enplane Drive Pier Cap #5 Both A and B Side at the Orlando International Airport. This project included structural repairs to Enplane Drive Pier Cap #5 on both A and B side to increase the size and strength of the pier camp providing a larger area for the end bearing of the deck girders.

BP-450 Bear Road and Cargo Road Resurfacing at the Orlando International Airport. The project included milling and resurfacing of the existing asphalt pavement for the west end of Bear Road near Tradeport Drive, repair of existing concrete slabs and joints for the east end of Bear Road new Cargo Road, and milling and resurfacing of the existing asphalt pavement for Cargo Road between Jeff Fuqua Boulevard North and Hangar Boulevard.

Highways and Bridge Experience:

Dec 2013- May 2014 US 98, from N of CR 540A to SR 540 for FDOT D1 in Polk County, Florida. Widening of 3.5 miles of US-98 from 4 to 6 lanes. Major items of work include adding new outside lanes and shoulders; constructing a pair of 3 span flat slab bridges over Banana Creek; milling and resurfacing; milling and overbuild cross-slope corrections; removing existing crossovers and adding controlled median access with designated tum lanes; excavating organic soil; installation of new mast arms and span wire signals; constructing new open and closed drainage systems; jack and bores under US 98; pond construction; utility relocations; and Utility Work by Highway Contractor. Mr. Brown was the Senior Project Engineer. Construction: $19.5 million.

SR 417 from SR 528 to Curry Ford (CFX 417-107). 4.1 miles of SR 417 from four basic lanes to six. Auxiliary lanes were added between the Curry Ford Mainline Plaza and the Lee Vista Boulevard ramps to/from SR 417 to the south and Curry Ford Road to the south. The project also included the widening of SR 417 mainline bridge over Lee Vista Boulevard. The milling and paving on this project were constructed with automated machine guidance to model profile elevations and became a model for the implementation of 3D paving statewide. Mr. Brown was the Project Administrator. Construction: $19 million.

SR 408 eastbound Exit Ramp on SR 50 and SR 408 (CFX 408-113). Construction of additional lane capacity to the SR 408 eastbound exit ramp at SR 50 and realignment of SR 408 between Woodbury Road and the SR 50 exit. Duties include constructability reviews, Pre-Bid, Pre-Award and Pre-Construction meetings, project correspondence and coordination with contractor, supervision of CEI inspection staff, preparation of all documentation for final estimate, preparation of all supplemental agreements, evaluation and analysis/resolution of all contractor


issues and claims, tracking and distribution of plans and submittals, and oversight of all inspection and materials testing. Mr. Brown was the Project Administrator. Construction: $4.8 million.

Nov 2008-Aug 2010 Construction Management at Risk Services, Phases II and Ill in Osceola County, Florida. Widening from two to four lanes from south of Crescent Lakes Boulevard to US 17/92. Responsible for CEI Services monitoring all facets of construction including modification of existing signals, installation of new signal, relocation of utilities, roadway construction and maintenance of traffic. Mr. Brown was the Senior Project Engineer. Construction: $16 million.

Jan 2010 - Jul 2010 CEI Services for the County's Economic Stimulus Plan Transportation Projects for Osceola County Public Works in Kissimmee, FL. This project consisted of milling and resurfacing on three separate projects including John Young Parkway, Buenaventura Boulevard, and Neptune Road. Mr. Brown provided technical leadership in resolving several constructability challenges as well as overseeing paving inspection operations during the coldest winter in recent memory. In addition, he coordinated the documentation and reporting for FOOT LAP and ARRA compliance.

Jan 2009 - Jan 2011 Miscellaneous Engineering Services for Osceola County Public Works. As the Deputy Project Manager, Mr. Brown managed tasks as different as oversight of CM at risk projects and signal installation inspection. He also personally served as Senior Project Engineer for several of the small tasks. These tasks include installation of traffic signal, drainage improvements, sidewalk improvement, and resurfacing.

Jul 2005 - Jul 2009 Miscellaneous CEI Services for CFX in Ortando, Florida. Work on SR 408, SR 417, SR 528 and SR 429 that included asphalt milling and resurfacing, roadway widening, drainage improvements, lighting installations, new closed circuit ITS components, and toll plaza expansions. Projects included Fiber Optic Network link between SR408 and SR429 via Florida's Turnpike and milling and resurfacing along SR417 and SR528. Other projects include micro-projects. Duties included constructability reviews, Pre-Bid, Pre-Award and Pre-Construction meetings, project correspondence and coordination with contractor, supervision of CEI inspection staff, preparation of all documentation for final estimate, preparation of all supplemental agreements, evaluation and analysis/resolution of all contractor issues and claims, tracking and distribution of plans and submittals, and oversight of all inspection and materials testing. Mr. Brown was the Senior Project Engineer. Construction: $110 million.

Central Florida Expressway Authority (CFX), Curry Ford Road Plaza {State Road [SR 417] Mainline) and Dean Road Plaza {SR 408) Mainline Reconstruction on these $27M {total) Projects. Both

A GROUP, INC. toll plazas were expanded from traditional 8-lane plaza configuration to the innovative open road tolling configuration with two open road lanes and four cash lanes in each direction while maintaining daily traffic flows of over 65,000 vehicles per day. Throughout the duration of this project, Ben managed a construction engineering and inspection {CEI) consultant. Ben intervened in a multitude of situations with successful results for the client including negotiation of major changes, interaction with surrounding residents, management of quality assurance program, contract compliance, and maintenance of working relationship with the contractor. Ben was part of the construction management consultant (CMC) team that met the many technical challenges of this demanding project with creative solutions that minimized the impact to the client while promoting a timely completion of the job.

SR 528 {Beachline)/SR 436 Design Build Interchange lmprovementsforCFX. Interchange improvements included a new eastbound SR 528 to southbound SR 436 flyover ramp and widening of various roadway segments and ramps to eliminate existing weave sections on SR 528 and SR 436. The project included modifications to airport signage and custom pavement markings with require coordination with the Greater Ortando Aviation Authority. The project was funded through the Transportation Regional Incentive Program {TRIP) and build in collaboration between CFX, GOM, and FOOT. Mr. Brown served as the Project Manager. He met the many technical challenges of this demanding project with creative solutions that minimized the impact to the client and stakeholders including GOM, while promoting a timely completion of the job. Construction: $26.7 million.

CFX Western Expressway and SR 408, East-West Expressway Widening and reconstruction, Ortando, Florida: engineer II for this new bridge/roadway multiple project program with construction totaling $500 million and 7-year CMC contract amounting to $10 million. Ben administers micro contracts from start to finish by writing scopes of services and requests for bids, preparing engineer's estimates, inspecting construction or managing a construction engineering and inspection consultant, preparing and tracking documentation, performing final inspection and project closeout. Ben coordinated the preparation of the independent assurance manual for use by CEI consultants, and is part of the Independent Assurance {IA) team that performs IA audits.

SR 207 Widening, St. Johns County, Florida: assistant project engineer who provided CEI Services for a 4-mile (6.4-kilometer), four-lane urban highway. Ben's responsibilities included preparing final estimate packages, back-checking final pay quantities against field records, calculating final quantities for earthwork, performing claim analysis for an outstanding issue from a previous job which led to a settlement favorable to the FOOT, administering Fast Response contract for advance selective clearing and


grubbing for utility work, and implementing a document management system.

Group 99-E Mahan Road, Tallahassee, Florida. Final estimates engineer on this multi-lane main arterial roadway reconstruction project of SR 10 (U.S. 90). The project consisted of widening the existing roadway and installing new signals at two intersections. Utility relocations were a key component to the project. Jun 2017 - Mar 2018 Wekiva Parkway 429-204 Systems Interchange of SR 429 & SR 453 for the Central Florida Expressway Authority (CFX). The contract included construction of the new 2.63-mile segment of the limited access roadway in Apopka, Florida. The project extended through 175-acres of greenfield, with nearly 150-acres of clearing and grubbing, 51 ,000-SF of house/associated structures demolition and 30,000-LF of existing fence removal. The project included eight bridges composed of 8,436 LF of 48-72" haunched concrete Ugirders, 72,792-LF of 18" concrete piling. It also included 201,000-SY of roadway construction, 243,000-CY of excavation, 2.48 million-CY of import embankment, a 6-month surcharge area, 7, 131-CY of sinkhole grouting, 47,800 tons of asphalt, 23,900-LF of drainage piping and structures. The longest of the bridges was a 2,550 LF flyover ramp composed of curved post tensioned concrete haunched U-girders. The work also included pond construction, FON construction, CCTV, highway lighting, sign structures, signage, pavement markings, sod and seed and mulch. Jul 2017 - May 2017 1-95 (SR 9) at North 1-295 Interchange Design/Build Project for FOOT District 2. The interchange was being modernized and reconfigured to increase capacity and improve traffic flow and safety. The scope included minor ramp improvements and auxiliary lanes were added to 1-95. The Cole Road Bridge over 1-95 was also replaced. There was also bridge replacement/construction and ramp reconstruction along US 17.

May 2016 - Feb 2017 SR 9B Phase 3 for FOOT D2. This was the final phase of State Road 9B. This project was 2.3 miles from 1-95 to County Road 2209 (St. Johns Parkway) in St Johns County and includes a new intersection at Peyton Parkway and County Road 2209. This project also included construction of five retention ponds and bridges/overpasses at Race Track Road, Peyton Parkway, Durbin Creek and Russell Sampson Road. 2015-2016 Construction at 1-95 and SR 202 (JT Butler Blvd) for FOOT D2. A GROUP, INC. 2005 - 2006 Trout River/1-95 Bridge and Roadway Widening for FOOT. The old bridge, which opened in 1958, was removed and the new bridge was built. Work also included widening of 1-95. Mr. Erickson served as a Lead Carpenter. Construction: $53.4 million.

2004 - 2005 Mr. Erickson served as a Foreman on various projects. He built, formed, and poured 8'x10'x30' box culverts. He also demoed box culverts, set pre-formed box culvers, built forms and poured concrete to tie existing culverts to new ones, sealed concrete pipes, formed and poured slope pavement, graded and leveled group for runway concrete pours, poured 611 pep lean concrete base runways, and poured 15" pep concrete for top runway.

2003 - 2004 1-275 Widening in St Petersburg, FL. Mr. Erickson was the Lead Carpenter. He set-up, operated, and maintained nightly lane closures. He also set-up Bidwell for deck pours, hung overhang jacks, built false decking, poured bridge deck, set temporary road barriers, set metal tub girders, and formed and poured footers, columns, caps, and diaphragms.

2001 - 2003 Palm Valley Bridge/Wonderwood Connector. Mr. Erickson served as the Lead Carpenter. He erected Simmons Forms for footers, columns and piers. He poured concrete footers, columns and piers. He also set girders, hung overhang jacks, built and later demoed false decking, poured bridge deck (750 -1000 cubic yards per pour), set up Bidwell for deck pours, formed and poured diaphragms, uploaded beams from bard to land with single triple 9 crane, pile driving 18" & 24" various steel sizes 70' to 11 O', set Fl BS, set Haunch Girders, set drop-in girders (longest in the state at the time), and 6' FIBS approximately 11 O' long.

Mr. David H. Howard has been working in the construction industry with increasing levels of responsibility since 1992 and has been working with A2 since 2006. His extensive experience includes projects for the Florida Department of Transportation (FOOT), the Miami-Dade Expressway Authority (MDX) and the Central Florida Expressway Authority (CFX) along with the Greater Orlando Aviation Authority (GOAA). Jan 201 o -Mar 2011 FOOT District 5's SR 50 from Good Homes Road to Pine Hills Road Project (3.6 miles). The improvements under this contract consisted of widening and reconstructing SR 50 from four lanes to six lanes, from Good Homes Road to Pine Hills Road. Additional scope of work included the widening of Good Homes Road from two lanes to four lanes, from SR 408 to 1000' North of SR 50. July 2008 - Dec 2009 SR-528/Narcoossee Road Interchange Project for the Central Florida Expressway Authority: Work required for this project included an eastbound mainline temporary bridge extension, demolition of existing bridges and reconstruction of two new steel plate girder bridges. The bridge construction was comprised of a combined total of 6,552 If of 18" square prestressed concrete piles and 2,160 If steel plate girders (six single spans of 180 lineal feet per bridge). Jun 2013 - Jun 2014 The Port of Miami Tunnel Project that links Watson Island to the Port of Miami. which was formerly served by a single highway. The two-tube tunnels required special techniques for boring through highly porous substrates of dead coral. This was the first large-diameter road tunnel ever construction in Florida with the use of a tunnel boring machine (TBM). It involved excavating two undersea tunnels for the fourlane road, creating four floodgates, widening the MacArthur Bridge, and constructing an interchange to join up with the port's existing road network. Oct 2015 - Dec 2015 SR 417/Boggy Creek Interchange in for the Central Florida Expressway Authority (CFX). This project encompassed 2.98 miles of construction along SR 417 mainline. The SR 417 Interchange at Boggy Creek provides connection to the South Access Road of the Orlando International Airport (OIA). The project included the widening and construction of seven bridges including three post-tensioned concrete box girders flyovers and the MSE wall panels. Ms. Alemany assisted with the Final Estimate. Mr . Callum concurred and stated that the staff was easy to work with . Chairman Patterson asked if there were remaining hot spots. Mr . Callum answered yes , that there are a couple rema ining hot spots . Chairman Patterson asked if there was any information on Cobb Site Development . Ms . Watson answered no . Ms . Chairman Patterson asked why the NTP was being requested for April 20, 2021, but the services are not projected to begin until May 15, 2021. Mr. Sorondo answered that the intent is to be able to access funding whenever the funds are exhausted through the existing contract with Schenkel & Shultz, Inc.

Summary:

Greater Orlando Aviation Authority (GOAA), on behalf of the City as applicant, has applied to the Federal Aviation Administration (FAA) for a grant in the amount $2,366,125.00 for Project BP-045, Taxiway F and G Rehabilitation at the Orlando Executive Airport. The total project cost is $2,629,029.00. Due to the required time limits for grant acceptance and funding, GOAA is requesting that City Council accept the grant prior to award. This will insure that GOAA does not lose the awarded funding due to the timing of acceptance.

Recommendation of the Construction Committee to Approve an Addendum to the Continuing Program and Project Management (OAR Prime Entity) Services Agreement with A2 Group , Inc . for Construction Phase Owner ' s Authorized Representative (OAR) Services for Project Bid Package (BP) No . 00045 , ORL Taxiways F and G Rehabilitation , at the Orlando Executive Airport

BACKGROUND

BP No . 00045 , ORL Taxiways F and G Rehabilitation , at the Orlando Executive Airport (ORL) , will consist of the rehabilitation and improvements of existing Taxiways F and G at the Orlando Executive Airport , and includes paving , lighting , markings and signage , the demolition of approximately 400 feet of Taxiway F, situated diagonally between Runway 13 - 31 and its parallel Taxiway Kand , as a result , the construction of a new Taxiway Kl . Taxiway Kl will be approximately 300 feet long , between Runway 13 -31 and its parallel Taxiway K. Construction work will include mill and overlay for the taxiway pavement and improvements for taxiway markings , lighting and signage . Updates to the Airport Lighting Control and Monitoring System and electrical vault are also included .

ISSUES

A fee has been negotiated with A2 Group , Inc . for a total amount of $229 , 595 for construction phase OAR services for BP No . 00045 . These services will provide , but are not limited to , construction management , oversight , inspection services and material testing for the above - referenced project . Additional assistance to the Aviation Authority in the areas of pre-construction , construction project management , project control , contract administration , construction coordination and close - out services in support of the project are also included . If approved , services will be effective the date of Aviation Authority Board approval and after receipt of the Federal Aviation Administration (FAA) and Florida Department of Transportation (FOOT) Grants .

The Aviation Authority has reviewed the proposal and determined that A2 Group , Inc . proposes to achieve 27 % Disadvantaged Business Enterprise (DBE) participation for this addendum .

On April 20 , 2021 , the Construction Committee recommended approval of an Addendum to the Continuing Program and Project Management (OAR Prime Entity) Services Agreement with A2 Group , Inc . for Construction Phase OAR Services for BP No . 00045 , ORL Taxiways F and G Rehabilitation , at the Orlando Executive Airport , as outlined in the memorandum .

On June 7 , 2021 , the Orlando City Council authorized the pre-acceptance of the grant offered by the FAA in the amount of $2 , 366 , 125 for BP No . 00045 on behalf of the Aviation Authority .
