How to successfully submit a training request

Start by clicking here. Or go to http://training/

Click on your name.

Click on Request Training.

FYI: This screen, your TRAIN Homepage, provides you a look at your training request history and status of each request.

Note: Your personal information is already populated into your training request!

Required fields are in

BLUE.

Don’t know your Index/PCA number?

To find it, click on either of these links, orYou can also find it at:

How to get your Index/PCA code:

1. Click on FDD Consolidation tab

2. Find your Region/Unit

3. Look in the “New Coding” section, if there’s nothing there, use “Previous Coding”

Category of Training

Course Title/Number

Enter the Class/Course name.

Course Date

Use the drop down calendars to selectthe first and last dates of training.

Type of Training

If the class is being given by a State Agency, select

In-Service. Otherwise, select

Out-Service.

Total Training Time In Hours

State: Number of hours (whole numbers, no decimals) you’ll attend training on

State time.If none, enter “0”.

Employee (non-State time):

Number of hours (whole numbers, no decimals) you’ll attend training on

your own time.If none, enter “0”.

Organization Providing Training

Select the training vendor/provider from the drop-down list. If they’re not listed,

select “Other” from the top of the list and enter name of vendor.

Location of Training

Address where training is being held.

Registration Fee/Tuition

Enter the cost/price of training class.

If there is no cost, enter “0”.

Book Fee

Enter the cost for books.If none, enter “0”.

Employee Contribution

Enter the dollar amount that you are contributing,

out-of-pocket, towards the cost of the class/books. (See General Training

Information).

Check Request

▪Select “Yes” only if the Organization Providing Training requires a check prior to training, and unwilling to invoice. ▪ Otherwise, select “No.”

Justification

Give your reasons why you want to attend this class.

… or to submit, select your supervisor’s name from the

drop-down and click Submit.

If the submit does not go through, the

missing input will be in RED.

Attach Supporting Document

Registration form and Course Outline are required for approval. Click on “Browse” to locate your

supporting document (Word or PDF only) and then click on

“Attach File”.

If no supporting document is needed,

click on“No Attachment”.

VERY IMPORTANT:Make sure that you get this message to

ensure that your submission was successful!

***PLEASE NOTE***

After you have attended training, you must complete a training evaluation. Go to your TRAIN

homepage and click on To evaluate.
