1 STATUTORYROLEOFTHEIMB
ThePrisonAct1952requireseveryprisontobemonitoredbyanindependentBoardappointedbytheSecretaryofStatefrommembersofthecommunityinwhichtheprisonorcentreissituated.
TheBoardisspecificallychargedto:(1)satisfyitselfastothehumaneandjusttreatmentofthoseheldincustodywithinitsprisonandtherangeandadequacyoftheprogrammespreparingthemforrelease.
(2)informpromptlytheSecretaryofState,oranyofficialtowhomhehasdelegatedauthorityasitjudgesappropriate,anyconcernithas.(3)reportannuallytotheSecretaryofStateonhowwelltheprisonhasmetthestandardsandrequirementsplacedonitandwhatimpactthesehaveonthoseinitscustody.
ToenabletheBoardtocarryoutthesedutieseffectively,itsmembershaverightofaccesstoeveryprisonerandeverypartoftheprisonandalsototheprison’srecords.
2 EXECUTIVESUMMARY
Mainjudgements
Onesentenceassessment
Considerableeffortshavebeenmadetoimprovetheprison,buttherateofprogresshasbeentooslowand,althoughstaffnumbershavereturnedtobenchmarklevels,thereremainproblemsaroundincreasingviolence,poorlivingconditions,perceivedunresponsivenesstoprisoners’concerns,lowlevelsofengagementineducationandconstructivepreparationforrelease.
Areprisonerstreatedfairly?Thetreatmentofprisonersregardingfairnessandequityismixed.AnindependentsurveyconductedinJanuaryfoundthat61percentofprisonerssaidthatmoststafftalkedtotheminarespectfulmannerbutonly28percentfeltthattheregimewasfair.Whilsttheprisonhasmadeimprovementstothepropertymanagement,complaintsandapplicationssystems,theresponsesareoftenstilltooslowandunclear.Drugtreatmentservicesarevaluedbyprisoners,buttherearestilltoomanydrugsgettingintotheprison.Thetrendofviolenceisupwardswithprisoneronofficerassaultsbeingmorethantwicethenationalaverage.Thementalhealthserviceisinadequateintermsofitsresourcesandcoverage.Areprisonerstreatedhumanely?PrisonersarenottreatedhumanelyinHMPBedford.Regularshortagesofbasicitemsremain,thereisoftenfartoolongbetweenkitchanges,pigeonsflyroundinsidethejailandthereareinfestationsofcockroachesand,morerecentlyrats,inmostareas.Thestruggletokeeptheprisoncleanisbeinglostandattimestheamountoflitterandrubbishlyingaroundisdisgusting.Thesearenotappropriateconditionsinwhichtodetainprisonersinthe21stcentury.

Despitenumerousattemptsatrefurbishment,theenvironmentofthesegregationunitissimplyappalling.Itisadungeon.Thetoiletsfrequentlyblock,therehasbeenaconsistentinfestationofcockroachesand,duringthesummer,therehasbeenaplagueofrats.Wefeelforcedtorecommenditsurgentclosureandrelocationtoanotherpartoftheprison.
Areprisonerspreparedwellfortheirrelease?Again,thepicturehereismixed.Whilstwestillhaveareasofconcern,therehavebeenareasofimprovement.Overallattendancelevelsatsomeeducationclasseshavegotmuchbetterandanumberofexcellentnewteachinginitiativeshavebeenrun.However,forbasicskills,thepositionisnotgood,withmanyprisonersrefusingtoattend.Thepaintingworkshophasbeenreopenedandtheconstructionsafetyqualificationisverysuccessful.Thecoreworkshopactivitiesremainstultifyinglymundaneandonly18percentbelievedthatenougheffortwasbeingmadetostopthemoffendingonrelease,while71percentbelievedthattheywere‘doingtime’,ratherthan‘usingtime’.Thepre-releaseboardworksextremelyhardtotrytoensurethatprisonersleavewithaccommodation,supportandwherepossible,employment.However,itisnotwell-integratedintomainstreamprisonoperationsandcommunicationbetweenpre-releasestaffandoperationalstaffisoftenpoor.NationalconstraintsmeanthatinadequatepreparationhasbeenmadeforthepaymentofUniversalCredit. MainAreasforDevelopment
TOTHEMINISTERi) TheUniversalCreditsystem,ascurrentlyproposed,willresultinprisonersonly
beingabletomakeanapplicationonreleaseandthenhavingtowaitafurthereightweeksforpayment.Thisisscandalousandcompletelyinefficient.Wecannotseeareasonwhyprisonerscannotbepermittedtomaketheinitialapplicationswhilstinside(12.8and12.9).
ii) InourviewtheMinistershouldundertakeafundamentalreviewoftheroleoflocalprisons.Theyareclearlythemostdisturbedandunrulyestablishmentsinthesystemanditisimportanttounderstandwhythisisthecaseandwhatcanbedonetoaddresstheproblemsthattheygenerate(3.5-3.7).

Accommodationandfacilities
3.1 HMPBedfordisaCategoryBPrisonwithanOperationalCapacityof487(310beingtheCertifiedNormalAccommodation).Thecapacityhasbeenreducedfromthepreviouslevelof508becausethatincludeddoublecountingofthecellsofhealthcareandthesegregationunit.Asabusy‘local’(nowResettlement)prison,HMPBedfordtakesbothsentencedandremandedprisoners,mainlyfromcourtsinBedfordshireandHertfordshire.
3.2 Itisimportanttorecognisethattheprison,initscurrentguise,wasdesignedforacapacityof310–anythingabovethatrepresentsovercrowding.Theprisonisthereforeovercrowdedandhasbeenformanyyears.
3.3 WingsA,BandCradiatefromacentralhubandwerebuiltin1845:§ Awingholdsupto146prisoners(allindoublecellsbuiltforoneman,withthe
exceptionof34cellsfurnishedassinglecellsforhighriskprisoners);§ Bwingholdsupto76prisoners,indoublecells.FloorsB2–B4areabove
ground,whilstB1istheSupportandSeparationUnit(SSU)withsixfurnishedandtwounfurnishedcells,aseparateexerciseyard,showerandadjudicationroom;
§ CWinghas99places.Aboveground,C2andC3consist,predominantly,oftwo-mancells,butincludealsotwofour-mancells.ThereisadedicatedcellonC2for‘listeners’(prisonerswhogiveuptimetolistentotheconcernsofothers)andadedicatedcellforadisabledprisoner.C4remainsasanormallocation.Underground,C1accommodatesthelibrary,threeover-flowcellsforthesegregationunit(SSU)andafurtherfivetwo-mancell,currentlyusedforvulnerableprisoners.
§ Ewing,with41beds,isthefirstnightcentrewherenewprisonersarelocated.3.4 Thefollowingfacilitiesareaccommodatedinseparatebuildingsonthecongestedsite,
withinaperimeterwallthatiscompromisedbytheproximityofcommercialandresidentialbuildingsontheoutside:
§ Fwingwasbuiltin1812andhousesa30-placeVulnerablePrisonerUnit;§ Dwingwasbuiltin1992andhousesa95-prisonerDrugTreatmentwing;§ TheHealthcareCentre,builtin1992,housesanInpatientUnitonthefirstfloor
comprisingninenormalcellsandagatedcell.Consultingrooms,treatmentroomsandapharmacyareonthegroundfloor;

§ Thegatehouse,builtin1992,housessomeoffices,aVisitsCentreandHall,andtheReceptionsuite;and
§ Inaddition,thefacilitiesincludealargemulti-faithroomandchaplaincyoffices,gymnasium,educationsuite,laundry,kitchens,teachingworkshops,maintenanceareaandanadministrationsuite.
Prisonersandlengthofstay
3.5 Asalocalprison,HMPBedfordservicesthecourtsofLuton,andStAlbans.Onoccasions,prisonersarealsobroughtfromLondoncourts.WhatthismeansisthatHMPBedfordaccommodates,predominantly,shortstayprisoners.Thisiseitherbecausetheirsentenceisshort(lessthan12months),ortheyaremovedontoamoreappropriatefacility.Typically,around80percentofprisonersstayforlessthansixmonthsandtheaveragestayisunder30days,orthereabouts.Thisimpactsontheopportunitiestocarryouteffectiveinterventions.
3.6 Itisalsoworthnotingthataroundathirdofprisonersareonremandandsubjecttoaslightlydifferentsetofrules(forexample,theydonothavetoattendeducationorworkshops).
3.7 Theshortlengthofstaymeansthatthereisahighturnover–duringtheyearofthereportover3,000inmateswentthroughtheprison(someonmorethanoneoccasion).
4 NOTESABOUTREADINGTHEREPORT
RiotofNovember20164.1 TheriotofNovember2016was17monthsago,butithascastalongshadow.Afterthe
riot,AandBwingswereunfitforuseandthecapacityoftheprisonwasreducedtoaround250.B-wingreopenedinJuly2017andA-winginSeptember.ItwasnotuntilDecemberthattheprisonwasoperatingatfullcapacity.WehavethereforeconcentratedondataandimpressionsfromDecember2017toJune2018.
Qualityoflifesurvey4.2 Between15thand18thJanuarytheNationalPrisonandProbationServicecarriedouta
surveyofprisoners’viewsinHMPBedford–itiscalledMeasuringtheQualityofPrisoners’Lives(MQPL).Thesurveyasked130questionsto119prisoners.Duringthereportwewillrefertothissurveyandusethefindingstoillustrateparticularpoints.Asummaryofthefindingsisshowninthegraphbelow

HMIPUrgentNotification4.3 ThisreportcoverstheyearfromJuly2017totheendofJune2018.Thesubsequent
collation,writingandreviewperiodmeans,inevitably,thatthereportispublishedafewmonthslater.Normallythisgapbetweentheendofthereportingperiodandpublicationdoesnothaveanyimpact.
4.4 However,sincetheendofthereportingperiodtheprison,inmanyways,seemstohavegonebackwards.Ourrotareportshavemoreoftenexpressedconcernaboutafebrileatmosphereandashortageofvisibleofficers.Onoccasionsithasnotbeenclearifofficersareinfullcontrol.Inaddition,thelitterandverminproblemhasgotworseandtherehavebeenfurthershortagesofbasicmaterials.MembersofanotherIMBwhovisitedtheprisoninAugustwereshockedatwhattheysaw.
4.5 LevelsofviolencecontinuetobeworryingandinSeptemberanincidentledtofourofficersbeingtakentohospital.Thecausesoftheseissuesarenotfullyclear,buttheshortagesofbasickit,inexperienceofstaff,coupledwithsummerleaveandtheuncomfortablenessoftheprisoninthehotweatherwerecertainlycontributoryfactors.
4.6 ItwasnotsurprisingtousthattheHMIPinspectioninSeptemberledtoanUrgentNotificationtotheJusticeSecretarybeingissued.Wehopethatthiswilladdresstheproblemsnotedinthisreport.Therehavebeenimprovementsinanumberofareasandwehaveseenadeterminationtobuildarehabilitativeculture.Butthishasbeencontinuallyunderminedbyinstabilityintheregime,inexperiencedstaffandafailuretoprovideaconsistentlydecentenvironment.Withsomereluctance,theBoardwouldthereforehavetoconcurwiththeissuingoftheUrgentNotification.
Evidencesections5–135 SAFETY
Violence5.1 Increasingviolenceinprisonsisanationalproblem.InHMPBedfordthenumberof
incidentsbetweenJune2017andMay2018almostdoubled(59to102).Incomparison,thenationalincreasewas13percent.Thefiguresinthefirsthalfof2018showsomeflatteningoutoftheupwardtrend,stabilisingataround50percenthigherthanthecorrespondingmonthsof2017.Thisisshownonthegraphbelow:

2018andaslightdecreaseinmorerecentmonths(20incidentspermonthdownto15byJuly2018).
5.3 For‘prisoner-on-staff’assaultsthepatternisdifferent.TherewasamarkedincreasefromJanuarytoFebruary2018,withamorethandoublingofthemonthlyfigures(9to21).TherewasthenaflatteningoutatthishigherfigurethroughMaytoJuly.Thepercentageincreaseoverthewholeyearwasagainaround50percent,comparedwiththenationalfigureoflessthan25percent.Theaveragenumberofincidentswasabout2.5timesthenationalfigure.
5.4 Inordertoaddresstheseproblems,thereisacurrentViolenceReductionStrategy(publishedinNovember2017)butthefigurescitedabovesuggestthatithasnotbeeneffective.Attemptstounderstandandreduceviolentincidentshavealsobeenrepeatedlydiscussedinthe‘SaferCustody’and‘UseofForce’meetings.Again,theseseemtohavehadlittleimpactonoutcomes.
5.5 Theprisonhasputmostofitsfaithinreducingviolenceinthedeploymentofvariousgeneraltrainingpackagesforstaff,forexamplethe‘SuicideandSelfHarm’(SASH)and‘FiveMinuteIntervention’(FMI)programmes.Althoughthesehavebeenwell-attendedandappeartobewell-constructedanddelivered,thereislittleevidencethattheyhavehadanyeffect.
5.6 Thismaybebecausethesetrainingpackagesaretoogeneralandnotfocusedenoughonunderstandingeachincidentandthecommonfactorscontributingtoit(knownasa‘conflict-centred’approach).Weunderstandthattheprisonmanagersintendtoadoptthisapproachinthefutureandwewouldwelcomesuchadevelopment.
5.7 Managershavealsobeenhamperedintheanalysisofviolentincidentsbytheconsistentfailureofindividualofficerstoproducedetaileddescriptionsofincidentsaspartoftheirroutinerecording.
5.8 Fromthedatapresentedabove(andtheanalysisundertakenintheSaferCustodymeetings)italsoseemsthattheremaybesomespecifictrainingneedsfornewofficersinrelationtospecifichigh-risksituations(forexample,gettingprisonersbehindtheirdoors).Theywillthenneedappropriatementoringtoembedthislearningandtograduallyincreasetheirconfidenceandskills.Thedearthofexperiencedofficersonthewingshasundoubtedlymadethismoredifficult.
5.9 Finally,therehasbeenlittleattempttoinvolveprisonersthemselvesinthepreventionandmanagementofviolence.Thisseemsoddgiventheirwealthofexperienceanddesiretoinvestinasafeenvironment.Theywouldalsoseemtohaveasgreatavestedinterestasanyoneinmakingtheprisonasaferplace.
Reception5.10 Wehavefollowedprisonersthroughthereceptionprocessandourobservationisthat
itiseffective,caringandwell-managed.Whilstrecentrefurbishmenthasprovidedgreaterprivacyforprisoners,itisstillanuncomfortableandcrampedspace.Theinstallationofairconditioningunitshasdonelittletolowerthetemperature.Giventhelimitationsofthebuilding,smallimprovementslikeafewfansandaworkingwatercoolerwouldgosomewaytowardsmakingtheenvironmentmorecongenial.
5.11 Unfortunately,theFirstNightCentre(E-wing)isnotoperatingasitshould,withtheresultthatprisonersoftengodirectlyontothemainwings.Thismeansthattheydonotgetthebenefitoffocusedinductionservices.TheimpactofthisisshownbytheMQPLresults:
§ 49percentfeltworriedandconfusedwhentheyfirstcameintoprison;§ Only20percentfelttheinductionprocesswaseffective;and

§ Only12percentfeltthatstafftookapersonalinterestinthemintheirfirstfewdays.
5.12 Wealsohaveconcernsaboutthetransportarrangementsforprisonerscomingintotheprison,assummarisedbythisrotareportextract:InspectedtheSercotruckandhadachatwiththedriver.Thesetrucksarereallycrampedandclaustrophobic.Ifaprisonerneedsrestrainingitseemsnearlyimpossibleinthesecrampedquarters(imagineaplanetoiletandyougettheidea).Thereisnoappropriatestoragespaceforproperty,thereisonetinycupboardwithadoorwidththatmakescramminganythinginimpossibleandasmallnettedareanearthestairs.Iwastoldthatthedriverisoperatingsolowithpotentiallydangerouspeopleinthetransport,butthatwasok,becausetheonlythingtheycandoisverballyabusethedriver.
Self-harm(ACCT)5.13 Themainmechanismthatisintendedtoreduceactsofself-harmandsuicideisthe
applicationofthe‘AssessmentCareinCustodyandTreatment’(ACCT)procedure.LastyearwewerecriticaloftheoperationoftheACCT;itisseenasessentiallya‘form-filling’exercise,ratherthanaprocedureaimedatmakingdistressedprisonersfeelsaferandlesslikelytoself-harm.Webelievethatthiscriticismremainsvalidformostprisoners.
5.14 AnexaminationoftenrandomlyselectedCAREMAPsfoundthatthecontentofCAREMAPSwasverylimitedandtheoutcomewasusuallyrestrictedtosimplyadjustingthefixedintervalsforobservation.Theyrarelycontainedinformationaboutpersonalcopingstrategiesorhowthesecouldbesupportedandintegratedintoanindividualisedplanforreducingrisk.
5.15 WewouldthereforewelcomerenewedattemptstoimprovethecontentofCAREMAPssothattheyreflected,moreaccurately,theexperiencesandcopingstrategiesofindividualprisoners.ThismeansidentifyingspecificactionstoaddresstheissuesidentifiedduringtheACCTassessment.GreaterinvolvementofthementalhealthteamintheassessmentandreviewofACCTdocumentswouldundoubtedlyhelpinthisprocess.
5.16 IntermsofthegeneralapplicationoftheACCTandoftraining,wewouldmakethefollowingobservations:
§ TheSuicideandSelfHarm(SASH)trainingfocussesonan‘actuarial’modelofriskassessment(thatis,identifyingwhetheraprisonerbelongstoaparticular‘atrisk’group),whichdoesnothelpinthejudgementastowhetherthisindividualprisoneris‘atrisk’ataparticulartime;
§ AnyevaluationoftheACCTshouldthereforeincludeseekingtheprisoner’sviews;and
§ PrisonersthemselvescouldcontributetothetrainingofofficersregardingapplicationoftheACCT,perhapsthroughjointworkshopswithstaff.
Drugsupply,useandtreatment5.17 Despiteconsiderableeffortstoreducethesupply,drugavailabilityandusageremains
asignificantprobleminthejail.BetweenJanuaryandApril2018,theprisoncarriedout90randomdrugtests(MDTs)ofwhichaquarterwerepositive.However,thenationaltestingguidancemeansthatthisislikelytounderestimatetheposition–testrefusalsarenottreatedaspositivetestsbutreplacedbyanother,whichmeansthatthestatisticalrandomnessofthesampleisskewed.Theprisonsimplydoesnotknow,withanyaccuracy,howmanyprisonersaretakingdrugsatanyonetime,butitiscertainlylikelytobewelloverathirdandprobablynearlyahalf.
5.18 Themajorityofdrugs(andphones)comeinoverthewall,whichisremarkablyeasyinatowncentreprison.Whilsttheprisonhasputastrongfocusonaddressingthis,andhas

adetailedactionplaninplace,itisdifficulttosayhoweffectiveithasbeen.Acapitalinvestmentinnewwindowgrilleswouldhaveasignificantimpact,butdespiterequestsbytheprison,andinourreportlastyear,HerMajesty’sPrisonandProbationService(HMPPS)hasnotprovidedthenecessaryfinance.
5.19 AnotherpointofentryforcontrabandisthroughtheVisitsHall–itisalmostimpossibleforofficerstomonitorallinteractionsinenoughdetailtobesuretostopanythingbeingpassedover.Dogsareusedtomonitorvisitors,buttheyarenotalwaysavailable–forinstance,inMay2018snifferdogswereonlyavailableononeoccasion.
5.20 Spiceandotherpsychoactivesubstances(PS)haveahighnationalprofileandcertainlycreaterealissues.However,theMDTtestsonlyshow7percentofresultsatHMPBedfordasbeingpositiveforPS.Therehasbeensomeevidencethatmarijuanaisbecomingmorepopularandthattobacco,whichwasbannedinJanuary,isvyingwithothersubstancesfortheillicitmarket.
5.21 Drugtreatmentiscarriedoutbyhealthcare(forprescriptions)andSORTT(supportingoffendersinrecoveryandtreatmentteam)whotrytodealwithpsychologicalimpacts.Itisadifficultjobcarriedoutbydedicatedindividuals,butsuccessisverydifficulttomeasure.Itisoftenaboutreleasingprisonerswithabetterunderstandingoftheimpactsandusingfewerdrugsmoresafely.Forprisonerswhodowanttogetoffdrugs,theservicesprovidearealopportunity.
Deathsincustody5.22 Therewasonedeathincustodyintheyearending31stJuly2018.Ofcourse,onedeath
istoomany,butitisasignificantreductiononthefourdeathsbetweenJuly2016andJune2017.
5.23 Wehaveexpressedconcernsaboutthecircumstancesofthedeath,particularlyinthecommunicationbetweenthementalhealthteamandtheofficerstaff.However,thecoronerfoundthatthementalhealthgiventotheprisonerwasappropriate.
Firstnightarrangementsandvulnerableprisoners
5.24 InJuly2016E-wingwasconvertedtobeaFirstNightCentre-wesupportedthisinourlastreport.However,theprisonhashadtoaccommodatemorevulnerableprisonersthanithasroomforinthe30bedF-wing.Theresulthasbeenthat10prisonershavebeenplacedonthesecondfloorofE-wingwhichmeans:
§ Thecapacityfornewprisonershasbeenreducedbyhalf;
§ OfficershavetotakevulnerableprisonersacrosstoF-wingeachday(andneedtoavoidmeetingotherprisonerswhomaybeout);and
§ TherehavebeenproblemsaroundinteractionswiththekitchenstaffwhoservefoodonE-wing.
5.25 Overallthesituationisunsatisfactoryandneedstobeaddressed.Visits5.26 Formanyprisonersmaintainingcontactwiththeirfamiliesisoneoftheirmost
importantpriorities;theMQPLresultsinJanuarysuggestedthatonlyaquarterofprisonersfeltthatstaffachievedthis.TheVisitsHallislargeandsuitableforpurpose,althoughtheair-conditioninginstalledlastsummerhasbeenunreliable-itwasnotworkingformuchofJuneandJulyandconditionsbecamevirtuallyintolerable.Thenewtablearrangementshavereceivedmixedreviewsfrombothprisonersandstaff.Theyofferpossibilitiesforintimacywhichisnotappropriateinapublicplace.
5.27 TheaccessramptotheVisitsCentrehasbeenunsafeforusebybuggiesandwheelchairsduetoslipperysurfacesandfoliageanddespiteourreports,littleattempthasbeenmadeatimprovement.

5.28 Thecafeisanimportantfacilityforthevisitshallthatalsoprovidestrainingforprisonersinserviceandbaristaskills.However,theprisonhashaddifficultyinprovidingenoughprisonersforthisusefulandinterestingworkopportunity–itisunclearwhythisis.
5.29 TheBoardwillbemonitoringtheoutcomeoftheFarmerReport(TheImportanceofStrengtheningPrisoners’FamilyTiestoPreventReoffending)andhowfamilycontactcanbeencouraged.Fourplannedfamilydays(10.00–15.00)didnothappenbecauseofashortageinstaff,whichismostdisappointing.TheOrmistonTrustareallreadytogowithenhancedchildren’svisits,especiallyduringschoolholidaysandhalf-term,butalwaysseemtobeheldupduetoindecisionorinconsistencyapplyingtherules.
5.30 FundinghasbeenreceivedfortheBreakingBarrierproject,whichsupportsfamilieswithinthecommunitywhosefathersareinprison.Therehavebeenalargenumberofreferralsandthereiscurrentlyawaitinglist.
Staffinglevelsandprisonerrelationships
5.31 Therecruitmentdrivefornewofficershasbeenasuccessandwecongratulatetheprisononthisachievement.Officernumbersarebackuptothebenchmarklevel(asdefinedbyHMPPS)of110.Mostoftheserecruitsarenewtotheprisonserviceandfromwhatwehaveseenoftenbringenthusiasmandafreshapproach.
5.32 However,thereisanexperiencegap,whichissometimesevidentinday-to-dayoperationsandinhandlingdifficultincidents.Webelievethereistheopportunitytobuildastrongcadreofeffectiveandcommittedofficers,butitwillneedaclearandwell-plannedapproach,withacrucialroleformoreexperiencedstaffasmentors.
5.33 Itshouldalsobenotedthattheseniormanagementteam,althoughverycapablyledbytheGovernorHelenClayton-Hoar,isalsorelativelyinexperienced.Thisisalwayslikelytoexacerbatetheverysignificantchallengesinherentinmanagingaprison.
5.34 Despitesometimeschallengingconditions,staffandprisonerrelationshipsare,generally,positive,whichwasrecognisedbyresidentsintheMQPLsurvey:71percentsaidtheygotonwellwithwingstaff;61percentthatstafftreatedthemrespectfully;51percentthattheyhadbeenhelpedbyamemberofstaffonaparticularissue.Wewouldalsoliketoputonrecordourrecognitionandrespectfortheworkofficersdo.
Fullcellsearch5.35 On9thand10thAprila‘fullcell’searchwascarriedoutinHMPBedfordby
approximately100externalofficers,dressedinfullprotectivegear.Mostcellswerethoroughlysearchedatleastonce–sometimestwice–andmanyprisonersweresubjecttoafullbodysearch.Thepurposewastolocateillicitobjects(phones,drugs,weapons)andthustoassistinthecreationofamorestableoperatingenvironment.
5.36 Whenwevisitedtheprisonafterthesearchweimmediatelystartedtohearnumerouscomplaintsfromprisoners,independentlyandindifferentwings.Theyrelatedtoallegationsofexcessiveuseofforce,nakedstripsearching(ratherthan‘half-and-half’asinguidance),damagetopropertyandthefactthatofficers’identificationwasnotvisible.Weadvisedprisonerstousetheformalcomplaintprocessandweunderstandthat12did.
5.37 Theprisonacknowledgedthattheofficers’identificationwascoveredbyprotectiveclothing.Otherwise,itsinvestigationsintothesecomplaintsdidnotfindanyevidencetosupporttheseclaims.Thiswasnotsurprisingasbodycameraswerenotwornandsomostofthekeyeventscouldthereforenotbeobserved.Recordsweremadeofthesearchesofindividualcellsandtheseweresummarisedintoarecordofwhatwasfound.However,nooverallreportoftheoperationstating,forexample,itspurpose,authority,timing,numberofstaffemployed,issuesencounteredandoutcomeswasproduced.Wefindthissurprising.

5.38 TheIMBunderstands,andindeedsupports,theprincipleofcellsearches.Inthiscasehoweverweareleftwithconcernsabouttheopennessandaccountabilityinthewaythatthiswasundertaken.
6 EQUALITYANDFAIRNESS
Diversityandequalitywithintheprison6.1 Inthe12monthssincethelastreporttherehasbeenverypositiveprogresswiththe
Equalitiesagenda,drivenlargelybytheGovernor-theEqualitiesActionTeammeetsmonthly.
6.2 Forumsforolder,younger,foreignnationalanddisabledprisonersaresuccessfullytakingplace,withpositivefeedbackfrombothprisonersandstaff.Issuesarisingfromtheseforumsarebeingdocumentedandfollowedupwherepossible;forexample,prisonersatthedisabilityforumraisedissueswithaccessforthosewithreducedmobilitytothelibrary,whichisbelowgroundThisisnowbeinginvestigatedtoseewhetheraccesscanbeimproved,orifalibrarytrolleyservicecouldbestarted.
6.3 Eachmonthamemberoftheseniormanagementteamtakesresponsibilityforpromotingawarenessofissuessuchasfaithawareness,LGBT,literacyanddyslexiaandanti-bullying.
6.4 Dataisnowbeingcollectedtorecordandtrack/monitortrendsinacoherentandaccessiblewayhighlightingareasofconcern.Examplesofthisare:
§ IEP(theincentivesandearnedprivilegessystem)levelsbeingmonitoredtohighlightpatterns/fairnessagainstpopulation;
§ AhighernumberofadjudicationswithBME(blackandminorityethnic)prisonersinitiatedaninvestigationthatfoundnoareasofconcernwiththeprocess;
§ Datacollectiononage/raceofthosemostcommonlyinvolvedinuseofforceincidents(young-white)-theresultsofthisfedbacktotheyoungprisonerforumanddiscussedfortheiropinions;and
§ DIRFs(discriminationincidentreportforms)-whilstlowinnumber,havebeentakingtoolongtoresolveandthisisnowbeingchased.
6.5 TheIMBhasreceivedanumberofcomplaintsfromprisonerswithregardtoaccesstoreligiousservices,particularlyforthoseinsegregation.Whilstitwouldappearthateffortsaremade,wherepossible,toenableattendance,itisoftenrestrictedbystaffavailabilityorsecurityissues.Wefeelthattheprisoncoulddomoretoensurethatallprisonerswereawareoftheprocedureforrequestingattendanceandthattheapplicationformsareroutinelyavailable.
6.6 WhilsttheGovernorisverycommittedtotheEqualitiesagenda,meetingsarenotalwayswellattendedwiththeresultsthatpeopleareoftenchasedupontheday.Inaddition,informationwhichhasbeenpromisedisnotalwaysdeliveredinatimelyfashionsothatissuesdonotcometolightassoonastheyshould.Thisallsuggeststhatthereisstillsomewaytogobeforetheimportanceofequalitiesisembeddedamongstallprisonstaff.
Complaints6.7 Althoughthepictureisnotconsistent,thegeneraltrendofcomplaintsisupward,as
shownonthegraphbelow.Inaddition,thelevelofcomplaintsinMarchandJunewashigherthanthe209onOctober2016,justbeforetheriotinNovember.

6.8 Theprisonhasputagreatdealofeffortintotheimprovementofcomplaintmanagement
anditdoesseemtobebearingfruit.Previously,performanceinformationwasnotevencollected.TheaverageperformanceNovember–Junewas71percentand,althoughwedonothavepreviousinformationforcomparison,ourexperiencesuggeststhatthisismuchbetter.However,thereisstillroomforimprovement.
6.9 Despitethisimprovementinresponsetimes,westillfindissuesaroundthequalityofresponse,whichisnotalwaysclear,writteninplainEnglishoraddresstheissue.Prisonersfindthesystembureaucraticanddifficulttoaccess.MostoftheapplicationstotheIMBaretheresultoftheprisonerfeelingthattheresponsetoacomplaintisunsatisfactory.
Boardmembership6.10 TheIMBhasbenefitedduringtheyearfromadiversemembershipintermsofgender,
age,nationalityandrace.Therehavebeen11membersofwhichthreearefromanethnicminoritybackground,sevenwerefemaleandfourwereunder40yearsold.
6.11 Unfortunately,wehavealsolostmembersduringtheyear–westartedwithseven,wentupto11andarenowdowntoeight.Atonestagewehadonlyfouractivemembers.Withsuchasmallnumber,ithasbeendifficulttoprovidementorsfornewstartersandtomanagecircumstanceswhereindividualmembershavehadtotaketimeoff.
7 SEGREGATIONANDSEPARATIONUNIT(SSU)
Physicalconditionsandclosure7.1 Despitenumerousattemptsatrefurbishment,theenvironmentoftheSSUremains
simplyappalling.Itisundergroundwithlimitedlightgettingintothecells,thetoiletsfrequentlyblock,therehasbeenaconsistentinfestationofcockroachesandduringthesummertherehasbeenaplagueofrats.Eachyearwehavecommentedonthedeplorableconditionsinthesegregationunit.Wenowbelievethatitcannolongerbeconsideredadecentandhumaneplacetoholdprisonersandrecommendthatitshouldbeclosedandrelocated
Operation7.2 Giventhedreadfulenvironment,theSSUactuallyworksremarkablywell.Thestaffare
highlyprofessional,well-motivatedandconsistentlypatientandhelpful.Fromourweeklyvisits,itisrarethatprisonersarenotgettingexercise,orashowerandeffortsaremadebystafftoprovidesomethingforthemtodo,evenifitisonlythe‘distractionpacks’.Therearesometimesproblemswithaccessingprisonphonesandwithmissingproperty,butwewouldsaythatthishasimprovedsincelastyear.Therearemorecomplaintsaboutthequalityandquantityoffoodcomparedwithlastyearandproblemswithprisonersnotbeinggiventheformstoapplytoattendreligiousservices.
7.3 Weattendnearlyalltheweeklyprisonerreviews.Amajorinnovationthisyearistheintroductionofamemberoftheadministrationstafftorecordthecontentofthesereviewsandbringthesenotestothenextmeeting.Thishasbeenveryhelpful.

7.4 Ingeneral,thereviewsarewellconducted.Prisonersaretreatedfairlyandwithrespectandtheircollaborationisroutinelysought.However,thepracticeofhavingadifferentGovernorattendeachweekdisruptscontinuity.WoulditbepossibleforGovernorstobenominatedtoattendthesegregationreviewsfor,say,amonthatatime?
7.5 MembersoftheHealthcareteamassiduouslyattendmostreviewseventhoughtheneedforphysicalhealthadviceisrare.InourexperiencethemajorityofprisonersinSSUhavementalhealthissuesand,regrettably,thementalhealthteamrarelyattend.Itwashopedbyconcentratingthereviewsononeafternoonperweektheirroutineattendancewouldbefacilitated.Thishasnothappened,andweremainconcerned.
7.6 .Thesegregationreviewsaretheplacewhereyouaremostlikelytoencounterthemostseverely,mentallydisturbedprisonersanditthereforefollowsthatthosestaffwiththegreatestknowledgeofmentalhealthissuesshouldalwaysbepresent.WethereforeurgetheGovernortoagreewiththehealthcommissionersthatthisshouldbeapartofthementalhealthcontract.
8 ACCOMMODATION(includingcommunication)
Generalconditions
8.1 TheBoardremainsconcernedaboutthechallengesofaVictorianprisonwithitsfailinginfrastructure.Planstoreplacethewindowsandupgradetheheating,whichhavebeenpromisedforyears,nowappeartohavebeenabandoned.Thismeansthatsectionsoftheprisonwillalwayssufferinveryhotandverycoldweather.Ill-fittingandbrokenwindowsarethenorm.
8.2 Portableheatersandfansareavailableforcertaincells,butthiscreatesasenseofinequalityamongstprisonersastheyhavenounderstandingofwhysomeprisonersgetfansorheatersandsomedonot.Theproblemisthattheelectricwiringdoesnothavethecapacitytosupportthesefacilitiesineachcell.Thisreflectsthelackofinvestmentinprovidingadecentenvironment.
8.3 Thehotwatersystemisbarelysufficientforitsneedsandprisonersoftencomplainofnotenoughhotwaterforshowers,or,conversely,thattheshowersareproducingonlyhotwater.Theboilerislikelytobreakdownatanytime.
8.4 TheMQPLsurveyindicatedthatthemajorityofprisonershadconcernsaboutthelivingconditions–53percentreportedthat‘thequalityofmylivingconditionsispoor’.Followingthisreport,theBoarddeviseda'BacktoBasics'surveywhichisregularlycompletedduringrotavisitstothewings,givingusanopportunitytoengagewithprisonersabouttheirconcernsregardingcleanliness,clothing,laundry,bedding,food,showersandsafety.Keyareasofconcernwerearoundthefrequencyofkitchange,notenoughblankets,smallfoodportionsandcockroaches.
8.5 Observationpanelsinthecelldoorsarecontinuallybeingbroken–weunderstandarearound30perweekarerepaired.Theyrepresentdangertostaffasprisonerscanspitandsquirtliquidthroughthemandthesmallpiecesofglassaroundtheedgesareadangertoprisoners.Thecostintime,moneyandriskissignificant.Wefeelthattheremustbeabettersolution.
8.6 ThecollapseofCarillionmadelifeveryuncertainfortheemployeesandthefutureoftheircontractwiththePrisonService.Thestaffareexcellent,buttheprocedureforprocessingtherequestsforworktobedoneismorecomplicated.
Cleanlinessandhygiene8.7 Thestruggletokeeptheprisoncleanisbeinglost.Whilstattimestheprisonappearsto
begettingontopofit,atothertimesitisreallyquitedreadfulanddepressing,withlitterstrewnallaroundtheoutsideareas.Timeaftertimeanewinitiativeisputinplace,with

somesuccess,buttheprisonverysoonrevertstoitspreviousstate.Unlessthiscanbeaddressedpermanently(andwedoacknowledgethatitisdifficult)itisgoingtobeveryhardtoengenderprideandrespectwithintheprisoner-andstaff-community.
8.8 Graffitihasstartedtoappearincellsthathavebeenrecentlyredecoratedandrefurbished.Thisisnotacceptableinplaceswhichare,toallintentsandpurposes,theprisoner'stemporaryhome.
8.9 SomerecentprogresshasbeenmadewiththeredecorationofHealthcare,SSUandD-wing.Theresidentpainter(aprisonerwhoseworkistobehighlycommended)willnotbeatBedfordformuchlongerandthereneedstobesomeotherprocessinplace.Thejetcleaninginitiatedbyapreviousgovernorhasnotbeencontinued.
8.10 Bedfordhasanongoingpigeonproblemrequiringsomeareastobenettedtopreventthebirdsflyingin,nestingandexcretingeverywhere.TheareaatthebackofBwingandthelibraryisparticularlyunpleasant.
8.11 TheIMBalsoreceivesregularreportsofcockroachesandothervermin.RatshavebeenaparticularprobleminFwingwherethewoodenplantersintheexerciseyardhavebeendemolishedandprisonersreporthearingtherodentsmovingaroundduringthenight.RatsandflieswerealsoreportedinthebasementoftheVisitsCentreinastoreroomcontainingdressuniformsandotherkit.
Housekeeping,kitchenandlaundry
8.12 Basichousekeepingisnotalwaysroutine.Thestoresdonotholdsufficientstockandtheorderingsystemispronetoperiodsoffailure.Recentlytherewasanothershortageoftoiletrollsandbasiccleaningmaterials,whichwasrectifiedbyanofficergoingoutandpurchasingdirectlyfromalocalsupermarket.
8.13 Washingmachinesanddrierswereoutofactiononsomewingsforseveralmonthsandtherewasadelayinbothmaintenanceandorderingreplacements.Thishasnowbeenrectified.Therehavebeenproblemswithreplacementclothesandtheissueofcleansheetsandblankets.
8.14 TherehavebeensomedifficultiesinthekitchenleadingtotheresignationoftheManagerafter22years.Mealsaresoimportantintheprisonthatitisimperativethatissuesareresolved,andnewstaffvettedandclearedpromptly.Potentialkitchenhelpers(prisoners)needtobespeedilyidentifiedandcleared.Generally,mealsareofagoodstandard,althoughrecentlytherehavebeencomplaintsaboutundercookedchicken.
8.15 TheBoardreceivesfewapplicationsaboutfood,althoughtheissueofthemainmealbeingservedlaterinthedayisexpressedfromtimetotime.AquestionnairewascirculatedtoprisonersinJune.Theresultsindicatedapreferenceforaneveninghotmealratherthanthelunchtimehotmealwhichiscurrentlybeingserved.Specialreligiousdietsarewellcateredfor.
Applications,communicationandphones8.16 Toobtainalmostanythinginprison–fromajob,aplaceineducation,ahealthcarevisit
toadditionalclothingorablanket-prisonersmustcompleteanApplication.Inourlastreport,wewerecriticalofthemyriadofunclearandconfusingforms.Wearepleasedtonotethattheprisoncarriedoutafullreviewandintroducedamuchmoresuccinctandfocussedsystemthatisaloteasiertouse.However,prisonersstillcomplainaboutApplications,beinglostorextremedelaysinreply.
8.17 StaffingintheBusinessHubhaschanged,includingtheretirementoftheGovernor'swell-respectedsecretaryaftermorethan30years.Thishasleftsomefunctionsneglectedforseveralmonths,leading,forexample,tolongdelaysintheprovisionofgoodsorderedthroughthecataloguesystem.

8.18 Theprisonwentsmokefreeon29thJanuary.Itistothegreatcreditoftheteamof‘smokefree’championsthatitwentsowell.Itwasachievedwithoutanygreatdramaandservesasagoodexampleofhowadifficultprojectshouldbedelivered.
8.19 TheBoardispleasedtonotethatthemonthlyprisonerforums(Fwingandmainwings)havecontinued.Thisisanopportunityforprisonerstoairproblemsconcernedwithaccommodationandtheregime.Theseareminutedandattendedbyseniormembersofstaff.Ourobservationisthattheyareworkingwell.
8.20 Prisonersreportthatstaffdonothavetimetojustlistentotheirconcernsandthenactuponthem.IntheMQPLsurvey82percentofprisonerssaid,‘togetsomethingdoneyouhavetoaskandask’.Theprisonhasstartedtointroducethekeyworkerscheme(nowcalledOffenderManagementinCustodyorOMiC)whichisspecificallydesignedtoprovideastructureforthesemeaningfulinteractions.Weapplaudtheworkoftheteamthathasorganisedthetrainingandimplementationofthepilotandlookforwardtoitbeingrolledoutacrosstheprison.Webelievethatitcouldmakearealdifference.
Property8.21 Althoughprisoners’propertyisincrediblyimportanttoindividuals,thesystems
surroundingitarecomplexandconfusing.ItremainsthecauseofthelargestnumberofapplicationstotheIMB.Mostissuesfallintothefollowingcategories:
§ Propertynottransferredfromapreviousprison;
§ Propertybroughtorsentinbyrelativesbutnotreceived;and
§ Propertymissingfromcell.8.22 Theproblemswithpropertyarenothelpedbythelackofanationalfocusonoutcomes
-noneoftheprisonperformanceindicatorsassessessuccessandefficiencyinmanagingprisoners’property.Itcantakeweeksforpropertytocatchupwithatransferredprisoner,bywhichtimehemayhavemoved.InonecasetheIMBwaschasingpropertyfromsixprisonsago!
8.23 WearepleasedthattheGovernorrecognisedtheineffectivenessoftheirpracticesregardingprisonerpropertymanagementandtheamountoftimespentbystafftryingtolocateandrecovermissingitemsofclothingandproperty,fromotherinstitutions,onprisoners’behalf.Thesteadystreamofcompensationclaimswasalsoamotivatingfactortoimproveproceduresandpractices.Asaresult,theprisoncarriedoutafullreviewofthemanagementofproperty.
8.24 Thefinalreport,boththoroughandrevealing,wassubmittedinFebruary2018.Implementationinitiallywasslowbutrecentlywehaveseensignificantprogressincluding:
§ ThePropertyRoominReceptionhasbeenre-organisedandisnowoperatingmoreeffectively-forexample,themanyblackbagsofprisonerproperty,whichoncecoveredmuchofthefloor,havebeensortedandallocatedtothealphabeticallyarrangedshelvingsystem;
§ Volumetricboxeshavereplacedtheinsecureandungainlyblackplasticbagsystem;
§ AnewformhasbeenintroducedfortrackingpropertyonceithasleftBedford;and
§ Asafurthersystemcheckforensuringaccountability,transparencyandentitlementofproperty,cellclearanceformshavebeenreinstated.
8.25 SERCO,thecompanyresponsibleforprisonertransfers,iscurrentlyoperatingathreebagperprisonerpolicyduringtransportation.Thisfrequentlyleadstosurplusbaggagebeingleftbehindfordispatchatsome,unspecified,futuredate.Thiscanthenresultina

prisoner’ssenseofdeprivationandfrustration,waitingformanyweeksormonthsevenforhismissingpropertytoarrive.Thisisnotsatisfactory,andwewouldaskthatHMPPSexploresabettersolution.
Transfers,foreignnationalsandHDCs8.26 Itisthepracticeoftheprisonservice,wearetoldforreasonsofsecurity,totransfer
prisonerstootherprisonswithnonotice.Thismeansthatprisonerscannotinformtheirfamilies,expressanypreferenceorprepareeffectivelyfortransfer.Thishardlyseemsdecentanditisnotunusualforprisonerstorefusetotransfer,whichcanresultinaphysicalaltercationwithstaff.
8.27 Wearepleasedtonotethatatthetimeofwritingthisreport,theprisonhaschangeditspolicysothatsomenoticeisbeinggiventoprisonersbeingtransferredtoacategoryCprison.Thisisastepforward.
8.28 TheBoardhasreceivedanumberofapplicationsfromCategoryDprisonerswhohavebeeninHMPBedford(acategoryBprison)foranunreasonablelengthoftime.Inonecase,aCategoryDprisonerwasthereforthreemonths,primarilybecausetransportcouldnotbearranged.Whilstweunderstandthecomplexitiesofprisonerallocation,itisnothelpfulforanyoneforprisonerstobeheldinthewrongcategoryjail.
8.29 AnincreasingnumberofapplicationstotheIMBfromforeignnationalprisonershasemergedasarecenttrend.Ingeneralprisonersservehalftheirsentence(consideringanyremandtime)andarethenreleasedonlicence.Thesituationismorecomplicatedforforeignnationals–theycanbedeportedafteraquarteroftheirsentenceorkeptinprisonafterthenormalhalfsentencehasbeencompleted,iftheHomeOfficehasregisteredaninterestinthem.Prisonersstruggletounderstandthesystemandhowtoengagewithit,particulariftheirgraspofEnglishislimited.Wethinkitwouldbehelpfulifaleafletwasproduced,inthemainlanguages,thatexplainedtoforeignnationalsthesystemandcontactpoints.
8.30 ThenewHMPPSguidelineshaveledtoabigincreaseinthenumberofHDCs(homedetentioncurfews,otherwiseknownastagging)beingissued.Itisamorestreamlinedprocess.ByMay2018HMPBedfordhadissued25HDCs,whichisaboutdoublethatforthewholeofthepreviousyear.Ingeneral,wewelcometheuseofthesekindsofnon-custodialalternatives,providingthatthesafetyofthepublicandsecurityisnotcompromised.
9HEALTHCARE(includingmentalhealth)
Healthcare9.1 TheHealthcareservicesatHMPBedfordarecurrentlyprovidedbyNorthamptonshire
HealthcareFoundationTrust(NHFT)andcompriseofasubstancemisuseservice(IDTS),includingpsychosocialservicessubcontractedtoWestminsterDrugProject(WDP),mentalhealthservices,pharmacyservices,inconjunctionwithLloyds,andprimarycareservices,includinganIn-PatientUnit(IPU).
9.2 Waitingtimescomparefavourablywiththoseexpectedinthecommunitywith,onaverage,atendaywaittoseeadoctorandfiveweekstoseethedentist.MoreconcerningisthenumberofDNAs(didnotattend).Overthefirst6monthsof2018anaverageof21percentofdoctor’sappointmentsweremissedand33percentofdentalappointments.Thesefigureswouldbeconsideredunacceptablyhighinthecommunitysetting.Themajorityofmissedappointmentswerebecausetheprisonerdeclined.
9.3 TheNHFThasscoredwellinuserfeedback.Itisregisteredwith‘iWantGreatCare’tomonitoritspatientsatisfactionsurveys.Prisonerresponses,onaverage20permonth,aresentto‘iWantGreatCare’tocollatethedata.Outofapossiblemaximumscoreof5.00

HMPBedfordscored4.68inJune2018,anincreaseontheaverage4.55intheprevious6months.
9.4 However,intheMQPLsurveyonly25percentofrespondentsagreedthat‘wingstafftakeaninterestinhelpingtosortout’theirhealthcareneeds.Thereneedstobeagreaterunderstandingofwhyan(ostensibly)goodservicehassuchhighratesofmissedappointmentsandisnotwellvaluedbyprisoners,
9.5 PrisonerswithsubstancemisuseissuescomingintoreceptionshouldideallyspendtheirfirstnightonthededicatedDWingwhichprovidesIDTSservices.Heretheycanbeobservedatregularintervalsbynursingstaff.Itisdisappointingthatthisdoesnotalwayshappenbecauseoflackofspace.
MentalHealth(MH)
9.6 Inourview,theprofileandlevelofprovisionofmentalhealthservicesisinadequate.Inlastyear’sreportwewerecriticalofthementalhealthteam’sfailuretoattendACCTandsegregationreviewsonaregularbasis.Thissituationhasnotimproved.Inourviewthisremainsasignificantfailurethatneedstobeaddressed.
9.7 Apartfromthevalueoftheirspecificexpertiseinthesereviews,webelievethatahigherprofilewouldhelpraisethegeneralawarenessofmentalhealthissueswithintheprison.Atpresent,fewcustodystaffconsidermentalhealthissueswhentryingtounderstandprisoners’behaviour.Whilstnotallstaffcanbeexpectedtohavetheexpertiseofthementalhealthteammembers,anincreasedawarenessofpossiblepsychologicalandpsychiatricexplanationswouldhelp.Thisisparticularlyimportantatatimewhenthementalhealthteamarenotengagedinanyin-housetrainingonmentalhealthforcustodystaff.
9.8 Therehasbeenoneexampleofatherapeuticgroupinitiativebeingdelivered(onmood/angercontrol),whichwasledbytheClinicalPsychologistandanAssistantPsychologist.Thisisverymuchtobewelcomedandwebelievethatmoregroupworkwouldbeaneffectiveandefficientwayofaddressingsomeofthedemandproblems,particularlyinrelationtothosewithcommonmentalhealthdisorders.Italsorepresentsanopportunitytoinvolveprisonersdirectlyinthedesignanddeliveryoftherapeuticinterventions.
9.9 Therehasbeennoprogressinthefollowingareas,thatweraisedinlastyear’sreport:§ Prisonersreportinglongdelaysinbeingseenbythementalhealthteam
members(andsometimesnotbeingseenatall);
§ Nosystemforauditingtheprogressandoutcomesofnewreferrals;and§ Nocheckonthequalityofassessmentsinreception,whichareundertaken
entirelybygeneralhealthcarestaff.9.10 ThementalhealthteamisnotformallyinvolvedintheprocessofInductiononE-Wing.
Giventheknownvulnerabilityofprisonerstoseriousself-harmincidentsearlyintheirsentence,webelievethatthesearrangementsshouldbereviewed
9.11 Wewouldalsowelcomearoutineauditofprisonersatisfactionwiththementalhealthserviceprovided,asiscurrentlycarriedoutbythemainstreamhealthcareservice.Itiseasytoforgetthatprisonersthemselvesareagreatsourceofexpertisewhenitcomestocopingwithsomeofthecommonproblemsoflivinginacustodialenvironment(forexample,sleepdifficulties,copingwiththoughtsofself-harmandresistingdruguse).
9.12 Themostimportant,andmostnumerous,resourcewithintheprisontodealwithmentalhealthissuesforprisonersaretheofficers.Wehaveseenmanygoodexamplesofofficersinteractingwithprisonersinwaysthatareclear,honest,sympatheticandempathic.Theseinteractionsfacilitatetheformationofpositiverelationshipsandare

likelytobethemostpsychologicallyhelpful.Indeed,theyarethebedrockofawell-runprison.
10EDUCATIONANDOTHERACTIVITIES
Coursesandattendance
10.1 Recentsurveyssuggestthatjustunderhalfofthoseenteringtheprisonsystemhaveliteracyskillsnohigherthanthoseexpectedofan11-year-oldchild.Formanyprisoners,educationisassociatedwithasenseoffailure,ratherthanasameansofself-improvement,ofincreasingasenseofpersonalworthandofaccessingrewardingemploymentopportunities.Forthesepeople,educationholdsnopromise,noattractionandnoenjoyment.
10.2 Itisimportant,therefore,thatthetypeofeducationofferedhelpstochangethatperception.However,thenationalcontractwitheducationsuppliersfocussesonstandardisedtests,whichdoesnothelpprovidethetypeofteachingthatmotivatesprisoners.
10.3 Theprovisionandrangeofshortandmedium-termopportunitiesregardingthecreativeartsandothermoreintrinsicallyappealingoptionsremainsunsatisfactory.Withtheexceptionofthecomputer-generatedmusiccourseandthebriefexcursionintoscriptwritingwithavisitingtheatregroup,therearenootheropportunitiesforprisonerstoexploreortodeveloptheircreativeawarenessortoincreasetheirsenseofachievementthroughtheexplorationofthesensoryskills.
10.4 Onlyaround15percentofprisonerschoosetoengageineducationandbookthemselvesoncourses.Eventhenattendancerates,atanaverageof70percent,aredisappointing.Butwedorecognisethatthisisanimprovementonthe50percentofayearago.
10.5 F-wingeducationhowever,bucksthistrend.Thevulnerableprisonerwinghassignificantlyhigherlevelsofbasiceducation,higherlevelsofengagementandmuchbetterattendancerates.Itisalsoveryconvenientlylocatedwithinthewing.
Basicskills10.6 Thebasicskillstestisakeyinitialassessmentsothattheprisoncandeterminea
prisoner’sleveloflearningandtheappropriatelevelofsubsequentcoursesneeded.Passingthetestisanessentialrequirementbeforemovingtoemploymentintheprisonortoappropriateeducationclasses.However,attendanceratesfortheseclassescontinuetobeverypoor.Manyprisonersrefusetoattend,andtheirnamesarethenreturnedtothebackofawaitinglist,onlytorefusetoattendforasecondandthirdtimewhentheirnameoncemoreappears.Allthewhile,thewaitinglistgrowsinnumberwitheachnewintakeofprisoners.
10.7 Theeducationdepartmenthasfrequentlyarrangedadditionalclassesinbasicskillsinordertocombatthelongwaitinglist,buttheproblemsremain.Inourview,thissituationhasarisenprimarilybecauseofbreakdownofappropriateincentives(and,wherenecessary,sanctions)atwingleveltoensurethatthoserequiredtoattend,doindeedarriveattheirclasses.
10.8 InJune2018,asameansofbreakingthedeadlock,theGovernorsuspendedtheoncemandatorybasictestingforallonentry,sothatmoreprisonersmightbeemployedandspendlesstimeintheircells.Whilstweunderstandthereasonsforthis,weareconcernedthatprisonersareremovingthemselvesfromthepossibilityofanyofthebenefitsthatcanbegainedthroughtargetedlearningexperiencesandfromachievinganyoftheeducationaladvantagesthatresult.Itisadifficultbalancetostrike.

10.9 OutreachandsupportworkinEnglishandmathematicsneedsafreshfocusandmoreconsistency.Weunderstandthatdeliveringthisprogrammeislinkedtostaffavailability,whichcanbevariable.Therehavebeenoccasions,throughouttheyear,whenbehavioronthewingshasbeenjudgedtoounpredictableforoutreachworktotakeplace.
10.10 Thereisalsoaclassroominthehealthcarewing,whichoffersapersonalandsocialdevelopmentcourse.Thisisespeciallyneededwithsuchvulnerableprisoners.Itiswell-attended,andwehavebeenimpressedbythequalityofteaching.
Educationinitiatives10.11 Despitethedifficulties,theprisonhasputinplaceanumberofinitiativestostrengthen
andvaryeducation,learninganddevelopmentopportunities.Thesearemuchtobewelcomed.Forexample,thepersonalandsocialdevelopmentcourse,withitsmodulesonapplyingforajob,thequalitiesandqualificationsneededinvariousemploymentsanditsintroductiontocounselling,isasuccessfulcoursethathasshownfurtherimprovementsinachievementsthisyear.
10.12 Similarly,the‘IntroductiontoCounselling’,isanewcourse,beguninJuly2017,thatfocussesonlistening,speakingandeducationskills.Thisisrelevantandpurposefullearningandcanaccommodatesixlearnersoverfoursessions.Atpresent,numbersarelow,althoughindoublefigures,butstaffconsiderachievementtobehigh.
10.13 Theeducationdepartmentalsoprovidesappropriatecoursesfornon,orlimited,Englishspeakerswhoarriveattheprisonandexperiencecommunicationdifficulties.Understandably,thenumbersrequiringthisprogrammefluctuatethroughouttheyear.Whennumbersquicklyrise,thereareoftenwaitinglistsforthecourseasitsdeliveryisnotsomethingthatcanbesupportedeffectivelyaspartofthein-celllearningfacility.Inotheraspectsofin-celllearningsupport,thementorsorganisedbytheShannonTrusthavebeenhelpful
10.14 TheSynergyTheatreGroupvisitedinJunetodeliverascriptwritingcourse.Thiswentwell,althoughnumbersinvolvedwerelow.
10.15 Reactiv8isagoodexampleofinnovativeandeffectiveworking.Itisa‘course’runbyasocialenterprisecompany,whichhasbeenoperatingforthelastthreemonthsintheprisonand49prisonershavetakenpart.Thethemeisaroundmotivation–tohelpprisonershaveadifferentviewofthemselvesandtodevelopconfidence.Thefeedbackfromprisonershasbeenverypositive,withnoticeablechangesinattitude.
10.16 InNovember2017,ateamofeducationspecialists,includinganOfstedinspector,carriedoutadetailedhealthcheckontheeducationdepartment.Theirconclusionwasthatthequalityofteaching,learningandassessmentwasgood,butthatleadershipandmanagementrequiredimprovement.Thisconfirmsourviewthattheteamhasthepotentialtodelivergoodeducationservices.
Library10.17 TheLibraryisavaluableresourceandanattractivelearningenvironment.Whilst
prisonersfromthewingscananddovisitthelibraryonarotabasistoborrowbooks,educationclasseshavebeenunabletomakeuseofthiswell-maintainedprovisionsinceitwasmovedfromitsoriginalbase,withintheeducationdepartment,toamoredistantandlessaccessiblelocation.Thisisregrettable,and,inourview,itslocationshouldbereviewedagaininthelightofthisexperience.
10.18 Inrecentmonths,educationandlibrarystaffhavetriedtoestablishapartsessioneachweeksothateducationclasseswillbeabletoattendthelibraryaspartofastructuredsessionratherthana‘freeforall’.Theyhavereachedagreementonthetimesanddaysandarenowwaitingforariskassessmenttobecompletedthatwouldaccommodatethemovementoflearnersinsession.Thisisawelcomedevelopment.

Chaplaincy
10.19 Werecognisehowimportantthechaplaincyteamistotherunningoftheprison.Notonlydotheyattendtospiritualandreligiousneeds,theyarealsoauniquelyrespectedandtrustedgroup.Theygiveadvice,guidanceandcomfortandarehighlyrespectedasanindependentvoice.
10.20 Inaddition,thechaplaincyteamplaysastrongroleinsupportingtheequalitiesagenda,particularlyinfaith-basedmatters.ThefactthattheleadclergyisanImamsetsahugelypositiveexample.Wewarmlycongratulatetheteamfortheirroleincontributingtoapositiveatmosphereinbothreligiousandsecularmatters.
Gym10.21 Thegymnasiumcontinuestobeaverypopularfacility.Weseequiteadifferent
atmosphereheretotherestoftheprison–thereisarespectfulandadultrelationshipbetweenofficersandprisonersandthisismuchtobewelcomed.Unfortunately,whilstwesaidinourlastreportthatopeninghourshadbeenextendedtoincludetheeveningsandweekends,thisisnownolongerthecase.
10.22 Thestaffarekeenandwillingtomakethegymaswidelyavailableaspossibleandwewouldverymuchsupporttheminthis.Inmanyways,staff-prisonerinteractionswithinthegymprovideamodelforgoodrelationshipsthatmightbeaimedforthroughouttheprison.
10.23 TheBoardwouldliketoseegreateruseoftheAstroTurffootballpitch,whichnowappearsabandoned,evenasanalternativeexerciseyard.
11 WORK,VOCATIONALTRAININGandEMPLOYMENT
11.1 Workshopactivitiescanhardlybesaidtobepurposefulandremainstultifyinglymundane.Theyusuallyconsistofsimpleassemblytasks(forexample,puttingtwoelementsofabadgetogether).PrisonersfeelthatthisisinadequatewiththeMQPLsurveyshowingthatonly18percentbelievedthatenougheffortwasbeingmadetostopthemoffendingonrelease,while71percentbelievedthattheywere‘doingtime’,ratherthan‘usingtime’.
11.2 Thelocationofthemainwingworkshopsmeansthatprisonershavetobebroughtfromeachwingseparately.Thiscontributestotheverylowlevelofattendance,whichisnormallylessthanahandfuloutofacapacityof17.However,agoodqualityofworkisproducedintheF-wingworkshop,whichisdirectlyoffthewing,andhasgoodnumbersattending.
11.3 WearepleasedtoseethatthepaintinganddecoratingtrainingfacilitywasreopenedinJuneafteralongperiodasastoragefacilityforriotgear.Whilstnumbersarealsolow,duetothephysicallimitationsofthespaceavailable,thisisawellorganisedandoversubscribedcourse.
11.4 ThepopularandsuccessfulBICS(BritishInstituteforCleaningServices)coursehashadtobetemporarilysuspended,duetoprolongedstaffillness.Itisnotknownwhenthissought-aftercoursewillbebackonthecurriculum.TheLaundrycourse,whichrunsintheprison’swell-equippedmainLaundryfacility,continuesasawellorganisedandmanagedoption,albeitforverylownumbers.
11.5 TheCSCScourse(asafetyqualificationforworkingonaconstructionsite)isverypopularandcontinuestobeoversubscribed.UnitsofworkaredeliveredinclassandviaaMobileTestingUnitbusthatparksintheprisononaregularbasis.PassratesarehighandtheCSCScard,whichthecourseawards,isvalidforthreeyearsandessentialforthosehopingtoworkonabuildingsiteonrelease.Therewasevenaninstanceofaprisonerdelayinghisreleaseinordertobeabletotakethetest.

11.6 Theother,highlyvalued,skillstrainingavailableare:§ baristatrainingisprovidedinthecafethatispartofthevisitsarea;and
§ thekitchenprovidestheopportunitytoobtainfoodsafetycertification.
12 RESETTLEMENTPREPARATION
Pre-ReleaseBoard12.1 AtHMPBedford,prisonerscomingtotheendoftheirsentencearelookedafterbythe
Pre-ReleaseBoard(alsoknownastheDischargeBoard)whichisout-sourcedtotheStGilesTrust.Theyalsotakeresponsibilityfor,andcomplete,Part2oftheBasicCustodyScreeningTool(BCST)foreachprisoner.
12.2 ThePre-ReleaseBoardtakesplaceeveryWednesdayafternoonandisaverypositivemeeting.Prisonersareinvitedtoattend12weeksbeforetheirreleasedate,givingthemtimetoengagewiththeprocessofreleaseandtodiscusstheirneedsonleavingcustody.ThemembersofthePre-ReleaseBoardworkcloselytogethertoofferadviceandassistanceincluding:
§ Accommodation/resettlement-inJune,ofthe53prisonersreleased,18wereclassedasNFA(nofixedabode)althoughthisismisleadingastheycouldbe‘sofasurfing’orlivingwithfamily/friends;
§ Employment-asBedfordisalocalprison,therearesomeprisonerswho,havingservedonlyshortsentences,arereturningtotheirformeremploymentandforthosewhowanttoworkthereishelpwithbothwritingCVsandapplyingforjobs;
§ Drugandalcoholsupport-thePre-ReleaseBoardworktowardsensuringthattheseprisonersreceivehelpwithboththeirproblemsandinsecuringthosebenefitsthattheyareentitledtoonrelease;and
§ Money,debtadviceandopeningabankaccount.12.3 Despitethisexcellentandcomprehensiveapproach,wedohaveanumberofareasof
concern.TheworkoftheBoardisnotwellintegratedwithintheprisonandthereisalackofreportingonkeyoutcomesoroperationalmetrics.Inaddition,noonefromthecoreprisonserviceattendsthemeetings.
12.4 ItisimportantthatprisonersattendthePre-ReleaseBoardsothattheycanbepreparedforwhentheyleave.Veryoftenattendanceislow–atarecentmeetingonlythreeoutof13attended.Thismeansawastedopportunityfortheprisonerandwastedworkfortheboardmembers.Inourviewitisaprioritytoaddressthis.
12.5 TheBoardisnotabletoadviseprisonersontravelwarrantsormoniesthattheywillbepaidonthedaytheyleave.Receptionholdthisinformation.ItwouldmakesenseforeitheramemberoftheReceptionteamtoattendthePre-ReleaseBoardeachweektoadviseor,ifthatisnotpossible,anemailtobesenttotheChaireachweekgivingthisinformationforeachprisoner.
12.6 Oftenhousingisnotconfirmeduntilthedaybeforeoreventhedayofreleasewhich,notsurprisingly,cancauseagreatdealofanxietyforaprisonerwhoisconcernedaboutwhetherornottheyhavesomewheretosleepthatnightonrelease.Alternatively,aprisonerisreleasedearlierthanexpectedandthereforedoesnothaveaccommodationinplace.ThereappearstobepoorcommunicationbetweentheprisonandthePre-ReleaseBoard:forexample,itsmembersarenotalwaysinformedwhenaprisonerisbeingreleasedearly.

12.7 Onceaprisonerleavescustody,itisdifficulttofollowupwhethertheyengagewiththeservicesthattheyarereferredtoandwhathappenstothem,particularlyiftheyarefromoutofarea-thereforesuccessrates,withregardtoaccommodationandemploymentarehardtotrack.Itisdisappointingthatsolittleinformationexistsabouthowprisonersgetonwhentheyleavejail.Werecognisethatthisisanationalproblem,butitisdifficulttounderstandhowGovernorsaremeanttoevaluatetheeffectivenessoftheirjailwhentheygetnoinformationabouthowreleasedprisonersleavingfare,onceinthecommunity.
UniversalCredit(UC)
12.8 TheIMBcontinuestohaveseriousconcernsabouttheprocessesaroundtheintroductionofUniversalCredit.Underthecurrentsystem,theJobCentrePlusstaffcomeintotheprisonthreetimesaweektoarrange,withtheprisoner,foranapplicationforJobSeekersAllowance(JSA)tobemadebeforeheleavesprison.Theprisonerwillthenleavewiththepre-releaseallowanceof£46andismeanttoreceiveJSAwithinonetotwoweeks.
12.9 However,whereaprisonerisreleasedintoaUCarea(andBedfordisone),theJobCentrePluscanonlymakeanappointmenttoseeaUCadvisorafterrelease.Onlythencantheapplicationprocessstart,andthereleasedprisonerapplyon-line(forwhichhemayhaveverylittleexperienceorskills)–paymentofUCmaythentakeafurthereightweeks.Thisisscandalousandcompletelyinefficient.Wecannotseeareasonwhyprisonerscannotbepermittedtomaketheinitialapplicationswhilstinside.
MAPPA(Multi-AgencyPublicProtectionPanelsorArrangements)12.10 ThepurposeofMAPPAistoensurethatmorecomprehensiveriskassessmentsare
completed,thustakingadvantageofthecoordinatedinformation-sharingacrosstheagencies,andtodirecttheavailableresourcestobestprotectthepublicfromsexualorseriousharm.
12.11 ItisalsotheprocessthroughwhichthePolice,ProbationandPrisonServiceworkstogetherwithotherAgenciestomanagetheriskposedbyviolentandsexualoffenderslivinginthecommunity,inordertoprotectthepublic.MAPPAisnotastatutorybodyinitself,itisamechanismthroughwhichagenciescanbetterdischargetheirstatutoryresponsibilitiesandprotectthepublicinacoordinatedmanner.TheNationalProbationServiceTeamatHMPBedfordattendallMAPPAmeetingseitherbytelephoneparticipation,orinpersonformorecomplexcases.Asfarasweareaware,MAPPAisworkingwellintheBedfordarea.
SentencesofImprisonmentforPublicProtection[IPPs]Cases.12.12 Currentlytheteamattendsbi-monthlyforumsforourliferandIPPPopulation.The
teamhasregulareventsscheduleduntiltheendoftheyear.IntermsofIPPmanagement,theSeniorProbationOfficerandanexpertfrompsychologymeetmonthlytoreviewourIPPpopulation.TheylookatwaystosupporttheOffenderSupervisorandtheindividualtoprogressintheirsentenceandtoestablishmentswheretheycanworktowardsParolerecommendationsand/oreventualrelease.
