The presentation of an advanced stage of skin cancer in an elderly patient poses clinical, ethical, and communication challenges. Surgery is the mainstay of treatment for these cancers, but elderly patients, their families, and their other providers may have unrealistic expectations, being either overly optimistic or overly pessimistic about surgical risk. This panel will help differentiate the elderly patient for whom aggressive surgical management is appropriate from the frail elderly patient for whom surgical management is risky.

This presentation highlights key recommendations from the AAO-HNSF clinical practice guideline on adult sinusitis, published as a supplement to the April 2015 issue of Otolaryngology – Head and Neck Surgery journal. The guideline is an update of the 2004 adult sinusitis guideline developed by the AAO-HNSF. New recommendations include extending watchful waiting as initial management for acute bacterial rhinosinusitis (ABRS), prescribing amoxicillin with or without clavulanate as initial therapy for ABRS, using topical intranasal therapy (saline irrigations, corticosteroids) for chronic rhinosinusitis, and not using topical or systemic antifungal agents for treating chronic rhinosinusitis.

The guideline is an update of the 2004 OME guideline developed by the AAO-HNSF, American Academy of Pediatrics, and the American Academy of Family Physicians. New recommendations are presented for managing OME in children who fail a newborn hearing screen, evaluating at-risk children for OME, and surgical indications for adenoidectomy.
