    REGISTRATION

    A place will be reserved for you upon receipt of your registration payment. Please make your payment either by Checkor Cash (within 30 days from commencement date) when you receive our invoice. All checks should be crossed and

    make payable to PhilEvents Management & Advertising Co. with the invoice no. indicated at the back of the check.

    REFUND POLICY, CANCELLATIONS/SUBSTITUTIONS

    Full refunds for the cancellations will be given until April 30. From May 1 through June 30, the registration fee less

    50% will be refunded. After June 30, no refunds will be given. There is no charge for substitutions, but you must

    submit a letter authorizing the transfer signed by the registered delegate one day prior to the event starting date.

    CONFIRMATION DETAILS

    1. Confirmation of your booking will only be sent when FULL PAYMENT is received. All posted registrations mustbe accompanied by a check or cash.

    2. Please treat this form as our request of payment3. If you do not receive a letter confirming the attendance two weeks prior to the event, please contact the

    PEMAC office.
