Introduction

This toolkit was designed with the audience intended for use kept in mind. In today’s era of healthcare, many providers are working overtime to keep up with the demands of their daily schedules and large patient panels. The idea of taking extra time from one’s busy schedule to read many pages of information surrounding the use of a new toolkit may not seem realistic or desirable to a busy healthcare provider, thus, we have attempted to make this toolkit as user friendly and efficient as possible for the busy healthcare provider caring for school aged asthmatic patients. With high efficiency and ease and simplicity of use, it is hoped these qualities will increase provider interest for education on and implementation of this toolkit within their daily practice. The purpose of this toolkit is to provide resources for healthcare providers and school nurses that will support them in improving pediatric patients and their caregiver’s asthma self-management skills, awareness of environmental risk factors, and preventing and managing asthma attacks. With improvement in these realms, it is hoped childhood absenteeism in the Manchester, NH school system will decrease and overall quality of life for asthmatic patients and their families will be improved.

Initial Survey

A thorough exam should be done on initial visit. Example of what initial visit should entail:

New Encounter

The Patients will then be given the same survey at 6 months, 12 months and 2 years and comparison of their scores will be made.

Re- Evaluation

The Practitioner is to obtain initial screening on all patients with asthma before initiating the tool kit. This will provide a base line score and help indicate if the patient is benefitting from the toolkit.

5 Question Asthma Assessment

1 The 5 Question Asthma Assessment should be used with every office visit in a patient with asthma. This will help guide the practitioner if the patients asthma is under control or changes need to be made.
