If the internal chemical indicator is visible, an external indicator is not needed. c. Do not use processed items if the mechanical (e.g., time, temperature, pressure) or chemical (internal and/or external) indicators suggest inadequate processing. d. Use biologic indicators to monitor the effectiveness of sterilizers at least weekly with an FDA-cleared commercial preparation of spores (e.g., Geobacillus stearothermophilus for steam) intended specifically for the type and cycle parameters of the sterilizer. e. After a single positive biologic indicator used with a method other than steam sterilization, treat as nonsterile all items that have been processed in that sterilizer, dating from the sterilization cycle having the last negative biologic indicator to the next cycle showing satisfactory biologic indicator results. These nonsterile items should be retrieved if possible and reprocessed.f. After a positive biologic indicator with steam sterilization, objects other than implantable objects do not need to be recalled because of a single positive spore test unless the sterilizer or the sterilization procedure is defective as determined by maintenance personnel or inappropriate cycle settings. If additional spore tests remain positive, consider the items nonsterile and recall and reprocess the items from the implicated load(s). g. Use biologic indicators for every load containing implantable items and quarantine items, whenever possible, until the biologic indicator is negative.”Guideline for Disinfection and Sterilization in Healthcare Facilities, 2008

How Mail-To-Lab Spore Testing Works:

Sign up for a mail-in test kit by contacting ATS. A custom kit will be shipped to your facility. Each spore is enclosed in a pouch, accompanied with a test form, Class 4 chemical indicator, and a return envelope.

Run the spore and chemical indicator strip in your sterilizer with your normal cycle. Complete test details on form. Mail the spore, chemical indicator strip and test form in the pre-addressed return envelope provided.

ATS, Inc. cultures the spore for 24 hours (for Steam sterilization) at which time results are posted online. If the test result fails (positive test) we immediately & confidentially contact the person who sterilized the test.

Are you confident your sterilizer IS sterilizing?“Biological indicators are the only process indicators that directly monitor the lethality of a given sterilization process. Spores used to monitor a sterilization process have demonstrated resistance to the sterilizing agent and are more resistant than the bioburden found on medical devices. B. atrophaeus spores are used to monitor ETO and Dry Heat, and G. stearothermophilus spores are used to monitor steam sterilization, hydrogen peroxide gas plasma, and liquid peracetic acid sterilizers. G. stearothermophilus is incubated at 55-60°C, and B. atrophaeus is incubated at 35-37°C. Steam and low temperature sterilizers (e.g., hydrogen peroxide gas plasma, peracetic acid) should be monitored at least weekly with the appropriate commercial preparation of spores. If a sterilizer is used frequently (e.g., several loads per day), daily use of biological indicators allows earlier discovery of equipment malfunctions or procedural errors and thus minimizes the extent of patient surveillance and product recall needed in the event of a positive biological indicator. Each load should be monitored if it contains implantable objects. If feasible, implantable items should not be used until the results of spore tests are known to be negative.”

Though a highly sophisticated tool, the Smart-WellTM incubator is easy to configure with its simple touch-screen interface. The incubator can evaluate up to ten Smart-ReadTM biological indicators independently, and contains one additional incubation cell for a positive control unit. Each biological indicator test result is automatically documented in a user-customizable printed report, and an alarm is sounded the moment that sterilization failure is detected. The incubator also contains an NIST-traceable thermometer marked at the appropriate incubation temperature to allow an operator to quickly verify proper operation.

For each incubated Smart-ReadTM indicator, the printout will show the incubation chamber number, the result of the Smart-ReadTM indicator, the incubation start time and date as well as the amount of time elapsed, plus the additional information programmed into the Smart-WellTM by the user, to include:
