Annual Achievement Report Form
What?
The Annual Achievement Report Form is a form that each club
MUST fill out prior to this year’s District Convention (April 5th-
7th). This form highlights all club achievements and covers areas
such as membership, service projects and overall performance.
Why?
Failure to submit this form on time disqualifies a club from all Dis-
trict Convention awards and scholarships. The form helps clubs de-
termine their own strengths and weaknesses and is required by Key
Club International as a way of assessing clubs.
How?
Simply fill out the form electronically or manually by District Con-
vention on April 5th-7th and then submit it to me!
