1. Certified true copy of Articles of Cooperation and By-Laws.2. Certified true copy of Certificate of Registration / Confirmation issued by CDA.3. Original copy of Certificate of Good Standing from the CDA.4. Latest financial statements duly audited by an independent CPA5. Certification under oath by the Chairperson / General Managera. The cooperative is transacting business with members only with both members and non-members.

b. Amount of accumulated general reserve fund and undivided net savings. c. At least 25% of the net surplus is retained to members in the form of interest on capital and a patronage refund.d. List of members and the share contribution of each member.
