PositionDescription:TheNursewillprovideperson-centeredsupporttoolderadultswithdementiawhileengagingthemininteractivelifeenrichmentactivities.UnderthedirectionofJamaicanCanadianAssociationNurses(JCAN)andincollaborationwithTAIBUCommunityHealthCentre(CHC),thispositionwilldirectlyreporttothePresidentofJCAN.TheprogramwilloperateatTAIBUCHC.TransportationsupportwillbemadeavailabletoprogramparticipantsthroughtheUbuntuVillagebus.DutiesandResponsibilities:

• Coordinateprogramsandservices• Provideinitial&ongoingassessmenttodetermineparticipantneeds• DevelopnursingcomponentofIndividualPlanofCareandsuperviseprovisionoftheservices• Documentallprogrammingprovideddailyincludingquarterlysigned/datedprogressreports• ProvideeducationandcounselingtoparticipantsandfamiliesasindicatedinIndividualPlan

ofCare• AssistparticipantswithADL’sand/orIADL’sasneeded• Maintaincurrentandaccuratewrittencasemanagementrecords,preparevarietyofwritten

reportsandcorrespondence• Participateindailyactivitiesofthecenterincludingassistingasneededwithmealservice,

clean-up,etc.• Performrelatedtasksasassigned• Maintainconfidentialityofclientandorganization’sinformation

Qualifications:• NursingDiplomaordegreefromarecognizedcollegeoruniversity• CurrentregistrationwiththeCNOandFirstAid/CPR

Asset:

• Atleastoneyearofexperienceworkingwithpeoplewithdementiaordevelopmentaldisabilities

• Behaviormanagementtechniques• Dayprogramfacilitationandinstruction•
