Authority/AdoptionThisSchoolofTheatre+Dance(SoTD)EmergencyPlandescribestheproceduresandorganizationalstructurebywhichtheunitwillrespondtoanactualorpotentialemergencywhichoccursattheUniversityofFlorida(UF)andimpactstheunit.ThisdocumenthasbeendevelopedinsupportoftheUFComprehensiveEmergencyManagementPlan(CEMP),andspecificallyaddressestheoperations,functions,androlesandresponsibilitiesthatSoTDwillhaveintheresponsetoandrecoveryfromanemergency.ThissignatureacknowledgestheacceptanceofthisPlanastheprimaryemergencyplanfortheunit,supersedingpreviousemergencyplans.Uponsigningthisletter,thedesignatedauthoritiesagreeto:

I. Introduction

A. PurposeThisEmergencyPlaniscongruentwiththeUniversityofFlorida(UF)ComprehensiveEmergencyManagementPlan(CEMP)whichdetailstheUniversity’sapproachtocoordinatingemergencies.TheSchoolofTheatre+Dance(SoTD)hasakeyroleinsupportingactivitiesinresponsetoandrecoveryfromanemergencythataffectsUFandSoTD.ThisdocumentestablishesactionstakenbySoTDinsupportofUniversity-wideoperationsdescribedintheCEMP.UFispotentiallyatriskfromvarioushazardsandthisPlansupportstheUniversity’soverallgoaltoprotectlife,property,andtheenvironment.DuetothevaryingnatureofemergenciesthatmayimpacttheUniversityanditsunits,thisPlanisintendedtoserveasaflexibleguidetomeettheunit’sneedsasdictatedbytheemergency.

B. ScopeThisPlanprovidesanoverviewoftheemergencymanagementsystemandorganizationalstructureforSoTDandtheUniversity.Thedocumentoutlinesthecoordination,management,andgeneralconceptofoperationswithintheunitandincludesadditionalemergencymanagement-relateddocumentsaswellasreferencestopoliciesandprocedures.ThisPlanappliestoSoTDandallstudents,faculty,staff,andexternalpartnersworkingwithintheunit.ThisPlanmaybeactivatedinsupportoftheUniversityCEMPorinresponsetoanemergencyimpactingSoTD.ThedocumentsupportsactivitiesdescribedintheCEMPbutdoesnotsupersedeit.

C. AuthoritiesTheSoTDDirectorhasapprovedthisEmergencyPlan,establishingSoTD’sprogramforemergencyoperations.AdditionalauthorityforemergencymanagementatUFisderivedfromavarietyofsourceslistedintheUniversityCEMP,includingUniversityplansandpoliciesaswellasfederalandstateauthorities.

II. PlanningAssumptions

ThefollowingplanningassumptionscanbemaderegardingemergencieswhichmayimpacttheUniversity.

A. UniversityAssumptions

• Anemergencycanoccuratanytimeoftheday,night,weekend,holiday,withlittletonowarning.

• UFwillrespondtoalarge-scaleemergencyfromtheUniversitylevelusingtheUniversityCEMPasguidance.

• Thesuccessionofeventsinanemergencyisnotalwayspredictable;therefore,plansandprocedures,suchasthisdocument,serveasaguide,andmayrequireflexibilityinordertomeetthespecificsoftheemergency.

• Whenanemergencyinvolvesmultipledepartments,operationsmightbecoordinatedthroughtheUFEmergencyOperationsTeam(EOT).

• AnemergencywhichimpactsUFmayalsoaffectthesurroundingcommunityandregion,limitingaccesstoandavailabilityofexternalresources.

• Extendedemergenciesmayrequire24-houroperations,andstaffingshouldbeplannedaccordingly.

B. UnitAssumptions

• Unitsareexpectedtorespondtoanemergencybyemployingunit-levelemergency

plans.• EachUFunitshouldbefamiliarwiththeUniversityCEMP,itsrespectiveEmergency

Plan,andspecificresponsibilitiesassignedtoitwithinthesedocuments.• LifesafetyisthefirstpriorityatUF.Iflifesafetyisatriskfollowinganemergency,

facultyandstaffmightnotbeabletoaccessUniversityfacilitiesincludingbuildings,offices,orlaboratoriesduringorafteranemergency.

• PersonnelfromSoTDwillbeavailableforUFofficialsinanemergencytoprovideinformationspecifictotheirunitincludingdetailsregardingoperationsandfacilitiesaffectedbytheemergency.

III. ConceptofOperations

TheConceptofOperationsprovidesguidelinesforrespondingandrecoveringfromemergenciesattheunitlevel.UnitsshouldreferencetheUniversityCEMPforafulldescriptionofemergencyresponseandrecoveryoperationsattheUniversitylevel.UnitsandtheirpersonnelmighthaveadditionalresponsibilitiesintheUniversityCEMPtosupportaUniversity-wideemergencyresponse.AnydepartmentresponsibilitiesoutlinedintheUniversityCEMPshouldbeperformedinconjunctionwithresponseactivitiesattheindividualunitlevel,usingthefollowingasaguideforprioritizingresponseactions:

A. University-WideEmergency

1. MonitoringThreatsandEmergencies

TheUFPoliceDepartment,theDepartmentofEmergencyManagementandEnvironmentalHealth&Safetyhavetheprimaryresponsibilityformonitoringthreatsandemergenciesandwilldisseminateappropriateinformationregardingactualorpotentialemergenciesandassociatedactionsasnecessary.

2. ActivationofUFComprehensiveEmergencyManagementPlanTheUniversityCEMPcanbeactivatedtoaddressoneormoreofthefollowingcircumstances:• Basedoninformationobtainedfromappropriateauthorities,Universityofficials

mightinitiallyactivateportionsofUniversityCEMPtocoordinatetheemergency.

• TheAssistantVicePresidentofPublicandEnvironmentalSafetymightactivatetheUniversityCEMPinresponsetoinformationregardinganactualorpotentialemergency.

• Incoordinationwithadeclaredlocal,state,orFederalemergency,theAssistantVicePresidentofPublicandEnvironmentalSafetymightactivatetheUniversityCEMP.

3. UFEmergencyOperationsCenterIfanemergencyaffectingUFrequirescoordinationattheUniversitylevel,UFmayactivateitsEmergencyOperationsCenter(EOC),whichservesastheUniversity’scentralpointofcoordinationforemergencyoperations.UnitsmayneedtocommunicatewiththeUniversityEOCinorderto:• AssistinsupportingtheUniversity’soverallemergencyresponseandrecovery,• Receiveupdatesonthestatusofemergencyoperations,• Coordinateand/orobtainassistancemanagingtheemergencyontheunitlevel,

and• Requestadditionalresourcestominimizedamageandimpactcausedbythe

emergency.

IftheUFEOChasbeenactivated,aunitmaycalltheUFPoliceDepartmentmainnumberat352-392-1111tocontacttotheUniversityEOC.

B. Unit-LevelEmergency

1. ReportingEmergenciesAllUFstudents,facultyandstaffhavearesponsibilitytoreportanyemergencywhichposesanimmediatethreattolifesafetybycalling911.Once911orotherappropriateauthoritieshavebeennotified,SoTDshouldfollowtheproceduresforspecifichazardslistedinAppendixA,asappropriate.Ifastudent,facultyorstaffinaunitreportsanemergencythatmightreduceorimpedeunitfunctions,theSoTDDirector,alongwiththeotherappropriateemergencycontactsasdescribedbelow,shouldalsobenotified.Iftheemergencyimpactstheunits’facilities,theappropriateworkmanagementcentershouldadditionallybenotified.Ifabuildinghasbeensignificantlydamagedorimpacted,UFPoliceDepartmentand/orEnvironmentalHealth&Safetywilldetermineifthestructureissafetooccupy.Iftheincidentinvolveshazardousmaterials,butdoesnotposeanimmediatethreattolifesafety,theUFPoliceDepartmentshouldbecalledat352-392-1111.ThePolicedispatcherwillcontacttheappropriateUniversitydepartments,includingEnvironmentalHealth&Safetyandlocalemergencyrespondersasnecessary.SoTDwillsupportEnvironmentalHealth&Safetyinprovidinginformationregardingmaterialsinvolved.Iftheemergencyincidentrendersacademicorclassroomspaceunusable,SoTDwillcontacttheOfficeoftheUniversityRegistrarat352-392-1374.

2. SchoolofTheatre+DanceEmergencyPlanActivationTheSoTDDirectorordesigneemayactivateportionsoralloftheDepartmentEmergencyPlantotheextentnecessarytocoordinatetheemergencyresponseandrecoveryoperations.IfallorpartofthePlanisactivated,appropriatepersonnelwithintheunitrequiredtoimplementactionsdescribedinthePlanshouldbenotified.RefertoSectionV.RolesandResponsibilitiesfordescriptionsofspecificrolesandresponsibilitiesinanemergency.

Essentialunitemployees(asdesignatedbytheSoTDDirector)mightberequiredtoassistinimplementingtheEmergencyPlan.Identifiedemployeesshouldtakepreparatoryarrangementstofacilitatetheirabilitytoreporttoworkasneededduringemergencyoperations.Thesepersonalpreparednessmeasuresincludeplanningforfamily,petandotherresponsibilities.

3. EmergencyResourceRequestsIfSoTDrequiresUniversityresourcesforemergencyresponseorrecoveryandtheUniversityEOChasnotbeenactivated,theSoTDDirector’sdesigneewillcontacttheappropriateUFdepartmentforassistance,suchasUFPoliceDepartmentandEnvironmentalHealth&Safety,basedonthenatureoftheneed.IftheUniversityEOChasbeenactivated,theSoTDDirectorordesigneewillcontacttheEOCwithrequestsforresourcesbycallingtheUFPoliceDepartmentmainnumberat352-392-1111.

4. EmployeeAccountabilitySoTDshouldaccountforemployeesfollowinganemergencyattheworkunitlevel.

DuringabuildingevacuationtheBuildingEmergencyCoordinators(BEC)anddesigneeswilloverseeaccountingforemployees.TheBEC’sanddesigneeswillutilizetheEmployeeAccountabilityChecklist(includedasAttachmentLofthisdocument).TheSoTDHealth&SafetyCommitteewithassistancefromtheAssistanttotheDirectorwillupdatethechecklistatthebeginningofeachsemester.

Employeeswillbeaccountedforviacalllistfornon-evacuationemergencies.TheSoTDHealth&SafetyCommitteewillmaintainthecalllistwithassistancefromtheAssistanttotheDirector.TheSoTDDirector,AssistanttotheDirector,BuildingEmergencyCoordinators,andHealth&SafetyCommitteedesigneeshaveaccesstothecalllist.ThedesignatedmeetingplaceforevacuatingtheMcGuirePavilionisthegrassyareasoutheastofthebuildingcrossingMcCartyDrive(directlyacrossthestreetfromG6&G10Studios).Evacueesshouldutilizethecrosswalkwhenpossibleandheadtowardthepicnictableareatoremain100’fromthebuilding.Seeimageonpage12.

C. RecoveryTherecoveryprocessfollowstheinitialemergencyresponsephase.Intheeventofalarge-scalerecoveryeffort,additionalassistancemightberequestedbytheUniversityfromlocal,stateandfederalemergencymanagementagencies.FordetailedinformationregardingtheUniversity’srecoveryplans,protocols,andprocedures,refertotheUniversityCEMP.UnitsaffectedbytheemergencyshouldcoordinatewiththeUniversityEOCtodetermineappropriaterecoveryactivitiesincludingdamageassessment,alternatefacilitiesandresumptionofnormaloperations.1. DamageAssessmentandReimbursement

Whendeemedsafebyapublicsafetyauthority,unitsshouldconductapreliminarydamage assessment of their buildings. The appropriate external and internalbuilding assessment forms should be completed. If UF’s damage assessmentwebsiteisfunctional,enterinformationintothesystem.Ifnotfunctional,theformsshouldbefaxedordeliveredtothebuilding’sappropriateworkmanagementcenter.(DamageAssessmentformisincludedasAttachmentEofthisdocument.)Departments shouldnot go into buildings that are suspected or determined to beunsafe. Any dangerous conditions or issues requiring urgent response should be

reported immediately to the appropriate work management center or UF PoliceDepartment.UFDamageAssessmentWebsitehttps://emergency.ufl.edu/programs/damage-assessment-program/(LoginusingGatorLinkIDandpasswordwhenprompted)It is important for units to document damage, including photographs, in order torecoveranypotentialinsuranceorFEMAreimbursement.Anotationofthelocation,includingbuildingandroomnumber,shouldbeincludedineachphotograph.(Thiscan be accomplished by including a hand-written sign in the picture or laterelectronicediting.)Units should take safe, protective actions thatmitigate or reduce hazards in theirbuildings and areas. (Example: Department placing barricade tape around adangerousareainabuildingtopreventpeoplefromenteringthearea.)Employeesshouldnottakeanyactionswhichplacethemselvesorothersindanger.

2. ContinuityofOperationsIftheemergencycausesdisruptionsinoperations,damagetofacilitiesorimpactstoemployees,theContinuityofOperations(COOP)planmayneedtobeimplemented.ACOOPaddressesthecontinuanceofaunit’sessentialfunctionsdespitedisruptionsfromanemergency.SoTD’sCOOPPlanislocatedinAppendixCcurrentlyunderdevelopment.

3. ResumptionofNormalBusinessOperationsOncerecoveryeffortsarecomplete,theUniversitywillresumenormalbusinessandacademicoperations.Insomecases,thismighttakeseveraldays,weeks,orevenmonthsandmayrequiretherelocationofeffectedoperationsdependingontheimpactandseverityoftheemergency.TheSoTDDirectorordesigneewillcoordinatewiththeUniversityEOC,UniversityadministrationaswellaswithSoTDpersonnel,onthereturntonormaloperations.

IV. EmergencyNotificationandCommunications

A. EmergencyNotification

TheUniversityemploysamulti-modalapproachtoemergencynotifications,termedUFAlert,tonotifytheUniversitycommunityofemergencies.ThesystememploysSMS/textmessaging,email,VoIPtelephones/speakers,socialmedia,webpostingsontheUFhomepageandothermethods.AllUniversitystudents,faculty,andstaffareresponsibleformaintainingtheircurrentemergencycontactinformationinmyUFLinordertoreceivetimelyemergencynotifications.SoTDwillencouragefacultyandstafftoparticipateintheUFAlertsystems.

2. DepartmentNotificationSoTDmaintainsaninternalnotificationsystem(s)includingmassemaildistributionandacontactlistofemployeeswithemergencyroles(seeAttachmentJ).Additionally,SoTDmaintainsacontactlistofvendors/partners(seeAttachmentK).College/Departmentalnotificationsmayincludespecificdetailsandinstructionsfortheunitincludingimpactedfacilities,alteredworklocationsandappropriateemployeeactions.IfinformationneedstobecommunicatedbeyondtheunittotheUniversitycommunityandpublic,UniversityRelationsshouldbecontactedforcoordinationandassistance.

B. EmergencyCommunications

1. UniversityCommunications

UniversityRelationsservesasthecentralpointforUniversitycommunicationsduringanemergency.AllpublicinformationandcommunicationshouldbecoordinatedthroughUniversityRelationswithsupportprovidedbySoTDcommunicationspersonnel.

2. DepartmentCommunicationsIftheunitisaffectedbytheemergency,SoTDisresponsibleformaintainingcommunicationsbetweentheDepartmentandtheUniversityEOCtoensurethattheoverallimmediateneedsoftheUniversityarebeingmet.

A.08Injury/MedicalAssistanceIntheeventofamedicalemergency,call911.Afterensuringthatanemployeereceivesemergencymedicalcare,AmeriSys,theStateofFlorida’smedicalcasemanagementvendor,shouldalsobenotifiedofanywork-relatedmedicalemergencyassoonaspossiblebycalling1-800-455-2079.Ifanon-lifethreateningon-the-jobinjuryoccurs:

1. The injured employee and supervisor should immediately contact AmeriSys by calling 1-800-455-2079.Treatment foranynon-life threateningwork-related injuryor illnessmustbeauthorizedbyAmeriSyspriortoobtainingmedicaltreatment.

2. AmeriSys will determine if medical treatment is necessary, and assist the employee andsupervisor in completing the First Report of Injury or Illness Form.
