With stops in five cities nationwide, this comprehensive professional advancement program will be accessible in a city near you.

The Advancement Forum is a three-part educational package designed to provide participants with education and training in core competencies and trends in case management.

PART 1A one-day workshop in one of five cities focused on core competencies and necessary skills in the evolving practice of case management. Here attendees will learn best practices, receive access to tools and gain insight into the future of health care.

PART 2A follow-up webinar with the faculty designed to help attendees implement techniques learned onsite. The webinar also provides attendees with an opportunity to ask questions post-workshop, explore real case scenarios and complete a challenge quiz.

PART 3Hard copy take-home guides distributed to all participants to serve as a reference in daily practice.

New Shop, New Toolbox: 5 Core Competencies and Essential Skills for Case Managers in 2014As provisions of the Affordable Care Act continue to take effect and new programs are implemented, case management will again undergo a transformation. New accountabilities will be in place for health care delivery and transitions of care professionals, which will require new skill sets and core competencies in order to effectively advance the patient throughout the continuum of care. This session will examine 5 core competencies and explore the essentials skills for case management and transitions of care professionals in the coming year. Core competencies include: population health, health literacy, transition planning, predictive modeling and working with community-based organizations (CBOs).

LEARNING OBJECTIVES:

n Identify the new areas of responsibility for health care delivery case management practice related to transitions of care

n Discuss 5 core competencies case management and transitions of care professionals will need in order to manage in the midst of regulatory changes resulting from the Affordable Care Act

n Identify the skills case management professionals will need to develop in order to prepare for practice changes in 2014

From Audit to ZPIC: Regulatory Requirements Every Case Manager Should KnowRegulatory requirements drive many of the case management team’s daily work processes. It is critical that practitioners are well-versed in federal and state regulations and are keenly aware of their impact on transition planning, case management’s role as advocate for both patient and organization, and the case manager’s personal liability. This regulatory primer will provide an overview of the key rules that impact case management practice and offer tips and resources to help practitioners stay ahead of the curve. The speaker will examine the impact of the fiscal year 2014 Hospital Inpatient Prospective Payment System (IPPS) final rule and its two-midnight provision, new Medicare Conditions of Participation regulations related to discharge planning, observation rules and more.

n Provide a high-level overview of the regulatory requirements that impact case managers’ daily work

n Review governmental requirements as they relate to case management

n Provide tips and resources to help case managers stay abreast of current regulatory requirements

Ahead of the Curve: Effective Discharge Planning in the Midst of Change

In light of recent developments in health care, now is the time for hospitals to evaluate discharge planning policies and procedures, incorporate best practices, and update staff competencies. Recent updates to hospital discharge planning guidance from CMS, revised inpatient rules included in the fiscal year 2014 hospital inpatient prospective payment system (IPPS) final rule, and the implementation of certain provisions of the Affordable Care Act all signal the need for case management professionals to ensure their practices are up-to-date. This session will examine these changes and their effect on discharge planning processes. The speaker will provide practical advice and insight into developing effective discharge planning processes and share strategies for measuring department and policy performance to support ongoing improvement and program success.

LEARNING OBJECTIVES:n Understand discharge planning-related requirements found in Medicare’s Conditions of Participationn Educate and empower case management staff to develop and deploy effective and compliant discharge plansn Implement ongoing department performance and policy measurement programs to support ongoing

Career Advancement 101: The What, Why and HowThis session is designed for the up-and-comers, for those who aspire to advance in the ranks of case management. The speaker will examine the case management career path and provide practical advice and recommendations that will allow attendees to set and reach career goals. The speaker will discuss the importance of networking, site visits, industry literature, research, association membership, certification options/industry standards, hospital and non-hospital career options, among other important areas of professional development in case management. The speaker will examine key aspects of aligning one’s goals with organizational priorities, understanding senior leadership perspectives when promoting leaders and the dichotomy of professional goals vs. personal goals.

The Must Haves: Requisite Case Management Leadership Skills and How to Use ThemThis session will explore the key characteristics of effective case management leaders, as well as leadership expectations and responsibilities. The speaker will discuss critical skills such as: interviewing, staff selection, HR processes, time and project management, presenting a business case and more.

n Identify the key characteristics of successful case management leaders

n Learn what skills and knowledge are essential for case management leaders

n Discuss the expectations and responsibilities of case management leaders

Touching Base: Post-Event Webinar and Practical ApplicationUpon completion of the live workshop, attendees will have the opportunity to participate in a webinar with the faculty. A few weeks after the Forum has concluded, attendees will have the ability to submit additional questions and scenarios from their workplace as they relate to the Forum’s topics. During the webinar the speakers will review attendee-submitted scenarios from daily practice and discuss these situations/questions while providing guidance in an open forum setting. In addition, attendees will have the opportunity to complete competency challenge quizzes to test their understanding of the content presented and apply newly acquired leadership skills. Open forum for Q&A will conclude the webinar, providing participants with an opportunity to ask questions specific to their daily practice. The webinar is included as part of the registration fee, and is prescheduled a few weeks after the live Forum.
