Vitality Basics

What’s Vitality?Vitality is a voluntary wellness program that rewards you for taking steps to maintain or improve your health.

Can I sign up for Vitality? You can sign up for Vitality if you’re eligible for McKesson benefits. You don’t have to enroll in a McKesson medical plan to join Vitality. If you’re new to McKesson, you can sign up 14 business days after your start date.

Your spouse/domestic partner can sign up for Vitality only if they’re enrolled in a McKesson medical plan.

How do I sign up for Vitality?Go to www.powerofvitality.com > First time logging in? Register Now and follow the prompts. You can also go to the Vitality website from McKNet at https://m.mckesson.com.

If you’re the spouse/domestic partner of a McKesson employee, enter the last four digits of the employee’s Social Security Number, not yours, when registering.

Why should I participate in Vitality?Participating can help you maintain or improve your health. You can also earn rewards, such as savings on your McKesson medical premiums, gift cards, fitness devices and rebates for gym fees.

How does Vitality work?As a Vitality member, you earn Vitality Points™ for completing healthy activities in categories such as Prevention, Physical Activity and Online Education. Your Vitality Points determine your Vitality Status®, and your Vitality Status determines your rewards.

For example, you need to earn 1,000 Vitality Points in 2019 to unlock your medical premium savings and/or increase your current savings.

If your spouse/domestic partner is enrolled in your McKesson medical plan and participates in Vitality, they also need to earn 1,000 Vitality Points in 2019 to unlock rewards. The higher your Vitality Status, the more you save. See the chart on p. 11 for more information.

As you earn Vitality Points in 2019, your Vitality Status and savings increase. Earn the initial 1,000 Vitality Points in 2019 and then keep earning points until you reach Platinum Status. The savings you earn in 2019 are yours to keep for all of 2020.

What’s the Vitality program year? The Vitality program year runs January 1 - December 31. See how the program year affects your Vitality Status on p. 11.

What’s my Vitality Age? Your Vitality Age™ is a reflection of your health based on your health risks and lifestyle. A Vitality Age equal to or lower than your actual age is a sign of good health. You can find your Vitality Age at Vitality > Health Profile > Health Results.

Where can I get the app?The Vitality Today app is available on the App Store® and Google PlayTM. Vitality Points

How do I earn Vitality Points? You earn points by completing healthy activities in a variety of Vitality categories — Vitality Reviews, Vitality CheckTM, Online Education, Physical Activity, Goals, HealthyFood, HealthyMind, Certifications, Prevention and Other Activities. Visit the Total Rewards Library to see a list of activities that qualify for Vitality Points — www.mckesson.com/totalrewardslibrary > Wellness Programs.

How many Vitality Points can I earn each plan year?Every Vitality category has a maximum number of points available. For example, the most you can earn in the Physical Activity category is 7,000 Vitality Points. To see how many points are available in each category, visit Vitality > Points > Points Planner.

Does my spouse/domestic partner earn their own Vitality Points?Yes. If your spouse/domestic partner is enrolled in a McKesson medical plan, they can sign up for Vitality and earn their own points. Your spouse/domestic partner’s points (and Vitality Bucks®) are added to your Vitality account. You and your spouse/domestic partner also share your Vitality Status. Your spouse/domestic partner can’t redeem your Vitality Bucks, except to order a free Fitbit® device or buy a more expensive fitness device. Learn more on p. 19.

Do my Vitality Points roll over each year?Ten percent of your Vitality Points from one year roll over to the next. This yearly rollover is known as your Kick-start Bonus. Your Kick-start Bonus doesn’t count toward the initial 1,000 Vitality Points you need to unlock savings. Once you’ve earned 1,000 Vitality Points, your Kick-start Bonus can help you reach a higher Vitality Status quickly.

If you earned McKesson medical premium savings in 2018, they’re yours to keep for 2019. However, if you want to increase your 2019 savings or earn savings for 2020, you need to earn at least 1,000 Vitality Points in 2019.

Can I earn Vitality Points for gym workouts?Yes, if your gym offers either cardio or strength training equipment. Pilates, yoga, CrossFit and boot camp programs count as gym workouts.

Check in at your gym using the Vitality Today™ app or track your workouts with an approved fitness device. Learn about how to use a fitness device or the Vitality Today app on pp. 16-19. You’ll see your workout posted on the Vitality website about 24 hours after you sync your device. Learn about the gym rebate on pp. 21-22.

Can I earn Vitality Points using a fitness device?Yes, as long as your device is compatible with Vitality. Simply link your fitness device to Vitality to submit your workouts automatically. Learn more about fitness devices, including how to link yours and which ones are compatible with Vitality on pp. 16-19.

Can I earn Vitality Points for a sports league or event that isn’t listed on the website?Yes, as long as the league or event meets certain requirements and you verify your participation. Learn more at Vitality > Points > Points Planner > Physical Activity > Sports Leagues.

What’s the Vitality Health Review (VHR)?The VHR is a voluntary online questionnaire that can help you assess your current health. Completing the VHR only takes 10-15 minutes and is worth 500 Vitality Points. Your results are confidential, and Vitality uses them to suggest goals and activities that can help you improve your health.

When can I take the VHR?You can take the VHR whenever you like. If you take the VHR within the first 90 days of joining Vitality or within the first 90 days of the Vitality year, you earn a bonus of 250 Vitality Points.

Where do I take the VHR?Go to Vitality > Resources > Health Resources > Vitality Health Review. You can also take it on the Vitality Today app.

Can I update my VHR answers?Yes, but you don’t earn more than the original 500 Vitality Points for taking the VHR. You may get updated goals and see changes on your Health Profile page.

To change the Key Measurements section of your VHR, you need to verify and submit your biometric screening results through Quest Diagnostics or online at Vitality > Your Account > Forms and Waivers > Vitality Check.

When is the last day I can use my Vitality Bucks? You can use your Vitality Bucks until the last day you work at McKesson.

I no longer work at McKesson, but have McKesson medical coverage through COBRA. Can I still participate in Vitality? Yes. If you continue medical coverage through COBRA, you can participate in Vitality. Your spouse/domestic partner is also eligible if they’re enrolled in a McKesson medical plan. However, you can’t earn savings on your McKesson medical premiums and lose any Vitality Points and Vitality Bucks you had. If you’re rehired by McKesson, you get your account back, but not the Vitality Bucks you lost.

Vitality Is Voluntary

Participation in Vitality is voluntary and activities are completed outside employees’ regular working hours. The program is intended to improve the well-being of employees, covered spouses/domestic partners and dependents.

Eligibility

Who is eligible for Vitality?Vitality is open to all benefit-eligible employees. If you aren’t enrolled in McKesson medical coverage, you don’t earn medical premium savings. But you can still earn other exciting Vitality rewards, such as Vitality Bucks to spend on the Vitality website. Your spouse/domestic partner can join Vitality if they’re enrolled in a McKesson medical plan.

If you’re new to McKesson, you can register on Vitality 14 business days after your start date.

What happens to my Vitality Points if my medical coverage changes in the middle of the year due to a qualifying change?

• If you add a child to your medical plan, there’s no change to your Vitality Status, Vitality Points or Vitality Bucks.

• If you add a spouse/domestic partner to your medical plan, there’s no change to your Vitality Status, Vitality Points or Vitality Bucks. However, the number of Vitality Points you need to reach the next status level is adjusted.

• If you drop a spouse/domestic partner from your medical plan, you lose the Vitality Points they earned. However, you keep the Vitality Status and Vitality Bucks you both earned. The number of Vitality Points you need to reach the next status level is adjusted.

I changed from working full-time to part-time. Can I still be in Vitality?Yes. If you’re scheduled to work at least 20 hours a week, your Vitality account, Vitality Status, Vitality Points and Vitality Bucks stay the same.

Can I earn Vitality Points during a leave of absence? If you’re covered under a McKesson medical plan during your leave, you still have access to Vitality and can earn Vitality Points. If you’re unsure of your medical plan coverage during your leave, call the HR Support Center at 855.GO.MCKHR (855.466.2547) and press 1.

Biometric Screening Basics

What’s the Vitality Check biometric screening? The Vitality Check biometric screening is a voluntary health assessment that uses physical measurements and blood tests to determine your Body Mass Index (BMI), blood glucose, blood pressure, cholesterol level and cotinine level.

Can I earn Vitality Points for taking the Vitality Check biometric screening?Yes. In fact, you can earn a whopping amount of Vitality Points. The biometric screening is worth 500 Vitality Points per year, and you can earn up to an additional 3,525 Vitality Points per year for in-range results. That’s easily enough Vitality Points to rocket you to Silver Status, or higher if you have enough Vitality Points before taking the screening.

Am I eligible for a free Vitality Check biometric screening? If you’re eligible to enroll in a McKesson medical plan, you can get a Vitality Check biometric screening at no cost to you once per year at a Quest Diagnostic Patient Service Center or a work-site event. Your spouse/domestic partner needs to be enrolled in your McKesson medical plan to get a no-cost screening.

Can I get a Vitality Check biometric screening at my doctor’s office?Yes, but you need to follow the steps below:

• Download the Vitality Check and Continine Screening forms from the Vitality website at Your Account > Forms > Vitality Check.

• Take both forms with you to the doctor.

• Be sure your doctor fills the forms out completely and signs them before you leave.

• Submit both forms to Vitality at Your Account > Forms > Vitality Check.

Your doctor needs to complete these forms even if they use a Quest Diagnostics facility for your screening.

I’m getting my biometric screening at my doctor’s office. Does my McKesson medical plan cover the screening? If you get your screening at your doctor’s office, ask our doctor to code the BMI, blood glucose, blood pressure and cholesterol level screenings as preventive care. Preventive care is covered at 100%, so you won’t need to pay a penny.

The cost of a cotinine screening is an out-of-pocket expense, but you can pay for it with money from your health savings account or standard healthcare flexible spending account (FSA). Be sure to submit a Cotinine Screening form to Vitality to get your Vitality Points.

How do I schedule a biometric screening?Log on to Vitality and go to Health Profile > Vitality Check > Schedule your Vitality Check with Quest.

How can I contact Quest Diagnostics?Call 855.623.9355 — 7 a.m. - 8:30 p.m. Central time, M-F and 7:30 a.m. - 4 p.m. Saturday. You can also visit My.QuestForHealth.com.

Do I have to take the biometric screening? No. The biometric screening, like all wellness activities, is voluntary.

How do I prepare for my biometric screening?• If you’re taking medications, keep taking them.

• It’s important to fast — don’t eat or drink anything but water — for 9-12 hours before the screening. If you have a medical condition that prevents you from fasting (e.g., diabetes), talk with your doctor about how to prepare.

I’m pregnant. Can I take the biometric screening? As always, follow your doctor’s advice. If biometric screenings are already part of your regular care, you may find it’s easiest to submit your results using the Vitality Check Form. You can download the form from the Vitality website — Your Account > Forms and Waivers > Vitality Check. Or, if you prefer, you can schedule a biometric screening at a McKesson on-site location or through a Quest Diagnostics Patient Service Center (PSC).

Vitality Check Biometric Screening

Update your VHR responses on the Vitality website to reflect that you’re pregnant, or submit the Prenatal Care Verification form (Vitality > Your Account > Forms and Waivers). That way, Vitality can adjust your goals as needed.

Biometric Screening Results

How do I get my biometric screening results? You can get your results online three to four days after your screening at My.QuestForHealth.com. You get a profile explaining your results in the mail 10-14 business days later. Your profile includes a medical summary page for you to share with your doctor. Your results also appear on the Vitality website under Health Profile > Health Results about two to four weeks after your screening.

What does it mean if my results are in range?In-range results are good indicators that you’re taking the right steps to stay healthy. Keep getting regular checkups, staying active and participating in our wellness programs.

How can I earn Vitality Points if my results are out of range? If you’re unable to get an in-range result, as defined by Vitality, for any biometric measure, you have a chance to earn the same number of Vitality Points by completing the Reasonable Alternative Standard. See p. 15 for details.

How does Vitality use my biometric screening results?Vitality uses your biometric screening and VHR results to suggest personalized wellness goals and adjust your Vitality Age. Find your personalized goals at Vitality > Health Profile > Goals.

How do I report my biometric screening results to Vitality? If you take your biometric screening at a McKesson on-site clinic or Quest Diagnostics PSC, your screening results are automatically posted on the Vitality website about two to four weeks after your screening. If you take your screening with your doctor, have them fill out the Vitality Check and Cotinine Screening forms.* They’re available on the Vitality website at Your Account > Forms and Waivers > Vitality Check.

* The forms need to be completed even if your doctor uses a Quest Diagnostics facility to process your screening results. You pay out of pocket for your cotinine screening.

Can I dispute my biometric screening test results? If you don’t agree with your screening results, you can retest (McKesson doesn’t pay for retesting). Discuss the results with your doctor to determine if a retest is right for you.

Vitality Check Biometric Screening

New to McKesson?

You may be able to earn Vitality Points if you completed a biometric screening within six months of your hire date. Go to www.powerofvitality.com > Your Account > Forms and Waivers > Vitality Check, fill out all of the information and attach proof of your last screening.

If my BMI is over 25 but my waist circumference is in range, can I earn in-range result points?Yes. If your BMI is between 25 and 29.9 but your waist circumference is within the healthy range, you earn 125 Vitality Points for the screening and an additional 1,000 Vitality Points for being in the healthy range.

If my BMI is over 25 and my waist circumference isn’t in range, can I earn in-range result points? No. If your BMI is over 25 and your waist circumference isn’t within the healthy range, you can’t earn 1,000 Vitality Points for in-range results. You earn 125 Vitality Points for getting the screening.

What does it mean if one or more of my results are out of range?Out-of-range results may be a sign of potential health risks. We encourage you to discuss your results with your doctor. He/she can answer your questions and use your results to suggest ways to maintain or improve your health.

Medical Accommodations

If you’re unable to meet any Vitality outcome-based standard, Vitality automatically offers you ways to earn the same reward by different means, regardless of any medical condition and without your doctor’s approval. If you think you’re unable to meet an activity-only Vitality standard, see p. 29 for more information.

Vitality Check Biometric Screening

Are screening results confidential?Yes. We respect the privacy of your personal health information. Quest Diagnostics handles every aspect of biometric screenings, including linking your information to Vitality. Quest Diagnostics provides McKesson with an anonymous summary of screening results for all participants. The summary doesn’t identify any individuals. (Although you can choose to share your results with your spouse/domestic partner, you don’t have direct access to each other’s screening information.)

Are my screening results shared with McKesson? No. McKesson only sees combined data for purposes of healthcare operations, such as quality assessment and improvement activities. Any individually identifiable health information, maintained or transmitted by Quest Diagnostics and Vitality, is protected health information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), as amended. PHI isn’t shared with McKesson unless you provide a signed release form. For more information, refer to McKesson’s Notice of Privacy Practices. Why should I complete a Reasonable Alternative Standard? When you receive an out-of-range result, Vitality automatically provides you with an alternative goal, or Reasonable Alternative Standard, designed to help you get closer to the in-range result. And, by completing the Reasonable Alternative Standard, you can earn the same number of points awarded for the in-range result.

What’s an example of a Reasonable Alternative Standard? If you have an out-of-range result for cotinine, you can participate in the Living Smoke Free program through Vitality. To start the program, set an eight-week Quit Smoking goal at Vitality > Health Profile > Goals.

Where can I find my Reasonable Alternative Standards?Find your specific Reasonable Alternative Standards at Vitality > Health Profile > Goals. Activate and complete these goals to get closer to the in-range result.

What happens if I can’t meet a Reasonable Alternative Standard? If you can’t meet a Reasonable Alternative Standard, you may qualify to earn the same reward by different means with your doctor’s approval. To see if you qualify, talk to your doctor and submit a completed Biometrics Physician Waiver to Vitality. You need to submit a letter from your doctor saying that you can’t meet the alternative in-range standard to get the waiver. Find more details at Vitality > Your Account > Forms and Waivers.

Heart Rate Monitors

How does a heart rate monitor work?A heart rate monitor uses two parts — a chest strap and a watch — to monitor your heart rate. The chest strap detects your heart rate and transfers that data to the watch, which displays and stores the information. To earn points for a verified workout using a heart rate monitor, complete a Light, Standard or Advanced workout.

Activity Vitality Points*

Light workout 15–29 minutes at 60% of max heart rate = 5 points

The fitness app or device needs to capture data equal to a Standard or Advanced workout at the gym, or continue to capture data equal to a Standard or Advanced workout throughout the day. How Medical Accommodations Work

I have a medical condition that limits my ability to meet a Vitality standard. What can I do?When a short-or long-term medical condition limits your ability to engage in physical activities or meet a Vitality standard, Vitality makes it possible for you to stay on track with a medical accommodation.

How do medical accommodations work? For each activity-based standard you’re physically unable to meet, Vitality offers an alternate activity that allows you to earn the same amount of Vitality Points. For example, if you typically earn 10 Vitality Points a day for a 30-minute workout, Vitality recommends a non-physical activity worth 10 Vitality Points.

For each outcome-based standard you’re unable to meet, such as a blood pressure range, Vitality automatically offers you ways to earn the same amount of points by different means.

How do I request a medical accommodation? You need to complete a form and submit it to Vitality. Go to Your Account > Forms and Waivers and select the appropriate form based on the standard or activity you’re unable to meet. If you need to, have your doctor complete his/her portion of the form and submit your completed form by email to [email protected] or fax it to 877.224.7110.

Submissions by email aren’t secure until they’re received by Vitality. For your protection, Vitality strongly recommends sending your form via their secure fax.

Why does my doctor have to complete a portion of the form?Based on your doctor’s suggestions, Vitality offers alternative activities you can complete to earn Vitality Points. Your doctor may also need to approve the alternative activities.

How Taxes Are Handled

What Vitality rewards are considered taxable?Taxable rewards include anything you buy on the Vitality website with Vitality Bucks, plus your rebates for gym and weight loss program fees.

Do I pay taxes on Vitality rewards? No. McKesson pays the taxes for all Vitality rewards by adding money (gross up) to your paycheck. Each month, McKesson processes a gross up record for the Vitality rewards you earned in the previous month. The record also includes the rewards your spouse/domestic partner earned. This process helps keep all taxes properly reported and reflected on your paycheck.

How is this reflected on my paycheck?Taxable reward earnings are shown in the box Taxable Benefits under a description of Vitality Power Points. This amount shows the taxable reward value. Under Hours and Earnings, an item with the description of Gross Up Vitality Points lists the amount that offsets the taxes. This amount is 63% of the taxable reward value listed in Taxable Benefits.

For example, Laura gets $35 worth of gift cards on the Vitality website. Her Taxable Benefit (Vitality Power Points) is $35, and her Gross Up in Hours and Earnings (gross up Vitality Points) is $22.05, as shown in the pay stub below.

McKesson is committed to helping you achieve your best health and offers all employees Vitality Points for participating in Vitality. If you are unable to meet any Vitality outcome-based standard, Vitality automatically offers you ways to earn the same reward by different means, regardless of any medical condition and without your doctor’s approval. If you think you are unable to meet an activity-only Vitality standard (or a reasonable alternative to such standard), you might qualify for an opportunity to earn the same reward by different means with your doctor’s approval. Visit the Forms and Waivers page on the Vitality website to download the applicable waiver form. If necessary, complete your portion of it, have your doctor complete his/her portion of it and then submit it to Vitality. Vitality will use your doctor’s recommendations to apply a medical accommodation for you in light of your health status. If you have any questions, contact the Vitality Customer Service Center at 877.224.7117.
