Register1. Visit the MMCC Registry website and click the

Create Account link under the Patients heading. Bit.ly/MarylandMedicalCannabisRegistry

2. Complete the ‘Before You Begin’ section to review and confirm that you have the required documents and information ready for upload.

3. Complete the online application form by uploading the required files.

Adult Patients (18+ years of age):Any non-Maryland resident currently in Maryland for the purpose of receiving medical treatment is eligible to register as a patient and, during registration, will be required to provide the address of and a contact name for the Maryland medical facility where the patient is receiving treatment.

Submit1. You must have a valid, accessible email account.

2. Complete and submit the online application form on the MMCC Registry website.

3. MMCC must review and approve your application.

• Approval indicates you are registered with MMCC

• You must still obtain a valid written certification to purchase medical cannabis.

• Once approved, registration is valid for two years. No fee to apply

Verify1. After submitting your online application,

CaregiverOnce approved by MMCC, you can log in to the MMCC Patient Registry to select a caregiver.

Minor Patients (Under 18 years of age):Minor patients are required to have at least one caregiver assigned to their accounts at all times. Only parents or legal guardians 21 years of age and older are eligible to serve as caregivers for minor patients. Caregivers must register with the Commission prior to registering the minor patient. Registering a minor patient requires additional forms, one of which must be notarized.

IDsID cards are optional for patients purchasing in Maryland. Care givers must aquire an ID card. An ID card cannot be purchased until a valid written certification has been ob-tained from a provider registered with MMCC Patient ID. Card fee: $50

If your photo does not meet the stated guidelines, your application will be returned.• Photo must look like a passport

or driver’s license photo.

• Must be a clear, color photo of the head and shoulders

• Must be an JPG, PNG or GIF file no larger than 3 MB in size

• 2” x 2” photo, minimum size of 600 x 600 pixels, 300 pixel resolution

• Recent within the last six months

DispensaryAfter your registration has been approved by MMCC and your pro-vider has issued you a valid written certification, you are ready to visit a licensed Maryland dispensary to purchase medical cannabis.

CertificationVisit a registered provider to obtain a valid written certification. Medical cannabis cannot be purchased without a valid written certification from a provider registered with MMCC Certifications are valid for a maximum of one year.

Please note: If nothing is dispensed from a certification within 120 days of issue, the certification expires and you will need to obtain a new certification from your provider.

Once registered, patients must obtain a valid written certification from a provider registered with the Maryland Medical Cannabis Commission. The patient is required by law to have a bona fide provider-patient relationship with the provider and the provider must approve medical cannabis as a treatment option for the patient for a qualifying medical condition. Qualifying medical conditions include cachexia, anorexia, wasting syndrome, severe pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, post-traumatic stress disorder and chronic pain.

The Provider Registry is open and the Commission encourages providers to register. Any provider with an active, unrestricted license in good standing with the Maryland State Board of Physicians and actively registered to prescribe controlled substances in Maryland is eligible to register with MMCC.

When both the patient and provider are registered with MMCC, the provider will need the patient’s MMCC-issued Patient ID Number to issue the certification for medical cannabis through the Commission’s secure online application.
