AMA State President’s Report 2015
2015provedtobeaneventfulyearand2016promisestobenolessbusy.
Asmemberswouldbeaware,afederalelectionistobeheldlaterthisyearandtheAMA’shealthpolicyagendawasdiscussedindetailatlastmonth’sPresidents’andCEOs’meetinginCanberra.Ourfocuswillbeonsecuringadditionalfundingforprimarycare,anendtotheMedicarerebatefreeze,therescindingoftheproposedwithdrawalofincentivesforbulk-billingofpathologyandmedicalimagingservicesandnewandongoinginvestmentinpublichospitals.
MemberswouldbeawarethatattherecentCOAGmeetinganagreementwasreachedwherebyStatesandTerritorieswillreceiveanextra$2.9billionfromtheCommonwealthforhealthexpenditureoverthethreeyearsfrom2017to2020.Tasmania’sshareofthisadditionalmoneywillbe$54million.Commonwealthgrantsaretraditionallyallocatedonthebasisofpopulation.Wehave2.2%ofAustralia’spopulation,soourshareshouldhavebeen$64million.Onecouldaskwhathashappenedtotheadditional$10milliontowhichwewerehistoricallyentitled.AnyadditionalCommonwealthfundingiswelcome,ofcourse,butthe$54millionwillbeinsufficienttocopewithincreasingdemandonthepublichospitalsystem.AMAwillbepursuingfundingcertaintyforthenextdecadeforourpublichospitals.
ThemostrecentAIHWdatafrom2014-15demonstratethatTasmaniahadthelowestnumberofpublichospitalpatientbeddays(659.8per1000populationcomparedwiththenationalaverage758.1)andseparations(208.3per1000populationcomparedwith240.2)ofanystateorterritory,whenpopulationdemographicswouldsuggestthatTasmaniansneedmorehospitalcarethanourmainlandbrothersandsisters.Inrecentweeks,wehaveseengraphicexamplesofthehumancostofover-crowdinginourpublichospitals.Tasmanianhospitalsdonothavesufficientcapacitytodealwithhighdemandperiods.
WhileAMATasmaniasupportstheprinciplesofClinicalRedesignandHealthServicesInnovation,thefactisTasmaniadoesnothaveadequatenumbersofpublichospitalbedsornursingstafftocareforpatientsinthosebeds.TherelativestayIndex,ameasureofthelengthofstayofourpatients,is1.00(i.e.thesameastherestofthecountry),suggestingthatmuchoftheproblemisduetoinadequatecapacity,notshort-comingsinefficiency.Indeed,ourweightedseparationsforpublicinpatientsarehigherthanthenationalaverage,confirmingthatwetreatmorecomplexandexpensivepatients.
TherecentAMAReportCardonpublichospitalperformancehighlightedthelongwaitingtimesforelectivesurgeryandoutpatientclinicappointmentsinTasmania.AnalysisoftheAIHWdatawouldsuggestthatTasmanianhospitalsareneglectingrelativelysimplecasesinordertotreatmoreseriousandcomplex(andexpensive)patients.TheStateGovernmentisspendinganextra$25millionthisfinancialyeartofundadditionalpublicelectivesurgeryinprivatehospitalsandonweekendsattheLauncestonGeneralHospital.WhileAMATasmaniaacknowledgestheimportanceofaddressingwaitinglistsforelectivesurgery;itbelievesthatfundingneedstobefoundforinfrastructuredevelopmentandadditionalcapacityinthepublichospitalsystem.IwillcontinuetoarguethiscasewiththeMinisterandExecutiveofboththeDHHSandTHS.
Dr.DavidAlcorn,therecentlyappointedCEOoftheTHS,hasannounceditsnewexecutivestructure.CraigWatsonhasbeenappointedChiefOperatingOfficerandwilltakeresponsibilityforimprovingtheperformanceofTasmanianpublichospitals,specificallyfocusingonemergencydepartmentsandelectivesurgery.AseniorclinicianistoberecruitedtothepositionofExecutiveDirectorofPatientSafety,anewstatewideroleaimedatimprovingclinicalgovernanceandpatientsafety.AMATasmaniawelcomestheestablishmentofthispositionasithaslongbeenworriedaboutthelackofeffectiveclinicalgovernanceintheNorth-WestofthestateparticularlyandhasmadetheseconcernsknowntobothGovernmentandtheGoverningCounciloftheTHS.TheExecutiveDirectorofPatientSafetywillworkalongsideanExecutiveDirectoroftheMedicalProfession(alsoyettobeappointed).AMATasmaniahassomeconcernsthatthenewexecutivestructureaddsanotherlayerofbureaucracybetweenseniorhospitaldoctorsandtheGoverningCouncilandwillbemonitoringitsperformanceclosely.IhavemetwithDr.Alcornbrieflyontheseissues,andregularmeetingsbetweenAMATasmaniaandDr.Alcornhavebeenscheduled.

InitsfirstmajorresponsetothePrimaryHealthCareAdvisoryGroupreportfinalisedlatelastyear,theFederalGovernmenthasannouncedthatitwilltrialHealthCareHomesasawaytoimprovecareforpatientswithcomplexandchronichealthconditions.TheAMAsupportsthismodelwhichacknowledgesthepivotalroleGPsplayinprimarycare,butithascalledontheGovernmenttodetailhowitwillinvestinthisinitiative.AdditionalfundingforgeneralpracticeandtheHealthCareHomeswillbeakeyelectionissue.
IndustrialissueswereagainamajorfocusforAMATasmaniain2015.MemberswouldbeawarethatonJanuary23rdthePresidentoftheTasmanianIndustrialCommission(TIC)publishedhisinterimdecisionregardinganapplicationbytheTSMPSandAMATasmaniatovarytheMedicalPractitioner(PublicSector)Award.TheHodgmanGovernmenthasappealedthisdecision.ThebasisfortheappealistheGovernment’swishtoexcludefromtheAwardtheprovisionrequiringIttoguaranteeaminimumPrivatePracticeallowanceforallTasmaniansalariedspecialists(thereiscurrentlyadiscretionarypaymentmadetothoseNorth-Westsalariedspecialistsnotonlocumcontracts,specialistsintheNorthandSouthofthestateearn35%oftheirbasesalariesfromtheoperationofPrivatePracticeschemesintheLauncestonGeneralandRoyalHobartHospitals,participationinwhichisarequirementforemployment).TheTIC’sdecisioncodifiedthispracticeandgavesecuritytothe35%paymentshouldthePrivatePracticeschemesfail.ThisisacomplexissueandAMATasmaniahasprepareditscaseandwillappearbeforetheFullBenchoftheIndustrialCommissionagainonApril29.AMATasmaniaisnowalsonegotiatingnewIndustrialAgreementsforbothsalariedmedicalpractitionersandvisitingmedicalofficers(despitemultipleattemptsbyGovernmentrepresentativestodelaythisprocess).ItisasourceofongoingfrustrationforAMATasmaniathatatatimewhentheGovernmentisexpectinghospitaldoctorstodrivethelongoverduereformofthepublichospitalsystemitwouldseektoblockwhatareessentiallycostoflivingincreasesinsalarieswhilstatthesametimespend15-20milliondollarsannuallyonanunsustainablelocumserviceintheNorth-West.IhaveconsistentlypointedouttheincongruityoftheGovernment’sapproachinmyregularmeetingswiththeMinister.
Costsassociatedwithindustrialissuesaresubstantialandhaveincreasedsignificantlyinthelast12months.AMATasmaniaisgratefulfortheongoingexpertadviceofGeoffO’Kearney,DirectorofWorkplaceRelationsAMAVictoria.GeoffisalsorepresentingAMATasmaniaontheSpecialistMedicalPractitioners’RemunerationReviewPanelwhichhasbeenestablishedbytheGovernmenttooverseepaymentstoseniormedicalstaffinTasmania.
WearealsogratefulfortheextraordinaryfinancialassistancefromtheFederalAMA,whichhasallowedustoretaintheservicesofMr.CraigGreen(Dobson,Mitchell,andAllportLawyers)duringtheappealprocess.
The5thTasmanianHealthConferencewasheldinHobartonOctober17andwasagainagreatsuccess.TheconferenceisorganisedundertheauspicesoftheAMAandhasbecomeanimportantannualopportunityfortheTasmanianhealthcaresectorandGovernmenttoreviewthequalityandcostofhealthcareinthestate.Lastyear’sconferencehadthetheme:‘Onesystem,manypeople:Howcanweworktogethertomakeithappen?’withtheaimofimprovingcommunicationandco-operationbetweenthevariouselementsoftheTasmanianhealthsystem.
MonthlymeetingsoftheSouthernDivisionoftheTasmanianAMAhaverecommencedandarebeingchairedbyAnnetteDouglas.Thesemeetingsarenowwellattendedandhavetheaimofre-engagingwithgeneralpractice.NorthernDivisionmeetingscontinuetobewellattendedunderthechairmanshipofGlennRichardson.
MembersmaybeawarethatIchairedlastyearasubcommitteeoftheHealthCouncilofTasmania,theHealthyTasmaniaCommittee,whichwastaskedwithprovidingrecommendationstotheGovernmenttowardsachievingitsstatedgoalforTasmaniatobecomethehealthiestpopulationinthecountryby2025.TheCommitteedelivereditsreport:VisionforaHealthyTasmania:TheHealthiestPopulationinAustraliaby2025totheMinisterinearlyJune.AsIreportedtoCouncilinSeptember,theHealthyTasmaniaCommitteewasdisbandedbytheMinisterbuta5yearActionPlan,whichIunderstandwasbasedlargelyonthereport’s

recommendations,waspresentedbytheSecretaryoftheDHHS,MichaelPervan,totheMinisterinlateAugust.TheMinisterannouncedtheGovernment’sresponsetothePlanimmediatelypriortoChristmas.IthasanemphasisonreducingobesityandsmokingratesinTasmania.CommunityconsultationwithrespecttotheGovernment’sresponseisbeingundertakencurrently.AMATasmaniaisandwillcontinuetobeactivelyinvolvedinthecommunitydiscussionsandtherecommendationsfromthesewhichwillbedeterminedbytheHealthCouncilofTasmania(ofwhichIremainDeputyChair).IcontinuetorepresenttheAMAontheprofessionalreferencegroupwhichhasbeensetuptoreviewtheredevelopmentoftheRHH.
Memberswouldbeawareofanumberofongoingconcernswiththeredevelopmentincludinginadequatebednumbersandfacilitiesforacutementalhealthservicesinthetemporarydemountablebuilding,therelocationofthehyperbaricchamberduringKBlockconstruction,inadequatebednumbersforgeneralandsubspecialtymedicinepatientsinKBlock,limitedofficespaceforseniormedicalandnursingstaffandinadequateITinfrastructure.TheHospitalre-developmenthasalsobeensignificantlydelayedbynecessaryworkstoremoveasbestosfromthesitesafelyandwaterdamageandmouldcontaminationofseveralofthedemountablebuildings.
IhavehadseveralmeetingswiththenewHeadoftheSchoolofMedicineattheUniversityofTasmania,ProfessorBenCanny,whoalsoaddressedawell-attendedSouthernDivisionmeetingoftheTasmanianAMAearlierthisyear.Hediscussedtheforthcomingre-accreditationoftheMedicalSchoolandhiswishtoworkcloselywiththeAMA,generalpracticeandhospitaldoctorstohelpcreatea“fit-for-purpose”nextgenerationofdoctorsforTasmania.
Onabrightnote,membershipofAMATasmaniacontinuestogrow,particularlyamongyoungdoctors.Engagementwithgeneralpracticeremainsaproblembutisimproving.
AMATasmaniahasalsorepaidtheFederalloanforAMAHouseinHobart,whichwenowownoutright.ThisisatremendousachievementwhichwillstandtheorganisationingoodsteadandistestamenttoyearsofdiligentworkbyourExecutive,Council,andBoard.
Finally,Iwouldliketotakethisopportunitytoacknowledgethehardworkofourexecutiveofficer,TonySteven,andtheadministrativestaff,JacquiQuinnandNadineCove.IremainverygratefulforthesupportofCouncil,chairedsoablybyChrisMiddleton,andourvicepresident,StuartDay.

Membership
Asatthe31stDecember2015ourmembershipwas630,excludingstudents,thefirsttimeinmanyyearswehaveexceeded600.DrivingtheincreaseisindustrialrelationsandaneffectiveinternrecruitmentcampaigninJanuaryeachyear.OuractivitylevelshavebeenveryhighonceagainingeneraladvocacytotheMinister,industrialrelations,mentalhealth,andworkerscompensation.
TheTasmanianwebsitehasbeenre-launchedusingtheAMAFederalplatformandensuresweareabletohavethebestavailabletechnologyandimpact.Thisincludesanewupdatedversionofourfortnightlye-newsletterAMANews.
In2015,wewelcomedNadineCovetoourteamasacommunicationsassistantandwithheradditionalskills,wehavebeenabletoadoptmoreofourmediaworkin-houseandproducemoreprofessionalcommunicationswithourmembers,especiallytheAMAeNews.
IndustrialRelationsIssues
Throughout2015,wespentthehighestamounteveronindustrialrelations.ThankstotheexpertiseofGeoffO’KearneyfromAMAVictoriawehavefinalisedtheoverdueVMOAgreement,whichwillbelodgedshortly.WehaveimmediatelystartedworkonthenextAgreement.
TheStateGovernmenthasappealedagainstthePrivatePatientSchemeclauseintheInterimSalariedMedicalOfficers’Agreement.ThismatterisbeingheardbythefullbenchoftheTasmanianIndustrialCommission,andweareduetoappearbeforethemagainonApril29.

Financial Results
In2015,wehadadeficitofjustunder$22,000whichhasbeencateredforbypreviousyearsfundsheldinreserve.Themainreasonforthisdeficitistheadditionalamountspentonindustrialrelations.
Attheendoftheyear,wewereontracktopayofftheloantoAMAFederalinMarch2016.Thisistheendofaseven-yearjourney,anditisatributetoallwhohavebeenontheBoardandBranchCouncil.
DrAndrewJacksonhasstoodovertheprocesswithdiligenceandfervour,designingfinancialreportsindetailalongwithmodellingtoolstogetustothispoint.HisreportsattheAGMshaveneverbeenboringandhaveallhighlightedthelessonsofthepast.
AsIsaidlastyearcashflowiscontinuingtochange.Asmoreandmorememberspaymonthlyorbysalarydeductionplusmoredoctorsintrainingjoinourranks,theyieldpermemberlessens.Wehavetomanagetodomorewiththeresourceswehaveandatthesametimefindwaystoincreasethoseresources.

Headlines
• MortgageonAMAHouseextinguished(originalprincipal$109,000)• Increaseinmembershipof6%butlower‘yield’• Deficitof$13,045(exclusiveofdepreciation)at4.6%oftotalrevenue
ThefinalisationofthemortgageonAMAHouseaftereightyearshasbeenamajorachievement(whichoccurredinAprilthisyear)freeing-uparound$24000perannumtouseelsewhereincludinghelpingtoensureasurplusin2016.
TheAssociationshouldaimtohaveasurplusofatleast5-10%ofBranchincomeandsomeofthisshouldbeusedtobuildacashreservefundtohelpensurefinancialstability.
TotalBranchincomewas$366,009representingasmalldecreaseofaround2%overlastyear.Thedecreaseintotalsubscriptionincomethathasoccurreddespiteanincreaseinmembershiprelatestoachangedmix of membership,wherethereisaloweraveragesubscriptioncontributionpermember(theYield)whichwas$581($620thepreviousyear).
Expensesat$384,543representedanincreaseofaround13%.Alargeproportionofthisrelatedtoasignificantincreaseinspendonindustrialrelationsincludingawardnegotiations.ThisspendhelpedresultinanewawardforsalarieddoctorsacrosstheState,butasavariablecostcontributedsignificantlytoourdeficit.Doctorswhobenefitfromthesehard-wonawardconditionsandwhoarenotmembersoftheAssociationshouldconsidertheirposition.
Membersshouldunderstandthatweactuallycollected$540,748insubscriptionsbutretainedonly$264,985oraround48%ofthetotal.Inmyview,asmallStatesuchasourswithnoeconomiesofscalewillalwaysfindithardtoexistwhenlosinghalfofoursubscriptionstofundfederalactivities.Iconsiderthatretainingaroundthree-quartersofcollectedsubscriptionsoraround$400kwouldputourTasmanianBranchonamoresustainablefootingwithouttheconstantmendicantneedforafederalAMAannualgrant,currently$40000(andstillleavesusalongwayshortofthesuggestedamount).
ThisisthesecondfullyearofaspendonMediaAdvocacy(Font PR)andaPresidentialStipend.YourHon.Treasurerregardsitasmoneywell-spent.TheMediaspendraisestheBranchmediaprofile(importantformembership),improvesourlobbyingcapabilityandprovidesmuch-neededmediaandlogisticalsupportfor thePresident.
