CCCWN Mission/Vision• Mission: To provide leadership to enhance the health of

communities through development and support for collaborative regional partnerships in Benton, Lincoln and Linn Counties.

• Vision: The Coast to the Cascades Community Wellness Network leads and sustains a system of partnerships of agencies and organizations working together to provide integrated services and programs to promote individual and community health.

The CCCWN has been successful in applying for and receiving local, state and federal funds to implement strategies that improve health.

Benton County Oral Health Coalition• Benton County Oral Health Coalition was formed in

2007 • Conducted a Needs Assessment in 2011• Reviewed/Prioritized/Implemented Prevention Services• Focus on Children• Boys & Girls Club of Corvallis – Johnson Dental Clinic• Collaboration with local agencies to provide adult

emergency treatment services - 2012• 14 Dentists participate• Average patients per year – 110• Emergency Department/Urgent Care use decreased

Linn County Oral Health Coalition• Linn County Oral Health Coalition formed in 2010• Conducted a Needs Assessment in 2011• Reviewed/Prioritized Community Needs• Focused on uninsured/underinsured adults• Collaborated with local agencies to provide adult

emergency treatment services with vouchers – 2013• Target east Linn County• 6 Dentists participated – Currently 3 Dentists• Average patients per year - 48• Emergency Department/Urgent Care use decreased

Lincoln County Oral Health Coalition• Lincoln County Oral Health Coalition formed in 2012• Conducted a Needs Assessment in 2012• Loss staffing in 2012• Reconvened in 2013• Reviewed goals/objectives/priorities• Identified prevention and treatment services• Provided only dental van services for uninsured and
