The past fiscal year caps an extraor-dinarily active and innovative decade inthe history of the Library. Highlights ofthe Library's accomplishments includeddevelopment of MEDLARS (Medical Lit-erature Analysis and Retrieval System);passage of the Medical Library Assist-ance Act of 1965 (which, in the past fiveyears, has authorized the appropriation ofapproximately $1O5 million for improve-ment of the Nation's health science li-braries); and creation in 1968 of the ListerHill National Center for Biomedical Com-munications, in which are focused nu-merous technical programs designed tospeed the flow of biomedical information.Other developments which have ex-panded the Library's role as an activeinformation center included the additionof an Office of Specialized InformationServices, which manages the ToxicologyInformation Program, and the integrationwith the National Library of Medicine ofthe National Medical Audiovisual Centerin Atlanta, which serves as a central re-source for the collection, cataloging, anddistribution of biomedical audiovisualmaterials.

Fiscal Year 197O contained its ownshare of important events, many of themlinked directly to programs and servicesbegun in the past decade. These included:1) a new MEDLARS publication. AbridgedIndex Medicus. designed to serve thesmaller hospital medical libraries and in-dividual practitioners; 2) designation,under authority of the Medical Library

Assistance Act of 1965, of the last twoof a total of eleven libraries comprisingthe Regional Medical Library network; 3)passage of legislation extending for threeyears the authority of the Medical Li-brary Assistance Act of 1965; 4) furtherdevelopment of MEDLARS II, which inFiscal Year 1971 will provide more so-phisticated information storage and re-trieval capabilities as it replacesMEDLARS I; 5) establishment of MED-LARS centers in Germany and at theWorld Health Organization (WHO) inGeneva, Switzerland; 6) publication of ADirectory of Information Resources in theUnited States: General Toxicology; and7) compilation of a computerizecf Inter-natipnal Index of Medical Film Data by theNational Medical Audiovisual Center. Thereports below expand on these and manyother developments during the fiscalyear.

As you examine this report, we hopethat you will sense the context in whichthe year's events took place: they arebased largely on past accomplishmentsas well as on the adaptation of new tech-nologies for improved communicationsservices to members of all the healthprofessions. I hope especially that thereport will convey something of the Li-brary's total commitment to improvedservice through the creation of a modernbiomedical library system which is, to thehighest possible degree, sensitive and re-sponsive to the information needs of thebiomedical community.

OFFICE OF THE DIRECTOR

In the task of administering and co-ordinating the programs of the Library,the Director is aided by his immediatestaff, a Special Assistant for InternationalPrograms, an Office of AdministrativeManagement, and an Office of Public In-formation and Publications Management.He is assisted also by the Library's Presi-dentially appointed Board of Regents,which meets three times a year to iden-tify problems affecting the medical librarycommunity and to advise on NLM policiesand programs aimed at solving theseproblems.

The coordinating and support activitiesof the Office of the Director are generallyreflected in the program reports whichfollow. Other activities relating to thework of the Office are reported in the textimmediately below. Figures 1, 2, and 3portray NLM's organization and financialand personnel statistics.

The Office of Administrative Manage-ment 1) processed about 7OO personnelactions; 2) prepared and presented twoseparate budget submissions to the Bu-reau of the Budget and Congress; 3)negotiated and processed 2O reimbursa-ble agreements with other agencies,totaling over $5OO,OOO; 4) prepared 62contract actions totaling $3,722,OOO; and5) procured supplies and equipment worth$835,OOO.

The Office of Public Information andPublications Management 1) respondedto more than 8,OOO public inquiries re-

garding NLM services; 2) arranged Li-brary tours for about 2,6OO visitors; 3)completed a slide series on the Libraryfor future presentation by WRC-TV inWashington, D.C.; and 4) prepared aseries of exhibits for the Library's lobbyon such topics as medical quackery andthe history of the conquest of tubercu-losis.

The Library continued to emphasize inits international programs those activitieswhich are cooperative in nature and in-volve a sharing of resources. During thefiscal year, the Library 1) concluded bi-lateral MEDLARS agreements with theGerman Institute for Medical Documenta-tion and Information, the World HealthOrganization, and the Japan InformationCenter for Science and Technology (bring-ing to seven the number of such quid proquo agreements); 2) continued as tech-nical support agent for the Pan AmericanHealth Organization Regional Library ofMedicine in Sao Paulo, Brazil; and 3) pro-vided interlibrary loan, demand search,and other services to institutions withinAID countries, under a specific agreementwith the U.S. Agency for InternationalDevelopment. Personnel and budgetarylimitations forced discontinuation, in Sep-tember 1969, of all services to foreigncountries not included in the AID/NLMagreement. However, foreign interlibraryloan and audiovisual loan services to non-AID countries will be provided for a feebased on costs for handling and postage,effective July 1, 197O.

LIBRARY OPERATIONS

Increasing network activities continued-to modify Library Operations (LO) pro-gram approaches to collecting, catalog-ing, indexing, and retrieving the world'sbiomedical literature. During the fiscalyear, the Library designated the last twoRegional Medical Libraries (RML's) of thenationwide network (see Appendix A)created to provide more widely availableinterlibrary loan and computer searchservices. Completion of the network hasaugmented significantly the total amountof service supported by the NLM. Therehas been a concomitant enhanced effortin coordination, training, and manage-ment, not only in the provision of servicebut in building the reference data basesas well: for example, foreign MEDLARScenters are indexing both foreign-language and many English-languagejournals in return for the use of MED-LARS tapes.

The Library Netwprk/MEDLARS Tech-nical Bulletin, published by LO staff forRML and MEDLARS personnel, is an im-portant tool for coordinating networkactivities. In addition, two meetings ofthe Regional Medical Library Directorswere held during the Medical LibraryAssociation meetings in October 1969and May 197O. There were two MED-

LARS workshops, one in November 1969for domestic MEDLARS centers and onein Stockholm during May 197O for Euro-pean MEDLARS centers. A meeting ofEuropean MEDLARS center directorswas held concurrently with the Stockholmmeeting.

Network training activities were ex-panded; more than 1OOMEDLARS UserOrientation Courses were presentedthroughout the country, with participationof both NLM staff and members of localMEDLARS facilities. In addition, LO staffmembers gave three four-month trainingcourses for domestic and foreign MED-LARS analysts, with instruction in index-ing, medical subject headings, searchstrategy, and related topics. To promotethe development of strong medical libraryservices at all levels, LO staff also pre-pared an exhibit entitled "The BasicHospital Library," incorporating a corecollection of textbooks, journals, and ref-erence works especially useful for smallcommunity hospitals.

All divisions of LO continued efforts todefine systems for automating libraryservices and improving bibliographic ac-cess under MEDLARS II, with concreteresults scheduled for the next fiscal year.

BIBLIOGRAPHIC SERVICESThe Bibliographic Services Division

(BSD) indexes the periodical literature ofmedicine, prepares Index Medicus andother published indexes, and formulatescomputer searches in response to individ-ual requests for bibliographic assistance(see Fig. 4). BSD trains personnel fromMEDLARS centers in the techniques ofindexing the literature and searching theMEDLARS files. It maintains technical.aison with the 11 domestic and sevenoverseas MEDLARS centers, and coor-dinates their operations.

IndexingAbout 21O.OOO articles were indexed

for MEDLARS during the fiscal year, therame as the previous record productionc " Fiscal Year 1969. Again, slightly overiialf of the indexing was performed undercontracts and cooperative agreementswith external agencies. Indexers weretrained for the MEDLARS centers inAustralia, France, Japan, and Germany.

The indexing backlog was further re-duced by an additional 2O percent. Cita-tions from many of the prime English-language journals now appear in IndexMedicus and Abridged Index Medicuswithin 3O to 45 days after receipt of theoriginal publication at the Library.

SearchingApproximately 14,3OO MEDLARS de-

mand searches were carried out fordomestic users, over 25 percent morethan during Fiscal Year 1969. Output in-creased at both NLM and the MEDLARScenters, with NLM continuing to formu-late one-fourth of all domestic searches.A new domestic MEDLARS search centerbecame operational in the Regional Medi-cal Library at the New York Academy ofMedicine; new MEDLARS centers wereestablished in Germany and at the WorldHealth Organization in Geneva, Switzer-land, while the MEDLARS center inFrance became fully operational duringthis fiscal year.

PublicationsThe 1969 edition of Cumulated Index

Medicus (CIM) contained over IO.OOOpages and was published in eight vol-umes. It contained 224,OOO citations, anincrease of eight percent over the 1968CIM.

In January 197O, the Library beganpublication of Abridged Index Medicus(AIM), an important new monthly bibliog-raphy specifically designed for the prac-ticing physician. Each issue containscitations to articles from 1OO English-language journals chosen from the 2,3OOjournals indexed in Index Medicus (IM)for quality and particular value to the

medical practitioner. The value of AIMstems from the fact that its select con-tent and low cost will make it particularlyuseful to the individual practitioner, andto the libraries of small hospitals andclinics.

In May, the first issue of a new recur-ring bibliography, Parkinson's Disease &Related Disorders, appeared. It is beingproduced by NLM for publication bythe National Institute of NeurologicalDiseases and Stroke, and replaces theParkinson's Information Center alertingservice.

Thirty-six new Literature Searches(selected individual computer searchesreprinted for wide distribution) wereadded to the list of available titles. Thesebibliographies covered such timely sub-jects as cannabis toxicology, drug abuseand crime, and non-nutritive sweeteners.

Medical Subject HeadingsBecause of the continuing technical

constraints of MEDLARS 1, the numberof new main headings introduced duringthe year was held to 248. Continued ef-fort was devoted to the development ofthe expanded vocabulary which will pro-vide the fundamental base for the im-proved MEDLARS II system.

TECHNICAL SERVICESThe Technical Services Division (TSD)

selects, acquires, and catalogs theworld's biomedical literature (see Figs.5-7) and publishes this data in the NLMCurrent Catalog. Proof sheets of NLMcataloging were issued twice a week toselected libraries to determine the valueof rapid dissemination of cataloging infor-mation, and received a favorable re-sponse. As a result, the Medical LibraryAssociation arranged to issue the proofsheets on a subscription basis, starting inJuly 197O.

Serial-record data for the 20,964journals currently received by NLM werefurnished to the Union Catalog of MedicalPeriodicals (UCMP) staff, which beganconverting the records to machine-read-able form. Work had been completed .on7,5OO titles by the end of the fiscal year.This project will form the basis both forautomating serial work at NLM, and forestablishing a national union list of health-science periodicals linked to a network ofregional union lists.

Use of external sources to select andacquire materials was further expandedduring the year. The procurement pro-gram with Keio University in Tokyo wasaugmented to include selection, acquisi-tion, and cataloging of available FarEastern biomedical literature for the Li-brary. In addition, arrangements weremade with the Library of Congress to re-

ceive their preliminary catalog cards forpublications received under the "All theBooks Program." The cards are beingused as one selection source for bio-medical materials published in the UnitedStates.

An on-line, in-process file to supportcooperative cataloging activities wasinitiated with the State University of NewYork Upstate Medical Library at Syra-cuse (SUNY Upstate) and the Francis A.Countway Library of Medicine at Harvard.Records of in-process cataloging can bequickly checked to avoid duplication ofeffort. These records are maintained inthe SUNY file, with the three librariesentering their data via 274O terminals,and searching the file via the sameterminals.

Library school students, working part-time, began to correct the card catalogto conform to the Anglo-American rules;computer file maintenance correctionswill also be made for 1966-7O cataloging.This will make possible the consolidationof computerized catalog data, thus fa-

cilitating its use by other medical libraries.

REFERENCE SERVICESThis Division provides reference, bib-

liographic, and reader services to usersat the Library, and supplies material fromthe Library's collections to the biomedicalcommunity at large by means of an ex-tensive interlibrary loan program in con-junction with the Regional Medical Li-brary Network. It also maintains andpreserves, through contract binding andthe microrecording of deteriorating publi-cations, the collection of library materialspublished from 1871 to date.

Reference, Bibliographic, andReader Service

The Reference Services Division (RSD)received over 21.OOO reference inquiriesduring the fiscal year (seeFig. 8). Morethan 25.OOO health science professionalsand students used the Reading Room andrequested over 78.OOO items.

Plans for a "Multi-Media Resource"within the Reference Section have beendeveloped, and the first steps are beingtaken to implement it, including spacerenovation and the securing of motionpictures, video tapes, and slide sets. Twomodel exhibits were installed in the Li-brary for a "Tutorial Environment System"and a "Medical Education Jukebox," bothself-instructional devices designed withNLM support.

The Division established a HealthSciences Bibliographic Clearinghouse tocollect, index, and distribute bibliograph-ies in the health sciences. Staff membersissued for evaluation a pilot publication,Bibliography of Medical Bibliographies,listing available bibliographies and pro-viding an index to them. A trainingcourse for medical librarians was devel-oped, including the preparation of a Ref-erence Services Manual. A list of refer-ence sources also was updated andpublished in the Bulletin of. the MedicalLibrary Association as "Selected Refer-ence Aids for Small Medical Libraries."

Intel-library LoanThe Division received over 1OO,OOO

interlibrary loan requests (see Fig. 9).Photocopy of journal articles was usuallyfurnished in lieu of lending the originalvolume. The average time required forstaff to fill requests improved during theyear.

Work progressed toward completion ofan automated Graphic Image System.Equipment with the capacity to store15O.OOO articles in microcopy form hasbeen designed to provide rapid responseto interlibrary loan requests for materialfrom a selected group of journals. De-livery of the system is scheduled in thenext fiscal year.

Preservation and PhotographicSupport

Over 21.OOO journal and monographvolumes were bound, and almost 6OO,-OOO pages of deteriorating and irreplace-able library material were microfilmed toarchival standards (see Fig. 1O). In addi-tion to interlibrary loan and preservationfilming, the Division filmed and processed38O,OOO catalog cards, processed 151,-OOO photocomposed pages for IndexMedicus and other publications, and pro-duced 2,2OO photographs and slides.

HISTORY OF MEDICINEThe History of Medicine Division (HMD)

acquires, organizes, and services the Li-brary's original source materials for studyand research in the history of medicineand related sciences (see Fig. 11). It pre-pares catalogs, bibliographies, and worksof independent historical scholarship. TheDivision has custody of manuscripts,prints, and all publications in the Library'spossession that appeared prior to 1871.

During Fiscal Year 197O, HMD pro-vided approximately 4,OOO books, manu-script boxes, and microfilm reels to read-ers in the Library. It filled over 2,OOOrequests for photocopies and interlibraryloans, and supplied l.OOO photographsand slides in response to requests for pic-torial material.

HMD added almost 1,5OOvolumes tothe historical collections, and over 16O,-OOO manuscript items, including papersof Drs. Ward Darley, Harold L. Stewart,and Jacob M. Ulmer, the Civil War lettersof Dr. Thomas Winston, and official rec-ords of the Association of AmericanMedical Colleges and the Association ofState and Territorial Health Officers.HMD cataloged over 3,OOO monographsand serial titles.

OFFICE OF COMPUTER AND ENGINEERING SERVICES

The Office of Computer and Engineer-ing Services (OCES) provides automateddata processing and related support tothe various programs of the Library, in-cluding technical planning, systems analy-sis and development, computer program-ming, and computer operations. A majorresponsibility is the systems developmentfor the design and implementation ofMEDLARS II. The staff of OCES directsthis development effort, is responsible forthe work of the principal contractor (Com-puter Sciences Corporation),and providesfor the necessary coordination with otherparts of the Library.

MEDLARS I ACTIVITIESThe Honeywell 8OO/2OO computer sys-

tems continued to function reliably insupport of MEDLARS I production, witharound-the-clock work shifts throughoutthe year. Computer processing at theArmy Map Service supplements the Li-brary's computer, and another capabilitywas established at two Department ofDefense computer facilities to be usedoy the Ohio State IBM 7O94 programs for

processing demand searches. Improvedflexibility of processing resulted, duringthe month of May, in the shortestthroughput time for demand search inthe history of MEDLARS, averaging onlyfive days from submission for input tothe computer to final output.

Three of the four production categoriesshowed an increase for the year: MED-LARS citations input increased from226,OOO to 228,OOO; demand searchesincreased from 6,5OO to 8,OOO; and totalphotocomposed pages increased from65,OOO to 75,OOO. Catalog entries re-mained at approximately the same level(19,600).

MEDLARS II DEVELOPMENTMEDLARS II has as its main objec-

tives: 1) more specificity in indexing, withan augmented vocabulary; 2) user-machine interaction for input, vocabularybrowsing, and retrieval; 3) improvementsin response time to bibliographic re-quests; and 4) automated acquisitionsand cataloging procedures. The systemis to be implemented in two phases—

INITIAL in the second quarter of FiscalYear 1971, and EXTENDED in the sec-ond quarter of Fiscal Year 1972. TheINITIAL is characterized by batch opera-tion with selected extensions over MED-LARS I mainly in the vocabulary area.The EXTENDED will provide more com-prehensive capabilities, with on-line ac-cess to the system through remotedevices.

Specific steps leading to the softwareimplementation are the Overall FunctionalDesign Specifications and Detailed Appli-cation Design within the overall frame-work for each phase of implementation.In conjunction with these design efforts,input and output requirements wereanalyzed.

An IBM 36O/5O computer was ac-cepted in June 1969 for the developmentand implementation of the MEDLARS IIsystem. The keymatic encoder (a key-board to magnetic tape device) was se-lected for input preparation to MEDLARSII, because of its ready adaptability to theINITIAL system design and expandedcharacter set capability, using a singlekeystroke.

The PHOTON 713-1OO-15 has been

approved by the Joint Committee onPrinting for production at the Library ofsmaller bibliographies and catalog cards.Larger publications will be produced onthe LINOTRON 1O1Oat the GovernmentPrinting Office.

OTHER SUPPORT ACTIVITIESComputer programming was accom-

plished to provide a file maintenancecapability for the Auxiliary ChemicalModule, and to segment the chemicalname data for input into the MEDLARSII vocabulary.

An automated audiovisual distributionsystem was developed for the NationalMedical Audiovisual Center by OCES, inconjunction with contract personnel fromthe Radio Corporation of America. Its ob-jectives are to: provide master file main-tenance, inventory control, and accurate,current booking accounting; systematizeshipping and receiving of audiovisuals;and speed the processing of unscheduledrequests. Transactions are sent fromNMAC offices in Atlanta, Georgia, viatelephone lines for processing at NLM;resulting instructions and booking dataare then transmitted back to NMAC.

SPECIALIZED INFORMATION SERVICES

Specialized Information Services (SIS)was established at the National Libraryof Medicine to meet national needs forinformation in toxicology and relatedfields. The Toxicology Information Pro-gram (TIP), an element of SIS, was de-signed to fulfill recommendations madeby the President's Science Advisory Com-mittee in recognition of the impact ofchemical contaminants on the environ-ment and their effects on human healthand welfare. "SIS receives guidance con-cerning scope and operational prioritiesfrom a committee in the Division of Medi-cal Sciences of the National Academy ofSciences, National Research Council,which maintains contact with the profes-sional, academic, and industrial communi-ties.

TOXICOLOGY INFORMATIONPROGRAM (TIP)

The TIP devoted almost all in-houseeffort, during the past year, to projectsconcerning pesticides information.

As the result of a Toxicology UserStudy, conducted by interviews and by alarge-scale national mailing of question-naires to toxicologists and related scien-tists, TIP is now better prepared todevelop specific products and servicesto meet national needs.

Through a university contract, TIP hasalso been studying various domestic andforeign sources of toxicology information.The study has identified relevant primaryjournals, secondary services, sources ofunpublished data, and non-serial publica-tions, as well as certain industrial firmswhich have expressed a willingness tocooperate with TIP. Another contract en-abled TIP3 to identify nationally some4,OOO individual toxicology professionalswho will volunteer to provide direct assist-ance to TIP.

The National Referral Center, Libraryof Congress, produced for TIP A_ Direc-tory of Information Resources in theUnited States: General Toxicology, listing

approximately 7OO potential informationresources. The Directory became avail-able for sale to the public through theGovernment Printing Office in January197O.

Initial efforts to develop a terminologyfor toxicology were begun during thefiscal year. A group of experts submittedcore and specialty lists of terms for re-view by TIP; this work will be continued.

TIP's continuing interest in drug inter-actions led to a contract to develop andproduce in 1971 a drug interaction bibli-ography, which will cite articles dealingwith drug-drug and drug-chemical inter-actions and their effects on humanhealth.

The Program is continuing work oncomputer input, file management, andsearch programs which will supportcomputer-based toxicology data files.Nearing completion by the close of thefiscal year were pjtot computer programsto provide capabilities for processing andediting of scientific text. Subscripts, su-perscripts, chemical structure diagrams,and many special characters would beacceptable in input documents as partof the scientific text. The input capabilityincludes hardwarefor handling the specialcharacters keyed directly onto computer-compatible magnetic tape.

TIP continued to work with industrialtoxicologists to learn more about theirneeds and problems as related to exist-ing secondary sources of information.

DRUG LITERATURE PROGRAM(DLP)

Preparation continued, in cooperationwith MeSH and other BSD staff, for theincreased indexing anticipated in MED-LARS II. The MEDLARS subgroup of theAcademy of Pharmaceutical Sciences'Subcommittee on Information was estab-lished and the initial meeting held.

The DLP staff was detailed to LibraryOperations in April, pending the Library'sconversion to MEDLARS II.

NATIONAL MEDICAL AUDIOVISUAL CENTER

The National Medical Audiovisual Cen-ter (NMAC) plans and administers a na-tional program to improve the quality anduse of biomedical audiovisuals in theschools of the health professions andthroughout the biomedical community.The Center thus contributes directly tothe mission of the National Library ofMedicine in strengthening the informa-tion services in support of the nationalhealth effort.

The Center acquires and distributesfilms and other audiovisual materials; pro-vides biomedicat audiovisual referenceand research services; consults with andassists medical schools and institutionsin the development and use of audio-visual systems and materials; conductsaudiovisual research, training, experi-mental production, and other activities inthe development of media to supportmedical education; and encourages pro-duction, distribution, and use of audio-visual materials in the health sciences.

In 197O, NMAC completed a major re-organization, shifting emphasis from mo-tion picture and television production toexpanded programs in the acquisition,organization, and distribution of healthprofessional audiovisuals, related clear-inghouse reference services, and ac-tivities involving educational research,consultation, training, and media devel-opment.

More than 72,OOO audiovisuals weredistributed in Fiscal Year 197O (see Fig.12). During the year, some 29 motionpictures and 41 television programs werecompleted (see Fig. 13). These coveredsuch areas as clinical pathology, conceptsand controversies in modern medicine,physical diagnosis, rehabilitation medi-cine, and teaching in medical laboratoryoperations.

NMAC provided over 26O consulta-tions, surveys, seminar/workshops, andpresentations for schools of the healthprofessions and related organizations (see

Fig. 14). During the latter part of thefiscal year, a series of six seminar/work-shops were presented at NMAC onmotion picture production, physicallearning spaces, sound/slide series pro-duction, photomicrography and photo-macrography, television production, andorientation to the computerized audio-visual reference services at NMAC. Inaddition, a national Conference of Direc-tors of Biomedical Communication washeld at the Center. Participation in theworkshops and other meetings waslimited to key physicians and professionalmedical educators representing manyschools of medicine, dentistry, nursing,and other health professions. All sessionswere filled to capacity.

NEW PROGRAMS

Cooperative Media DevelopmentProjects

During the fiscal year, NMAC enteredinto cooperative projects with schools of

the health professions, national profes-sional health organizations, and agenciesof the Federal Government to help de-velop audiovisual media for medicalteaching. Each cooperative proposal mustmeet specific criteria as to subject matterand scope; relate to medical teaching;have national significance and appli-cability; and be acceptable to a targetaudience of health professionals. Coop-erative projects are now being consideredfor the development of audiovisual corematerial in such fields as pathology, psy-chiatry, anatomy, orthopedics, and ob-stetrics.

NMAC Videotape DuplicationProgram

A videotape duplication program wasinitiated this year to make available, with-out charge, duplicates of NMAC's video-tape master material. Eligible requestersprovide NMAC with raw videotape towhich the Center transfers requestedmaterial from about 175 master tapes ofprograms produced at NMAC or acquiredfrom other sources. CONTINUING PROGRAMS

Clinical Pathology SeriesDuring 197O, NMAC completed a

series of 56 videotape programs whichconstitute a core course in clinical pathol-ogy. The series is available for both loanand purchase as transfers on 16mm blackand white film, and will be available invideotape format through the new video-tape duplicating service.

Concepts and Controversies inModern Medicine Series

This series was continued, with com-pletion of the following programs: "Hos-pitalization of the Psychiatric Patient:

Should It Be Abolished?"; "The Antico-agutetion Dilemma"; "The Oral Contracep-tives"; and the "The Medical School andthe Community: How Are They Related?"Programs in this series are also availablefor loan or sale on 16mm black and whitefilm, and will be made available throughthe new videotape duplication project.

Rehabilitation Medicine SeriesA cooperative project with the New

Computerized International Indexof Medical Film Data (IIMFD)

The IIMFD was computerized and hasbeen operating successfully for a year.All data can now be retrieved by: (1) anycombination of 65 major subject catego-ries or 1,8OO sub-headings, (2) any com-bination of 3,OOO distribution and salessources, (3) specific titled address, or (4)major cataloging project designation. TheCenter furnished more than 7,OOO sub-ject-oriented searches from the Interna-tional Index during the fiscal year.

NMAC/VA Cooperative ProjectThe Center continued a program begun

in 1969 with the Veterans Administration(VA) to provide information about mate-i ials available for education in hematol-

ogy, psychiatry, gastroenterology, andsurgery. With support from the VA,NMAC has also provided consultative andadvisory services to the American Col-lege of Orthopaedic Surgeons in the de-velopment of 6O sound/slide productions.

PublicationsToward Improved Learning, Volume M_,

was published by the Center in FiscalYear 197O and distributed to the medicalprofession. The 1970 National MedicalAudiovisual Center Catalog and the FilmReference Guide for Medicine and AlliedSciences, 197O edition, will "be releasedearly in Fiscal Year 1971 by the Govern-ment Printing Office.

THE LISTER HILL NATIONAL CENTER FORBIOMEDICAL COMMUNICATIONS

The Lister Hill National Center for Bio-medical Communications (LHNCBC) hasthe responsibility within the National Li-brary of Medicine for improving healthcare delivery, medical education, and bio-medical research through new applica-tions of computer and communicationtechnology. It also serves as the focalpoint within DHEW for coordination ofbiomedical communications systems andnetwork projects.

During Fiscal Year 197O, the Centercontinued its efforts to define the educa-tional and information needs of the medi-cal community, and to relate these needsto services which a national BiomedicalCommunications Network could provide.A contract with the Council of AcademicSocieties of the Association of AmericanMedical Colleges produced informationand advice concerning the educationalneeds of the medical community. A steer-ing committee of medical educatorscon-centrated specifically on relating theseneeds to the content and mechanisms

which the Lister Hill Center could bestuse in the selection, production, evalua-tion, and delivery of education services.

The Center has also worked activelywith a newly formed committee of theAmerican Medical Association entitled"Computers in Medicine." This committeeis focusing, with staff of the Center, onproblems of standardization and sharingof computer records and resources. TheUniversities Associated for Research andEducation in Pathology (UAREP) is nowunder contract to develop selected edu-cational materials to support learning inspecific areas of pathology.

BIOMEDICALCOMMUNICATIONS INVENTORY

The Center, as DHEW focal point forbiomedical communications project andnetwork activities, continued to performspecial analyses such as the BiomedicalCommunications Inventory (BCI) preparedin Fiscal Year 1969. Using information

from this inventory, the Center presenteda preliminary overview of DHEW biomedi-cal communications activities to the Li-brary's Board of Regents in November1969; subsequently, a revised analysisof survey information was submitted tothe Director, NIH. The Center arrangedfor agency briefings for the AssistantSecretary for Health and Scientific Af-fairs, DHEW, as a step toward formulat-ing a Departmental program for improvedcoordination of DHEW biomedical com-munications projects.

BIOMEDICALCOMMUNICATIONS NETWORK

The Center concentrated its efforts onthe development of experimental proto-type services of a Biomedical Communi-cations Network.

Video NetworkingThe Center undertook three specific

projects related to linking medical schoolsand hospitals by television. The first wasa comprehensive design and cost analysisof a video network connecting all U. S.medical schools. The second looks to thedesign of an experimental television net-work which would allow interactionamong Dartmouth Medical School, theUniversity of Vermont Medical College,the University of New Hampshire, andsome 16 community and Veterans Ad-ministration hospitals in the Vermont-NewHampshire area. (A contract for the engi-neering design of this network—whichwould be used for purposes of both medi-cal education and the delivery of healthcare—has been initiated with the Jansky& Bailey Department of the Atlantic Re-search Corporation.)

The final project focuses on using avideo network to relate the C9ntinuingeducation resources of a university medi-cal center to specific, patient-orientedproblems confronting those who deliverhealth care. A contract has been negoti-ated with Dartmouth College for the de-sign, implementation, operation, andevaluation of such a network linking theDartmouth Medical School-Mary Hitch-cock Memorial Hospital complex andClaremont General Hospital. The Centerhopes to produce, through this project, amodel system which could be adapted foruse in areas where one or two medicalcenters must serve a large number ofisolated, small communities.

On-Line Computer NetworkingThe Center, in cooperation with Library

Operations staff, has initiated a new ex-perimental service, called the AIM-TWXSystem, to provide rapid, responsivesearching of the medical literature. Bibli-ographic information on articles published

during the last five years in over 1OOjournals of clinical medicine is beingstored in a large, time-sharing computerin Santa Monica, California, run by theSystem Development Corporation. Thejournals include those in the new AbridgedIndex Medicus (AIM). This computer canbe called from either TWX terminals orTeletype terminals connected to the tele-phone network. After placing a call to thecomputer and signing in with his number,the terminal user receives instructionsfrom the computer, explaining how tosearch the stored bibliographic informa-tion in a simple, conversational way. Thisexperiment is being conducted to identifythe usefulness of such services: 1) tohelp medical practitioners in isolatedareas, 2) to assist in undergraduate,graduate, and continuing medical educa-tion, and 3) to provide information pre-cisely when it is needed for health care.About 15 interested groups were author-ized in Fiscal Year 197O to use the sys-tem.

Audio NetworkingThe Center, in conjunction with the

University of Wisconsin and the RANDCorporation, completed studies whichanalyzed the need for telephone dial ac-cess or audio services, and produced anetwork design, including costing data,which would make such services availablenationwide. Implementation awaits a re-examination of priorities for availablefunds.

SATELLITE COMMUNICATIONS

The Center initiated an experimentalsatellite communications project duringFiscal Year 197O, using the NationalAeronautics and Space Administration'sApplications Technology Satellites (ATS).The project's initial objective is to experi-ment with satellite communications as ameans for medical consultation betweenremote areas and participating medicalcenters; a long-range objective is the ex-ploration of the degree to which satellitecommunications technology can be ap-plied to the spectrum of biomedical com-munications. The LHNCBC, using itsown satellite communications terminal lo-cated in the NIH Clinical Center, formed anetwork consisting of a component of theUniversity of Alaska, and the medicalcenters and engineering facilities at theUniversity of Wisconsin, the University ofWashington, and Stanford University.During April 197O, the LHNCBC initiateda series of satellite communications tests.The series produced the successful trans-mission of live electrocardiograms, slow-scan television and color facsimile, andthe interrogation of a time-sharing com-puter through consoles linked to thecomputer via satellite.

ON-LINE INDEXING

The Lister Hill Center has also beenactive in exploring the potential of tech-nology for library-related activities. Sincethe fall of 1967, the Center has beenworking with the Lincoln Laboratory ofMIT on the development of an interactivesystem for using and manipulating thecontrolled vocabulary of MbDLARS,Medical Subject Headings (MeSH). Tomeet this objective, Lincoln Laboratory

has built a data base which contains allof the existing MeSH terminology. Thesystem has been modified to allow useof a cathode ray tube display terminal atthe National Library of Medicine. A firstversion of an on-line indexing capabilityhas been developed to operate using thisterminal, and the first experiments in in-dexing medical literature on-line havebeen completed.

EXTRAMURAL PROGRAMS

The Office of Extramural Programs(EP) administers grants and contracts au-thorized by the Medical Library Assist-ance Act of 1965, and an internationalprogram funded by special foreign curren-cies. Of major significance to the librarycommunity was the passage in March197O of a three-year extension of theMedical Library Assistance Act. TheMedical Library Assistance Extension Actof 197O increases the previous authoriza-tion of $21 million per year to a maximumof $27.5 million at the end of the thirdyear. Other changes include the policyfor making resource grants, the additionof demonstration project authority, anda provision for planning grants for Re-gional Medical Libraries.

REGIONAL MEDICAL LIBRARIES

This program supports a national sys-tem of Regional Medical Libraries to sup-plement the services provided by existinghealth science libraries. Fiscal Year 197Osaw a total cpverage of the nation withRegional Medical Library services, withawards made for the South Central andMid-Continental regions (see AppendixA).

RESOURCE GRANTS

These grants provide funds to expandand improve medical library resources.Since the enactment of the Medical Li-brary Assistance Act of 1965, 1,418awards have been made to 4O2 institu-tions. During Fiscal Year 197O, newgrants were awarded to 12 libraries, andcontinuing support provided to 385 li-braries. Most grants are used to expandbasic collections of books and journals orto acquire indexing or reference tools.Some larger libraries use grants for as-sistance in automation of processing pro-cedures, production of union lists, im-provement of catalogs, and expansion ofbibliographic and interlibrary loan service.Recent amendments to the Medical Li-brary Assistance Act offer greater flexi-bility in the operation of the program. Forthe first time, grants may be made toassist in the establishment of new ji-braries, and the size of an award isbased on the needs of the institution andthe quality of its proposal. (Previously,the amount was based on the library'soperating budget for the past threeyears.)

TRAINING GRANTS

More than 1OO health informationspecialists received support from NLMtraining programs during the year. Fiveof these programs provided internships to

21 librarians for specialized training ininformafen processing and medical li-brarianship. Six grants supported 65trainees for master's degree programs inbiomedical communications or health li-brarianship. Four doctoral programs sup-ported 11 Ph.D. candidates for careersin health information research. Onespecialized program for medical editorssupported five trainees. Applicationswere received for two training programsin a new area: multi-media specializationfor health education, research, and serv-ice programs. Three postdoctoral re-search fellowships were supported. Fournew programs will not recruit until nextyear.

SPECIAL SCIENTIFIC PROJECTS

Five scholars continued to receive sup-port for studies of major scientific ad-vances related to health.

RESEARCH GRANTS

NLM research grants support four pro-gram areas: (1) library-related researchand development; (2) information scienceresearch and development, e.g., projectsto improve information storage and re-trieval, and studies of information needsof health workers; (3) historical researchin medicine and health; and (4) projects todevelop and analyze information searchtools, or to review and analyze the healthliterature. A total of 46 research projectswere active during the fiscal year, al-though only 11 new projects were funded.

PUBLICATIONS GRANTS

Among the projects funded in this pro-gram during Fiscal Year 197O were thepublication of critical reviews and trans-lations of Russian literature in psycho-physiology and psychosomatic medicine;the preparation of an internationallystandardized comparative nomenclaturein histology; and the preparation of astereotaxic brain atlas.

INTERNATIONAL PROGRAMSThe Library serves as the principal re-

source within the Department of Health,Education, and Welfare for the improve-ment of international exchange of pub-lished biomedical information in thespecial foreign currency program (PublicLaw 48O). During Fiscal Year 197O, proj-ects were developed in Israel, Poland,Yugoslavia, and India. These projects in-clude translations of biomedical mono-graphs and journals, critical reviews,histories of medicine, conference pro-ceedings, abstracting, indexing, andaudiovisual projects.
