b. Development of teaching techniques 1997 – present
Sharp Needle Regional Anesthesia Demonstration on Volunteers These workshops teach regional anesthesia techniques. Superficial skin is anesthetized with local anesthetic. Needles are inserted to locate with nerves but without injection into the nerves. Nerves are located by electro-stimulation, or ultrasound guidance. Only selected “highly safe” nerves are so demonstrated (e.g. pneumothorax risk “blocks” are avoided). Volunteers are un-sedated and accept deep insertion of needles and perineural catheters. No deep drugs are injected. This live education technique has only ever been done in South Africa and Iowa, USA. Dr. Raw was not the first in the world but did pioneer this in his region together with Dr. Andre Boezaart. Much use is made of hypnotic analgesia. Surface Nerve Anatomy Demonstration Using Doppler Nerve Mapping and Electro-Nerve Mapping This demonstration is to teach regional anesthesia techniques. A volunteer is used who tolerates mild discomfort without any tissue damage. Technique utilizes (i) washable felt-tip pens to mark the

skin; (ii) pen-doppler’s to locate arteries and non-invasive electro-stimulation of superficial peripheral nerves. I believe I was the first in the world to evolve this concept into teaching and “coin” the term “Surface Nerve Mapping” and to pioneer this mode of workshop teaching. Dr. Raw made a serendipitous observation of a colleague managing to accidentally stimulate a nerve without penetrating flesh. Dr. Raw investigated this and explored its possible clinical benefit; presented it first at a conference in 1998 and published it later with a coworker. Subsequently commercially designed nerve stimulators became available.
1997 Porcine Fresh Meat Nerve Block Workshop This technique uses a ‘double loin” cut of fresh pork meat. The butcher’s cut extends from vertebrae T8 to L5 of a freshly slaughtered commercial pork pig. The teaching specimen is obtained from a commercial butcher. It can be used to practice paramedian epidurals, midline epidurals, psoas compartment blocks, and Paravertebral blocks. The model simulates human anatomy very well and has the advantage that the “inside” aspect of the rib-spinal structure can be examined to see the needle and catheter positions.
1998 Animal-based Hands-on regional Anesthesia Workshop Utilizing an anaesthetized pig, a large variety of nerve blocks about the forelimb, hind limb and spinal column can be located repeatedly by students. All nerve and catheterized and position verified by stimulation of the catheter. Dr. Raw did not originate this idea, but has evolved it considerably.
2007 Ultrasound Transducer Handling Gelatine Phantom “Nerve in Tissue” Four combinations of nerve views and needle views are simulated and four transducer movements are tested on each of the four phantoms. After structured teaching a structured assessment with score awarding done. Used on RASCI courses.
2008 Plastic Tissue Phantom Designed for Structured Teaching on the Handling of a Styletted Stimulating Catheter Kit Used on RASCI Courses
2008 Pre-testing Before a Practical and Didactic Regional Anesthesia Course with retesting of the same Questions at Completion of the Course This assesses both the pre-knowledge of the delegates as well as the efficacy of the teaching of the course afterwards. In addition the pre-questions are designed to be fact-retention aids when didactic information is presented, and questions are on specific key points that need emphasis.

The idea of the paper was jointly thought up by me, as the author group periodicall discussed our growing experiences with reactions to the bone cement and we speculated on the etiology of the reactions.
