ACUPUNCTURE IN RHEUMATOID ARTHRITIS(ABSTRACT). SHEN AC ET AL. arthritis andrheumatism. 1973,16(4),559 (eng). ref:0A double-blind, cross-over study of the effects ofacupuncture in 20 adulte with classic or definiterheumatoid arthritis is in progress. The precise site anddepth of insertion of acupuncture needles and the useof moxa are determined by the acupuncturist who is

Chinese and Chinese-trained. Previous medicalregimes are undisturbed throughout the study. Tenpatients are treated appropriately for 5 weeks, inappropriately for the next 5 weeks. The second groupof 10 patients is treated reversely . Inappropriatetreatment consists of the insertion of needles in sitesadjacent to but not in traditional acupuncture points, atsuperficeal depths and without the traditional twirling.Subjective response of the patient, tris lever of functionand physical findings are monitored by an examinerwho is "blind." Serial erythrocyte sedimentation rates, tests for rheumatoid factor and x-raye of target jointsare performed. To determine the susceptability tosuggestion psychiatrie interviews and MinnesotaMultiphasic Personality Inventories are conducted.

Analysis of results will be available for presentation. Forthe purpose of this abstract the blind examiner canmake the following observations. Of 10 patients whosestudy is complete 4 were subjectively improved through

both the inappropriate and appropriate treatmentperiods (sequence unknown) and wish to continueacupuncture. One patient was improved subjectivelyduring 1 5-week peripd, 4 were unchanged while l wassubjectively worse throughout the 10 weeks ofobservation. Physical signs of active diseasediminished during a 5-week period in 1 patient, showedno change in 7 patients and progressed throughout the

period of observation in 2 patients. [18.04 / ecr- ]7- gera: 14103/nd/rePRELIMINARY CLINICAL STUDY OF

Amost remarkable cure rate was obtained inosteoarthitis deformans of the knee when treated withacupuncture on the normal side and flection-extension

The curerate was low in rheumatoid arthritis, and the therapywas non-effective concerning disorders in motility of theknee joint efter cerebral hemorrhage or thrombosis. Theimprovement rate, however, was extremely low inosteoarthritis deformans and rheumatoid arthritis of theknee after the acupuncture on the affected part of theaffected side with Reha on the affected part of theaffected side, or Reha on the lesion, and these meansof therapy were completely non-effective concerningdisorders in motility after cerebral hemorrhage orthrombosis. Our previous reports No. I and 2 accordwith the evidence obtained in this study that theacupuncture on the normal side and Reha on the

The low improvement rate in rheumatoid arthritisand non-effectiveness concerning disorders in motilityof the knee joint after cerebral hemorrhage orthrombosis may be explained by differences inmorphology of the diseases.

There wereno findings of bone metastases. Although themechanism of accumulation of I-131 in this patient is

and 5 cases with no efficiency (16.1%). The rate of totalefficiency was up to 83.9%. Several signs such as thegripping power of two hands, the swelling and painnumbers of joints, tenderness as well as lab indexessuch as erythrocyte sedimentation rate (ESR), mucin, rheumatoid factor (RF) and blood rheology wereobserved in patients with RA. It was found that allexperimental indexes and clinical signs except RF weresignificantly improved after acupuncture treatment, which indicated that the curative effects of RA byacupuncture were achieved by promoting bloodcirculation to remove blood stasis, eliminating oralleviating the state of high blood viscosity, diminishinginflammation and easing pain. Following treatment, there wasimprovement in the immune function, specifically: theerythrocyte sedimentation rate decreased, hemoglobinincreased, the rheumatoid factor became negative, thelymphocyte transformation rate and E-rosette formationrate increased, complement C3 increased andimmunoglobulin concentrations changed favorably. These results suggest that the treatment of rheumatoid

    arthritis with moxibustion may influence the immunefunction. [18.04 / 23.02- 05.09- ]

It isregarded as an autoimmune disease, but thepathogenic mechanism is still to be clarified. In order toobserve the effects of the treatment of RA withacupuncture and moxibustion on IL-2, 41 patients weredivided into warming needle and point injection groupsat random, and 19 healthy subjects as control group. The results showed that the IL-2 levels in two RAgroups before treatment were obviously lower than thatin control group (P 0.05). After treatment the IL-2 levelin control group was unchanged, but increased

considerably in two RA groups (P 0.01). Generally, IL-2is considered as a very important, signal for regulatingimmune response. Our results have shown that thedecrease of IL-2 in patients with RA should be one ofmain causes of internal environment disorder, acupuncture and moxibustion as a stress stimulationexerts an influence on the immunity system throughneuroendocrine system to improve the IL-2 production.[18.04 / cta- 05.09- il- 05.15- ]

NIHONYANAGI K.journal of thejapan soc iety of acupuncture. 1994,44(3),227-32(jap*). ref:0Effects of pretreatment with gold ion or coexistence ofgold and silver ions on silver-induced contracture were

studied to evaluate whether different uses of gold andsilver needles in patients with rheumatoid arthritis aredue to different actions of these heavy metals.Simultaneous application of 20 M gold ion and 5Msilver ion prolonged the time from ion application totension development in single fibers of frog skeletalmuscle 10 fold, compared with that observed uponsilver ion alone. Such fibers showed no striationdisorder. When 20 M gold ion was given to fibersimmediately after development of silver-inducedtension, the duration of a transient component of silver-contracture was shortened and a subsequent toniccontracture no longer observed. Delayed application ofgold ion fibers at which the tonic contracture is alreadyappeared did not inhibit silver contracture. Even after

pretreatment with gold, muscle fibers deteriorated uponsilver application. These findings suggest that chemicalinteraction of both metal ions with membrane proteins isnot simple, but depends of concentrations used, application order and time of gold application afterexposure of fibers to silver.

[18.04 / 23.02-05.15- ]

The effects of tripchlorolide, an active ingredient (T4) ofTripterygium wilfordii Hook on the production ofprostaglandin E2(PGE2) by synovial cells of rheumatoidarthritis (RA) patients were investigated. Six cases ofdefinite RA (female 5, male 1, mean age 45 with anaverage course of disease of 9 years) were selected.Surgically obtained synovium specimen weredissociated into digested synovial single cells (DSSC).

The cells were incubated with various concentrations ofT4 for 48 hours. Using radioimmunoassay T4 wasfound to significantly inhibit the production of PGE2(control 6. 10 _ 2. 30 vs T4 treated 0. 58 _ 0. 47 x 10(-5) mol. L-1) by short-term cultured DSSC of RApatients. The results of this study suggests that T4 maybe useful in the forthcoming treatment of RA due to itsinhibition of production of PGE2 by synovial [18.04 / - ]

ADRENAL CORTEX IN RAT WITH ADJUVANTARTHRITIS. ZHANG MM ET AL.journal of tong jimedical university. 1994,14(3),158-61 (eng). ref:0This paper reports the effects of Tripterygium Wilfordii(TW) on adrenal cortex in rats with adjuvant arthritis.Forty rats were divided into 5 groups. Adjuvant arthritis(AA) models were made with complete freund'sadjuvant (CFA) in groups I- IV. Each of which wastreated with sodium carboxyl methyl cellulose, TW,

prednisone and cyclophosphamide respectively. Theuntreated rats allocated to group V served as normalcontrols. The swelling of AA markedly subsided ingroup II, III and IV as compared with group I (P < 0. 01),whereas no significant differences were noted amonggroups II, III and IV (P > 0. 05). The obviously increasedplasma corticosterone levels and decreased adrenalascorbic acid levels were observed in group II, whereasdecreased plasma corticosterone levels and increasedadrenal ascorbic acid levels were noted in group III.There was a striking contrast between groups II and III.The morphological changes of adrenal glands underlight microscope revealed hypertrophic adrenal corticesin group II, and atrophic adrenal cortices in group III.

The above findings suggest that the effect of promotingproduction of corticosteroids may be one of themechanism by which TW can effectively treatingautoimmune diseases. [18.04 / eap+rat- ]
