First of all, I wish to congratulate the workshop organisers particularly,the Organising Chairman and President of the Malaysian Society ofGastroenterology and Hepatology 2008/9, Dato’ Dr Muhammad Radzi and theCourse Director, Professor Dato’ Dr K L Goh for organising this year’stherapeutic endoscopy workshop – Endoscopy 2009 with the theme – ‘Exploringthe Limits of Endoscopy’. As with previous years, “Endoscopy 2009” promises tobe a highly beneficial and exciting course and I wish to welcome all participants.
I wish to specially welcome our international faculty who are all top world classendoscopists: Professor Jerome Waye from New York, USA, Professor KulwinderDua from Milwaukee, USA, Dr Ichiro Yasuda from Gifu, Japan, Dr HirofumiKawamoto, Okayama, Japan, Professor Lee Yong Chan from Seoul, Korea andDr Amit Maydeo from Mumbai, India. It is a great honour for the University ofMalaya to have you come and participate in this workshop.

I am proud once again that the Faculty of Medicine and the University MalayaMedical Centre in collaboration with the Malaysian Society of Gastroenterologyand Hepatology is organising yet again an important endoscopy workshop.This continues with the tradition of organising such top class endoscopyworkshops since 1993 and I wish to congratulate the organisers.
I wish to welcome our distinguished guest faculty from overseas in particular,Professor Jerome Waye, who will deliver the Distinguished Lecture this morning.I am also honored to welcome our other guest faculty, all famous namesin their own right: Professor Kulwinder Dua from Milwaukee, USA,Professor Lee Yong Chan from Seoul, Korea, Dr Ichiro Yasuda from Gifu, Japan,Dr Hirofumi Kawamoto from Okayama, Japan, Dr Yasushi Sano from Kobe,Japan, and last but not least, Dr Amit Maydeo from Mumbai, India. I am alsoinformed that a special highlight this year is the live transmission of cases onNatural Orifice Transluminal Endoscopic Surgery (NOTES) from Hyderabad,India.
I am also extremely proud that the GI Endoscopy unit has been awarded the“Center of Excellence” award by the OMED (World Organization of DigestiveEndoscopy) and am pleased that Professor Jerome Waye, President-Elect of theOMED will officially present the award to us at this meeting. It is a testimony tothe untiring efforts of the unit in achieving and maintaining the highest standardsof practice and I congratulate Professor Dato’ Dr K L Goh and his team.

I wish a warm welcome to all the participants, faculty members and participatingbiomedical industries. Without the participation of the BMIs, we would not havebeen blessed by an astounding and mind boggling array of equipment, tools andmachines. I would like to especially welcome the members of the faculty fortaking time off their busy and packed schedule to attend this EndoscopyWorkshop. The presence of the participants is a show of strength of theircommitment and a wonderful gesture to our Society. The presence of legends inendoscopy among our participants, speak well of the support and importance ofthis Endoscopy Workshop in the scheme of things.
Having said that, organising a world class Endoscopy Workshop is a tough taskand I must congratulate Professor Dato’ Dr K L Goh and his team for puttingtogether this event year after year and improving it every time. They have beenputting together a very high standard and internationally recognised workshopfor many years and I am sure this year will not be different. This year’s Endoscopy2009 marks another important milestone as it is endorsed by OMED WorldOrganization of Digestive Endoscopy and American Society of Gastroenterology(ASGE). The endorsement by these two highly prestigious institutions show therecognition we have achieved from our fellow international colleagues andI know that we are as proud as Professor Dato’ Dr K L Goh and his team of thismomentous achievement. I think Professor Dato’ Dr K L Goh and his teamdeserve all the credit due to them for this.
In our busy schedule and work, it is sometimes impossible to have first-handexperience at close quarters and exposure to groundbreaking techniques overseas.One of the main aims of the Endoscopy Workshop is to bridge this gap by invitingthe giants of endoscopy to share their skills and experience with us and thereforebringing the techniques closer to home. This also gives us the chance to interactand learn from them personally, and I hope this golden opportunity will not bewasted.
In addition to a comprehensive programme which will cover extensive aspects ofendoscopy, an emerging technique called NOTES will be the highlight of ourEndoscopy Workshop. To put icing on the cake, this technique will be transmittedlive from Hyderabad by the well-known Dr Nageshwar Reddy and Dr G V Rao.This technique was chosen to complement the theme of this year’s workshopwhich is “Exploring the Limits of Endoscopy”. It is my hope that we willcontinue to embrace new techniques and technologies.
Lastly, I must thank the UMMC team again for the effort and skill in organisingthis workshop and not forgetting the industries which have been untirelesslysupport our educational programmes and helping us push the limits of endoscopy.In view of the many exciting cases being planned over the next two days,I hope that we can all sit back, enjoy the show and bring back learning memoriesfrom this event.

Welcome to Endoscopy 2009 with the theme “Exploring the Limits ofEndoscopy”.
We are indeed delighted and honored to have a distinguished faculty. It is truly aprivilege to have Professor Jerry Waye, one of the pioneers and a “giant” incolonoscopy, deliver the University of Malaya / MSGH Distinguished Lecture.In addition, we have Professor Kulwinder Dua from Milwaukee, USA,Dr Amit Maydeo from Mumbai, India, Professor Lee Yong Chan from Seoul,Korea, Dr Hirofumi Kawamoto from Okayama, Japan, Dr Ichiro Yasuda fromGifu, Japan, and Dr Yasushi Sano from Kobe, Japan, who are all,expert endoscopists in their own right.
We have planned an exciting program with live cases showcasing the bestendoscopic cases which will be interspersed with practical lectures.This year, we will demonstrate the use of several new modalities including thenarrow band imaging and autofluorescence.
This year’s extra highlight will be the live transmission of cases on NOTES(Natural Orifice Transluminal Endoscopic Surgery) from Hyderabad, India, byDr Nageshwar Reddy and Dr G V Rao. This will be the first time such procedureswill be shown in Malaysia!
The GIAs have their own special workshop on 20th March. Our guest faculty forthis event will be Dr Ang Tiing Leong (Singapore) and Dr Ichiro Yasuda (Japan).The program promises to be both informative and “fun” for all participatingGIAs and the day will end with the usual gala evening event, only for the GIAs.

OBJECTIVES AND FORMAT OF WORKSHOP
Emphasis on therapeutic endoscopy: technique, indications limitations and pitfalls. Procedureswill include difficult ERCPs with placement of new types of metallic stents, pancreaticendotherapy and transnasal endoscopy. We will show the whole array of new sophisticateddiagnostic modalities including NBI and autofluorescence scopes with HDTV and wide anglecolonoscopy and again demonstrate techniques of detection of early colorectal cancers andendotherapy of these lesions.
GIAs are invited to participate in all sessions. A specially designed GIA symposium will be heldon 20th March 2009, Friday.

Natural Orifice Transluminal Endoscopic Surgery
(NOTES) is a new experimental surgical technique
whereby “scarless” abdominal operations can be
performed with an endoscope passed through a
natural orifice (mouth, urethra, anus, etc) then
through an internal incision in the stomach,
vagina, bladder or colon, thus avoiding any
external incisions or scars.
NOTES was originally described in animals
by researchers at Johns Hopkins University by Dr Anthony Kalloo and
colleagues. The first NOTES procedure on human beings – a transgastric appendectomy was
performed by the team in the Asian Institute of Gastroenterology (AIG), Hyderabad, India,
headed by Dr Nageshwar Reddy and Dr G V Rao.
The AIG is an outstanding GI centre recognised as one of the top centres in the world. It has
received many accolades for their outstanding work in gastroenterology and GI surgery including
the “OMED Center of Excellence” award.

Professor Lee is an expert endoscopist specializing in upper gastrointestinaltract. He has worked throughout his career at the Yonsei University College ofMedicine obtaining his PhD in 1998. From 2000-2003, he worked in ProfessorMartin Blaser's laboratory in New York University, USA on various aspects ofHelicobacter pylori infection. Dr Lee spent several of his school going years inKuala Lumpur.

Professor Jerome Way, teacher and colonoscopist par excellence is truly alegend in his own time. A long time Professor and Chief of Endoscopy in thefamous Mount Sinai Hospital, New York, Jerry Waye is a household namethroughout the world of endoscopy. Professor Waye has held many importantposts in the American College of Gastroenterology, American Society of GIEndoscopy and is currently President-elect of the World Organization ofDigestive Endoscopy (OMED). Professor Waye is Distinguished Lecturer forEndoscopy 2009.
KULWINDER S DUA (Milwaukee, USA)
Professor, Medical College of Wisconsin, Milwaukee and Director, Diagnostic andTherapeutic Endoscopy, Froedtert Memorial Lutheran Hospital,Medical College of Wisconsin
Professor Dua is well known for his expertise in esophageal stents where hehas devised an anti-reflux self expandable metallic stent which bears his name– the Dua stent. He has performed excellent research in this area. In addition,he is also an expert in ERCPs. He is Chief of Endoscopy at the FroedtertMemorial Lutheran Hospital, Medical College of Wisconsin and is a wellrespected teacher and therapeutic endoscopist throughout the USA.
ICHIRO YASUDA (Gifu, Japan)
Associate Professor, Gifu University and Consultant Therapeutic endoscopist, Gifu University Hospital,
Dr Yasuda belongs to the long line of expert younger set of Japaneseendoscopists. An alumnus of the famous Endoscopy unit at Hamburg underProfessor Soehendra is an expert in ERCP and Endoscopic ultrasound (EUS)and is head of the pancreatobiliary section in his hospital.

Dr Kawamoto is an expert in ERCP where he has special interest and expertisein the use of metallic stents for biliary strictures. He worked in Hiroshimabefore moving to Okayama in 2002 where he is now the head of the biliarysection at the Okayama University Hospital
AMIT MAYDEO (Mumbai, India)
Head, Department of Endoscopy and EUS, Jaslok Hospital and Research Center,Mumbai India
Dr Maydeo is a “man for all seasons”. An expert in so many aspects oftherapeutic endoscopy, Amit trained initially as a surgeon. Following thefootsteps of his mentor professor Nib Soehendra he now dedicates himselfpurely to endoscopy. He was one of the earliest endoscopy fellows in Hamburgin the late 1980's. He now runs a top class therapeutic and research unit at theJaslok Hospital in Mumbai.
YASUSHI SANO (Kobe, Japan)
Director of Gastrointestinal Center, Sano Hospital, Kobe, Japan
Dr Sano is an expert in colonoscopy and colorectal diseases particularly in thediagnosis and treatment of early colorectal tumors. He worked previously inthe National Cancer Centre East, Chiba prefecture and is now working inprivate practice in the Sano Hospital in his hometown of Kobe, Japan. He isfamous for his skills in endoscopic submucosal dissection (ESD) where he isconsidered a true world’s expert.

It is my pleasure to introduce Jerry Waye who is our distinguished speaker forthe MSGH Special Lecture in Endoscopy 2009. Jerry is a person who does notneed much of an introduction seeing that his reputation far preceeds him and I dare say there is little or none of us that have not heard of him before.It is indeed our privilege to get a chance to hear him speak and he will be talkingto us about “Exploring the Limits of Endoscopy” – the theme of the Workshop.
Jerry is one of the pioneers of endoscopy and one cannot study endoscopywithout coming across his name or read any groundbreaking endoscopy paperswithout seeing his name. Being one of the giants of endoscopy, there is no otherbetter man to give us this lecture on the limits of endoscopy.
When I was given his curriculum vitae which was probably an abridged version, it shocked me that itwas 51 pages. If I recall correctly most of the CPGs that I know of, are less than 51 pages. This speaksvolumes of his experience and knowledge. Condensing this into a citation is a monumental job andI hope, I did not do him any injustice during the next few minutes.
Jerry graduated from the Boston University School of Medicine, MD (Cum Laude) and started hisinternship nearly half a century ago at Mount Sinai Hospital, which was already a world renownedhospital at that time and completed his gastroenterology fellowship four years later. As we alreadyknow, he subsequently excelled himself in the field of endoscopy and is a member of no less than20 professional societies, of which he became the President / Chairman of six of them. Currently, he isthe President-Elect, World Organization of Digestive Endoscopy (OMED). He is also actively involvedin 30 workgroups and affiliated to 15 medical journals. A man of his stature is never short of awardsand the lists of awards bestowed upon him currently stands at 18 with more to come, I am sure.
Needless to say, it is unsurprising that a man of his standing would have 243 published articles,59 abstracts, 9 books, 69 chapters, 4 books in ancillary publications, 44 ancillary articles and have given251 lectures and courses. I am sure that there will be more publications to come this year and the next.
Currently, Jerry is the Clinical Professor of Medicine of The Mount Sinai Medical Center and alsoChief of Gastrointestinal Endoscopy Unit, The Mount Sinai Hospital. It gives me great pleasure indeedto welcome Jerry to Malaysia and to invite this great man to give us the lecture.
