Your eye care doctor John Doe, MD has made your medical records available online. Through this online Patient Portal you can

also send secure messages directly to your doctor.

You can access your patient portal account online at https://secure.myeyecarerecords.com

Once there, you will need to complete a one time registration in order to confirm your identity and establish your account. Please

register your account by clicking the 'Register' button under First time users. You will need the Initial Password below.

INITIAL PASSWORD: 1234

If your registration is not accepted, your information may have been entered incorrectly by the Doctor's office. Please call your

Doctor's office, have them verify your information, and ask them to resubmit your information to the web portal.

Note: This is a ONE TIME email to inform you of the availability of your records online. This service has not made you part of any

newsletters.
