Diagnosing Team Failure When teams fail to perform as expected, an attempt is usually made to

pinpoint and blame the people responsible. An ineffective team or team failure may at times be nothing more than just a

consequence of the wrong combination of people. However, the answer doesn't always lie in replacing the team but in

answering the question, ‘What went wrong with the group as a whole & why?
