The Rock Island County Metropolitan Mass Transit District (MetroLINK) is issuing this Addendum

Number FIVE (5) to the Invitation for Bid (IFB 2016 MMS-03) for the Renovation and Addition General

Package for the Moline Multi-Modal Station (MMS) Project.

Item 1: Bid Date, Addendum, Substitution Request Clarifications – Below are the updated project bid

schedule date clarifications: i) Bid Date: CHANGE to 6/22/2016 at 3:00 PM

ii) Final Addendum: No. 05

Item 2: Legat Drawings and Specifications: REVISE Legat Specifications and Drawings per attached

Specification Section 009005 instructions, drawing revisions, and sketches dated June 14, 2016 (4 Pages).

Item 3: Section 5.1 Scope of Work – REPLACE Bid Package Scope of Work with attached Bid Package

Scope of Work, including, project milestone schedule and construction logistics drawings.

Item 4: Bidder Question: Spec 22 0500 & 23 0500/1.4 indicate the requirement for coordination

drawings, with the Mechanical Contractor as the coordinating contractor. Is this really wanted, or is this a

boilerplate … specification, and, will onsite field coordination suffice instead to save project cost? If

drawings are required – are they to be 2-D coordination or a complete BIM coordination? ANSWER: A

minimum 2D coordination drawing will be required. Prime Contractor shall submit coordination drawings

as noted in the specifications.

Item 5: Bidder Question: The cmu wall infill located on the north elevation basement that is to be

removed for access to building with a ramp per addendum 1 sheet 21. Is this construction indeed cmu

infill? Or is it concrete foundation? And is this part of the contract scope? ANSWER: Yes, this is an

existing masonry wall. Yes, it is part of the base bid contract scope.

Item 6: Bidder Question: The exterior windows at the basement level and east addition that are to be

removed and restored per note D-2. It's our knowledge from the other windows in the building

that they have lead paint and need abated. Is this true? If so can you clarify that abatement is to be done by

owner. Therefore, the abatement contractors will do the removal of windows.

ANSWER: See Lead Paint Report from Addendum No. 001 and Bid Package Work Scope Listing. Lead

paint abatement will be completed by the Owner. Prime Contractor is responsible for window removal,

salvage, restoration, etc. per work scope.

Item 7: Bidder Question: Are the remaining historical windows on first thru sixth floors part of this bid

package for restoration? ANSWER: Refer to Drawings A-200 and A-201.

Item 8: Bidder Question: On the drawings Sheet AD-103 has a dashed rectangle along column line 4

between lines C and D that is not referenced as to what it is. ANSWER: This opening will be provided

by the future Developer.

Item 9: Bidder Question: At detail 5/A700 and 1/S400 – What quantity of drilled piers (caissons) are to

be included under the bridge support? ANSWER: There is one pier located at the center of the

bridge. See detail 10-S401 for information on pier and column size.

Item 10: Bidder Question: What is the location of each drilled pier (caisson) under the bridge

support? ANSWER: See 4’-7” dimension in attached sketch: Larson 01. This dimension will need to

verified in field to avoid interferences with existing foundations and utilities.

Item 11: Bidder Question: SSK-3 indicates 2 guide rail support columns as alternate however I believe

one of the two would be required for the base bid. There is also not support columns shown on the East

side of the East elevator for the elevator and weight guide rails (as shown on detail 3 A-402). Are these to

be mounted to the masonry wall? ANSWER: Intent is for the east elevator to be braced off the new

masonry wall.

Item 12: Alternates Schedule: DELETE Item G - Alternate No. 07 from the Alternates Schedule.

Item 13: Section 3.4 Alternates: DELETE Alternate No. 07 from the list of Alternates on the pricing

form.

This addendum forms a part of the bidding and contract documents and modifies the original bidding documents dated May 10, 2016. Acknowledge receipt of this addendum in the space provided on Bid Form. FAILURE TO DO SO MAY SUBJECT BIDDER TO DISQUALIFICATION. I. PART 1 - ADDENDUM TO THE PROJECT MANUAL

A. Section 21 13 00, 2.1A: ADD mechanically coupled grooved fittings for 2” and under pipe. Plain end fittings or couplings are not acceptable.

B. Section 22 07 19: ADD 1” Type B insulation for all sump pump discharge piping.

C. Section 23 52 33.14: REVISE the boiler basis of design from Thermal Solutions EVAM to EVS, burner turn-down

from 4:1 to 3:1, and the heat exchanger warranty from 3-years to 5-years. The boiler controls and electrical parts shall have a 1-year warranty

II. PART 2 - ADDENDUM TO THE DRAWINGS

A. Drawing G-002; ADD note to structure on west side of East Stair Tower to read: “Provide 2 hour applied fireproofing system to new steel in order to maintain the indicated fire rated stair assembly”

B. Drawing AR-100; REVISE Roof Area #1, Note 4 to read “4. INSTALL 5/8” COVER BOARD SET IN INSULATION ADHESIVE”.

C. Drawing A-312; REVISE 2 layers of 5/8” gypsum board and 1 layer of exterior gypsum board on exterior face of East Stair tower to be 3 layers of 5/8” exterior gypsum board. Update all Drawings/Details to incorporate this change.

D. Drawing S-102; ADD note 13 as indicated on SSK-8. This revises the bearing height of Vestibule 118 and 119 framing indicated in 2/S-102.

E. Drawings P-100 & P-101: DELETE roof drains RD-1 and ORD-1 that are near column line 9D that serve the future hotel courtyard including the branch piping down to the 12” storm main in the basement. Install a 6” capped wye fitting in the basement 12” storm line for future extension to the courtyard roof drains by others. Disregard earlier addendum items related to this.

F. Drawing FP-101: REVISE Keynote 4 from closed head window sprinklers noted in Addendum #4 to a deluge system with open head sprinklers on both sides of the windows. The deluge valve can be located within Custodian 113. Provide deluge valve including all associated piping, supervised shutoff valve, sprinklers, detection system and wiring for a complete and operating deluge system for the windows. Include all associated fire alarm connections.

G. Drawing E-500, ADD the following the Approved Manufacturers list for Types S3 and S3A to the Luminaire

Schedule: DECO LIGHTING D1440-LED SERIES III. PART 3 - CLARIFICATIONS

A. All steel to have intumescent or spray applied fire proofing shall meet the substrate requirements of those systems.

B. Sections 21 05 00, 22 05 00, 23 05 00, 1.4 and 26 05 00, 1.6: CLARIFICATION: 2D or 3D coordination drawings are acceptable.

C. Section 21 05 29, 1.2: CLARIFICATION: All sprinkler piping supports shall comply with NFPA 13 and 14.

D. Section 21 05 29, 2.6: CLARIFICATION: Pipe sleeves are required for penetrations through masonry walls and through floors

E. Drawing M-500, Boiler Schedule: CLARIFICATION: The boiler capacity listed is in MBH, not Btuh.

MULIT-MODAL STATION PROJECT SECTION 5.1

SCOPE OF WORK

BID RELEASE #03 – General Construction and Street, Sidewalk, and Utility Improvements

The following shall be included from this workscope:

Item Description

001 SECTION 1 of 3: UNDERSTANDINGS AND STIPULATIONS

002 The following items listed are to assist the Prime Contractor preparing their proposal for the work defined in this Bid Package; such listing shall not be interpreted as limiting the work required for this Prime Contractor.

003 Although portions of the foregoing documents are applicable to work scopes of bid packages not included within this Bid Package's scope description, Prime Contractor shall familiarize themselves with all work shown and described in all the documents and shall include in their proposal any costs for assumed impacts on their work.

004 Notwithstanding anything in this Agreement to the contrary, the Scope of Contractor's Work includes but is not limited to the following: (i) The Scope of Work contained within IFB 2016 Elevator-01; (ii) Notice to Proceed dated TBD (to be determined); (iii) Contractor's Bid Submission TBD (to be determined); and other Owner/Designer-issued documents as agreed by Owner in writing, or attached hereto and incorporated by reference.

005 In any event there is/are deemed conflicting terms and/or conditions within this agreement, any/all regulatory and/or bid submission requirements previously executed and attached as to the Agreement shall first govern.

006 Prime Contractor understands the project is being funded by federal monies via a Tiger Grant. The City of Moline is Owner of the project property and Metrolink is the grantee of the Tiger Grant.

007 Prime Contractor agrees to reach agreement with Illowa Construction Labor and Management Council regarding IMPACT Agreement as condition precedent to first pay application being approved.

008 Prime Contractor shall provide a payment bond and a separate performance bond in amounts equal to the Prime Contract Sum. Costs of the bonds are included in the Prime Contract Sum.

009 Prime Contractor work activities shall comply with all applicable OSHA work rules, project specific work rules, assigned project responsibilities, safety provisions as noted within the Agreement, and any/all industry standard regulations governing standard safety practices or protocols.

010 Prime Contractor understands this is a tobacco-free project. Workers on this project understand use of tobacco products on this project is prohibited. Workers found in violation of this project requirement are subject to removal from the project.

011 Prime Contractor shall provide any/all required public protection related to Prime Contractor's work. This may include but not be limited to fencing, signage, barricades, and any/all materials and/or equipment.

012 Prime Contractor shall report any/all project incident(s)/accident(s) to the Owner within 24 hours of said incident(s)/accident(s).

013 Prime Contractor shall provide any/all drinking water, cups, and ice to workers for their portion of the project.

014 Prime Contractor shall provide any/all porta-toilets for their portion to workers for their portion of the project.

015 Prime Contractor shall with comply with limited onsite parking and coordinate site access and

deliveries with Developer General Contractor.

016 Prime Contractor's employees will strictly adhere to a no harassment policy. All workers shall comply with Prime Contractor's policy posted at the job site and shall remove any violators from the job site.

017 Prime Contractor understands although lead and asbestos abatement work has been completed by the Owner, with exception for several existing to remain structural items identified with orange paint markings, some unknown hazardous materials may exist. Prime Contractor shall familiarize themselves with any/all hazardous material reports prior to mobilizing to the project site and agree to stop work and report any potential findings of hazardous materials to the Construction Manager/Owner.

018 Prime Contractor understands the building has historical significance and preservation requirements, specifically the South elevation. Prime Contractor shall use care working around adjacent structure. Any damage to the South elevation of the building and/or any assemblies of historical significance shall be repaired only by corrective action direction from the Owner's historic preservation consultant and/or supplemental consultants.

019 All work included under this work scope shall comply with the applicable codes and standards adopted by the City and State where the project is located. Prime contractor shall notify, coordinate, and pay fees for any work related inspections or permits. Prime contractor shall notify the Architect and/or Owner/Owner Agent 48 hours prior to work inspection requests. Prime Contractor is responsible to document all inspections in writing and submit documentation to the Architect and/or Owner/Owner Agent on an approved Daily Mancount Form (DCR). Prime Contractor is also responsible to provide a copy of any inspection document received from the inspecting party to the Architect and/or Owner/Owner Agent within 48 hours of a completed inspection. Prime Contractor shall correct any deficiency items noted by the inspection authority within 48 hours of receiving verbal or written direction of said requirement and before work progresses. If work progresses before corrections occur; Prime Contractor shall bear any/all retesting costs and rework costs to complete corrective actions.

020 Prime Contractor understands the project site will have multiple General and Prime Contractors working on building renovation and addition projects. Prime Contractor agrees to participate in weekly, day/time to be determined and agreed upon, coordination meetings. Prime Contractor shall also attend supplemental meetings upon request by Developer General Contractor, Architect/Engineer, and/or Owner Representative.

021 Owner shall hire an independent testing agency that will be responsible for onsite and laboratory soil, concrete, reinforcing steel, and structural steel testing. Any/all offsite testing, inspections, certifications and/or any/all testing(s) for materials/equipment not mentioned herein are the responsibility of the Prime Contractor. Owner shall provide Owner provided testing contact information to the Prime Contractor. Contractor shall coordinate all testing and inspection activities as to not affect Project Schedule and is responsible for providing access to, connections, and any/all temporary provisions to complete partial/final system inspections and testing as required to comply with Project requirements and work scope. Multiple tests and mobilizations are to be expected.

022 Prime Contractor shall, at their expense, provide additional labor or enact labor overtime provisions necessary to meet Project Schedule milestones. Prime Contractor agrees to do so upon receipt of written delay notifications per the Contract Agreement terms and Conditions. It is to be assumed by Prime Contractor that work will occur in multiple areas during the same timeframe, thus, multiple working crews performing the same task may/will be required to meet Project Schedule Milestones.

023 Project available work hours are 6:30 am to 5:00 pm Monday through Friday. Owner reserves the right to modify work hours to suit project conditions and/or daylight conditions. Any work outside of these hours, must be approved by Owner/Owner Agent. Prime Contractor shall provide, at their expense, increased work crews and/or overtime as necessary to meet scheduled milestones. If additional manpower/hours are not the fault of Prime Contractor, this needs to be communicated to Owner/Owner Agent ahead of time so appropriate party(ies) can be notified of potential

backcharge(s).

024 Prime Contractor shall submit all project specific material and equipment product data, closeout submittals, operating manuals, as-builts/record drawings, maintenance and/or operations manuals, etc. in electronic PDF format on CD. If electronic files are not available from vendors, Prime Contractor shall be responsible for providing electronic PDF files (including oversize drawings). The files on the CD should be separated out per closeout item. Files will be named by specification section followed by the name of the closeout item.

Prime Contractor shall submit one hard copy and three electronic copies of each closeout item in a 3-ring binder with closeout items separated by heavy paper dividers with clear plastic tabs labeled by spec section. Oversize drawings should be placed in binder sleeves. This binder shall be submitted to Architect for review. Once the binder and oversize drawings are complete and approved by Owner and Architect, Owner and Architect twill return the binder and drawings to the Prime Contractor for three CDs to be created. The foregoing procedures constitute the minimum requirements for the closeout process. These procedures shall not supersede any more stringent requirements set forth in the Contract Documents, but rather shall serve to supplement said requirements, and in the absence of said requirements, serve as the minimum Owner and Architect requirements. Prime Contractor shall remain responsible for providing additional hard copies of closeout information as specified in the contract documents should Owner require it.

025 Prime contractor is responsible for any/all layout, staking, and surveying required to complete their work. Work installations deviating from Coordination Drawings or the Contract Documents must be approved in writing by the appropriate designer prior to field installation.

026 Prime Contractor shall furnish any/all hoisting facilities for Prime Contractors material, equipment and personnel.

027 Prime Contractor shall repair any/all disturbed public and/or private items/areas adjacent to construction activities back into their original state prior to the end of construction.

028 Prime Contractor understands they are working around existing to remain structure(s) and/or finishes. Any existing-to-remain structure, walls, floors, ceilings, and/or finishes disturbed by Prime Contractor shall be patched back to match its' existing condition prior to construction completion.

029 Any/all patch back/patching of existing conditions by disturbing Prime Contractor must be done in such a fashion so that it is deemed permanent. Disturbed areas must be shaped, formed, bushed, and prepped to receive proper patching per the patching material requirements. Some patching may require additional removal of disturbed surfaces to properly patch and all patches must be compatible with new or matching finishes. Any/all repairs must be approved in writing by the appropriate Architect/Engineer.

030 Prime Contractor shall submit Construction Document Drawing Sets and fill out and submit Building Permit application to the City of Moline. The Owner shall pay any/all application costs/fees for the building permit.

Prime Contractor shall apply for, purchase, obtain, and coordinate any/all other necessary City, County, State, and/or Federal permits and inspections related to completing their work.

031 Prime Contractor shall be allowed to install (1) 4'x8'project sign (print approved by Owner) with company's name, logo on the project site at a location approved by the Owner.

032 It is the responsibility of Prime Contractor to follow all regulations and request any/all required City, State, Utility, and Federal approvals for utility locates required to complete their work scope. Locate request must be requested in a timely fashion as to maintain and/or improve project schedule.

033 Prime Contractor shall, on an on-going basis or at the direction of the Owner, keep the Project site and surrounding area free from accumulation of waste materials, debris or rubbish caused by performance of the Prime Contractor's Work.

034 Individual/personal scraping will not be allowed on this project. It is understood that the Owner owns any/all demolished items until these items are unloaded offsite.

035 Prime Contractor shall provide a professional final cleaning for the Great Hall, Restrooms, and

Penthouse. Future retail/tenant spaces shall require a finish broom cleaning and general wipe down.

036 Prime Contractor shall only use cleaning materials which will not create hazards to health or property and which will not damage surfaces. Prime Contractor shall only use cleaning materials and methods recommended by manufacturer and clean surfaces/parts/areas recommended by cleaning material manufacturer.

037 Prime Contractor shall maintain at least one access drive onto the project site from 4th Avenue at all times that allows for any/all equipment and material deliveries to be driven onto the site for loading/unloading.

038 TEMPORARY:

Prime Contractor shall reference attached Temporary Conditions and Utilities Maps and include any/all directions, understandings, and/or responsibilities within bid.

039 Perimeter site fencing is provided by Owner. Prime Contractor is responsible for maintaining and/or modifying site fencing as required to complete work. Multiple access gates shall be required through the duration of the project. Any fencing damaged by Prime Contractor shall be repaired or replaced by Prime Contractor. Prime Contractor shall be responsible to coordinate with the Owner's fencing contractor to remove any/all remaining fencing at the end of the project and shall provide a 48 hour written notice to the Owner of upcoming fence removal coordination.

040 Initial erosion controls have been provided by Owner. Prime Contractor shall fill out, submit, pay for, obtain, maintain, and closeout for any/all permits, documents, and/or record keeping for project erosion control. Prime Contractor is responsible for modifying and expanding existing controls to comply with building and civil improvement erosion control requirements. Prime Contractor is responsible for maintaining and/or modifying erosion control measure as required to complete work. Prime Contractor is responsible for removal of any/all erosion control measures at the end of project but only after approval of removals has been granted and site has been deemed stabilized by Authority Having Jurisdiction. Prime Contractor shall include copy of any/all erosion control documents within closeout binder.

041 Prime Contractor shall provide mowing/weed control of job site upon Owner Request, including, but not limited to, any area inside construction fence with weed trimming around job site construction fence. Mowing/weeding will not be requested more than three times during project.

042 Prime Contractor is responsible for any snow removal for access to jobsite and building. The cost for this work is considered an allowance, see bid form. Prime Contractor must notify Owner prior to completing snow removal work in excess of the allowance. Prime Contractor must submit time and material tickets for this work with monthly pay applications.

043 Prime Contractor is responsible for dewatering any/all of their excavations, as applicable, to complete their work. Any dewatering in the basement and/or any basement access location(s) will be the responsibility of Prime Contractor. Prime Contractor to furnish and install, maintain, and remove temporary sump pumps and/or pits and ensure any temporary pumps/pits are operating for the duration of the project or until permanent sumps and pumps can be installed, permanently connected, and are fully operational.

044 Prime Contractor shall provide any/all dust control as required.

045 Prime Contractor shall provide any/all street cleaning as required.

046 Prime Contractor shall provide any/all construction dumpsters as required. Prime Contractor shall adhere to any/all project waste tracking requirements and shall submit documentation to Owner once a month with pay application request.

047 Owner shall furnish to Prime Contractor same-keyed padlocks that fit a standard hasp assembly and 20 keys. Prime Prime Contractor shall place, use, and maintain project padlocks on exterior gates and any/all temporary and/or permanent entryways as needed to secure project site/area. Prime Contractor shall return padlocks to Owner Representative at end of project.

048 Prime Contractor is responsible for protecting, securing any/all tools, materials, equipment on the

project site. Prime Contractor understands that Owner is not responsible for any lost and/or stolen items.

049 Prime Contractor shall furnish and install (10) permanent-to-remain tie-off points on the main building roof evenly spaced and a minimum 15-0 from edge of building. Tie-off points are to be engineered and flashed in with the structure/roof.

050 Prime Contractor is responsible for completing work within and/or under building structure's allowable working loads. Prime Contractor includes furnishing, installing, and removing any temporary support systems needed to properly support and protect the existing structure during modification, demolition, and new installations directly associated with this work.

051 Prime Contractor understand that existing building has wall openings with temporary partitions install to secure the building. Prime Contractor shall take ownership of any/all temporary enclosures for openings shown to receive new doors, windows, frames, infills, and/or any finishes for this work scope.

052 Prime Contractor shall provide and install temporary covers over any existing or new exterior wall openings resulting from demolition work by this Prime Contractor. Prime Contractor shall maintain all covers and remove covers for other Prime Contractors needing to install new work. Temporary covers must be adequately secured and weatherproof.

053 Prime Contractor understands that existing building has some temporary water service, including but not limited to, a backflow, service connection, and temporary distribution piping/fittings. Prime Contractor is responsible for maintaining, modifying, and/or extending temporary services to complete their work. Prime Contractor shall assume sole ownership of the temporary water services item(s) and will have priority of the service/system, meaning, Developer General Contractor may request permission to extend service to complete their work as part of their cost without impeding Prime Contractor. Prime Contractor shall remove temporary water items/service and return any/all parts to Owner at end of project.

054 Prime Contractor understands that existing building has some temporary electrical service, including but not limited to, a transformer, distribution panel, underground service, electrical panels, receptacles, and a temporary lighting circuit on each floor. Prime Contractor is responsible for maintaining, modifying, and/or extending temporary services to complete their work. Prime Contractor shall assume sole ownership of the temporary electrical item(s) and will have priority of the service/system, meaning, Developer General Contractor may request permission to extend service to complete their work as part of their cost without impeding Prime Contractor. Prime Contractor shall remove temporary electrical items/service and return any/all parts to Owner at end of project.

Prime Contractor is responsible to coordinate with appropriate authorities and follow any all right-of-way railway and highway work requirements, including any/all temporary means, flagmen, and any/all other requirements.

055 DEMOLITION:

056 Prime Contractor shall complete any/all demolition work as shown on the Contract Documents.

057 Prime Contractor shall excavate, with appropriate layback, and remove a section of basement wall from foundation to bottom of 1st Floor concrete deck between Gridlines 6 and 7 along Gridline A to create an access ramp allowing for labor, material, and equipment access. Temporary opening shall be secured with a temporary wall and temporary doors with a security hasp. Temporary access ramp and wall shall be open for a minimum 4 weeks before enclosing is allowed. Prime Contractor to coordinate access ramp installation and re-build work with Developer General Contractor. Prime Contractor shall install new structural masonry wall when infilling opening and shall treat exterior of wall with an approved sheet membrane and/or approved equal liquid applied waterproofing membrane system (including 2" insulation board), and, infill excavation with appropriate fill that will meet project compaction requirements.

058 Any minor demolition not specifically shown on the plans, but required to complete Prime Contractor scope of work, shall be by this Prime Contractor if it is deemed reasonable that the

demolition was obvious and had to occur to perform the new work. This includes patching back any surrounding finishes affected by demolition work.

059 Prime Contractor shall make all penetrations through walls and floors required to complete their work, whether shown or not shown on the drawings, but required.

060 Prime Contractor shall furnish and install any floor fill and shall grind down any high spots of existing slabs required for leveling floors as noted on the Contract Documents.

061 Section 2 of 3: WORK SCOPE

The Prime Contractor's Work includes all labor; supervision; materials; equipment; services; supplies; tools; facilities; transportation; storage; receiving; licenses; inspections; certifications; overhead; profit; or other items required or reasonably inferable to properly and timely perform and complete all work and services to be performed by the Prime Contractor pursuant to this Agreement. Prime Contractor's Work shall be performed in accordance with the Drawings, Specification Section(s), Addenda, and any/all supplement instruction and/or work scope furnished by Architect and/or Owner, attached hereto and/or incorporated into bidding documents or herein by reference.

062 Prime Contractor shall furnish any/all labor, material, and equipment to complete all demolition, selective and otherwise, for new installations. Prime Contractor understands work will take place around existing to remain finished product and shall take care to protect any/all adjacent surfaces and/or materials. Any/all damage to existing to remain finishes shall be repaired with matching materials and methods by the disturbing Prime Contractor at the Prime Contractor's cost. Patches and/or repairs shall be reviewed by Architect and/or Owner for compliance and acceptance, any/all rejected work shall be repaired by Prime Contractor at Prime Contractor's cost.

063 Prime Contractor shall furnish an/all labor, material, and equipment to complete all concrete work, including, but not limited to, all maintenance of concrete, cast in place concrete, formed concrete, concrete floor finishes, footings, foundations, piers, slabs on grade, suspended slabs, toppings, walls, curbs, equipment bases, stairs and/or stair pans, patchwork. Prime Contractor shall furnish and install any all mock ups, reinforcing, adhesives, injections, formwork, form liners, ties, embeds, admixtures, vapor retarders, cures, hardeners, densifiers, sealers, anchors, finishes, components, fasteners, and any/all accessories for complete installations.

064 Prime Contractor shall plan, coordinate, furnish and install any all concrete system joints and/or expansions systems.

065 Prime Contractor shall plan, coordinate, and execute any/all hot weather and/or cold weather concreting requirements.

066 Prime Contractor shall complete floor flatness and levelness tests for new slab on grades within 24 hours of finishes and publish report results to Owner Representative within 48 hours of test completion.

067 Prime Contractor shall furnish and install any/all precast structural concrete, including but not limited to, delegated design requirements, hardware, anchorage systems, embeds, reinforcings, grouting, bearing pads, insulation assemblies, finishes, patches/repairs, and any/all components and/or assemblies for a complete installation.

068 Prime Contractor shall furnish and install any/all cast underlayments for a complete installation.

069 Prime Contractor shall furnish any/all masonry work, including but not limited to maintenance of masonry, mortar and masonry grout, unit masonry, and cast stone masonry. Prime Contractor includes any/all brick units, concrete masonry units, grouts, mortars, accessories, protections, preparations, cleanings, flashings, coursings, placings, lintels, joints, fasteners, anchors, reinforcings, and any/all components for a complete installation.

070 Prime Contractor shall furnish and install all metals required for project, including but not limited to structural steel framing, steel decking, cold-formed metal framing, metal fabrications, metal stairs, laddders, pipe and tube railings, joists, purlins, handrails, guardrails, plates, anchor bolts, miscellaneous metals, reinforcing angles, frames, tubes, columns, beams, deckings, bolts, washers, nuts, anchors, fasteners, pour stops, fillers, primers, finishes, galvanizing, approved

temporary connections, bracings, and any/all components for a complete assembly.

071 Prime Contractor shall furnish and install all wood and plastics required for project, including but not limited to rough carpentry, millwork, caseworks, blocking, nailers, supports, accessories, hardwares, countertops, countertop supports, and components for complete assemblies.

072 Prime Contractor shall furnish and install all thermal and moisture protection for this project, including but not limited to all sheet waterproofings, membranes, thermal insulations, batt insulations, board insulations, foamed-in-place insulations, weather barriers, ventilated rainscreen systems, fiber cement sidings, and any/all components for complete assembly installations.

073 Prime Contractor shall furnish and install all roofings, including but not limited to preparation of re-roofings, elastomeric membrane roofings, sheet metal flashings and trims, insulations, roof accessories, and any/all components for complete assembly installations.

074 Prime Contractor shall furnish and install all fireproofings, including but not limited to applied fireproofings, intumescent mastic fireproofings, firestoppings, through penetration firestopping, and any/all components for complete assembly installations.

075 Prime Contractor shall furnish and install all joint sealants, including but not limited to any/all sealants, primers, fillers, tape, backings, mastics, adhesives, and any/all components for complete assembly installations.

090 Prime Contractor shall furnish and install a complete mechanical system, including but not limited to basic requirements, supports, anchors, vibration isolation, identification, testing-adjusting-balancing, insulations, controls, instrumentation, piping, pumps, refrigeration piping and specialties, chemical treatments, ductwork, ductwork accessories, power ventilators, air inlets and outlets, air cleaning, breeching-chimneys-stacks, boilers, chillers, air handling units, air conditioning units, heat transfer units, heaters, and any/all associated components for complete assembly installations.

091 Prime Contractor is responsible for any/all temporary heaters and/or air conditioning units required to complete installations. Permanent equipment may be utilized for temporary solutions but must still meet criteria for warranties and require temporary filters/protection and cleaning at substantial completion.

092 Prime Contractor shall finish and install a complete electrical system, including but no limited to basic electrical requirements, wiring, cabling, grounding, bonding, supports, anchors, conduit, boxes, identification, service entrance, transformers, panelboards, motor controls, disconnects, switches, contactors, variable frequency drives, lighting, emergency lighting, any/all temporary requirements, and any/all associated components for complete assembly installations.

093 Prime Contractor shall furnish and install all electronic safety and security including but not limited to rescue assistance communication systems, fire alarm and detection systems, accessories, and any/all components for complete assembly installations.

094 Prime Contractor is responsible for all earthwork, including but not limited to gradings, excavations, subgrade and subbase preparation, erosion and dust control, and any/all components for complete installations.

095 Prime Contractor is responsible for all exterior improvements, including but not limited to pavement construction, sidewalks, concrete paving, asphalt paving, pavement markings, chainlink fences and gates, decorative metal fences and gates, segmented retaining walls, seedings, plantings, landscaping, and any/all components for complete installations.

096 Prime Contractor is responsible for all utilities, including but not limited to testing and quality control, trenching, excavations, backfill, concrete for utilities, water main, sanitary sewer, storm sewer, water distribution, gas line development coordination, stormwater detention chamber, miscellaneous sewer and water constructions, and any/all components for complete installations.

097 Prime Contractor is responsible for any/all dewatering and any/all temporary pipings/routings/connections for sewer systems to complete the work.

098 Prime Contractor is responsible to create and maintain a moisture control plan based upon the materials associated with this project. Prime Contractor shall ensure that materials exposed to or damaged by moisture be replaced immediately. Any/all costs associated with removals/repairs are the Prime Contractors responsibility.

099 Prime Contractor is responsible for all street, sidewalk, and utility improvements as shown on Shive-Hattery Drawings, as per IDOT Standards, City of Moline Standards, and per any supplement information provided within project addendums.

100 Prime Contractor is responsible for all building additions, building renovations, and site improvements as shown on Legat Architects Drawings and per any supplement information provided within project addendums.
