Accident Recording and Reporting Reference Guide (OSHA) What types of accidents or injuries must be recorded? Every accident or injury, no matter how minor, must be recorded internally; therefore, it is the responsibility of every employee to immediately report every accident and/or injury that occurs at work to his/her supervisor. Injuries requiring only first aid will be designated as such. What types of accidents or injuries will be investigated? Every accident and/or injury will be investigated when it occurs. The form entitled “Supervisors Accident Investigation Report” will be completed during this process and forwarded to the respective HBMA Safety Department upon the completion of the accident investigation. What types of accidents or injuries must be recorded for OSHA? Every incident resulting in occupational injuries or illnesses must be recorded on the OSHA No. 200 form and maintained on a calendar year basis. A supplemental record, which includes additional information, must be recorded on the OSHA No. 101 form and maintained on a calendar year basis. The OSHA No. 200 form must be posted on the employee’s bulletin board no later than February 1 of each year and must remain posted until March 1. All completed OSHA forms must be retained for a period of five years from the end of the calendar year to which they relate. What types of accidents or injuries must be reported to OSHA? 1
