SPECIALTY DOCTOR / GENERAL PRACTITIONER / MEDICAL PRACTITIONERGLENROTHES & CAMERON COMMUNITY HOSPITALSHOURS: FULL OR PART TIME AVAILABLEREF NO: ED457/02/19

We are looking for a Specialty Doctor, General Practitioner or Medical Practitioner to work within 2 of our community hospitals.

Community Hospital based care includes Rehabilitation for frail people,Neurological rehabilitation divided into under 65 years and over 65 years stroke rehabilitation, Palliative Care and Hospital Based Complex Clinical Care (HBCCC)

The care that we provide in community hospitals for adults is evolving. The overriding model of rehabilitation for adults is now provided in community settings through various models such as the Integrated Community Assessment and Support Service (ICASS) and Short Term Assessment and Reablement (STAR).

The rehabilitation wards are for people who require sub acute care following a period of illness or injury. At present many complex frail adults are admitted to the acute hospital when they could have their care needs met within the community hospital setting. The evolving models of care provide an opportunity to develop an alternative to acute admission. Direct admission to rehabilitation wards from areas such as Hospital at Home and Community Health and Wellbeing huddles would ensure that people are cared for in a more appropriate setting, enhancing the opportunities for step up care.

The neurological provision for over 65 years at present is solely for people who have suffered a stroke. People who are over 65 and have other neurological conditions such as traumatic brain injury receive general rehabilitation. We also know that people are waiting in the acute hospital to access stroke rehabilitation.

You will have full GMC registration with a licence to practice. If a GP you will also be on the GMC Register as a qualified GP.
