General

• European Athletics uses IAAF forms

• Must be completed in English

• Important it is completed accurately as it will form part

of the case when pursuing an adverse analytical finding

or other anti-doping- rule violationor other anti-doping- rule violation

• Documentation is the official record of the test

• Ensure the forms are fully completed and legible

General

• European Athletics will send sufficient forms to LOC prior

to the testing

• Testing agencies may not be familiar with the IAAF forms

so before testing commences ensure that you brief how

they should be completedthey should be completed

• Chaperones will be particularly unfamiliar

• Monitor their completion during testing

Member Federation’s subvention

Covered by the athlete/representative. Meeting Organizer is

responsible to inform the athlete about payment modality
