Final Report on Grid Disturbance at Akola, Akola2 and Indiabulls
(Rattan India) Sub-stations in Maharashtra on 29th March 2015
1.0 Introduction :
The disturbance at Akola and nearby substations in Maharashtra Systemon 29.03.2015 was
discussed in the 124th
, 125th
PCMs held on 21.04.2015 and 05.06.2015 respectively. In the
125th
PCM, it was decided to form a group comprising of members from MSETCL (Shri.
Tijare), WRLDC (Smt. Pushpa Sheshadri), PGCIL (Shri. Anand Dubey & Shri G. Shinde),
APL (Shri. Uday Trivedi), TATA Power (Shri. G.T. Jawale) & WRPC (Shri Satyanarayan S.
& Shri P.D.Lone) to analyze the disturbance in depth, as per preliminary report of WRLDC,
protections at many locations have mal operated/not operated resulting in a wide spread
disturbance.
Accordingly meeting of the group to analyse the disturbance was held on 16.10.2015. The list
of participants is enclosed at Annexure-I.
Based on the discussions & WRLDC preliminary report, following report is prepared.
2.0 Brief on the disturbance:
The single line diagram of the affected area and flows prior to the disturbance is enclosed at
Annexure-II.
400kV Akola2-Taptithanda-I was already in service. 400 kV Akola2 – Taptithanda – II was
open from Taptitanda end and was closed from Akola2 end for trial operation on 29.03.2015
at 13:34hrs. After charging the line at 13:34:45.335 Hrs, ‘PT fuse fail’ alarm appeared.
However the staff of Akola2 S/S didn’t pay required attention to this alarm. The DPS (both
main I & main II) at Akola2 end got blocked due to PT fuse fail. After 30 minutes i.e. at
14:03:56.610 Hrs a fault occurred on the line and was not cleared from Akola2 end due to
blocked DPS. The reason for PT fuse failure was investigated after the occurrence & it was
found that PT connection wires loose at terminal block. Thereafter at 14:04 Hrs, tripping
occurred on many lines of WR due to non-clearance of fault on 400 kV Akola2 – Taptithanda
– II. This event has led to complete blackout at Nandgaon Peth (Indiabulls), 400 kV Bus at
Akola1 and Akola2 sub-station. During this, unit 2, 3 & 5 at Indiabulls (Rattan India)
generating station (Gen: 440 MW) and Unit 8 (Gen: 110 MW) at Koradi – II generating
station tripped on standby E/F protection.
With this event and tripping of lines, power on Wardha –Parli D/C has crossed 850 MW
resulting in SPS-5 operation i.e. Wardha-Parli D/C crossing 850 MW (As 765 kV Wardha-
Aurangabad (PG) 1 & 2 were under planned outage and other line in corridor i.e. Wardha-
Akola-Aurangabad (PG) D/C and Akola2-Taptithand S/C tripped). In addition, the power
from Wardha s/s got routed through Wardha-Seoni-Khandwa-Dhule corridor and
Bhadrawati-Parli and Chandrapur-Parli corridors.
The source of the fault was located on the newly idle charged 400 kV Akola2-Taptithanda II
circuit which was charged from Akola2 at 13:34 Hrs and kept hanging from remote end. The
line was charged based on Maharashtra SLDC confirmation that they were receiving the data
for this line. The circuit was test charged for 15 minutes as per the approval of WRLDC and
it had to be tripped by 13:49 Hrs. However, the same was not followed by the Akola2 sub-
station, which is a MEGPTCL sub-station as well as Maharashtra SLDC who were having the
Data for this line. After 29 minutes of charging of this circuit, B phase to earth fault has
occurred on this line (14:03:55.090), which has to be cleared from Akola2 end distance
protection as lines was only ideal charged from Akola2 end. This fault was not cleared from
Akola2 end relay of the 400 kV Akola2-Taptithanda 2 circuit, which resulted in fault feeding
by various feeders connected with Akola2.
The above incident was serious in view that already 765 kV Wardha-Aurangabad 1 & 2 along
with 765/400 kV ICTs at Aurangabad (PG) were under shutdown. The above incident had not
resulted in any major event due to reliability measures taken for the planned outage of 765
kV lines. The Tirora generation was limited below 1800 MW and Maharashtra drawal was
restricted to 5300 MW along with WR-SR TTC reduction by 200 MW to take care of
contingencies. Further the interstate corridor of Maharashtra i.e. 400 kV Seoni- Khandwa-
Dhule D/C, 400 kV SSP-Dhule, 400 kV Vapi-Boisar, 220 kV Navsari-Nasik and all lines of
Bhilai corridor were kept in service. During the loss of the lines in the Akola –Bhusawal,
Akola2-Taptithanda 1, Akola-Wardha D/C and Koradi2-Indiabulls, the power got routed
through the 400 kV Wardha-Parli D/C and other parallel corridors and interstate corridors
mentioned above. The loading of 400 kV Wardha-Parli D/C has crossed 850 MW each which
has led associated SPS operation at 14:04 Hrs. It was further observed that no generation
backing down as designed in SPS, has occurred either at Tirora or at Mouda generating
station. The load shedding in SR has led to reverse power flow on 765 kV Solapur-Raichur
D/C and thus relieving the line loading on 400 kV lines from Eastern Maharashtra to Western
Maharashtra (load Centers) thus helping in safer operation of the Indian Grid during this
extreme contingency.

3.0 Sequence of events :
Initially the fault was B-Ph fault and another fault have occurred which was Y-phase fault as seen
from the DRs and following protections operated during the fault fed. The relay indications and
Sequence of events is enclosed at Annexure-III & Annexure-IV respectively.
(i) 15:03:56:610 Hrs : 765/400kV, 1500MVA ICT tripped on initiation of 51N function of the
ABB REL670 relay on 400kV side, which is connected to neutral CT of ICT with a setting of
20% of FLC (2165 A) & time delay of 1.5 secs.
(ii) 14:03:57:684 Hrs : 400/220kV, 315MVA ICT at 400kV Akola1 tripped from LV side on Dir.
E/F.
(iii) At 14:03:58.250 Hrs, 400 kV Akola1 – Wadha – II tripped in Zone-2 at 400kV Wardha S/S
and DT received at Akola 2.
(iv) 14:04:09:058 Hrs : 400kV Nandgaon Peth –Akola1 tripped on O/V
(v) 14:04:09:064 Hrs : 400kV Wardha-Akola1 – I tripped on Zone-II DPS at Wardha end.
(vi) 14:04:09:069 Hrs : 400kV Wardha-Akola1 – II tripped on O/V at Akola1 end
(vii) 14:04:09:073 Hrs : 400kV Akola1-Augangabad(PG) – I tripped on O/V at Akola1 end.
(viii) 14:04:09:073 Hrs : 400kV Akola1- Akola2 – II tripped on Z-III(L2-L3) at Akola1 end.
(ix) 14:04:09:074 Hrs : 400kV Akola1- Akola2 – I tripped on O/V at Akola2 end.

4.0 Event Analysis:
To analyze the disturbance the data was collected and the DRs/ELs triggered/logged were co-
related. MSETCL Akola (TCC) gave a well co related presentation on the disturbance and is
enclosed at Annexure-V. The relevant DRs is enclosed at Annexure-VI.
Tripping of the lines/ICTs from remote ends was analyzed in the meeting and the
observations of the group is as follows :
The fault current recorded due to initiation of stub protection pick up of 400 kV Akola2-
Taptithanda II line at Akola2 end was around 3.448kA. The fault current contribution by
765/400kV ICT at Akola-II was seen to be around 1.635kA. The remaining contribution of
around 1.813kA was contributed by : 400kV Akola1- Akola2 D/C, 400kV Nandgaon Peth –
Akola2 D/C and 400kV Taptitanda - Akola2 – I.
The fault (Y-phase jumper flash over) on 400 kV Akola2-Taptithanda II was located at a
distance of 147Km from Akola2 end during patrolling of the line. However DRs initially
show B-phase fault. Therefore two faults occurred, initially the fault was B-phase fault and
later on second fault(Y-Ph) occurred.
(i) 400 kV Akola2- Taptithanda II line should have tripped from Akola2 end in Z-I DPS.
However the Main-I & Main –II relays were already blocked due to VT fuse failure.-
MEGPTCL have already admitted that on receipt of VT fuse failure and blocking of Min-I &
Main-II protection the S/S staff should have handtripped the line. The cause for VT fuse
failure was loose connections in both the terminal blocks of the PTs. The DRs show that the
fault was on B phase, initially. The non clearance of fault by primary protection of this line
led to clearing of fault from remote ends as follows.
(ii) Tripping of 765/400kV, 1500MVA ICT on initiation of 51N function of the ABB REL670
relay on 400kV side : Since the sensitive E/F protection was non directional with DMT, it
operated. The setting adopted is 20% of FLC (2165 A) & time delay of 1.5 secs.
(iii) Tripping of 400/220kV, 315MVA ICT at 400kV Akola1 tripped from LV side on Dir. E/F
was due to fault fed for the set time delay as per the IDMT time characteristics.
(iv) Since the 765/400kV ICT was the major in-feeding source for the fault, the other lines could
not sense the fault (due to infeed effect of approximately 2x impedance of the fault section on
400 kV Akola2- Taptithanda II was seen till the ICT tripped).
(v) Tripping of 400 kV Akola1 – Wadha – II in Zone-2 at 400kV Wardha S/S and DT received at
Akola 2 : The fault feeding path for this circuit is 400kV Wadha –Akola1(DC)– Akola2
(DCs). The line lengths of 400 kV Akola1 – Wadha is 162Km and line lengths of 400kV
Akola1-Akola2 is 30Km. The setting for Z-II 400kV Wadha –Akola1 lines is 150% of the
line length(i.e.243Km). The setting for Z-II 400kV Akola2 –Akola1 lines is 150% of the line
length (i.e.45Km). Therefore 400kV Akola1-Akola2 could not sense the fault in Z-II,
However as per settings it was within Z-II reach of 400kV Wadha –Akola1. The 400kV
Wadha –Akola1 lines could not trip ahead of ICTs was due to the fact that there was major
contribution of fault current from ICTs, resulting in high in-feed effect for 400kV Wadha –
Akola1 lines before tripping of ICTs. However the 400kV Wadha –Akola1 lines tripped in Z-
II after tripping of ICTs(i.e. reduction in in-feed fault current). This suggests that the fault was
between 8Km to 66Km from Akola2end.
The DT in Z-II on 400 kV Akola1 – Wadha – II was inadvertently wired up and hence
tripping of this line at Akola1 on DT receipt occurred, which has now been disabled.
(vi) Tripping of 400kV Wardha-Akola1 – I in Zone-II DPS at Wardha end –same as described at
Sr.(iv)..
(vii) Tripping of 400kV Nandgaon Peth –Akola1 on O/V : After loss of major fault feeding lines,
O/V might have occurred.
(viii) 14:04:09:069 Hrs : 400kV Wardha-Akola1 – II tripped on O/V at Akola1 end : After loss of
major fault feeding lines, O/V might have occurred.
(ix) 14:04:09:073 Hrs : 400kV Akola1-Augangabad(PG) – I tripped on O/V at Akola1 end.: After
loss of major fault feeding lines, O/V might have occurred.
(x) 14:04:09:073 Hrs : 400kV Akola1- Akola2 – II tripped on Z-III(L2-L3) at Akola1 end : : The
fault feeding path for this circuit is 400kV Akola1– Akola2 (DCs). The line length of 400 kV
Akola1 – Akola2 is 30Km and the fault was between 8-66Kms. The setting for Z-II 400kV
400kV Akola1– Akola2 (DCs) lines is 150% of the line length(i.e.45Km).. Therefore 400kV
Akola1-Akola2 could not sense the fault in Z-II since the fault was 8-66Km from Akola2 end
and due to DC the faulty section impedance seen by the relays was 2xfault impedance after
the tripping of 400kV Wadha –Akola1 DCs. Therefore one of the 400kV Akola1- Akola2
lines tripped in Zone-III due to racing around and thereafter the other circuit tripped on O/V.
(xi) 14:04:09:074 Hrs : 400kV Akola1- Akola2 – I tripped on O/V at Akola2 end : After loss of
major fault feeding lines, O/V might have occurred.
(xii) 14:04:15:694 Hrs : 400kV Akola1-Augangabad(PG) – II tripped on O/V at Akola1 end :
After loss of major fault feeding lines, O/V might have occurred.
(xiii) 14:04:15 Hrs : 400kV Akola1-Bhusawal tripped on Z-III(Y-B) at Bhusawal end : : The
fault feeding path for this circuit was 400kV Bhusawal - Akola1(SC) - Akola2 (DC). The line
length of 400 kV Bhusawal- Akola1 is 180Km and line length of 400kV Akola1-Akola2 is
30Km. The setting for Z-III 400kV Bhusawal- Akola1 line is 100% of Bhusawal- Akola1 and
100% of Akola1-Nanedgaon peth (longest line from remote bus) which is 288Km and
therefore tripping of this line resulted after loss of major contributing in feeds.
(xiv) After 14:04:15 Hrs: 400kV Koradi-Nanedgaon peth : 400kV Koradi-Nanedgaon peth line
length(i.e.158Km). The setting for Z-III 400kV Koradi-Nanedgaon peth line is 100% of
Koradi-Nanedgaon peth(158Km) and 100% of Akola1-Nanedgaon peth (longest line from
remote bus) which is 262Km and therefore tripping of this line resulted after loss of major
contributing in feeds.
(xv) Once the primary protection had failed to isolate the fault, protections at remote end
operated and the reaches sensed by these relays were telescopic or reverse telescopic.
This might seem surprising at the first look, but were by and large in order.
5.0 Change in the voltage during the event can be seen in below figures:
5.1 Load/Generation Loss: Generation of Around 550 MW was lost with tripping of Units
of Koradi-8 and Indiabulls 2, 3 & 5. There was no direct load loss, due to tripping of above
ICT’s or lines. However due to tripping of Akola 315MVA ICT, 220 kV network at Akola
was over loaded (Paras unit-3 &4 were out prior to occurrence). Hence, planned load
shedding from A to G groups and distress load shedding was carried out in Buldhana and
Akola district. Approximate 80 MW Load was affected due this. Load shedding was
withdrawn at 15.50 Hrs after charging of Akola ICT.
5.2 Frequency Change: Due to loss of generation frequency dropped by 0.0564 Hz due to
loss of generation as shown in below figure.

6.0 Conclusions & Recommendations :
Human Factor at 765/400 kV Akola2 Sub-station
After analyzing the disturbance, it is observed that one the major factor which led to the
event was the human error at Akola2 Sub-station. The event logger has clearly shown the
VT fuse failure alarm immediately after the ideal charging of 400 kV Akola2-Taptithanda
2 circuit. The VT connections were loose as admitted by M/s MEGPTCL, the voltages
displayed on the control panel & the alarm itself would have been taken seriously and the
line should have been immediately hand tripped rather than trying to search out for the
cause. Moreover this was idle charged and could have been tripped easily.
The Group recommends that the well known protection principle of tripping a line once
both DPSs are blocked and the cause for blocking the DPS could not be fixed within
shortest possible time, should be strictly followed. Otherwise operation of protections
from remote ends would disrupt larger areas and their operation can be bit surprising at
times. This issue may be discussed in OCC and suitable policy adopted.
Other protection issues :
1. The commissioning should have been done with proper testing and checking of all the
wiring and logic prior to charging of the lines at 765/400 kV Akola2 Sub-station.
Proper testing to be done for all the connection prior to charging of new lines. All
utility must do a proper check of all the connections and verify the connections using
injection kit.
2. The system near about Akola (Akola, Wardha, Koradi, Chandrapur, Khaparkheda &
Aurangabad) is complex and have many complexities as regards to Main/Backup
protection settings. A coordinated setting of Main/backup relays at the complex of
Akola is required to be arrived at, considering the In-feed/Out-feed effect by
simulating faults at various locations. Hence the traditional/Ramkrishna Committee
recommendations would be required to deviate.
PGCIL, Maharashtra STU, M/s MEGPTCL and indiabulls representatives in co
ordination with WRLDC may sit together and decide proper settings.
3. Utility intending to connect new elements to either STU network/CTU network

4. WRLDC/SLDC instruction to be strictly adhered while carrying out any work in the
Interconnected Grid. Any online relay work if need to be performed should be first
informed to respective SLDC or RLDC for permission.
Overvoltage grading of Akola and nearby stations is required to be done.
WRLDC instruction :
The line was charged based on the confirmation of details from Maharashtra SLDC that
they were receiving the online data of this line at their SCADA system. In view of critical
system conditions, WRLDC have permitted test charging of the line for 15mins only.
After 15 minutes of the line charging, the line should have been switched off.
Unfortunately the fault occurred on the line just after 15mins.Therefore in case of any
issue this should have also been reported to WRLDC as the data of this line was not
available at WRLDC end. Also WRLDC may review its policy, as to why test charging of
lines for such a small durations are permitted. Policy may be discussed in OCC.
Non-Availability Data of Maharashtra System at WRLDC SCADA
The most of the Maharashtra system real-time data from Maharashtra system was not
available at WRLDC, which makes system monitoring in Maharashtra at WRLDC very
difficult. WRLDC has to rely completely on Maharashtra SLDC.

• M/s MEGPTCL reported that Y ph subconductor of the line got loosened and touched the tower member at loc. 327 at 147 km.
