As always we are looking forward to a tremendous conference filled with opportunities to learn, celebrate, network, and spend time together. Our conference theme, ‘Physical Therapy the Frontline of Musculoskeletal Care’ summarizes the importance of our profession in assisting individuals receive the services/ care that they deserve as well as supporting our natural role as the portal to the healthcare system for individuals with musculoskeletal conditions. Highlights of the conference will include keynote addresses by Dr. Bill Vincenzio, Dr. Eric Robertson and Dr. Tim Flynn – true leaders of our profession who will be providing sessions that you will not want to miss. Dr. Stanley Paris is the recipient of our first Distinguished Lecturer Award and I have no doubt that his presentation will be outstanding! President’s MessageIn just a few weeks we will be merging in Anaheim, CA for the 2011 AAOMPT conference. It is hard to believe that a year has passed since last years conference, but I guess my battery charge is a little low and nothing recharges the old OMPT battery like the AAOMPT Conference. As always we are looking forward to a tremendous conference filled with opportunities to learn, celebrate, network, and spend time together.

Our conference theme, ‘Physical Therapy the Frontline of Musculoskeletal Care’ summarizes the importance of our profession in assisting individuals receive the services/

care that they deserve as well as supporting our natural role as the portal to the healthcare system for individuals with musculoskeletal conditions.

Highlights of the conference will include keynote addresses by Dr. Bill Vincenzio, Dr. Eric Robertson and Dr. Tim Flynn – true leaders of our profession who will be providing sessions that you will not want to miss. Dr. Stanley Paris is the recipient of our first Distinguished Lecturer Award and I have no doubt that his presentation will be outstanding! The Saturday Breakout Sessions will be fantastic and as always our conference ends with a “bang” with the Oral Research Presentations on Sunday including the Research Keynote address by Dr. Gerard Brennan.

We will take valuable time to celebrate via several events with the first one being the Fellowship Recognition Ceremony on Thursday. One of the most important aspects of our conference is the recognition of our highest awards. aaoMPT international Delegate Report

Background: The AAOMPT is a member organization (MO) of the International Federation of Orthopaedic Manual Physical Therapists (IFOMPT).

The mission of IFOMPT is to:

• Promote and maintain the high standards of specialist education and clinical practice in manual/musculoskeletal physiotherapists.

• Promote and facilitate evidence-based practice and research amongst its members.

• Communicate widely the purpose and level of the specialisation of manual/musculoskeletal physiotherapists amongst physiotherapists, other healthcare disciplines and the general public.

• Work towards international unity/conformity of educational standards of practice amongst manual/musculoskeletal physiotherapists.

• Communicate and collaborate effectively with individuals within the organisation and with other organisations.

annual highlights:

1. The IFOMPT working group that I am a member has been working for a year and a half on the docu-ment entitled International Federation of Orthopaedic Manipulative Physical Therapists International Framework for Examination of the Cervical Region for potential of Cervical Arterial Dysfunction prior to Orthopaedic Manual Therapy (OMT) Intervention. The framework is intended to be informative and not prescriptive and aims to enhance a physical therapists reasoning as part of the process of patient assessment. The framework is designed to give guidance for the examination and management of the cervical region for patients in advance of planned ortho-paedic interventions. The document has gone through 3 drafts and has had input from all Member Organizations. The final draft is nearing completion at which point it will be available for all members.

2. The AAOMPT Standards Committee submitted an extensive review of the AAOMPT Fellowship programs and credentialing process to the IFOMPT Standards Committee. The IFOMPT conducts International Monitoring of postgraduate educational programmes in Orthopaedic Manual Therapy and successful review of these standards allow our continued status as a Member Organization (MO) in IFOMPT. See Standards Committee report for additional details.

3. The IFOMPT held formal and informal meetings at the World Congress of Physical Therapy (WCPT) in Amsterdam in June of this year. Past AAOMPT President Dr. Ken Olson currently serves as an Executive Member of IFOMPT. The IFOMPT presented two focused sympo-sium one on Advancing International Post-Professional Educational Standards: Learning from the IFOMPT Experience and another on Spinal Manipulation – Evidence for Physiotherapist Delivery of Effective Procedures.

Upcoming Collaborations:

The American Academy of Orthopaedic Manual Physical Therapists (AAOMPT) will be conducting our 2012 annual conference in conjunction with the International Federation of Orthopaedic Manipulative Physical Therapists (IFOMPT) Conference 2012! The Canadian Academy of Manipulative Physiotherapists (CAMPT) is the host country for the 2012 IFOMPT Congress and AAOMPT will have the opportunity to work collaboratively with CAMPT. This outstanding event is the result of unprec-edented collaboration and has been years in the making. Please join your AAOMPT family and your international colleagues in Quebec City, Canada from September 30th to October 5th, 2012.

aaoMPT Research Committee Report

The AAOMPT Research Committee has been working to accomplish the following activities:

1. Stimulate submission of OMT related Research abstracts for the AAOMPT conference poster and platform presentations. For the last three years the number of submissions has more than doubled, from about 45 in 2008 to about 100 in 2011.

2. Stimulate submission of OMT related Research proposals for the AAOMPT for funding of OMT related research. Nine research proposals were submitted this year.

3. Stimulate and support the submission of OMT related case studies and research projects by OMT Fellowship students/programs by providing feedback/review/mentoring.

4. Build a Panel of Reviewers (currently 24 Members) made of licensed physical therapists in clinical practice and/or academia to help in

a. The review process and selection of abstracts for the poster and platform presentations

b. The judging of poster of platform presentations at the conference

c. The scoring of Research proposals

5. Solicit Grant funding support for funding of OMT related research.

6. Work on Programming of Research Day and Poster presentation sessions

7. Evaluate the feasibility for electronic submission and automation of abstracts directly on the AAOMPT website (cost analysis)

8. Honor the Recipients of the 2010

The aaoMPT Practice affairs Committee has been working to accomplish the following activities:The AAOMPT practice affairs committee continues to work very closely with the APTA manipulation task force, the Orthopaedic Section practice committee, and the APTA state governmental affairs department to assist state chapters and individuals confronted with practice issues related to OMPT. In August, 2011, due to APTA restructuring, the APTA manipulation task force has been changed to the manipulation workgroup. The members, goals, and function remain much the same with strong AAOMPT representation on the workgroup and a close working relationship with Justin Elliott of APTA state governmental affairs. The three primary issues we have dealt with over the past 12 months have been related to the practice of manipulation, intramuscular manual therapy (i.e. dry needling), and Referral for Profit.

Practice affairs Committee

Members of the committee have assisted AAOMPT members to work with several state licensing boards and APTA state chapters to pursue clarification regarding the practice of intramuscular manual therapy (dry needling) as it relates to each state PT practice act. In 2010, the Federation of State Boards of Physical Therapy developed a Resource paper on Intramuscular Manual Therapy (dry needling) with the assistance of members of the AAOMPT and copies can be obtained by contacting FSBPT or APTA state governmental affairs department. Much of the positive activity surrounding dry needling resulted from the position statement passed at the 2009 AAOMPT annual meeting in support of dry needling being within the scope of PT practice. This has now evolved into development of a dry needling workgroup within the APTA.

The members of the practice affairs committee also developed and published a critic of the Federation of Chiropractic Licensing Boards paper titled: High-velocity, low amplitude spinal adjusting / manipulation performance: Minimum criteria for safety and adequate competence. Please see the February, 2011 issue of Articulations at www.aaompt.org to review this paper. A new flyer on the legislative issues surrounding manipulation was also developed in 2011 and can be found at www.apta.org/manipulation.

2011 state legislative Recap (From aPTa state governmental affairs)Nebraska: The Nebraska Department of Health and Human Services (DHHS) Division of Public Health has scheduled a hearing on September 16, 2011 to consider extensive revisions to both the physical therapy and chiropractic regulations. The proposed regulations deal with several different issues, from continuing education to disciplinary action against providers with substance abuse problems, and both sets of revised regulations include changes affecting the practice of spinal manipulation.

 The regulations proposed by the Nebraska Board of Physical Therapy add the following new definition of “normal physiological range of motion”:

 “Normal Physiological Range of Motion means all motions available at a joint in response to forces and loads that move the joint to the limit of its flexibility or extensibility. These motions may or may not be accompanied by a popping sound from the joint commonly known as joint cavitations. Motions that result in movement beyond the range of motion of a normal, unimpaired joint or that result in partial or full dislocation are not part of the normal physiological range of motion of a joint.”

 The regulations proposed by the Nebraska Board of Chiropractic add the following new definition of “chiropractic adjustment”:“Chiropractic adjustment means the same as the term joint manipulation and means a high velocity thrust, which carries a joint beyond the normal physiological range of motion and is accompanied by joint cavitations whether audible or inaudible. The patient cannot prevent motion beyond the physiological range. Joint manipulation commences where manual therapy or grades one through four mobilization ends. Motion beyond the physiological range of motion occurs without the patient’s ability to control such motion. This includes, but is not limited to, a high velocity low amplitude thrust.”

APTA is assisting the Nebraska chapter in preparing for the hearing. The chapter is developing comments and is working to generate comments from chapter members in support of the proposed PT regulations and in opposition to the proposed chiropractic regulations. They are also planning a legislative strategy should the chiropractic rules be approved.

CT SB 528 (patient consent) FAILED: Concerns consent to chiropractic cervical spine treatments; requires a chiropractor to obtain informed written consent from a patient prior to performing a procedure involving treatment of the patient’s cervical spine.

GA SB 135 (chiro scope clarification) PASSED: States that a licensed chiropractor may perform a chiropractic adjustment. Provides exemption for other licensed health care providers working within their scope.

HI SB 657/HI SB 961 (chiro scope expansion) FAILED: Major scope of practice expansion to include therapeutic and “physiotherapeutic services.”

IN HB 1186 (require minimum hours of education for PT spinal manipulation) FAILED: This bill once again was a major factor in the ongoing battle with the chiropractors opposing our chapter’s direct access bill (HB 1151). INAPTA’s direct access bill made significant progress this year and the chapter continued negotiations with the state chiropractic association. Legislators continued to press both parties to come to a compromise. However, at the conclusion of the legislative session, the chiropractors communicated to us that they no longer see a point in continuing to try to negotiate on this issue and that they will continue to oppose direct access and try to prohibit PTs from performing spinal manipulation.

MN HB 1334/SB 1065 (chiro scope expansion) FAILED: This was the most problematic bill of the year. Massive expansion of chiro scope (copy of text below) that would have defined manipulation and mobilization.

MS SB 2129 (minor chiro scope) PASSED: Provides clarification of what a chiro may delegate to a chiro assistant.

NM HB 127 (major chiro scope): FAILED: Provides for a certified advanced practice chiropractic physician to prescribe and administer dangerous drugs and to perform certain other procedures.

WA HB 1673 (Removes manipulation prohibition from PT Act) FAILED

Referral For ProfitReferral for profit is another area that AAOMPT is interested in assisting. A $5,000 grant awarded to South Carolina chapter in 2010 assisted in their legislative efforts to protect their current law that restricts referral for profit physical therapy practice arrangements.

Illinois – Illinois was engaged in a grassroots effort to fight legislation proposed by the IL State Medical Society to legalize referral for profit practice arrangements in the state during 2010. The Governor vetoed this legislation in July, 2010, but the ISMS successfully overturned this veto during the November, 2010 veto session to essentially legalize referral for profit in IL.

PaF grantsThe AAOMPT practice affairs fund grants are available to state chapters dealing with OMPT related legislative and regulatory issues. The grant application is available on the AAOMPT website. We hope members will continue to contribute to the AAOMPT PAF fund and participate in the PAF fund raisers. The funds are used to support the grant program and to put on additional education programming to assist state chapters in obtaining their OMPT related practice goals.

California – An AAOMPT practice affairs fund grant was awarded to the California Orthopaedic Manual Physical Therapy Special Interest Group to assist in obtaining a legal opinion on the interpretation of California law as it relates to the practice of thrust joint manipulation by physical therapists. This legal opinion will provide greater clarity of the interpretation of the practice act and related laws in California to assist in the development of further strategic planning on this issue in the State.

The AAOMPT practice affairs committee wishes to seek AAOMPT member input throughout the conference and throughout the year on OMPT practice related issues that are important to our members.

Clinical PearlFlexion Mobilization with Movement for the Thoraco-lumbar Junction

Technique: Flexion Mobilization with Movement for the Thoraco-Lumbar Junction

indication: Patients with mid- or low-back pain often present with decreased flexion mobility at the thoraco-lumbar junction. They may exhibit decreased active range of motion in flexion, or a limitation to the reversal of the lumbar lordosis while performing a flexion movement. Spring testing or other joint mobility assessments may reveal decreased mobility at thoraco-lumbar segments and / or result in the provocation of pain symptoms. Patients may report difficulty with sitting or driving or with other flexion-biased tasks, such as squatting or reaching for dressing activities (ie: donning / doffing shoes). Physical therapy literature has demonstrated posi-tive effects of lumbar ‘gapping’ mobilization / manipula-tion in cases of low back pain – including pain reduction and improved multifidus muscle recruitment.1,2 Other studies have demonstrated decreased thoraco-lumbar mobility with mechanical ‘constraint’ to the ribcage and thoraco-lumbar region (as would be present in a patient with an acquired hypo-mobility of the thoraco-lumbar junction)3, as well as improvements in pain symptoms for patients with back and proximal leg pain after adding manual therapy for the thoraco-lumbar region to a program that had previously resulted in limited positive response after lumbar-specific mobilization.4

Position: The patient is positioned in quadruped on an exercise mat or a low treatment table. The thera-pist stands to the side of the patient, facing cranially. The therapist’s mobilizing hand is positioned so that their thumb and flexed 1st digit are over the transverse processes of the superior vertebra of the segment to be mobilized (Fig 1).

instruction: The patient is instructed first to flex the thoraco-lumbar spine by arching their back ‘like a cat’ while actively performing a posterior pelvic tilt (Fig 2). During this movement, the therapist takes up soft-tissue slack by providing a flexion force at the targeted spinal segment. The patient is then directed to slowly sit back onto their heels, as attaining a yoga ‘Child’s Pose’ (Fig 3). During this movement, the therapist maintains a flexion force (cranially) at the superior vertebra of the target segment, encouraging further flexion at the segment. The patient then returns to the starting position while the therapist maintains hand position without any force. This action is repeated several times. This sequence may be repeated at several different spinal segments.
