Choosing a scenario

• Take note of risks but need to focus on the issues you want the team to manage

• Bigger is not necessarily better

• Intermittent problems and ‘Rising Tides’ are (usually) more complex

• Time of day, day of the week and date are all important

Barriers to learning

• Outcomes are commonly delayed and not easily attributed to a particular action

• Variability in the environment degrades the reliability of the feedback , especially where outcomes of low probability are involved

• Most important decisions are unique and therefore provide little opportunity for learning

• There is often no information about what the outcome would have been if another decision had been taken
