Better Choices, Better Health Program The Better Choices, Better Health program (also known as the Chronic Disease Self-Management Program, or CDSMP) is a free workshop for adults 60 years of age and older that is given two and half hours once a week, for six weeks. The program aims to help its participants feel better, regain control of their health and start Older Adult Health Insurance Counseling APPRISE is a free health insurance counseling program designed to help Pennsylvanians, age 60 years and older. APPRISE volunteer counselors are specially trained to answer consumer questions and offer education about Medicare, HMOs, long-term care insurance, supplemental insurance, and Medicaid benefits. APPRISE counselors are available to assist an individual in the following ways: Determine if a Medicare HMO is right for the individual by explaining the way Medicare HMOs work. Better Choices, Better Health ProgramThe Better Choices, Better Health program (also known as the Chronic Disease Self-Management Program, or CDSMP) is a free workshop for adults 60 years of age and older that is given two and half hoursonce a week, for six weeks.

The program aims to help its participants feel better, regain control of their health and start doing the things they want to do.

The workshop is offered in a community setting, and provides support for continuing normal daily activities and dealing with the emotions brought about by chronic health conditions.

Older Adult Health Insurance CounselingAPPRISE is a free health insurance counseling program designed to help Pennsylvanians, age 60 years and older. APPRISE volunteer counselors are specially trained to answer consumer questions and offer education about Medicare, HMOs, long-term care insurance, supplemental insurance, and Medicaid benefits. APPRISE counselors are available to assist an individual in the following ways:

Determine if a Medicare HMO is right for the individual by explaining the way Medicare HMOs work.

Understand Medicare benefits Select a Medigap insurance

policy Obtain assistance to pay for

prescription drugs through government and private programs that offer this service

Find government programs that will pay Medicare deductibles, co-payments, and Part B premiums and assist consumers with the paperwork.

Understand long-term care

APPRISE services are free, objective and completely confidential.

Senior Farmers’ Market Nutrition Program

The annual Senior Farmers’ Market Nutrition Program (SFMNP) is available to eligible consumers across the county. If an individual is eligible they may receive one set of checks to be spent at participating farmers’ markets. One set of checks is worth $20.00 (issued as four checks worth $5.00 each).

Eligibility: Allegheny County resident 60 years of age or older (by

December 31, 2014) Meet the income guidelines

(self-reported): One person household: At or

below $21,590 Two person household: At or

below $29,101**Married couples may each receive one set of checks Older adults living in a nursing

home, convent, or residential facility that provides meals on a regular basis are not eligible to receive checks

Distribution Day: Second week of JuneDistribution will take place at most senior centers in your area. Checks are distributed on a first come, first served basis. At the time of distribution be sure to bring identification of proof of age and residency.

Training for low income individuals that meet:

Services include: Community-service, work-

based training in a variety of government or non-profit agencies

Resume preparation assistance Interviewing skills training Job searching
