Appendix A: Decision Trees for Differential Diagnosis

Note: Prepared by Michael B. First, M.D., Allen Frances, M.D., and Harold Alan Pincus, M.D.

The purpose of these decision trees is to aid the clinician in understanding the organization and

hierarchical structure of the DSM-IV Classification. Each decision tree starts with a set of clinical

features. When one of these features is a prominent part of the presenting clinical picture the

clinician can follow the series of !uestions to rule in or rule out "arious disorders. #ote that the

!uestions are onl$ appro%imations of the diagnostic criteria and are not meant to replace them.

The &s$chotic Disorders decision tree is the onl$ one that contains disorders that are mutuall$

e%clusi"e 'i.e. onl$ one disorder from that section can (e diagnosed in a gi"en indi"idual for a

particular episode). *or the other decision trees it is important to refer to the indi"idual criteria

sets to determine when more than one diagnosis ma$ appl$.
